My father was a patient at Decatur Center for Nursing and Healing from December 13th until his final days in early February. I have a mixed perspective on the facility, as my experience was shaped both by my own observations and my father's perspective. Many of my concerns with the facility could be considered relative, as they stemmed from communication and language barriers. There were instances where my father’s needs (which I will elaborate on shortly) may not have been fully understood because the staff did not make an effort to dig deeper and determine exactly what he required. This issue primarily involved food options. My father did not eat cheese or pork, and it took 2.5 weeks”and my persistent advocacy”to get this documented. He had only one direct issue with a specific staff member who worked at the nurses’ station. He described her perfectly, and I personally witnessed her lack of professionalism (unrelated to my father) while standing at the nursing station. She’s not worth describing in detail. Another minor issue I encountered arose when my father had two separate emergency room visits during his stay. Toward the end, I received a call from someone informing me that paperwork regarding my father’s stay had not been completed. At this point, he was on life support, and two months had already passed. I had been at the facility every other day (if not daily) to the extent that the staff recognized me. It was disheartening and extremely unprofessional for a medical facility to demonstrate such negligence in documentation”especially when I was so deeply involved in every aspect of my father’s care. Regarding the rehab team, I want to express my love and appreciation for Vanni. We spoke on many occasions, and she was informative, humorous (which was much needed), and genuinely had my best interests at heart when advising me on how to proceed with my father’s care. I also appreciated the thoughtful questions and updates I received from the speech therapist. I have a list of staff members I want to acknowledge for making my father’s stay as positive as it could be. Beharne (I may have misspelled his name) and Chris were very forthcoming and accommodating on the nursing and tech side of things. I never met Solo, but I appreciate the behind-the-scenes work he did to support my father’s care. When my father had to go to the ER, LaKeshia was a Godsend”warm, understanding, professional, and informative. Ms. Bonita at the front desk was always a joy to see, and she helped me with any needs I had. Ms. Mary was also someone I deeply appreciated for her support and kindness. Towards the end of my father’s stay”while he was in the ICU”I noticed that some of his key belongings (his smartphone, glasses, etc.) were misplaced, even though I had already picked up most of his other personal items. For about a week, I was sent on a frustrating search between the hospital (which diligently attempted to locate them) and the facility. Initially, the facility claimed they could not find anything, and there was a great deal of miscommunication. Just as I had resigned myself to seeking compensation for the missing items rather than continuing the search, I received a call”at the absolute last minute”informing me that the facility had located them. This leads me to highlight Nikki. Nikki works on the business side of patient cases, and when I say she held my hand throughout my father’s time at the facility”I truly mean it. She is working in her calling. Nikki kept me well-informed about my father’s potential discharge details, provided phenomenal service in helping me locate assisted living facilities, communicated back and forth with insurance personnel on my behalf, and played a crucial role in coordinating with the custodial/laundry staff to find my father’s missing items. Thank you, Nikki. I hope no one has to ever experience a loved one in sub-acute rehab. What I will say is that you MUST advocate for them in the event that they are placed in one and locate the people that you know will get the job done as well.

As soon as I entered the front door, I was greeted with a very friendly smile from the receptionist. She was very helpful in making sure I signed in correctly and how to sign out. Every staff person I encountered spoke with a friendly greeting. I have 2 love one's here and I feel great about their care.

During my visit to the Decatur Center of Nursing and Healing, I found the overall experience to be organized and efficient. I arrived before 5 PM and was able to engage with both the case manager and social services. They provided me with a comprehensive discharge plan and outlined the next steps, although they were unable to confirm a specific discharge date. They assured me that they would keep me informed of any updates. Upon entering the facility, I was greeted promptly by the staff, which created a welcoming atmosphere. I was pleased to see that the staff recognized the patient I came to visit, indicating a personal connection to the residents. Throughout my visit, the team remained focused on the care of the patients and demonstrated a high level of attentiveness, even amidst their busy schedules. Ebonie, in particular, took the time to discuss my father's health with me, which I found to be very supportive. Overall, the visit was productive, and the staff's professionalism contributed positively to my experience.

My Auntie was at this facility and it was really sad how the residents are taking care of. No soap to wash there hand. ™ The nurse that was there on June 21, 2026 at 7pm acted as if nothing was a priority. My auntie was supposed to be taken to the hospital at 3pm and was still there at night. She had thrown up on herself, eye was dropping, couldn’t see me when I came in. Then to top it off one of the residents had fallen. I heard the cry for help from one of the other residents. I went and saw him on the floor. I rushed and yelled for help she took her time. I ask do you need me to help you. She said no but with no urgency. We called 911 to help my auntie. Then the nurse acted like she cared. This facilities need to stop using AI to hire because being unstaffed is hurting people we love. My auntie ended up with staph in her blood and pneumonia. My auntie had a stroke that was the reason she was at this facility. The two women who were taking care of her was very helpful. Some who look’s out for the elderly should checkout this facility. This place has a wonderful rehab facility and the therapist are extremely skilled. Especially Michelle, Evida. And Annessa was a wonderful CNA I loved her and she was always there when called and extremely helpful. Other than that I was very unhappy with my stay.

This was my first time dealing with a rehab center, and I must say the individuals at this center are truly professionals. I was taken care of as a visitor.,I was able to speak with nursing personnel, the information desk was more than accommodating. This was my first experience with a rehab center. I will definitely be telling others who may need this type of nursing care how great this place is.

This place is good for therapy but awful for everything else. The nurses are professional but have no bedside manner. We only encountered about 4 nurses the whole month that we were there who were helpful. Only 2 CNA’s really helped. It was hard to get anyone to help at night. They are short staffed and need more assistance to help with the patients. I wouldn’t recommend this place as a 1st option. We left early!

As I was walking towards my brother's room, every employee that I passed said hello and when it was obvious that my brother had changed room, I was immediately asked " can I help you" ? I left believing that my brother is getting best care.

My sister is at the Decatur center every time I go I feel welcomed, I feel my sister is safe. They’re always people around at the front door, at the nurses station , and they’re constantly checking in on her. I have been there doing meals, lunch, and dinner and find the meals to be visually appealing and my sister says that they’re good. The caregivers seem more like family than staff Nd

I recently spent 6 weeks here and need to compliment the staff for making the effort to make me feel as comfortable as possible. I would specifically like to thank the CNAs: Demetria, Mia, Balanchine, Charlene, Jane, and Dawn. These ladies were tremendously helpful and kind. The nurses: Jeremiah, Vanessa, and Charlene who stepped out of their way to help when no one was around but them. The Rehab Team was outstanding. With their help I was able to recover stronger than I was before I came thanks to: Alex, Asha, Smitha, Lacie, and Michelle. With special thanks to Terry for working with me almost daily to navigate the stairs in my home. I am eternally grateful for the care offered at this rehab facility.

Ms. Solo is the best Unit Manager. She makes me feel very comfortable with the care of my parent. She's always available to address my concerns and goes out of her way to make sure each patients' needs are met. She also very knowledgeable and great at explaining what's going on at all times. Ms Karen is a great tech. She is extremely patient with the patients. She has a very calm and nurturing voice and devotes special time and attention to taking care of everyone.

My Uncle has been here for 4/5 months now. He has wounds on both feet. Panchertta, his wound care nurse is very helpful and keeps me updated with his progress. She is very approachable and willing to address all of my concerns. The staff here is great here. From Angie in the business office to Danah in admissions, they are all wonderful and helpful.
