I moved into independent living at the Forum five months ago and felt at home immediately. The staff is just the friendliest and most helpful, as is everyone who works here. The food is delicious, and what a diverse menu! The residents, many of whom I now call "friend" love living here, and I can see why. An added bonus is location because of the proximity to so many great restaurants and shopping. My children are delighted that I'm here, and so am I!

Witnessed a maintenance woman with a ponytail propping the doors open at the memory care facility. Gigantic no no. The lady at the memory care facility desk was on top of it! Made sure that door got closed and let the maintenance lady know that's not protocol. Good on her. Then the maintenance lady was on her golf cart driving and texting, completely unaware of the cars behind her attempting to leave the property.

My father in law has been here for about 1 years now in the memory care area. they take great care of him each day and provide him with activities that help him be engaged as he can. I have seen baby kittens and puppies even show up to help bring interest to the daily activities.

I am a relatively new resident, having moved in December, 2021. I would recommend The Forum to my friends and family. The residents and staff are friendly, and the cuisine is outstanding. The Maintenance and Housekeeping staffs go above and beyond to insure that The Forum is well-maintained and is very clean. The Forum Management is very friendly and is very responsive to our needs. I am happy living here.

Personnel in memory care excellent at enriching my mothers life with activity that is structured to her needs. The rooms and especially the bathrooms are always clean. I am contacted immediately when my mother has issues, and "hand- off" information to next shift always appears to be accomplished well. I appreciate the personnel and work done at memory care in the Forum.

When my parents reached the point where they could no longer stay in their own home, they chose The Forum at Lincoln Heights for their retirement years. They moved into a large, lovely apartment in the Independent Living building. That was in 2009. When Dad had to move to Skilled Nursing, it was great for Mother to be able to stay in their apartment, but within walking distance to visit Dad everyday without leaving the safety of the building. Later, when Mother could no longer stay by herself, she moved to another lovely apartment in the Assisted Living area. She was so well cared for and our minds were at ease for her. Now she is 100 years old and lives in the Bridge to Rediscovery area. Again, she has a lovely room and lots of loving care and attention. We are so pleased with her accommodations and the wonderful people who are like family to us. The Forum goes out of their way to make everything as secure and pleasant as can be. Having Mother stay in one center where she knows the staff and feels at home has been a great gift to her and to her family.

My husband is in memory care and has been for almost a year. He has done very well. The staff is very attentive. The activities are really wonderful and keep him engaged and busy. They play Bingo and Black jack for really cute prizes. Field trips are often and to interesting places. There are caring and plenty of staff to care for their needs. The nursing care is extremely wonderful. This is the best place for my husband. I am very glad that we have him here. The Forum is a great facility.

My mother, Frances Swinny, had the privilege of living at The Forum at Lincoln Heights for two years. She has always been an extremely independent woman, therefore she had hesitation about becoming a resident. She was 95 when she entered The Forum in the independent living section. It literally took her about a week to adjust. For the next two years she was healthy, happy, and involved. From the day she arrived, The Forum residents and staff welcomed her with open arms. Her independent apartment was beautifully renovated to meet her needs. It was sparkling clean and ready for her to personalize. While living at The Forum, my mother was involved daily with activities she found interesting and entertaining. My husband and I commented often that it was like living at a beautiful resort where the activities were in abundance. My mother enjoyed sing along, choirs, pianists, drumming, yoga, tai chi, happy hour, (always with entertainment), ladies wine parties, the choices were endless. In addition to all the activities, The Forum has a wonderful chef who prepares delicious meals with a generous selection of choices. The facility is always clean and odorless. Although mother was in independent living she was lovingly watched over not only by staff but by the other residents. There is a real sense of community. From my observation all the residents seem to thrive at The Forum. It is not depressing like many assisted living facilities, quite the opposite. I visited my mother almost daily for the two years she was in residence. I visited at all times during the day and night. I got to know or recognize all the staff, administration, security, and residents. It was hard to move out of The Forum, I will miss seeing people I came to know, enjoy, and respect. My entire family recommends the Forum. For the two years mother was a resident we were truly without concern. I would highly recommend the facility.

I've had relatives staying at the Forum for almost two years. Communication has been quite good, with information provided to residents regularly and questions and service requests mostly fielded in a timely and satisfactory manner. The apartment is well lit and cleaned regularly. Staff is pleasant and helpful for issues like managing incoming packages and assisting residents who fall. The food is good quality, varied, and prepared to be appealing. The response to keeping the residents healthy without being draconian during the virus concern has been mostly reasonable and well-planned. Visits with family can be arranged outside, which is great, but the designated location needs a mobile ramp to make it safe for residents with mobility issues. Other on-campus social activities such as haircuts and exercise groups are currently tightly controlled in accordance with state regulations, but the requirements and plans moving forward are communicated regularly and clearly. Overall, the Forum is doing a good job.

Do not go here for rehab if the patient needs pain medication in a timely manner. My loved one has received pain medication more than an hour late on 2 occasions. One of those was after being put on a schedule (every x hours instead of PRN). Also, the nursing staff is impossible to contact by phone and there is no way for patients or their families to contact the doctor (who is only there 3 days a week).

Donna Gaultier and crew were awesome recarpeting my mom’s room! They put every piece of furniture back in place. It only took a few hours and all was done. The aides take the time to tomeet my mom’s needs with a smile. Thank-you!

My Father has been here at the Forum in the skilled nursing rehab for almost a month and we are beyond pleased with the exceptional care he receives daily! Everyone is truly kid, caring, and goes above and beyond. Each nurse, CNA, Doctor, and all who walk the halls have made a huge difference in his recovery. Let me tell you with this being his 4th rehab we are very experienced in rehab life and they are hitting it out of the ball park here! Thank you to the whole team here at The Forum for making a difference in his life and recovery.

As a musician who plays at over a dozen homes, communities, and facilities throughout central Texas, I see a wide range of approaches with regard to activities and their effect on residents. Here at The Forum at Lincoln Heights, residents truly look forward to gathering in a room dedicated to various forms of entertainment and community, and I have the privilege of joining in on the fun, singing songs that residents will sing with me. Some like to listen attentively, while others prefer to catch up in an adjacent room, but all wear smiles during the 4 o’clock happy hour!

The Forum at Lincoln Heights is definitely a GEM! The saying "Home is where the heart is" epitomized this place, particularly, their Independent Living and/or Retirement Home. Being a care provider with a long history of caring for the elderly through an agency, my job took me around serving clients from almost 22 other independent living facilities in the SA area. What made The Forum stood out is the staff, the cozy, fresh setting that gives you the warmth and comfort of your own home...and most 'specially their activity programs that keeps the residents active - physically and mentally. I must say they have the best of the very best! Tai chi, Chair Yoga Aroma Therapy, Stretch and Flex exercises in the morning, Saturday Movie at the lounge in the afternoon, Happy Hour on Wednesdays and Fridays (live performers performing in the lounge), Soup & Salad Meet and Greet on Sundays - are just a few of the many activities all geared to keeping residents socially and physically healthy. My client who is currently living in The Forum had a stroke last year and has made tremendous progress, which could be attributed to The Forum's support focus in the overall well-being of their residents. So 5 thumbs up to THE FORUM!

The Forum at Lincoln Heights has proven to be an ideal place for my 94-year-old mother. There are many wonderful activities, parties and amenities. The grounds are always beautifully kept and the food is always delicious, with many choices available. I am especially appreciative of the kind and committed staff, most of whom are long-term employees. I feel it speaks well of a company when there is so little attrition among the employees. The staff at the Forum know all the residents by name and are all interested in their well being and bend over backwards to be helpful and accommodating. I couldn’t be happier with our choice to have her at the Forum. They excell in every way.

I provide programs for the Forum at Lincoln Heights every month and have for many years. The staff is always interested in and cares for the residents. Residents are a lively mix of interests and talents and a delight to be around. Activity directors also provide a mix of entertainment and activities. My mother is leaving skilled nursing at the Forum tomorrow. It was the best experience she and I have had with this type of facility. The nurses are competent and attentive; they get things done. The CNAs are superb. The PT/OT staff is also excellent and hardworking. The facility is very nice. (And I am a bit of a curmudgeon about this sort of thing.) Big rave!

After touring and visiting other nursing/rehab facilities we are so pleased we found The Forum at Lincoln Heights. They are top notch. The care my grandfather is receiving has been outstanding. I had to do my research before selecting a facility for him much like everyone else. I compared reviews, ratings, positive comments, concerning comments and negative comments of around 30 facilities. After our tour here we knew this was exactly where he needed to be. He recovered from his first surgery here two months ago. The staff from the aides, nurses, therapists, housekeepers to the administrators and directors were phenomenal. They truly cared about him and did all they could to make his stay a comfortable one. His only complaint the first go round was "they wake me up too early for breakfast, they're always after you to give you a shower and they push you hard in physical therapy"-my response was good they're doing their job. He recently underwent another surgery and decided to try a facility closer to home. BIG MISTAKE..he checked in Friday night and Saturday morning my phone was ringing for me to get him out of there. The phone calls and even during visits it was non stop to get him out of there. I asked where did he want to transfer to he said take me to the place I was before. After his transfer his demeanor and spirit did a 180. He is joking, laughing catching up with the nurses, staff and friends he made there. I am beyond ecstatic he is back at TFaLH and starting to make progress again. I can't recommend this place enough!!!

I spent hours researching and reading reviews to find a reliable place to care for my aunt. We had a terrifying experience in 2016 at a facility that DID NOT care nor attend to my Aunt's medical needs. It was so frightening at that facility, I HAD to call 911 to have her rushed to The ER at St Luke's. The next day, we were informed that 14.5 oz of fluid had to be removed from her lungs. My 95 yr old Aunt had the same issues as Millie's. I coordinated with the hospital's case worker, who worked on my aunt's discharged to the skilled nursing facility at the Forum. The Forum provided transportation from the hospital to the skilled nursing facility. Dr to the RN'S, CNA's to the medication nurse's, the nutritionist and physical therapist's and the case worker, kept us closely informed and answered all of our questions. This place is absolutely AMAZING and I would recommend this place to anyone who wants the TLC for any of their loved ones! We love the Forum at Lincoln Heights!!! THANK YOU SO MUCH!!!! My dearly loved grandparents lived at the forum many years ago. It was fantastic. Close to family and non intrusive for the residents who are encouraged to be independent for as long as possible. I have visited several times in recent years to see friend of my mom and family and I'm always impressed by the genuine care and kindness of all staff. The residents are obviously very happy. The food is fabulous and this is a top notch home. I day home, rather than facility because that is how it feels. I just trust that the best care is here. And it's real. The staff are chosen carefully and it shows. Those lucky residents get respect. Knowledge. And people who have chosen to work in this field ( geriatric care). It makes a big difference and I highly recommend those looking for options for themselves or family to visit The Forum. There are many choices. There are many newer facilities. Here in San Antonio, new does not equal better. I have found an astonishingly pitiful lack of eye contact or personal attention when visiting other "high end" facilities... As if it's all business. Here's a hint to those places... Lives matter. Memories and people dreams matter. Listening matters. The Forum gets that and it makes all the difference in the quality of life you or your loved ones will experience. Choose love!

Six years ago my Mom and I spent several days going to different retirement communities in and around San Antonio trying to find just the right place for her. After three days of visiting different locations, she and I both knew without a shadow of a doubt that The Forum was the right place for her. And for over six years now they have they have proved us right! It is evident in all they do that the staff at The Forum cares deeply for the residents that live there. We have seen them go above and beyond time and time again to provide incredible care and support to both the residents and their families. Knowing that my Mom is safe, loved and receiving excellent care each and every day is a gift that our family will never be able to adequately repay. The staff’s actions the evening of February 20, 2017 however deserve special recognition and are the most recent example of their dedication to serving residents. A tornado struck that area of San Antonio that Sunday evening at about 10:30 pm. The night staff responded quickly and efficiently, as the entire staff was recalled, to calmly and effectively evacuate resident, many to safe areas within the facility. This task was especially difficult given the large number of older and infirmed residents and the time of night, especially on a weekend. The facility received moderate damage, the weather threat passed, and no residents were harmed. Clean-up and repairs began the next morning. I visited The Forum on March 6, 2017. Repairs continue but operations have returned to normal. It surprised me to learn that many residents, my mother included, still did not realize the extent of the danger or damage. I conveyed my thanks to Kristi Allen, Director of Community Relations, but words can not adequately express the gratitude my family and I have for the staff. This was, quite simply, another example of the excellence we’ve come to expect from the staff at The Forum at Lincoln Heights.

I just had my 73 yr old mother taken to the Forum for rehabilitation after an extended hospital stay and although we havent been here long, the welcome was outstanding and she is really happy with all areas if her care. Even though she is now on a heart healthy and diabetic diet, the food is amazing and the menu has a variety of choices. Her medical needs are being met very well and they jumped right in to begin rehabilitation immediately. They encourage her and make her feel like she is more of a friend than a patient. I had her at another facility that was so neglectful that they allowed a violent patient to enter my mom's room who verbally and almost physically assaulted her. Nobody would check on her even when she pushed her call button and because she couldnt go to the dining room to eat they fed her 1 1/2 hrs after everyone else was done and her food was cold and disgusting and they paid no mind to her diet. I am so grateful to the staff at the Forum for their care and dedication and I feel blessed and lucky to have found such outstanding care for my precious mother

I saw the 1 star and had to leave something in response. I've been visiting my loved one at the Forum frequently and the staff are top notch. We've never complained - even during the construction that's been going on for a while now... Kristi Broadbear is an EX employee & left a bad review on the way out. I didn't know her well - she wasn't the happiest person while here. This is a great community with an excellent reputation. Tenured staff who CARE.

This place is a mix of independent living, assisted living, Skilled Nursing Facility and hospice all in one, pretty well managed mostly clean has little events for the residents also throughout the week.
October 2015
Last February 2015, I placed a 93 year old friend at the Forum. His mind was perfectly clear but he was physically failing rapidly. The care he received at the Forum was exceptional. From the nursing assistants to the dietary staff, nurses, physical therapists and administration, the warmth and compassion they demonstrated was exemplary. He was in skilled nursing and then hospice care. I was there daily, at all hours, and never found anything out of place or the care lacking. Additionally, the food was very good - and I am very finicky about food. The place is immaculate and very pleasant. I highly recommend the Forum for anyone who has a loved one who needs their services.
