2nd attempt. The people here are very hard workers. Today people expect you press a button and a person appears. There is hot coffee steaming in a cup. Wrong!!! The Jetson's never happened. Our day and younger who accually do know less than most older people (even with slight OTDD [Old Timmer Dimming Disease] Our food is good, getting better every day. I personnaly eat to live, but I won't bore you with the list of things why I am here...unimportant. Anyway I am bragging on a excellent group of people here. The Activities & Fun for All. They never stop, amazing group. We use left and right sides of our brain for balance. Coloring, word games. Some draw, listen to music, converse with each other, play BINGO, RING TOSS. Kudos and Thank You to the Rehab Therapists (Physical and Occupational Departments). I would like to thank this Department for exceptional services. Under the guidance and direction given by Chetna, this team represents one that is united, purpose driven and results oriented. I worked with Shri, Aileen, Mike and Djahida. Each person brings their own style. ,They each show patience and empathy. With them, I felt comfortable They paid specific detail regarding progress safety. !!!What a great team

The staff has been so helpful with my recovery. When I first came here I needed help just sitting up in bed or even transferring to my wheelchair from my bed. Now I'm able to do those things with no assistance and now Im also able to stand albeit just for a few seconds, but can stand with no assistance. My thanks to Mike, RJ(?) and Erica. Please keep up the good work.

Thank you to the physical therapists that worked with my mom(Mary) when she was at the facility in May 2024. Aileen and Shri were the best. She hasn't stopped talking about you since she came home on 5/29/24. You were able to help her get some her mobility back. The only issue she had was with her food. She was on a heart healthy diet and they didn't get her the proper food until she was about to leave. The nurses and attendants were always there when she needed something. She truly appreciates the time you all spent with her and the conversations you had with her. to you all. The White family.

I would like to appreciate all staffs working here Director, Case manager, HR , ss worker.....and special all nurses here , they r super nice and take care My Mom during time she here and even another patients too . I would like to thank Fair Oaks Health & Rehabilitation for their care of my mom. I would like to specifically thank Rhonda Lewis. My mom was previously in another rehab center and the level of care and attention she got at Fair Oaks was just incredible. Specifically, Fair Oaks Health & Rehabilitation did the following very well: Fair Oaks is well kept. The facility is clean and the staff is attentive and friendly When my mom pushed the call button, the aids and nurse came right away The nurse bathed and attended to my mom. One of the aids, braided my mom's hair is such a beautiful and stylish manner My mom is a strict vegetarian. The chef came to my mom and asked if the entrees suited her needs The Kaiser doctors would also seek me out and speak to me regarding my mom's case. They were very attentive and went through potential diagnosis and recommendations with me What impressed me the most at Fair Oaks Health & Rehabilitation was Rhonda Lewis. Rhonda Lewis called me and my brother every day updating us. She would visit with my parents every day, sometimes multiple times daily, confirming that my mom was comfortable. My mom was at Fair Oaks for about a month. Towards the last two weeks of her care, Rhonda setup daily meetings with nurses, aids, doctors, my dad and mom deciding on mom's next steps. Rhonda also had my dad meet with Sunrise and Care with Love. Rhonda also meet with these two outside parties with my parents. My parents decided on private nursing care and after my parents left Fair Oaks Rhonda continued to follow up daily with my parents, my brother and I. I am very impressed Rhonda Lewis. Her attention to detail and care is just incredible. She has done more for my parents than any other person so far. Thank you Rhonda Lewis and Fair Oaks Health & Rehabilitation

Facility needs to be renovated-dated, dirty. Staff very professional, compassionate, respectful, caring with detailed attention to needs. We left to be closer to our home.

Excellent service and staff is good. Food is not my type but I make it work. Rhonda social worker is very helpful, nice, and helps everyone. I get a lot of respect here. I enjoy seeing the Administrator walk around and greet everyone.

My mother had a stroke in early Feb. 2024 and spent almost 4 weeks at FOHR. Dre was so attentive to my mother and all other patients at the facility. My mom would push her button for a change and Dre would coming in with a smile and something friendly to say within minutes. Dre always explained what he was doing so my mom knew what to expect. FOHR is lucky to have Dre on their staff. Heartfelt thank you. AR

Fairoaks rehab is one of the best rehabs in the area.. Rhonda and Mayona are both amazing at making Dr.appointments and having transport come on time every time. They are the sweetest and nicest there.. highly reccomended

Hannah was the best speech pathologist my grandma could have had with her time here! She was so patient with her and I could tell how excited Hannah was every time my grandma completed a task correctly! Really appreciative of everything she did for her.
