This is an review on Nurse Jasmine Cooper. I have been at Legacy of Mentor Nursing Home and Rehabilitation Center for the last month and in that time... I have had excellent service from nurse's aide namedVictoria and Bertha and a nurse named Jasmine Cooper. Not only do they botheydo their job, but more because we look for them to come to work and they do more than the average workers, because they try to assist with the things that we complain about; They go over and beyondtheirjobs by listening to the residents when they talk and And by replying with insightful and empathetic responseor some type of assistance(within their campiblities). It makes a difference when the nurses or assists treats the residents with dignity and takes the time to be empathetic and listen to the residents and assist in any manner that they can. These three named employees have done this while working with me and around my presence.

I have had over 30 years experience caring for elderly relatives in rehab and nursing homes. Now, it has been my turn. Legacy Mentor came as a pleasant surprise. Aides, nurses, and particularly therapists all did their jobs with a high degree of professionalism. My needs were promptly met on a daily basis, with real concern for my healing journey. I will recommend the facility with confidence. Thank you, everyone at Legacy!

The food was horrible they need to work on that. If you didn't like what was being served you had a choice of grilled cheese or Peanut butter and jelly spread so thin you could hardly taste it. I was there for 4 months The staff was great.

Took great care of my aunt. She can be very ornery and hard to deal with at times but they went above and beyond to make her happy. Very impressed!

I have nothing but good things to say about our facility. It's full of amazing staff with a great management team! The longevity of our staff is amazing. Staff are fun, friendly, and most importantly caring! Our Activities department is the best I've seen with various weekly activities including trips out and about on the facility bus so residents can continue to engage with the community. I highly recommend Legacy Mentor!

My parents were transferred here after being weak in the hospital for different reasons but they both had to get rehabilitation. There were a few bumps along the way, but I had some very good people step in swiftly and help fix issues I had. No one wants to have their parents in a place especially when it's further away from them, but with the people stepping in and helping, I felt more like they cared for them so I am grateful for that.

My mom has been at legacy for 2 yrs. I love it here and she is safe and I never have to worry about her. She is always in good hands. If something is going on with her as to her not eating or not sleeping we'll legacy always contacts me. The aids and directors are the best. This is my 2nd home when visiting. Wonderful place for mom.
