Clean, friendly nurses and staff. My dad is taken good care of. I like that the facility offers Bible studies. I like that there is plenty of land to walk around. The only down side is the food is not so great.

Centerville is an amazing place! We put our family member in the facility she was working only there for a short time! When I say the staff was amazing to our loved one! We had no issues at all! At one point we had to send her out between the paramedics and the staff our loved one was held facility within three minutes, heading towards the hospital! When I say they’re amazing I do mean they treated our loved one with the respect she deserved and we thank u all for this!

I see many good changes with the new management staff. The social worker goes over and beyond, the administrator is accountable, the new house keeping manager ensures Covid cleaning as well as regular cleaning is done properly and the business office manager is fabulous working with the families. I encourage all staff and family members to address concerns at the time they occur and allow staff time to correct any issues.

I have a son that is a resident of CHR. The staff has been very nice and helpful to my son and I. They are very fast at responding to his needs, for example getting him a new bed mattress as soon as possible because he is bed bound, making sure his medical procedures are accomplished on a daily basis. Overall rating for this facility is four stars.

High turn-over...under staffed, hard to make contact by phone...food/menu leaves a lot of room for improvement...wierd pricing scheme...SOME of the best, hardest-working folks I've ever met and some of the worst! Way better than other places I've seen but the business plan is not to make people better but milk the insurance, then the individual and then the government of resources...definitely NOT value-priced.

My fianc© had to go to rehab and I’m happy I picked Pristine over Heartland. They are very courteous and kind. We loved the nurses. Ok my son really like the nurses. They are a bit under staffed and the food was not that great. But overall we would recommend Pristine. If he had to go into rehab again I would choose them!

The negatives about this facility are as follows: The food taste bad on more occasions than not. They are always short staffed. At times prescribed medication was not given to my mother as needed. Several times my mother waited for over 1-2 hrs to either get on or off the bed pan. I started doing her laundry myself because her new t-shirts were lost. On several occasions I've walked into the nursing home and could smell a foul odor. The social worker is slow about getting things done. And the director of nursing literally ignored a request to speak with my mom about complaints. Residents have to buy there own briefs and wipes. The positive things are as follows: They have since hired a new director of nursing. There are a few good aides there that are very kind and pleasant. The Rehabilitation side is much nicer living than the Acute-long term nursing side. Unfortunately my mom is in the Acute long term side.

Great facility for rehabilitation! Therapists were professional and very devoted into getting my father back to the shape he was in before his accident. He is now fully recovered & we thank your facility and staff for all your help.
