My Dad was a resident at TLC back before covid he was there for about a year. So this isn’t someone complaining after four days. TLC is a very nice place to put a loved one. My Mom and I were at TLC everyday to see my Dad. The nurses were very kind and always gave complicated medication correctly. During covid they made sure that everyone who wanted the shots had them. It was very difficult at the time because they wouldn’t let us visit but, let us see my Dad through a window and let him have a cell phone so we could talk to him.

My wife was admitted to TLC five weeks after suffering a stroke. She was at TLC for a month. The staff at TLC were outstanding. In fact, the staff gets 5* out of 5 from us. We couldn't be more grateful for the nurses, CNA's, and therapists. If not for them, I'm not sure where we'd be regarding my wife's recovery.

TLC Care Center is a great place. The rehab department is excellent as well as the long-term care. The patients are happy and well cared for. Many patients recover and return home to the prior level of function. They have a wonderful respiratory unit which handles all levels of respiratory needs. The staff is friendly and very helpful, and if they can't assist in your needs, the staff will direct you to who can. I highly recommend this facility.

"TLC CARE Center is a excellent location and high CMS star ratings. While no place is perfect, the management is very transparent and communicative whenever we have questions about care plans. We did notice minor issue, staffing can be lean on weekends, but the caregivers who are there work incredibly hard and are very affectionate with the residents. Overall, it's a warm, home-like environment."

My husband was admitted to this facility in December for rehab after a massive stroke, when he was brought in, the receptionist greeted us with a smile and a hello, Mae the nurse came in and spent time doing her assessments and answering questions. The following day, he started therapy and every therapist encouraged and explained the processes and the expected results. He is home now and still continuing to progress and I know that this is possible because of the encouragement and hard work done the Alan's team. I am grateful for this team of caring therapists.

I can’t say enough good things about the respiratory team here. They were incredibly kind, patient, and encouraging every step of the way. very good staff some of the nurses are top notch and there are a few that efforts need to be more thorough. the rehab staff on the gym is really good very nice and know there jobs as they make you feel extremely comfortable. it is a center I would def tell people to go there for long term or short term care. only draw back is the few whole sometimes tasty is often cold and not big enough portions

quality care facility and I would recommend staying there. There are a number of very friendly and knowledgeable people who work here mainly Ashley, Lynn, and Theresa in 100 hall. there are also great CNAs like cookie, carmella, and Veronica. they all give quality care and seem to love their jobs. also the rehab department is top notch. also shoutout to deena from housekeeping who is very thorough and always has a smile on her face. However, there are also a few negative things. For example, the kitchen department is surely lacking quality and quantity. food is often cold and there is a lack of imagination. also certain parts of the building could use renovation. overall, there are many good things about the facility I have enjoyed my stay. there are people I will remember for a long time and I hope they hire more people like the ones mentioned above. with that being said, I would recommend this facility and would not mind coming back.

I was fortunate to have the opportunity to place my brother who is a total dependent into such a caring and committed facility as TLC! TLC had cared for my brother since 2017 after I had him transferred from another local and disappointing facility. The level of care he has received is better than that of any nursing facility I’ve encountered, and I have been in many. Amazingly, the CNA,- Ebell, who cared for my brother in 2017 is still at TLC nine (9) years later and still so wonderful to my brother. Raquel and Mae, two of the top administrators have been so kind and willing to help. Alan, Jason, and Mike have exercised the utmost in due diligence to try and rehabilitate my brother with their PT expertise: From Dee Dee at the front desk to Kashimir and Roselle, Ayo, Jeannie, Sarah, and Jacque, to name a few, everyone at TLC had been so kind and caring to my brother and me. The new Director, Tracy Brantley, is a thoughtful leader. She has so much responsibility bit found the time to listen to me and to go the extra mile to help my brother. Emily; one of the head social workers cannot do enough for her patients and goes above and beyond to reassure family members. Emily spends a lot of time making the patients comfortable. I wish that there were more skilled nursing facilities like TLC in Nevada and throughout the US. In my experience this one is the best I’ve encountered! It is very reassuring to me to know that my brother, a self-care deficit, is being so well cared for. God bless the staff of TLC Care Center.

Thank you to the CNAs and Nurses on all the halls. You are appreciated for all you do considering the circumstances. You deal with the high maintenance and super over the top needs of more than a few patients. 700 and 500 halls have been my experience with fantastic ladies doing their best. Tia, Barbara, cashmere, Blythe, Lashley, Valerie, Wendy, Kim, Michelle and the list goes on. Housekeeping with Jennifer and Tia doing top awesome work while handling laundry, cleaning and even light maintenance. Can't remember everyone's name, but you know who you are. Professional, interactive and great personalities who bring a little sunshine to what can at times be a depressing existence. From the bottom of my heart, I really do appreciate you. Almost forgot.. the food is terrible.

I was at TLC from 3/25/25 to 9/27/25. On multiple occasions, TLC ran out of one or more of my much needed pain medications; and, even though my pain meds were scheduled, by my doctor, to be administered at specific times so we could stay ahead of the pain, there are nurses who are employed by TLC, not agency nurses, who NEVER brought the meds on time. On multiple occasions, they wete 3+ hours late. Several times, I receivd incorrect meds or incorrect quantities. One nurse gave me double the amount of medication because the computer said to do it. The computer was wrong but she didn't question it, I had to. I would like to give a great big shout out to nurses Tina, William and Kendra in 800 hall who took excellent care of me the entire time I was there and I will always be grateful. Some of the CNA's are there just for a paycheck and it shows in the manner in which they care (or don't care) for the patients. Patients should not be treated as a burden or inconvenience. I encountered several just-for-paycheck CNA's while I was there but on the positive side, there are a few who truly do care about and want the absolute best for their patients. Kristin Pallotti and Anne Jones, I thank you from the bottom of my heart for never treating me like I was a burden. I appreciate you more than words can say for taking such excellent care of me. Also, thank you to Purity, Bernadette and Alfonso for your kindness and for taking such good care of me. One department at TLC was flawless and that's the Therapy Dept. During my stay, several of the therapists I'd worked with left to go to a different facility. I had developed good working relationships with some of them and was concerned that the new staff wouldn't be as good. I was wrong. The new staff didn't miss a beat and I am so thankful to Alan, their fearless leader, for having such an awesome crew. The entire department works together harmoniously and they encourage all patients, even the ones they're not working with. HUGE thank you's go out to Roy, John, Rex, Enrico, Jaylene, Mike, Audrey and Eden. I can't thank you enough for your kindness, patience and encouragement. I appreciate you all. Finally to the Administration. You could have a decent facility if you cared about your staff and patients more instead of focusing on filling beds to make more money. In addition to my comments above, I experienced horrible food, terrible TV reception, air conditioning in the physical/occupational therapy room going down twice in the middle of the summer, fire and other alarms constantly going off and some patients in my hall that made me question if I was in the psych ward or not. Some who would wander into my room at all hours. Three times, I woke up to one guy standing in my room. He also walked the hall lifting his gown and playing with his privates among other distasteful things. I complained to nurses and CNA's. One CNA said she went to Admin and was told he (the patient) has rights. I, as a patient had a right to not wake up with the 800 Hall Master Bater in my room, so I complained to Admin and finally he was moved. One person in Admin had the duty of going from room to room to ask if there were any complaints or if everything was OK. I answered but could tell by her comments/questions she wasn't listening. One day, my roommate told her about a negative experience she had with a CNA and Admin responded, "so, everything's ok?" In my mind I was thinking she's not listening to her at all and when Admin left the room, my roommate said out loud, "she didn't hear a word I said." Admin, do better, be better and care more. You have some great people working there but you have some not so good people who are still getting paid. - talking on their cellphones when they're supposed to be passing out meds or attending to the patients (I have video). Cut them loose and take the time to find better nurses and CNA's. Treat the great ones you have better, give them a reason to stay and even recommend the place to other quality nurses and CNA's they might know.

I wanted to start by saying , don’t go based on the reviews only, there are amazing people in this facility who actually care and work by heart. When my mom was first sent here I was petrified because based on the things I read I mean how can you not be concerned but I have to say , we’ve been here for 4 months already and we can’t be more thankful with the staff nurses and the facility itself. There haven’t been one thing we have pointed out and request that haven’t been addressed and taken care of. It is a peace of mind knowing you’re loved ones are well taken care of and indeed in good hands, and that’s exactly the feeling we have thanks to all the team and wonderful staff that have worked and continue working w my mom. Everyone was pretty much nice except one person. I talked to case manager prior to talk about discharge and was told check with nurse.. I did exactly what I was told . I asked one of the nurses and she said talk to her. I think her name was Cynthia she was Asian but didn't have much of Filipina a but we have accent. I told her case manager told me to check. She said Case manager was not in the office right now so I asked her if I can wait She started to bark at me, this is first time I met her . She talk down to me like I was stupid. All I said was if I can wait for case manager. By the way case manager is great no problem at all .n That nurse keep telling me I already told you but this is first time I met her. I told her case manager already told me to check with you She didn't even ask me what my mother's name is or room number. She must have woken up on wrong side of by bed. Also food portions need to be larger. My mother doesn't eat alot but even she said it's not enough.
