My mum spent 3 months here. The staff were warm and that made my mum's stay alot easier and a pleasant experience for us. Every aspect of her care Nursing, PT, OT and speech was so well coordinated. The recreational team were also a joy to engage with and my mum found that quite pleasant. The post nursing care coordination by Parris and her team was also very seamless for us. Clean nursing home, friendly and pleasant front desk person. One of the nursing homes in MoCo, which has rehab for acute care as well as post acute care. Very few places have that option. One of the best rehab teams to work with. Definitely a 5 star for Rehab. Administrator Ben and DON Brenda are always on the floor talking to the residents and staff and are super helpful. Some of the best nurses at this place - John, Daisy, Natalie, Samuel and Aida..

Greetings one and all. I Wanted to share my experience here at Shady. I have been working here for a little over 3 years now. In the beginning it was some what touch and go, as the Building and the management staff at the time was not NSYNC I Could say that they didn't have the same goals or intentions for the facility. Fast forward to a couple years ago we had some changes in management including a new Admin. That came in and had a vision, not many saw the same vision so they resigned. The Admin has created a team of managers that all share the same goals and vision and this team has became a family. I can happily and honestly say that I went from wanting to exit from the facility to wanting to come in to work everyday. Here at Shady We as Managers put our residents, family and staff needs and necessities first! And strive for nothing short of the best. Shady is now a comfortable home for our staff residents and family, and we pledge to continue to provide the best as well as improving where's necessary. Walking into the office first time to meet Paola, Administrator and then having opportunity to meet Parris and Annie, respectively Office Manager and Activity Director, we knew we were going to experience only the best. The culture they exemplify in terms of energy and persona, they were very welcoming and you can see they run an amazing Skilled Nursing Facility, with happy residents. My 83 year old mother fell and broke her back. She was at this facility for 3 and a half months. She required intense physical & occupational therapy and pain management. I visited her daily, so I was very familiar with everything going on. My mother and I both thought this was an excellent place. The doctors, nursing staff, aides, physical & occupational therapists, the recreational director, social workers, and many volunteers were all fantastic!! Each of them coordinated together seamlessly to make my mother's healing progress go smoothly (both physically & emotionally.) They were always professional, but they were also quite personable, friendly, attentive, compassionate, and super encouraging. I also was pleased that it was quick and simple to get in touch with any of the employees if I needed to speak to someone. Everyone was accessible, knowledgeable, and very helpful. My mother is back at home now. The staff coordinated her transition to home beautifully. They ordered all the equipment she would need at home and set up home health aides and home PT/OT. They made everything pretty much stress free from beginning to end. We very much appreciate all of the hard work and dedication the entire staff provided to get her back to living her life again. She wrote a card to the staff and I have included a photo of it.

I was recently a short term resident at this facility. While you do have some employees that are friendly and care, some just don't give a sh!!. The nursing and gna staff are overworked. I was discharged nearly 2 months ago, I have yet to receive a piece of medical equipment that is really needed due the the fact I can not put pressure on one of my legs in which the PT supervisor mysteriously had notes that I could. This in turn caused more damage than was already done. It took 15 or more calls to get in touch with the social worker, who seems to refuse return calls. I should have received in home physical therapy, which i have not yet received nor gotten any feed back about. (2 months have passed). Would not recommend. HORRIBLE

Like the place and the people are good, but seen twice that the food stated on the ticket was not what was received. A good example was yesterday it said turkey with mashed potatoes smothered in gravy, tomatoes and a roll with butter. She received Mac and cheese, tomatoes and a roll no butter. No protein! Also her shower head will not Hang up, just hangs by cord because it's broken.

Bernard the physical therapist is great always has a smile on his face and shows how much he cares about his patient's. I visit my aunt here often. It is a very welcoming place, with welcoming people. I'm greeted as soon as I enter the facility. The customer service rep is extremely accommodating, she never seizes to pop her head into my aunt's room during my visit to make sure we're alright. I feel better leaving everyday knowing that there will be people there regulary checking in on her.

Not what we expected Our experience did not go well at Shady Grove Rehab Center. I visited dad 5/6 days a week. I witnessed a lot of wrong and a few good things with this place. There is a new DON (Evelyn) and I'm not too sure if she has the capability to turn the place around. I don't want to put the blame on all, just to name a few of the bad nurses; Mr. Freddie and Tony were rude and lazy. The younger female GNA's would always make an excuse when asked a question or unresponsive (bathing/feeding) and they would have their name tags reversed. Medication was not administered on time and not correctly. I had shared my concerns with the DON and I never received a call from the doctor. We had our meeting to finalize Dad's discharge plan with the social worker Brandi. She interrupted and was very difficult to talk with. Apparently she didn't like us asking questions and abruptly ended the meeting. If you plan to send your loved ones; please visit daily and if your social worker is Brandi ask to be reassigned. And get this, my dad was assaulted by his roommate. They seemed more concerned on the aggressor saying how nice he was. The front desk staffs, therapist Brandon and his team are phenomenal. The social worker Debbie S seems more responsive and reasonable. Debbie was transparent since Brandi is a pitiful unethical rude individual. It was delightful when dad was assigned with nurse Natalia, Oliver or Ms. Freddy (PRN). The GNA's Jean, Ricky and Yvette are very passionate individuals. Update: 1/10/21. I was supposed to receive a call for home therapy services. I called the provider myself today. They were not aware so I sent an email of Dad's discharge summary. Apparently Brandi did not do her job. Brandi discharged dad without a proper aftercare plan. They will not provide care for dad since he resides in DC. Which Brandi was aware of. I called Brandi, DON Evelyn and the liaison all to no avail.

I was a patient for a couple of months while i received intravenous antibiotic treatment. I was pleasantly met with fantastic care. Upon arrival from the hospital they were right on top of making sure my required meds came in a timely manner as to not interrupt my IV regiment as well as my pain stayed in a manageable state. I heard horror stories of how it can take up to 48hrs before pharmacy delivers arrive. Not here, i arrived at 9:00 pm and my meds were right behind me mid morning. That was due to staff diligence. From Ms. Ngimbi (she went out of her way to make me feel welcome and followed up with me to make sure any concerns if any were addressed ) to the entire nursing and aid staff, physical therapy, maintenance and food preparation (covid restrictions called for meal brought to room but very good considering) and the admin staff was very nice and could tell they cared. It is looking like i will be needing additional surgery I can breath a little better knowing that i can come back for post care if needed. This has been a very difficult and trying time for me and my family but this facility and the people that make it what it is have made it better. As for the building and amenities, the pictures on the site are accurate and it is kept very clean. 5 stars from this Marine.
