My mother in law was admitted here immediately following a hospital stay and I will say the services were mediocre. The first two days she was there one of the aids told me that they had no idea she was even in the room. She fell twice and it was denied by the staff. They even told that the scar that she obtained came from the hospital staff. I think they were my first focused on how they were going to get paid rather than care for her. We never got a care plan on paper even after discharge there were services that were supposed to be offered to us. IE Mom‘s meals that we were told about and a month later I called to see about getting that information and was told that they can no longer help me due to her, not being in their care anymore. Not what I expected for someone caring for the elderly

We were checking for a memory care facility for my mother and I was very impressed with Prairie Lakes. I liked how it was down the street from my house. Jessica was very helpful giving us a tour and all the information we needed. They did a great assessment on mom, the facility was beautiful and felt my mom would do well here. We eventually picked another facility only because it was a lot closer for my dad to visit. I recommend giving them a visit if you’re looking for a nice and safe community for your loved ones.

I was there during covid recovering from surgery from a benign brain tumor. I was there a month. The staff did not treat me well. The intake nurse caused me pain by taking a required test. She poked my arm with a needle and then twisted it in my arm. It felt like a hornet sting. I cried in pain and looked at her face. She was expressionless like a psycho. My therapists told me if I asked someone over the weekend to go outside in a wheelchair maybe someone would take me. I asked a nurse Saturday and she coldly told me no. She said maybe Monday someone could take me. I would ask a question to the staff always promised to get back to me for answer. Never did. Staff was disrespectful to me. It was just a job for them. I won't recommend this place. I hated being there.

This facility truly deserves its five-star reputation, recently earning a top award from Trilogy, their corporate owner, and an impressive near-perfect survey from the Indiana Department of Health. The team at Prairie Lakes is nothing short of extraordinary. Holly, Kristen, Stephanie, and the entire staff consistently go above and beyond to ensure that every resident, family member, and coworker is treated with genuine care, compassion, and respect. Their professionalism is matched only by their heartfelt dedication to creating a warm, welcoming, and supportive environment. In addition to the exceptional care, the facility is currently undergoing a complete remodel to ensure the quality of the physical space matches the high level of care provided. Cleanliness is a top priority, and Eric and his team deserve high marks for maintaining an immaculate environment throughout the building. The dining experience is also worth highlighting. Patrick, Max, Tyrese and the entire kitchen team bring a personal touch to every meal, serving delicious food with a smile. Basem, Anna also stands out for always going the extra mile, ensuring residents’ needs are met quickly and thoughtfully. A simple gesture, like bringing my mother a banana because they know it will brighten her day, speaks volumes about the care and attention to detail provided here. The activities team also deserves immense recognition for the incredible “Circle of Friends” gatherings each evening. This special time after dinner brings residents together to sing, share stories, and connect in meaningful ways. It’s been a joy to see my mother come to life during these moments, finding companionship, laughter, and a renewed sense of purpose through these heartwarming gatherings. They would also help accommodate connecting my mother through her iPad to my sister that lives in Australia to have FaceTime conversations as needed. Now that Service! We also want to give a special shoutout to the team on Conner Drive. The nurses—Bri and Angela, whom we see most often during the first shift as well as all the nurses and medical staff —and the dedicated CNAs, including Dylan, Shawn, Mia, Shelby, and so many others, provide care with deep compassion while striving to provide highest level of professionalism. If you are looking for a place where your loved one will not only be safe and well cared for but also genuinely loved and valued, Prairie Lakes Health Campus is the place. We give them our wholehearted five-star, two-thumbs-up seal of approval. Highly recommended!

One of my favorite places for my clients to recover. Their rehab is well staffed and the therapy department is top notch. I have placed several clients with horrible situations and they all leave stronger an able to go back to their life. The majority of the current employees have been with this Trilogy building for many years. The Assisted Living section of the Health Campus is adorable with well trained team to care for every patient’s needs. Top notch building for Rehab, Long Term Care, Memory Care and Assisted Living, Prairie Lakes can help you or your loved one in many ways!

Legacy, which is part of the Prairie Lakes Health Campus, was my Mom's second Memory care unit and we did not have a positive experience at the first place. The difference was the staff and how they truly care for the people that reside there. From the nursing staff, to the cleaning staff, to the chef, to the grounds keeper, they were all so kind and caring to both my Mom and to our family. The staff is consistent without the normal turnover and has been there for a long time. No place is perfect but I found that this caring attitude truly made all the difference in her experience there. I can't thank them enough.

Staff really care about patients and strive to meet all needs. Food is much better than a lot of facilities I know. The therapy team works with you to maximize your time. I feel that this place is one of the best buildings in Hamilton county, definitely recommend it.

We are so thankful for the amazing people at Prairie Lakes. Every single person, from leadership, to healthcare providers, to the dining team, and on have been truly caring and involved with the residents. The team on my mom's wing are all angels!

My Dad has been a resident at PL for about 2 1/2 years now. Unfortunately he is bed ridden and only has use of one arm. He came in during the pandemic. I tried to be very understanding of the staff issues and other problems because of short staffing due to illness. (Also I work in healthcare, and understand the struggles) But as we have come out of the pandemic things have not become better. My husband and I are only able to get to see my Dad once a week due to our busy schedules. So I rely on the staff for daily care, especially since he can’t do simple tasks for himself anymore. Every week when we show up my fathers nails are long and hands are dirty. He has food on his face all down his shirt and under his neck. His teeth don’t look brushed and I have to constantly ask to have him put on the hair cutting list (even though they ask me if I want him on a 4-5 week rotation and I say YES! every time). Now he is developing eczema on the sides of his face, ears and hair line. I am beyond frustrated. Like one of the other reviewers stated. I keep trying to find who is in charge and I get the run around. We also have to see him on the weekends which is another problem because none of the full time staff are there. So we get the… “I’m just here on weekends, I’m not sure what they do with your Dad during the week” Bottom line…HE IS YOUR PATIENT! No matter if it is the week day, weekend, or night shift. He is your patient to take care of!! When we come to visit we try to take him outside in the sunlight. We do this because he is sitting in a dark room all day almost everyday. In the end I wish my Dad (and all of the residents) could get the care and respect they deserve.

They have really gone down hill fast. We have been patient due to covid but they are really taking advantage of that. They have done little to address how short staffed they are. They refuse to acknowledge they may have to keep up with fast food workers pay to get some people on board. They have contracted with the worst doctor imaginable when it comes to communication and response time. He will not communicate even with specialist doctors who try to connect him. They are so short on nurses they have people making decisions that are unqualified to do so. In general if this is what we saw when looking for a place, we most certainly not have chosen Prairie Lakes. I am positive some of the 5 star reviews that are here are from employees.

If you don't mind not having food this is a great place to be. I have beenhereseveral weeks and they have not brought lunch and or diner at least six times. Not to mention sometimes you order food and they bring you whatever they feel like bringing you. I have let them know I don't eat peas or carrots I get them about 3 times a week. The back of my menu sheet says no less of carrots. Rediculious.

I can’t say enough wonderful things about Prairie Lakes. From the moment I first walked into the facility and met with Kelly Ponsler and the rest of the amazing staff, relief washed over me. I knew my mother would be in safe, capable hands while she rehabbed there after a release from the hospital. The campus is beautiful and well-maintained. The interior is clean, organized, and feels like home. Every staff member I met, regardless their job title, was nothing but kind and helpful. And the food...oh my! My mother raved about the meals. Honestly, I wasn’t sure I could get her to leave upon her discharge date. The staff is knowledgeable, professional, patient, and just plain nice. My mother worked with a physical therapist, speech therapist, and occupational therapist during her stay and not only did she return to her baseline goal, she improved in all areas. The peace of mind I had during her stay, the smile on my mom’s face during each visit, that all says a lot about her time there. 10/10 would recommend. Thank you Prairie Lakes for helping my mom and getting her home. You truly are a life saver!

Good Morning, Went to visit a family member at the Legacy at Prairie Lakes Health Campus a few days ago. When I walked in the building was so clean! You can tell that the Housekeeper really cares about her work, resident's and there families! Way to Go. While walking to my family member's room I saw the Housekeeper sweeping the floor, picking up trash making the building shine, one residence walked up to the Housekeeper and wanted a hug the Housekeeper gave her a big hug, was smiling and talking to the lady, this just melts my heart knowing there are such amazing people working with my family member and other residents. Not only was the housekeeper making sure the hallway's were clean, she was cleaning the rooms and bathrooms wonderfully. The housekeeper greeted me with a smile and a hello, how are you today, that is when I got to see her name was Amanda is what they badge read I do believe. (Not sure of the last name). Also noticed the nurse was very helpful when was as a questioned she answered and was so polite, not sure of her name. After I entered my family members room I noticed that it has been cleaned, bathroom looked fabulous was well stocked. Floors have been swept and mopped, this sink and toilet was cleaned, the carpet in the room was clean and vacuumed you could tell, trash was taken out, the furniture was all dusted and the room was smelling amazing smelled like a baked cake! This has not been done in awhile I was happy to see that someone knew how to do there job and took it serious. Thank You:) Also while I was there I saw a lady that was tall, had glasses and curly hair with a dark blue long sleeve shirt on cleaning the dining room tables, she was so nice and caring she stopped what she was doing to help a resident. Would also like to let you all know Holly the Director is absolutely amazing, we just adore her so much. Holly you have a great staff with the three ladies I saw going above and beyond and making my family fill welcomed and cared for! Please let them know I really appreciate all the are doing, continue what you ladies are doing because you are making a difference! Thank You, Tinsley

Excellent rehab team that focuses on getting resident strong and to appropriate level of care. Food is restaurant quality and multiple choices. Truly a loving and caring environment all around

I cannot say enough about this campus. The caring staff, the amazing leadership team, and the outstanding food is just above and beyond what you'll find elsewhere. The staff cares for the residents as if they were their own family. The atmosphere is just different at Prairie Lakes and I certainly encourage you to consider them for therapy, assisted living, memory care, or skilled care.

Prairie Lakes is an amazing place! I love all the activities they offer and that they come up with ways to help residents live out their dreams! They just recently took a resident up at the campus in a hot air balloon. Can't say I've ever seen anyone else do that for a resident! I would recommend them for a loved one.

I have been so pleased with Prairie Lakes and the care my mother is receiving there. Jess was the first person I met with and she was so extremely helpful with getting my mom all setup there. Her caring attitude makes everyone feel so welcome there! The entire staff is wonderful and so caring! The facility is gorgeous and very homey! My mother's room is spacious and beautiful. The food is excellent and they even have made to order breakfast. My favorite part of Prairie Lakes is the many activities that the residents can attend. I love how busy my mom is, going to pet parades, bingo, choir, and so much more. They do a great job keeping the residents busy and stimulated! I am so pleased that my mom is at Prairie Lakes where I know she is being well cared for and I love that I am only five minutes away so I can spend a lot of time with her.

My observations from Prairie Lakes from back in March was the best. You can tell Staff care about their individuals under there care. They are emotionally connected with thier individuals. The night before i was there, they had one of thier individuals under their care. They felt the loss just as they lost one of their own family members. This is a prime example of the care they give. Grand mothers care while here is been great. Enjoys pictures on her tv. Everything has been great.
