They have helped me so much with everything. I would recommend them to anyone needing rehab. It is not what I thought it would be, if I have to be somewhere for rehab, it will be Wellbrooke of Avon.

I was sent to Wellbrooke of Avon from the hospital because I needed physical therapy. The PT staff began working with me immediately with a measure of aggressiveness wrapped in a blanket of kindness and concern. I was able to regain a level of mobility equal to my previous abilities. The food and activities were positive. The facility is clean and the grounds are lovely. The aides who carred for me were cheerful and hardworking but in my opinion they are spread much too thin. They have a lot of responsibilities for each patient in their group from getting some out of bed to showering and dressing each person. In addition they had to make the beds and clear the bathroom surfaces, collecting all the towels and wash clothes as well as collecting the patient’s dirty clothes and then either take the patient to The dining room for their meal or make sure that the meal was delivered depending on the patient’s choice. The call system is a call pad which turns on a light at the door to your room. There is no voice part of the call system so the patient cannot tell anyone what is needed. I think this is unacceptable because the need could be immediate or not. No one knows until the aide comes to your room to check on your situation. I would give Wellbrooke of Avon 5 stars for facility and PT but I would give them 3 stars for patient care. The aides try very hard to take care of the needs of all of their patient load but it is impossible for them to complete their expected duties in a timely manner.

I am here for rehab and I have has a great experience! The nurses are kind and helpful. There are some students that have been learning here and Alexa is so kind! The dining staff even came down to check on me one day when I hadn't come down to eat yet. The food is fantastic! I would recommend anyone to try this facility for rehab! Wellbrooke of Avon has been a blessing. We recently moved in a loved one to the ALF, and the transition was seamless. Victoria in Admissions, is a true pleasure to work with. She was upfront and honest throughout the tour process, ensuring that we were fully informed and aware of all the details, without any hidden surprises. Sherry, the Admissions Nurse, was incredibly patient and helpful. She guided us through the process of transferring all medications and ensured that everything was properly arranged and accurate. Cathy in Activities, is an absolute blessing to the facility! She has been incredibly caring and attentive to our loved one, making her feel right at home. Our family wasn’t sure how this transition would go, but the Wellbrooke of Avon team has made us feel at ease and has made this transition easier to accept. THANK YOU to the entire team for your kindness and compassion and for taking such good care of our loved one!

I was at Wellbrooke for almost 9 weeks after falling and fracturing a kneecap and shoulder. I received excellent care and have only good things to say about Wellbrooke. Thanks especially to the therapy team who is top notch. They all work well together and are very kind and encouraging in providing excellent care. I worked mostly with Robert, Chai, and Jaladhi and want to thank them for helping me reach my OT and PT goals. The rehab director Jodi is amazing and keeps everyone organized and on track. Her trainee Courtney was very personable and helpful. Kathy and Heidi in the Activities Department always encouraged residents to participate in fun activities. They are the best! Samantha took care of the insurance side and arranged my home therapy after discharge. In addition, the facility is clean and inviting and has staff who genuinely care for the residents. I can't mention everyone but thanks to all at Wellbrooke who helped me through a very difficult time.

Wow what a great place! My Mom was recently at Hendricks Regional Hospital & they suggested she go to Rehab after she was released. We chose Wellbrooke of Avon from a list provided by the hospital. I have nothing but praise for this place!!! My sisters & I were scared . the workers at Wellbrooke of Avon welcomed us with open arms. We were allowed to tour the facility & ask questions even before Mom moved in. Since Mom has been in Wellbrooke of Avon, her therapy has been amazing & she is getting much stronger. Whenever I have a question or a concern I immediately get an answer from someone that works there. Audrey at the front desk has been wonderful. I can stop at nurses station, there is always someone there and willing to help. The Social Worker, Samantha, is the BEST! She has walked us through so many issues. I have stopped by her office, talked to in the hallways & called her from home she makes herself available & listens so well. Thank You Wellbrooke of Avon! Mom is leaving Rehab . but we are keeping her there. I met Victoria in Assisted Living who is just as caring as the Rehab Employees. My sisters & I have agreed to move Mom into Wellbrooke of Avon Assisted Living.

I was here 3 yrs ago with a broken hip. Stayed for 4 weeks. Everything was great then. This time I was here because of ear cancer with a lot of stitches on my neck, a wrist where surgeons took out an artery & vein & a skin graft on my thigh. Everyone here has been so nice & caring. The nurses, OT/PT people & dining people really take time to treat me with care & concern. I would greatly recommend this facility for anyone needing rehab.

Great place to stay after hospital stay. I pinched a nerve and couldn’t stand on left leg at all. The pt/ot staff was amazing and had me walking with aid within a week. Food was good and always would make you something if nothing on the menu sounded good. Outside area was peaceful and beautiful to sit. We had lots of activities that were so fun and activities director was amazing. One night we had casino night happy hour including alcohol, soft drinks, and food. We often had special food, and fresh cookies at 9:30 every morning in activities area. I met many new friends and am coming back to visit some. Don’t want to need again but if I ever do I won’t hesitate to come back to this facility. Special shoutout to the head of laundry, activities and rehab floor nursing staff Folake rn and Pat Lpn

The therapy team worked well with me. The nurses gave me all my medications as they should. All the nurses and housekeepers are good and work with you well. They always ask if they can help you. I have been to a few nursing homes before this and this has been a better experience. Everyone is friendly, even the other patients said hi. Overall I enjoyed my stay here. In a way I don't want to leave, but I need to go home.

Wellbrooke of Avon has been such a pleasant surprise! I had such low expectations of nursing homes and when my mother was in need, we chose here after touring 8 different facilities! The staff are friendly and professional and all have called my mom by name since the very beginning. It is clean and does not smell like a nursing home. My mom raves about the food and enjoys all of the activities, especially bingo and when there is live music. My mom will move over to the assisted living side to participate in their Almost Home program to give her a few more weeks to rehab and build strength before going home. The staff communication regarding insurance has been timely and effective. All in all this has been a much better experience than we ever expected.

At Wellbrooke, Social Services Director Sidney Foxworthy provided super customer-centric care. Our calls were returned promptly with extremely informative responses to our questions regarding our first time use of a rehabilitation facility, as well as recommendations, which expanded our purview of resources available to my 82 year year old aunt. Once admitted, we found the facility consistently, clean and sparkling and fresh! Staff were extremely attentive. Made my 82 year old aunt feel right at "home" the 10 days she was there. Scheduled telephone meetings with staff were promptly conducted by the multidisciplinary team assigned to my aunt. Each member of the care team was very prepared with detailed and thorough assessment information, and next steps concerning my aunt's care. Wellbrooke exceeded our expectations with their professionalism, organized corroboration between and with multidisciplinary team (Nurse, OT, PT, Administration), as well as personal attentiveness to both client-patient and follow-up with me as caregiver. Upon my aunt's discharge, a very comprehensive and thorough continuation of care plan had been created Sydney to include a 30-day supply of pharmaceuticals, conveniently segmented by day, date and time to be taken. This was extremely valuable and helpful to my aunt not confusing her medications and timof day to take the medications. Wellbrooke wowed my family with their superior services and organization. My Sister and I have entrusted Wellbrooke of Avon with the care of our Mother, who has stage 5 Parkinson's Disease. They provide 24-hour skilled nursing with a tremendous amount of help from the awesome aides employed by Wellbrooke (not outsourcing help). Jonna, Lisa, Chantel, Robyn, Mary, Wesly, and the rest of the staff all rotate halls so all aides get to know our Mother and provide the personalized care she needs. This is important to us when we are not there, as it gives us comfort that she is not just a patient, but a person receiving individual attention. The activity staff, Heidi, Neta, and John, all go out of their way to make sure Mom is not left out, even though she can't participate by herself. Even Cody, one of the chefs in the cafeteria, has learned her likes and dislikes in food and knows she loves her ice cream. The entire staff is professional and polite, and I must mention Sonya in the financial department; she has been patient, assisting with sorting out the Medicaid nightmare. I know there are no perfect assisted living facilities, our Mom has been in 3 different ones in the last year and a half, but Wellbrooke has been the only one that I can walk out of and be able to sleep, not stressing if she's being cared for. My name is James Roth. I have just spent 5 weeks at Wellbrooke of Avon rehab learning to walk again. The experience was exceptional. From the first day of my arrival and for the next 35 days the people worked diligently, getting me back on my feet. Nurses Jenny, Pat, and Grace were always so kind. The nurse practitioner, Jessica, did routine check-ins with me. Monte helped me with my questions and answered them. Kauther and Frida helped me with my showers and were very gentle and courteous. Candice, an aid helped whenever I needed it. Heidi, Micheal, and Netta in activities were always trying to get people involved and out of their rooms for activities. Nola in housekeeping was very thorough and went out of her way to help me with something not even in her job description. Marsha in the salon did my hair and did a wonderful job. The culinary team really tried to make sure the food is prepared well and warm for the residents.

My 91 year old Sister had gotten too weak to stay alone. Wellbrooke was able to offer her a respite stay. We choose two weeks. She loved it there and ate better and willingly participated in the various therapies. We are grateful for the outstanding care and concern she received. After my rehab experience at Wellbrooke of Avon with two aunts (2016 & 2020) and my mother (2018), it was a no-brainer that Wellbrooke was the #1 choice when my brother required inpatient rehab three weeks ago. Four years after our last experience at Wellbrooke, my whole family has still been extremely impressed with and grateful to the entire Wellbrooke staff. The facility is "home-y" and clean (with no lingering odors). The nursing staff has been right on schedule with medications and care - which is extremely important if you are dealing with pain medication. The therapists have really taken the time to understand my brother, his goals, and what methods of therapy work best for him - and motivate him. The administrative staff members have been an office-peak-in or a phone call away to answer all our questions - carefully explaining and making sure our questions have been totally answered. But the thing that stands out the most (in my experience of 22+ years of caregiving) with Wellbrooke of Avon is the consistency. The level of care, individual attention, and thoroughness has not changed, in my experience, over the past 8 years. All of the staff are attentive, funny, and comforting (to patients AND families), making it a good experience - if/when someone is faced with the necessity of inpatient rehab. I have dealt with a number of rehab facilities over the last 22+ years, and THERE IS A DIFFERENCE! In my humble opinion - and for what it's worth to help other caregivers decide where to take a loved one - I wholeheartedly give Wellbrooke of Avon 5 stars...along with a huge thanks to ALL the staff who have taken such good care of my family over the last 8 years.

My mom had been in and out of rehab several years ago. We choose Wellbrooke of Avon. She was very pleased with her experience both times. Absolutely loved the PT department, and her Nursing staff. When it was time for her to move into an Assisted Living, there was no question on where she wanted to be. I can’t say enough about the staff, the care and compassion at this facility. There are plenty of activities to keep her active and her apartment is alway clean. Mom said If I have to be in a place, this it is the place to be. We are truly blessed.

My father is at Wellbrooke of Avon and both he and the rest of our family are extremely pleased with this facility. He is in the assisted living unit and all of the staff there has been wonderful to work with and very helpful. They all seem to truly care about his well-being. We are especially thankful for Mike, the head nurse in the assisted living area. He goes above and beyond in caring for all of the residents. All of the CNAs that help my dad are always willing to pitch in and are reliable and caring. The activities staff here is great and they are always trying to do special things for the residents to make their time here entertaining. The administrator of the facility, Danielle, is very hands on and responsive. We have been to 4 other facilities and Wellbrooke of Avon is by far the best we’ve experienced.

The staff at Wellbrooke is so caring. My family had a wedding while my Dad was in their care and Sydney and Heidi made it their mission to help us get him to the wedding. They arranged for him to ride their bus and stayed with him in case he needed anything so my siblings and I could enjoy the wedding without worrying about his care. I was so upset when the doctors at the hospital said he would need rehab care, but this has been a great experience for my Dad and our family. My dad was admitted to Wellbrooke of Avon for PT/Rehab & was in for about 3 weeks in late July/early August of 2024. Overall, it was a VERY GOOD experience during his entire admission! Especially when considering the bad experiences he had at the two rehab facilities he was at in 2023. Wellbrooke staff members were ALWAYS accessible & appeared to always be genuinely concerned about my dad. For the most part, they addressed our questions & concerns within 15-20 minutes of us voicing them. The one issue (complaint) that I have was that I didn't receive a call when my dad fell one morning. Fortunately, there were a couple of staff members who heard him fall & tended to him immediately. Unfortunately, I didn't find out about it until about an hour after the incident when I went to visit. Even tho my dad was not injured, & even tho someone was (very fortunately) right there when it happened, I explained to the nurse that I should have been called immediately, even before my dad was back settled in his bed. While not being called immediately is a pretty big oversight, that is the only negative that I can think of regarding this facility. There are SO MANY folks that I want to spotlight; unfortunately I don't know all of their names. But, a great big THANK YOU to SYDNEY (Social Services), JENNY (one of his nurses), GRACE (another nurse), ALL of the folks in PT, and ALL of the AIDES who assisted in my dad's care! I also want to say a special thank you to the staff members in the DINING ROOM/FOOD SERVICE area; they were super friendly, polite, professional, & helpful too!! If anyone asks me my recommendation for short-term PT/Rehab on the west side of Indianapolis, Wellbrooke of Avon gets my vote!

I helped care for a friend who spent several months at Wellbrooke in the past year. I honestly never had any complaints. The rehab and skilled nursing staff all seemed to care about him and were invested in his progress and well-being. He was always dressed and clean when I saw him. Despite resisting being in a care facility, he never complained about being at Wellbrooke. I never ate there, but the food always looked good and my friend never complained about it.

Their customer service is second to none! My uncle repeatedly complimented the employees on their caring nature. There were three employees that truly stuck out to us, Robin, Nicole, and Jonnia. I commend them on the wonderful services and care toward my uncle for his recovery. He stated that he loved the service because it was way more helpful than any doctor he's been to. By him being obese he was glad to be treated equally to any other patient. For this reason and his added medical condition, he required a larger chair for his room. So Rochelle, the director of nursing, came up with a plan by using a chair from the common area to accommodate him. Going into the facility he was not able to walk but after a 3 week stay he was able to walk on his own again. So the rehabilitation truly works! The services all together made him feel at home and safe with the workers. The workers also gave me a lot of insight into his well being so I was never in the dark about his current health status. Whenever I called they always had time for my questions. I felt comfortable leaving my uncle in their care!

My mother has been in the memory care unit at Wellbrooke of Avon for several years. The last year has been exceptional. The new director has hired more staff, hired a new cleaning staff with an excellent head of housekeeping, more activities people, additional client supervisory staff, a new Director of AL and Legacy,and the Food Services director continues to be top-notch. Mom's care has improved as has the cleanliness of her room and unit. I can only speak for the Legacy unit, but I am pleased with the care my mother is receiving.

Would never send a family member back to this facility. The staff all seems friendly, but not always very responsive. Very dirty and smells of strong urine. Never cleaned the room or bathroom the entire time we were there. Evening and early morning staff ignored call button when she needed assistance getting to bathroom. She had trouble with IV and was not properly attended. Was there almost a week before we had her "plan of care" discussion. They never got her medication correct or labs taken on time.

My mother was a recent resident of Wellbrooke of Avon. We cant say enough about the fantastic care she recieved. Nurses and aids on her wing were kind and attended to her every needs. Physical therapy was absolutely the best we have ever recieved in the past. Facility was clean and well maintained. Cathy at the front desk made us fell at home during our entire stay. I am staying at Wellbrooke of Avon for rehabilitation services. This facility is: 1. Clean 2. The staff is friendly, helpful, and as nonintrusive as possible. 3. The dining room is well maintained and the food is generally healthful and good. The variety is good. The food could be a bit hotter but otherwise is good. 4. The rehabilitation team is very proactive. 5. The nursing team is very responsive to my needs 6. The CNA staff has been very good. 7. Wellbrooke provides a variety of activities to keep the patients involved. Overall, I am very satisfied with Wellbrooke of Avon and highly recommend them.
