my grandma lives here and she loves it. We’ve even started getting together for the holidays here. The facility is always nice and clean, and the food is actually great. The activity and events schedule is always pretty lit. The double set of automatic doors in the front are slow. It’s like you’re going through some hazmat decontamination chamber before going in and out.

I've been here 2 and a half years. I thoroughly enjoy the exercise program. Food is fairly good. I also perform with fellow residents in the radio acting troupe which is such fun. Great location. Nice courtyard. Soup is very good

I have lived here for almost 3 years. It's been great; I've gone through a couple of physical changes, but Church Creek is made for you to age in place. They are very accommodating & so many great people live here. Lots of friends I have made here. Staff is very caring; always a reason to smile at Church Creek.

I am a resident here. All of the people are extremely friendly & always willing to help. I had fallen a couple of months ago, pressed my button, & someone came immediately to come help me. They are very thorough & kind always. They listen, & are kind.

Some good, such as the incredible staff providers, caretakers, and hospice nurses-- specifically Marina, Lou, and Mae, all of whom are godsends and were incredibly helpful and comforting during our time working with them. Some bad, such as just about everyone in the front office/management who couldn't seem to care less about what happened to our relative once we signed on the dotted line, and were befuddled when faced with doing their jobs once a senior resident passed away in a senior living community-- an event that has never once happened before, judging by the level of incompetence we faced when trying to handle her affairs. Let Lou run the place.

Honest Suzy was a bit too sincere clearly expressing our dobuts. But shines a lite why this place is for us.

From the consultation so far and the price and knowledge of the women Anissa, probably the most i have understood the process, adventure for my mom to be there, I felt at home relaxed, even the people that are there very friendly, welcoming, down to staff itself. Customer desk was polite, friendly also with me and the people living there even if the question was off the wall. Thank you so much for caring in a way words are not expressed.

In early 2023 my parents moved into a wonderful 2 bedroom apartment at Church Creek. Recently I was able to attend an annual Veteran's Day ceremony and lunch Church Creek held for all the military veterans and "Rosies" living there. Army officers from the 85th Support Command facility across the street were also involved in not only the ceremony, but also sitting with each table of veterans at the luncheon. Holly C. the activities director put on a meaningful and touching ceremony, honoring all branches of the service as well as a number of women who served at home as "Rosies" in factories, etc. There were tears of emotion as they were recognized for their service, many of whom has served 50+ years ago, and reminded of our gratefulness for their sacrifice. It was a truly wonderful event. Thanks Holly C. and Church Creek!

My mom has been at Church Creek memory care since February. The nurses and CNAs are wonderful and treat my mom with so much love and respect. I can’t say enough about them. Unfortunately it appears that the resident to staff ratio has decreased recently. No kitchen staff available so the CNA and nurse on duty are serving meals while also trying to distribute meds and care for the residents. Memory Care residents need constant monitoring and care and it’s just not possible for them to tend to all their needs any longer. The nurse on duty is also assigned to dispense meds at Assisted Living leaving one or two at the most CNAs. The staff is wonderful and many have worked at Church Creek for decades. Hopefully they won’t get burned out and leave.

I would like to send out a huge thank you to the nurses team in Memory Care....Fe, Seema, Virginia, and Nirvi. One of the main reasons why we chose CC for our beloved Mom! Their combined experience has reassured us. The 3 star rating does not reflect off the employees at Memory Care. Unfortunately, the past few months, I have a concern regarding the environment. The number of staffing levels has been reduced, experienced team members are leaving and the staff/residency ratio is not up to par. I am hoping things will improve and CC will stick to their "5 Star Senior Living" in Memory Care!

My mom and dad have been at CC for 1 1/2. Thank you to ALL the staff -reception, dining, housekeeping, maintenance-the behind the scene people who make it home! Thank you to Staff Provider caregivers for your kindness to my parents, especially to my dad ; ). They all try so hard to please and are delightful! Thanks to Prime Care home health for your professionalism and guidance! Mae and Lou-I don’t know what I would do without your ongoing kindness and caring!!! Thank you to the Agility Therapy team for knowing just what dad needs when he needs it! Thank you to Ken and Renee who manage the dining program and the flow of mealtimes! The food is outstanding!!! Thanks to Holly for spreading joy, the beautiful seasonal decorations, and programs! Thank you Kelly and Erin for your dedication to providing the best experiences possible at CC!!!

Facility is very clean and staff is extremely helpful. Once loved one has moved in, questions are not answered. Phone calls are not acknowledged or return, specifically Kelly. Promises are made regarding room and no follow up or action taken. : (

The staff in the Memory Care Unit take wonderful care of my elderly mom. They are attentive, kind, and work hard to help my mom live the best quality of life she can have at this time. i appreciate Seema, Fe, and Virginia for all of their help!!

My mom (Sue Fahey) age 97, has been staying at Church Creek Memory Care in Arlington Heights, Illinois since January of 2021. If anyone has their loved one at the Church Creek Memory Care, they can rest assure that they’re being treated and cared for properly. My brother and I visit regularly and the entire staff are always attending to the residents needs. My mother has a great relationship with the staff especially Fe, Seema and Virginia who allow my mother to engage with them as if she were a colleaguelol They Get It and they get my mom! It’s very comforting for the family of the residents to know that Church Creek has got their loved ones best interests at heart. Great job Church Creek!
March 2022
My mom is in the memory care unit at Church Creek and has been there for a little over a year. I am extremely pleased with the care she receives! She was in assisted living at a different facility but we moved here as it seemed she would receive much better care - she is, for sure!! The people who work there are extremely competent and compassionate. I continue to be amazed at the high level of care mom gets and the level of communication I get to make sure mom gets what she needs. Seema andVictoria are my main care contacts and always are pleasant and friendly. Fe is the unit director and always knows what goes on there and who is doing what. If someone needs something, she will always make sure they get it. All three of these ladies make the entire stay pleasant and manageable for the family. Could not be happier!!! I give the facility 5 stars and the people 7!!

We moved my mother into Church Creek recently. Understanding the importance of the decision, I talked to several senior living facilities. The folks at Church Creek stood out for their compassion and professionalism in handling this painfully difficult decision for my family and me. They walked us through the process and allowed us to move at our pace when we were ready to do so. The staff members have been incredibly helpful from on-boarding my mother and me to the new surroundings to consulting on medical issues to handling my questions about day to day activities and necessities. They are always there to take my call and put my worries to rest. I really appreciate the way that everyone has handled my mother with patience and grace while maintaining her dignity throughout. Maribeth, Kelly, Fe, Seema, and the rest of the team have made an incredibly difficult time in our lives a time for a hopeful new chapter in our lives. I highly recommend anyone interested in starting their own chapter to speak to the Church Creek team.

I am writing this review one day after my father sadly passed away at Church Creek. While he was only there for less than a week after suffering a stroke on Christmas Eve, I was very grateful for the compassion, attention and care he received. We experienced a "team approach" in which my father came first not the job description. I spent a lot of time researching SNFs because I anticipated that this would be where by beloved father would spend his last days. I am writing to help others who are searching for guidance and insights into this very tough decision. While no SNF is perfect, I was happy with our choice and appreciative of the staff.
