Mom moved into Lifestyles Transition at Wesley Heights this past October. What an impressive place! The facility is bright and sparkling clean with beautiful art work on the walls. The atmosphere is warm and feels like home.What struck me most were the smiles and warmth with which we were greeted upon our arrival. They knew exactly what to do in order to make my moms transition a smooth one. All staff, from management to front desk to caregivers to housekeeping, were so kind and welcoming. The nursing staff is great! They handle all the medications, can be easily reached, and do a great job keeping me informed when need be.There is always something for residents to do, and most important always someone to talk to. The activities and programs are both fun and meaningful. Often when I call, my mom is not in her apartment but busy attending one.Life Styles has been great for my mom, and no doubt has improved the quality of her life. Its great staff makes Lifestyles Transition what it is!

My father lived in congregate for over a year until his passing and loved living there. The staff was wonderful and he made several friends.

I dont know why someone gave only a one star review 3 years ago, but it compels me to write. We moved my mother from an assisted living facility in Colorado and the move literally saved her life. We arrived in Colorado to find an infected skin tear on Moms leg that went unnoticed. So, our first stop was the ER where we were given a course of antibiotics and wound care supplies. By the time we settled her into Lifestyle Transitions she had completed the antibiotics. She moved in on Tuesday and I got a call on Friday that she had a fever. A Certified Nurses Aide (Joy) had noticed that Mom didnt seem like herself and alerted the RN. The fact that after only 3 days of knowing Mom the staff was observant enough to see a change and act on it is remarkable. Mom had developed an infection that entered her bloodstream and required a weeks hospitalization to clear up. Had the staff of Lifestyle Transitions not been so observant I am sure Mom would have developed sepsis and we would have lost her.Currently Mom is very happy with her apartment because it doesnt feel like a nursing home. Her window looks out at a manicured lawn. She loves the staff and participates in many of the activities (just not bingo). There is always something going on in the way of therapeutic recreation. Of course, mealtimes are a favorite activity! And there is plenty of space to walk, either roaming the halls or on nice days outside in the courtyard. We couldnt be more satisfied with the care the Mom is receiving at Lifestyle and will always be grateful for keeping her safe and content.

We are extremely pleased with the care at UMH-Lifestyle Transitions. Our Mom feels as if she is at home with this community and the caring staff. The meals, diverse activities, and open line of communication with the family has made us comfortable with our decision to have Mom live here. Our frequent visits have always showed that Mom is well cared for and respected by the staff. The activities have stimulated her and added more quality to her life. She always has a new event or project to share with the family. A clean, lovely place for our Mom.

My mother has been a resident at Lifestyle Transitions for two years. The staff is extremely kind , caring and friendly to everyone. My mother is very happy living there. The atmosphere is comfortable and home-like. I would definitely recommend living at Lifestyle Transitions to anyone.

My aunt has been cared for at Lifestyle Transitions for over 2 years and we have been very pleased with the entire experience. She always comments to me on how nice everyone there is, and has adjusted very nicely to living away from her beloved NYC where she lived all her life. My wife and I have also found everyone there from the director to the aids to the front desk personnel to be very kind, accommodating and caring. The nursing staff is very tuned in to my aunts health and does a great job of communicating anything I need to know. We had a brief disappointing experience at another facility and have been extremely happy we made the quick decision to move my aunt to Lifestyle Transitions.
