If I could give this facility less than one star, I would. Whatever you do, do not bring your loved one to this location. When my mother became a resident, she was in a shared room and we paid $3,750 per month. At her annual renewal in May 2026, [name removed] and [name removed] were unable to find another resident to share the room, they changed her to a single-occupancy rate and increased the monthly cost to $6,195. I do not understand why it became our responsibility to absorb such a significant increase simply because the facility could not find a roommate. In my opinion, Prema has gone downhill since [name removed] became Executive Director after taking over from Terese Askew in July 2025. Throughout our experience, I found [name removed] to be rude, unprofessional, disrespectful, and dismissive of family concerns. Rather than addressing repeated complaints, I felt she defended certain employees and described them as “misunderstood” instead of holding them accountable. Several family members, including myself, raised concerns about two care staff members who frequently appeared to be on their phones or engaged in conversations with one another or with a private sitter instead of focusing on resident care. On multiple occasions, I personally observed residents in distress who were expected to come to the staff instead of staff responding promptly to assist them, creating what I believe were unnecessary safety concerns. I also experienced what appeared to be favoritism from management. Instead of addressing legitimate concerns, I often felt blamed or belittled for advocating for my mother. Families who speak up about the quality of care seem to be labeled as “aggressive” or told they may not be a good fit for the community. During our time there, the facility also experienced significant leadership turnover. There currently no full-time nurse and no Memory Care Director. My mother lived at Prema for just over a year and, despite my understanding of the facility’s assessment requirements, she received only one formal assessment. The assessment occurred in March of 2026 when her level of care was increased. This caused her rate to be increased which I challenged and they held off on the $200/month increase they planned in March/April 2026. When I spoke to other family members they experienced the same issue of an assessment with a rate increase. Not sure if this was a way for Prema to recover the losses for the company due to very few residents coming into the memory care verses those that left or passed away. State requirement requires the facility to have quarterly assessments which Prema in violation of. Because of my concerns, I filed a formal complaint with the Georgia Department of Community Health and have documentation, including emails, supporting the issues I raised throughout my mother’s stay. I was also disappointed that I never saw meaningful engagement from the owner, [name removed], regarding the concerns raised by families. The promotional videos present a very different picture from what we experienced day to day. In my experience, the meals and overall services did not consistently reflect what was advertised. When we moved my mother out on July 17, 2026, Memory Care had only about 13 to 14 residents despite having capacity for 32 residents. During our time there, we also witnessed many residents pass away, which was difficult for families and residents alike. The physical condition of the facility was another concern. Furniture and carpets were stained, the rocking chairs on the patio were often dirty unless families repeatedly complained, and routine maintenance did not appear to be a priority. Even the staff bathroom needed repairs, which reflected poorly on the overall upkeep of the building. Many other memory care communities provide better value and higher-quality care for the same—or even lower—monthly cost. Based on my family’s experience, I cannot recommend Prema. I strongly encourage families to thoroughly research other options before making such an important decision for their loved one.

Where do I even begin?? I was at my wits end trying to find a reputable community for mom, she was coming from a different state and to say the move was stressful is an under statement!! Enter the Oaks @Sawnee Creek and their amazingly skilled, professional, caring staff!! They held OUR hands thru the whole process, out my family at ease, were always available and made an incredibly difficult situation seamless!! Highly recommend!! 5 out of 5!!

WHY OH WHY did we wait so long to find Oaks at Suwanee Creek? What a godsend this has been for our parents!!! They were in another local assisted living facility, and without being ugly, I'll just say a good Southern "Bless their hearts"! Little did we know what great service and care REALLY looked like! We love the facility, the people, the staff, the amenities...all of it. In the next week or so, we will be transitioning our mom to the lovely Memory Care unit, and we feel so much comfort knowing how well she's going to be cared for. Plus, with their policy of inclusion for memory care patients, our dad will be able to be with her as much as he pleases! This company GETS IT. They care about their clients as well as the family members caring for them. Great communication, great kindness, and great care all combine to make us VERY happy daughters!!!

Our Mom has been at the Oaks at Suwanee Creek since September 2021. We have been very pleased with her care. She now considers the Oaks her home. The staff is always friendly, caring and sweet to Mom. Everyone there has a big heart and have truly been a huge help to our Mom and our family. My mother recently has been experiencing challenges at home alone due to health and mobility concerns. This is what started my families journey in looking into various senior communities around Suwanee. We recently did a tour of Oaks at Suwanee Creek and immediately felt comfortable with the staff we met at their community. Very family oriented, easy to talk with and helpful in navigating us through some of the challenging conversations my family needs to have to get mom situated. While we aren't ready for Mom to move-in just yet once we have everything organized, i'm confident this is the community I want my mom living in. Oaks at Suwanee creek has been an absolute amazing transition for my dad who is residing in the memory care unit. Evan has been a great help to my family. We moved from New York to Georgia and he made the transition a smooth one. The staff is very friendly and helpful. They always have activities and nice holiday things to do. It’s a very family oriented environment and also a very beautiful place. My dad is well taken care of and me and my mom are at ease knowing the staff is great. I would definitely recommend this a new home for someone’s family member.

My husband has been a resident in Oaks in Suwanee for almost a year now. We moved from New York to Georgia and I couldn’t find anywhere that was suitable for him. After speaking with Evan I was comfortable and very sure my husband would be well taken care of, and attended to. He is in the memory care unit and I have to say that he is very comfortable there. Although he misses being home that is his home now. They keep him occupied and make sure that he’s safe. Evan is always available at a moments notice. If there is something that needs to be addressed you can be sure ,that it gets taken care of soon after its requested I would highly recommend Oaks to anyone who had to leave their love one with people who are caring, giving and compassionate. Thank you to all of the staff at Suwanee.

My parents are residents in Horizons (memory care) at the Oaks at Suwanee Creek. It is a wonderful facility. *Please don't let the negative reviews influence your decision in finding a place for your loved one. (there are several from ONE family, regarding one person..since those reviews, they have hired a new nurse and E.D.) After having loved ones in several different facilities, and also having toured almost everything in the area, we realize this is a very special place. First and foremost, the caregivers, nurses, directors and managers are all loving, caring people that value the dignity of the residents. This is so important! They put a huge emphasis on the culture of this place so that it is the best it can be for our loved ones. Call, texts, emails are answered immediately. The maintenance team is quick to respond to work orders and takes a lot of pride in their facility. Besides that, its a beautiful "home" with a chapel (weekly worship services), movie theatre, beauty salon, delicious food served in the warm dining rooms, fish tanks, bird aviaries, fountains, courtyards, etc. Even memory care residents are able to come and go from the outdoor courtyard any time they would like some fresh air. They have daily activities and encourage all residents to participate. They have a bus and take lots of field trips....they make it fun! Before coming here, my parents were stuck indoors 24/7 and left in their rooms with nothing to do. They are so much happier now and spend their days with their friends in a loving, caring community. I am forever grateful to the Oaks for providing a wonderful quality of life for my parents.

Horizons, the memory section of the facility, provides the support that our family member needs to live a better life with dignity. Anyone with a loved one with Dementia knows this is a devastating disease. We feel so fortunate that the Oaks at Suwanee Creek opened this spring. It is absolutely the right place for our loved one.

My mother in law says she’s received the best physical therapy there that she’s ever had. She feels cared for and loved and she isn’t afraid of falling anymore. I highly recommend this place!

We are so pleased with Oaks at Suwanee, and how the staff and residents are really like a family for my mother-in-law. When we moved her into the memory care facility three months ago, she was pretty depressed and I think had kind of given up. Advanced dementia is so hard on a person, and also on the family caring for them as well. Anyway, since moving to Oaks, she has rallied. And I think it has alot to do with the overall morale of the caregivers, support staff, and even the maintenance workers who keep the building kept up. Yes, our mom has good days and bad days, but the positive energy overflows to the residents, and they care about each other. The front desk staff are so cheerful and encouraging too! I told my husband just the other day on the way over to see his mom that it’s such a comfort having her in such a loving place. Any time we’ve had questions or a concern about her care, they have been responsive and quick to resolve the issue. I wholeheartedly give Oaks at Suwanee an A++!
