The staff at Azura are great. The caregivers do a great job taking care of the residents (family) there. Mom was there for 2 1/2 yrs and the staff was always good to mom and to us as family members. The activity people do a great job keeping them busy, taking them on imaginary trips and real trips and so much more! Well no one wants to see a loved one in memory care, we always felt more at ease knowing she was being cared for by the great people at Azura! We never regret putting mom in memory care cause it gave us more time to spend quality time with her, while the staff dealt with the things that can stress a family relationship. Highly recommend the facility! Clean, Friendly and always welcoming. Hats of to the staff at the Oak Creek facility!

My husband has been at Azura Oak creek since July. He has been given lots of loving care. He has some very difficult days but the staff always make sure his needs are met. The food is very good, ( I eat with him occasionally). He sometimes has double portions He stays in the clean dining room until he totally finishes his meal. I am very happy he has gained 10 lbs already. He has a very nice clean sunny room and large bathroom. I feel content with Azura in Oak Creek. Thank you staff.

Azura staff here at the Oak Creek facility were patient and kind when my mother came to them, madder than a wet hen that she needed a place like this to help her. They were interested in her as a person and took into account what she wanted based on her history. As the dementia caused more communication issues for her and she became more aggressive, the Azura staff took it in stride and with a sense of humor. My son, a paramedic who works in another part of the state, came with me to visit her towards the end of her life. He said, "Mom, I been in a few places like this now. It's clean. It's really nice. She's clean. Her skin is so soft and in good shape." The activities staff put together many interesting experiences. Even though my mother wasn't expressing excitement anymore, they would send pictures of her involvement to us with a report on how she was doing. They listened when we, as her family, asked about medications, podiatry, psychiatry, palliative care and salon amenities to be scheduled. It may have taken a day or two for responses yet our requests were heard and on their "to do" list. Mom consistently was able to indicate that she liked the food that is served there. I was particularly touched by the Oak Creek Azura staff's love and compassion when Mom was on hospice care. I witnessed their gentle caresses, their kisses on her cheek and their "I love you's" during the days before she slipped away. My family was very happy with Azura Memory Care, especially the caregivers on Mayhew Drive. Thank you so much for everything you did for our mother. We will be thinking of you often as you continue your loving care of others.

My husband has been at Azura Oak Creek for a little over a year. This is not our first facility. Navigating this journey is very difficult on the family. One thing that I have learned is, you don't need all the bells and whistles in a facility. What you need is compassionate and caring caregivers. Don't get me wrong, Azura is a beautiful facility. You will always be greeted with a smile. If you are unhappy about something, say something. They listen to you. They really listen and will make every effort to meet your needs! A+++ to the wonderful staff! It also says a lot when you have staff that have been there for awhile.

Azura in Oak Creek is a fabulous place to work! Azura really values their employees and it shows from my interactions with the owner all the way to my director and team I work with. I have never worked in a home that had more compassion and specialized care toward dimentia. Azura's MOSAIC philosophy and training has really opened my eyes to what our residents with dimentia go through. I am so blessed to be part of a team that gives our residents a better quality of life everyday. I am proud to tell people what I do. t is with humble gratitude that I submit this entry. My sad face is the result of a very emotional day for me. You see, today was my last day at Azura Memory Care. I truly am saddened and affected to be leaving. When I started at Azura, I was welcomed with open arms, listening ears and leaning shoulders. I became part of an elite team that I was so very proud to call my extended family. Bonds were made and friendships gained. I’ve never worked for such an amazing company. The incentives available and the appreciation from this corporation for their employees goes without saying. I have established relationships with both my Residents and their wonderful families. Although my move has taken me to another facility, I will never forget the amazing people I met here and what a great facility this was to be a part of. I want to Thank my dear friend and Execcutive Director, EldaTujani, for giving me a home when I needed one. For she knew I would be an asset. I hope I made you proud Azura! I wish the company much prosperity and growth and hope that I can come back and visit everyone when ever I return to town. Thank You Azura Memory Care for allowing me to be a part of improving the quality of care for our elderly population, it’s where my heart is and will always remain. We are stronger together!!!!

The first thing that struck me when walking into the building was the cleanliness and coziness of the facility which is so rare to find these days. I would give Azura excellent rating. The staff was wonderful, not matter the time of day. My mom enjoyed helping out, and the staff was able to accommodate in multiple ways. Management responded to questions and concerns in a timely manner. They were extremely supportive when mom started hospice. 2020 was not a good year to have someone in long term care, but Azura had a variety of options for visits. I would highly recommend Azura Oak Creek.

I have thoroughly enjoyed working at this community as a therapist. The administrators, nursing staff and caregivers have assisted me and helped to ensure that the residents are cared for and have the help that they need. The staff is receptive to my concerns and help to increase the safety and independence of their residents. They are able to provide information about daily routines and each community members needs and desires. The staff is knowledgeable and helps to ensure that communication between the facility, family members and other agencies is completed in a timely manner. The facility is kept clean and tidy, and has beautiful outdoor areas to enjoy!

My Father has been a resident at Azura Oak Creek for about 17 months now. Its very hard to watch a loved one be affected by dementia. Its important to find a place where dementia is understood. My overall rating of Azura is very high. Thanks to everybody there for all they do.The building is fairly new and is very nice. It is very clean. I have been to other facilities where the smell of urine in the hallways is overpowering. I have never smelled urine here, it always smells fresh and clean.The caregivers are very compassionate and seem to truly care about the residents. I understand that being a caregiver is a very hard job to do and I commend them. Ive had interactions with the activities coordinator, RN, and Executive Director and all have been pleasantI think the only negative thing I can think of is that it can take a long time for somebody to answer the door when stopping by for a visit. But other than that I rate Azura 5 stars.

I am a therapist who comes into Azura to work with many of the residents at this community and have had nothing but positive interactions with the staff (care providers, RN and facility manager). The staff have been very helpful in providing information on their residents and look for help when they dont know the answer themselves. Each time I am in this community I feel very welcome and can tell the care providers really care a lot about their residents. I can tell the staff take time to get to know each individual in order to help them strive in their new environment. The staff are responsive to recommendations and are looking for ways to help their residents be safe and maintain their abilities.

We recently placed our mother into the Azura Memory Care facility located in Oak Creek Wisconsin. Mom moved into a new room providing her more space than she needed. The room as set up for easy use, certainly as moms physical condition deteriorated during her stay. The move was a learning experience for the family. Sign up was very easy as the staff took the time to learn the family members, they learned of our needs and expectations and openly communicated with us on a regular basis. As weeks passed, issues did surfaced, name a facility that does not have any issue. Each concern was addressed immediately when possible. We recognized rules and regulations, at times, prevent complete satisfaction or expectations of the correction. We did learn no caring facility is perfect, yet Azura was willing to do all they could to meet the families needs. Billing was easy and user friendly. The facility allowed pets to visit which provided a comfort and pleasure each day. Dining, arts and craft, live entertainment were provided with no pressure to join in. Mom was moved from the facility for family reasons. I plan to consider Azura should my time come where I need assisted living.

The nurse and care providers work well with therapy and help to provide a collaborative approach in caring for each individual. The Azura team has been a wonderful advocate for their residents and reach out to therapy if they notice a change in a residents condition and are proactive in reducing a risk fall

I am an occupational therapist that has been treating patients at Azura - Oak Creek for nearly two years. During this time, I have seen the wonderful care and compassion that the care giving staff provides to the residents of the community. The staff is aware of the many challenges that the residents may face with a dementia or memory-cognition-related diagnosis, and the staff members are able to advocate for their needs and desires. The community (care givers, administration, nursing, homemakers, activity directors - every member of the Azura team) is very supportive of our therapy team, and they work with us and our recommendations to achieve positive outcomes and the best quality of life for every resident. I am thankful to have them as a resource during my OT treatments.

This last year has been very difficult especially if you have a loved one in a assisted living facility. Elda and her staff have made it a little easier to deal with. The communication, I feel, has been excellent through this pandemic. I truly believe that my mother is being well taken care of. I also want to thank Elda for helping me get mom on Medicaid assistance. I didnt know where to start, so she provided contact information for me. It was not an easy task but Azura always supported us.

They are always open to suggestions and receptive to our recommendations to help improve the residents ability to participate in daily activities. They give us feedback on how residents are following through and let us know when a resident needs help with a particular task, so that we can better focus our sessions.

I am a Physical Therapist and the company I work for provides therapy services in peoples homes. One of those homes happens to be Azura in Oak Creek. I absolutely love working in these buildings. For starters, the collaboration is great with the staff there. We work very closely with Linda and Elda who are constantly advocating for their residents and communicate with us and the doctor involved. If they have a concern about a resident, they will often ask one of us to consult with them because sometimes you just need a fresh set of eyes - I think that is a true sign that they have their residents best interests at heart. I also love how they treat the residents. So often people with dementia are overlooked and underestimated. But Linda, Elda, Nikki and the rest of the staff at Azura look at their residents for who they are - wonderful, beautiful people who have lived a long and full life and have experiences and stories to share. They have so much potential to improve their quality of life and mobility, and Linda and Elda recognize the role of therapy in unleashing this potential, and they are huge advocates for therapy because of that. I have recommended Azura to many people already and I will continue to do so because I believe in their mission and their dedication to providing quality care for their residents.

This has been a trying year with the Covid-19 and not being able to see my mom in person. Azura has done a fabulous job with my mom she is happy and loves all the staff. I have worked for Azura Oak Creek for close to 1year now we have recently opened up 1year ago in August a friend of mine referred me and I am very pleased to work for Azurá Memory Care. I have worked for different establishments in the past and over all I have to say this one is one of the best ones just like any other facility we do have ups and downs at times over all weve had more ups than downs. I have met amazing residence and their families. I consider being a part of Azurá a family you get to know certain things about residents their likes their dislikes its a very challenging job but at the same time you need to be at outgoing, caring person to take on a job like this
