"After an extensive stay I was transferred from IMC Hospital in Murray, Utah to Mission at Alpine Rehabilitation Center. I was extremely physically and mentally taxed. I was only able to walk about 25 ft and my extremities were weak. I had reservations about going anywhere but home. That was not an option though. My daughter that arranged for my extended physical therapy stay insured me that after the many centers she had visited, Mission at Alpine was definitely the best. With my reservations I arrived at Mission at Alpine Rehabilitation Center. I was taken from the transportation van into Alpine. I was met at the door by the caring staff of Alpine. The first thing I noticed and put me at ease was that there was a lack of strong smell I thought was going to be there. I was immediately shown everything Alpine had to offer me from a peaceful relaxing area outside to my prepared and inviting room. My room was clean and very organized. The bedding had fresh sheets as well as a beautiful bedspread. I had a very clean bathroom area. In a short time the kitchen director came to my room and informed me of the options I had. I was thinking that maybe I would be receiving a lesser quality meal. I actually liked the meals there better than an upbeat hospital meal. I had plenty to eat every single meal! For snacking they had small sandwiches and fruit cups and puddings... that were quite filling along with several beverages to choose from. They even had a snack cart that went to each room in the evening. Any reservations I had of staying in an in-house PT residence were gone! Because of my extensive physical problems I started PT the next day. I was wondering if there was going to be something like a drill sergeant directing the therapy. What I did find is two friendly, knowledgeable and educated staff that were very professional. They provided excellent PT as well as intellectual therapy my doctor directed. Day one I could hardly make it to the PT area next to my room. One month later my body felt completely different. I had not only the strength to walk further and my extremities stronger but also increased mental ability to stimulate my mind to learn better again. The medical staff that interacted with me were exceptional! Not only did they take care of my needs but I observed that they took care of ALL the residents. They were always kind, compassionate and proactive. I pressed the call button one time in a month. I would most definitely trust my loved one to reside at Mission at Alpine Rehabilitation Center. Additionally, I would highly recommend Alpine to anybody needing in-house physical therapy. Mission at Alpine Rehabilitation Center changed my life. I am very thankful to all of them!"

Our family cannot say enough about the staff at Alpine Valley Care Center. The staff was there for our mom, very kind and attentive. It wasn't just a job, they cared. A few of the girls that worked there would drop by on their days off just to say a quick "hello". You couldn't ask for more. When she was passing, they were there with us as family. When she did pass, they bathed and dressed her without our asking. The memorial we had was filled with people that loved her and cared for her daily. She mattered and so did they. Thank you for the love you showed our mom... you made the difference.

My Mom was at Mission for only a short while, but her time spent there is one we will be forever grateful for. They gave her the care we could not. They loved her and made her last days better. It isn’t aesthetically pleasing, but the care will make up for that. I have nothing but love and admiration for the staff. I also have a deep love for the Residents. Our Mom was our world. Having to place her in a care facility was one of the hardest decisions my siblings and I had to make, but Missions was the right choice. A decision I would make again. Thank you for caring for our Mom as you did.

I have been experiencing Mission at Alpine for almost a year now. So I must give them a review. Honestly, I don't know how they handle this very difficult job, day to day? But, they do, and they do it so well! The 4 star rating is only because the owners need to invest in some physical improvements! The excellent staff deserves better surroundings, as well as the residents! I really don't believe you can find a more caring staff anywhere else. These people are good! And they care!! That's the most important part! THANK YOU!!!

My mother has lived at this care center for the past few years and they take very good care of her. It is a small Care center in Pleasant Grove, and we have been very happy with the service here. My mother is clean, well fed and has great relationships with staff and is friends with other residents. They are also very helpful with paperwork issues. They help me successfully navigate Medicaid rules on behalf of my mother, which is incredibly helpful.

my grandma was here for about 1 1/2 years intel passing, the nurses and cnas were the best people ever. they went out of their way to get to know us and treated my grandma like their own. they gave us a lot of special memories and things. everytime we had a problem they would try to help.we had her in a different nursing home and they were not as good. it might look run down from the outside but inside it’s so homey and loving. they never had problems with medications, the nurse has to give meds to every single resident so it might’ve been late but they are trying and i never had that issue. they get 2 meals (breakfast and dinner) and snack in between (some times mac and cheese, pizza, chilli etc) so it’s basically lunch but if they are still hungry they allow sandwiches and yogurt and more. they check water cups and drink cups every 2 ish hours. the nurses and cnas here treated us so good and i can’t stress that enough. if they had a concern they will call and let you know. if you ask they will tell you the percentage of food eaten. they do field trips about once a month or more. they go to walmart once a month and do a craft daily and games daily. they also have church on sundays if you like that. i highly recommend this place for anyone

My sister had been at Mission Alpine for 2 1/2 months. So far things have been fine. I like that there is a locked back yard area that she can go out to when she wants. There is room for her to walk around both inside and outside. There are lots of activities. The center meets with the guardians every couple months to go over care, changes, or any questions you have. Staff were very helpful getting everything transfered over and in order. They have been quick to let us know when there have been issues and have sought thoughts and ideas about how to help my sister adjust to her new home. They have been easy to work with, welcoming, friendly, helpful, and knowledgeable when I have come and have remembered me.

My mom was recently diagnosed for dementia 6 months ago. The good people at Utah Senior Planning recommended Mission at Alpine for memory care. At first I wasn't sure how she would do here after being alone for the past 14 years after my dad passed. It turned out after a few days she really enjoyed being around other people. The staff is really helpful and friendly. They make sure to call me if they're are any issues. The food they serve is nutritius and they make sure that all diet issues are met. I feel good knowing she is in a good place that takes care of her. The staff always make sure she gets her hygiene done every day. They also make sure her hair and nails are done so she can feel pretty. They always have daily activities to help with their memory. In the past 6 months I have seen my mom look more healthy and happy then I have in a long time. Thank you Mission at Alpine.

I feel welcomed at each visit and am grateful for the loving and kind care my son receives at this facility. His meals are nutritious and the help he needs in feeding himself is handled with patience and encouragement. I witness kind interaction and workers who attend to individual patient needs with soft words and attention to details with proper help given. I leave feeling much gratitude that my son is shown love by those who handle whatever the day's needs might present. No better place is found where the total dedication and service is so evident. It gives me peace of mind and I feel grateful and confident in knowing all is well there.

We were happy to find Mission at Alpine. I hadn’t left my wife’s side for more than a day or two in 40 years. 13 years ago she was diagnosed with Alzheimer’s. I was very reluctant to take a respite and leave her for several days. Ask the staff lol Anyway, the staff is extremely caring and really good about listening to your concerns. The RN Sarah that was on shift when Theresa stayed there went way beyond to see that the staff took really good care of her. I have recommended this facility to others and would continue to do so.

My brother in law has been a patient at Mission at Alpine for the past year and has received very good care. The building is a bit dated, but it always smells clean and the staff are very caring and friendly. Every time I ask my BIL if he is likes it here, he always smiles and says he LOVES it!

My dad was here for about 6 weeks following his hip surgery. As former doctor having alzheimer's, he would ask many questions about his medication and treatment. The staff was so caring and patient with him. They were respectful, understanding, and professional. So grateful for the care he received during his rehabilitation.

My dad recently stayed here for a few days for respite. It came recommended from hospice and I’m glad he stayed there. The staff made him feel very welcome and were wonderful to work with. They took great care of him and he enjoyed his short stay there. It is an older/outdated building but I think it’s probably perfect for the majority of residents who have dementia or other mental disabilities. The staff interacted with the residents very well and were very kind and patient. My dad’s wheelchair leg went missing and the staff was excellent about looking for it and ended up finding it in another resident’s room!

This is a nice little place that focuses on patients that need memory care services. The team cares so much for the residents and I love how they sit with them and hold their hands and do their hair. They are perfect people for the population they work with.

My dad lived his final years here before passing recently. It doesn’t look like the nicest place driving by but this is a great facility. The staff is caring and attentive and does a great job communicating with family. I will never forget the kindness they showed my father and how they treated him like family.

The caring of patients goes above and beyond here. The attention to detail and individual needs of residents is a high priority of the administration and staff, which is honestly an amazing attribute of facilities in this fast paced age. At first glance you’d probably want to pass over this care center as it looks a bit out dated, but please look deeper! My mom has dementia and this has been the perfect place for her. It is clean and smells clean. The staff is very caring and attentive. She is bathed regularly. She is on a regular schedule with her medications, meals and snacks. There is a lovely outdoor area that is secure for my mom to walk to her hearts delight (she is a pacer). When she tires she enjoys sitting in their recliners and looking out the window. They have a hair salon which always makes ladies feel good to get their hair done! They call regularly with reports and updates and genuinely try to find out as much as they can about her to make her comfortable and happy. My family feels so grateful to have such a wonderful safe place for our mom to be cared for. Anyone who has experience with dementia knows how extremely difficult and exhausting caring for your loved one is and this facility does a stand up job!

I used to volunteer here with my church group when I was a teenager and I enjoyed the experience. I have heard a few horror stories as they do hire some young individuals to take care of the mentally disabled that live here. Hopefully that has changed. I think it's always good when there's a place that takes in people that can no longer be cared for or no one wants to care for them anymore. Overall when I went as young women I found that they were feeding them healthy meals, they allowed us to play games with them, I even painted some of the girls nails and did their hair.

Their "Eden" approach of attempting to make a skilled facility as much like home as possible, has worked so well for many of our patients.
