I am sharing this critique on Crescent Rehab Facility in Sugar Land, Texas, to let potential clients and families know of the extremely poor and pitiful care that they can expect. I rated them a "One" because that is the lowest rating allowed. My wife contracted an infection from her hospital stay that required her to be in a room by herself, so she was transferred from Ft. Bend Healthcare Center to Crescent. FBHCC is under the same corporate umbrella as Crescent. We assumed she would receive the same excellent care she had been given at FBHCC but was that ever wrong. The following experiences are just a few of what my wife encountered for 15 days before she passed. Her passing was not caused by anything that she experienced at Crescent Rehab. Except for three nurses at Crescent, the rest of the staff on the Harmony Lane side did not know what the caring of a resident meant. When I told one of the physical therapists that I had never seen a place where the staff did not care. The physical therapist told me that I was the fifth person that MORNING that had said the same thing. In 15 days my wife received ONE sponge bath. On the day my wife passed, four nurses were lifting her out of the bed to perform CPR, and they dropped her on the floor. Only one person who was one of the three caring nurses said he was sorry, but no one else from Crescent facility has expressed that they were sorry that she passed. After the Cantex Corporation received my three-page letter listing all the incidents of pitiful care provided to my wife, one executive called and said they were sorry. When I asked the executive what they intended to do to address this total lack of caring at this facility, he said they tell the employees to be caring. I told him that "telling" without holding the employees "ACCOUNTABLE" is worthless. These people need to be held "ACCOUNTABLE" for the pitiful way the way they treat the residents. There are many, many more examples of not caring at this Crescent facility I could share, but I would like to conclude with this comment. "If you really care about your loved one, send them to Ft. Bend Healthcare Center in Rosenberg"; where from the Executive Director, to the nursing staff, and the housekeeping staff, they really care about the residents.

My dad was transferred here for physical therapy recovery after brain surgery at SL Methodist. The entire team there took very good care of my dad during his 2 week stay. Only issue is calling the phone will ring for long time or may not answer until multiple calls.

I had an extremely disappointing experience with Crescent Nursing Facility in Richmond, Texas. I wouldn’t recommend this place to anyone looking for a reliable and caring environment for their loved ones. Inattentive and Inadequate Staff: The staff at this facility seems overwhelmed, inattentive, and undertrained. There were numerous occasions when my concerns and requests were ignored, and my loved one’s needs were not met in a timely manner. The lack of attentiveness was evident in their approach to patient care, which was far from satisfactory. Poor Emergency Action Plan: During a recent natural disaster, it became clear that Crescent Nursing Facility was completely unprepared. The emergency generator was broken and had not been tested in advance, leading to a catastrophic failure during the emergency. As a result, patients were left without lights and air conditioning for three days, which tragically resulted in the deaths of several patients. The facility's inability to execute a proper emergency plan and provide even the most basic necessities like lighting and air conditioning in a crisis is unforgivable and highlights a severe deficiency in their ability to protect and care for their patients. Inadequate and Chaotic Patient Intake Process: The patient intake process at Crescent Nursing Facility needs a complete overhaul. When my loved one arrived on a weekend, the staff was not prepared to properly receive them. There were no medications available upon arrival, and it took an unacceptable two weeks for the necessary medications to be obtained. This delay in providing essential medication is inexcusable and could have had serious consequences for my loved one’s health. Furthermore, the ineffective communication between the facility and the contracted staff doctor regarding my loved one’s needs only exacerbated the issues, leading to confusion and substandard care. Medication Mismanagement: One of the most disturbing issues was the improper control and mismanagement of medications. The contracted pharmacy frequently failed to deliver necessary medications, leading to my loved one being without prescribed and pain medications for almost two weeks on one occasion. This lapse in medication delivery caused unnecessary and inhumane suffering, highlighting severe flaws in both medication management and pharmacy coordination. Lack of Safety Equipment Maintenance: The facility's safety equipment, especially for patients at risk of falling, was poorly maintained. My loved one experienced a fall due to faulty equipment, which could have been easily prevented with proper maintenance and checks. The call button in their room was inoperable, the bed rail was broken, and there were no fall mats near or around the bed—basic safety measures that should have been in place but were completely neglected. The disregard for basic safety measures is unacceptable. Poor Patient Accountability: There is a noticeable lack of accountability regarding patient whereabouts and safety. In one alarming incident, a disoriented patient wandered into my room and remained there unnoticed for at least an hour until someone finally came to check on me. It was evident that no one had missed the resident, and she had been completely unaccounted for. The situation left me deeply concerned about what could have happened had the patient left the facility unnoticed. The facility's inability to keep track of its residents and ensure their safety is not only irresponsible but dangerous. Ongoing Investigations: Perhaps the most concerning fact is that during my loved one’s stay, there were at least 16 ongoing investigations related to the facility. This indicates a systemic issue with the way the facility is managed and raises serious concerns about the overall safety and care provided. Unsatisfactory Transport to Medical Appointments: The transportation provided for my loved one to and from medical appointments was also unsatisfactory. There were numerous delays, and on one occasion, my loved one missed an important appointment due to miscommunication and poor scheduling on the facility’s part. This lack of organization in coordinating necessary medical transport further underscores the facility’s overall inefficiency. Refusal to Consult with Outside Physicians: Another major issue is the facility's refusal to consult with outside physicians in a timely manner, if at all. This resulted in poor aftercare for my loved one and likely worsened their condition. The refusal to seek expert medical advice from outside the facility is unacceptable and highlights a disturbing disregard for patient well-being. Ineffective Communication with Insurance Company and Appeals Process: The facility also demonstrated ineffective and severely lacking communication with the insurance company and the entity responsible for appeal approvals. There were long delays and frequent miscommunications, leading to unnecessary stress and confusion regarding coverage and care approvals. This lack of competence in handling insurance matters further contributed to the overall dissatisfaction with the care provided. Inoperable 24-Hour Access and Poor Communication: Although Crescent Nursing Facility claims to be a 24-hour facility, my experience suggests otherwise. When visiting my loved one outside of normal visiting hours, we were left standing outside for at least 30 minutes, if not more, trying to gain access. No one seemed to know or was willing to provide the access code to gain entry. This lack of responsiveness and poor communication during non-standard hours raises serious concerns about the facility's commitment to the well-being and security of its residents. Subpar Food and Maintenance Issues: The food provided at Crescent Nursing Facility was consistently subpar, lacking in variety and quality. Additionally, the refrigerator door in my loved one's room would not close properly due to it needing to be defrosted. This issue was not addressed until we brought it to the staff’s attention, reflecting poor maintenance practices and a lack of attention to basic facility upkeep. Poor Communication from Administration: Communication with the administration is virtually non-existent. When issues were raised, they were either ignored or addressed with dismissive responses. There is no clear line of communication, making it difficult to resolve concerns or get updates on patient care. In summary, Crescent Nursing Facility is severely lacking in several critical areas, including patient intake processes, staff attentiveness, emergency preparedness, medication management, patient safety, communication with both families and external parties, and overall administration. I would advise looking elsewhere if you are seeking a nursing facility that genuinely cares for its residents and ensures their safety and well-being.

The facility is very nice, lunchroom, food great, from the cna's to nurses. Only couple of misunderstandings in communication but overall good nurses, physicians, PA,s, Doctors, really care, clean facility. Do what they can to make patients comfortable.and care for their overall health...compared to other facilities, it's clean, and in weighing my views I considered the shortage of people interested in going into this type career. Now the Therapist, OMG!!!. They are really blessed by Creator. These Therapist work magic. I witnessed my 92 year-old cousin start with not able to stand at all, much less walk and now after about 11 weeks, she is walking, she can get up and stand and go from sitting on edge of Bed can stand up, hold on to walker and can walk. They truly are amazing and work with patient diligently. Can't praise them enough. They care, they are polite, respectful, , you're kept informed on patients progress , they motivate patients, gosh, Just can't say enough how great they are. May God Bless these folks for being so attentive and having so much empathy. Thank you to staff of every level of service at Crescent Facility in Sugarland for your superior professional help. My cousin is so happy and grateful and family also are very grateful. Thank you

I have been here since October of 2023 as a dietary aid and I can say that I have had the best experience I ever had working at this facility . the nurses are great , staff are wonderful would definitely recommend visiting .
