I was very pleased with my rehab experience at this rehab facility. The staff was caring, efficient, and professional. The physical therapy program was excellent, and my 'dream team' consisted of Phuong and Kevin who were dedicated to my success, working hard to help me get stronger, while remaining encouraging and fun. The nurses and caregivers were all excellent and professional, everyone of them friendly, professional, and efficient I would like to specifically commend Amador Torres who went out of his way to help me feel safe and well cared for whenever I needed to be transferred on the hoyer lift. The hoyer lift was specifically difficult for me because I was hurt by a caretaker at another facility while on it. Torres was so experienced, patient, and kind sensing my fear, he really took his time and went out of his way to help me feel comfortable. I am thankful for this special effort to calm my fears and make it so much easier for me. The place is kept clean, no need or request I has went unaddressed and taken care of. The food is good for institutional type centers like hospitals, I found everyone from management to housekeeping pleasant, hard working, professional, and friendly. They work as a team, everyone dedicated to the patient's wellbeing. I highly recommend NHTC, and will request to go back if I should ever need this type of health care facility again.

Tghis is a bad place to stay, I would only give it a one star because that is all it deserves, Room mate playing blues all night or eithet the tv is on loud, the food is the only thing nice, never enough staff her to answer the call button when adk for the nurses station never get an answer and this supppose to be the nurses station

This place is great and has potential, but a lot is out of order. The med aide Bridney was in a bonnet which looked like she got up from bed with it and came to work, wore a puffer jacket and her attitude was just disgusting - loud & yelling across hall

First off, the staff are phenomenal! They truly care about each resident which goes into the level of care they put into each person. The building was well kept, and very clean. They have a great variety of activities and food! Highly recommend!

My grandpa is a veteran and he's pretty picky. It was really hard to find a place that he liked. We looked at many facilites but this facility is very veteran friendly, they even had a big Veterans Day breakfast for all the residents. Grandpa felt so special at this breakfast. They have big private rooms with private bathrooms and the facility just feels good.

My grandmother was recently discharged after staying here for three months covered under her insurance. I give the facility three stars because no one answers the phone and they have a front receptionist, my grandmother loved the care she received. She loved it so much she didn't want to leave. She received physical therapy, which helped her tremendously. She could have visitors 24/7 and she was taken care of very greatly.

The place is aestheticlly pleasing, but when someone is sick, caring for them takes presedence over pretty pictures. The physical and occupation therapy is good. The nursing staff is to be of concern. They are just not that competent. The doctor and staff do not return calls or call to let the family know of any changes in medical condition, treatment, or results from tests. There is a reason why they will not let you visit and see a room. Transparancy is not in their vocabulary. It is just a shame and so sad the way the elderly is treated. Example: It took a week of us telling them daily that she needs a diabetic diet before she received the correct tray. Her blood sugar stayed between 235 and 415! At home her blood sugar is around 100-130 Their solution, just give her more insulin. Incompetent, and non empathetic staff and administration. Also they wanted her to sit in her urine and feces while in a diaper. Therefore, she has MRSA now and back in the hospital.

I have visited friends at other facilities like this. This is the best facility of its kind that I have seen. They have treated my good friend Sid very well. I recommend this place to people who need this kind of service.

The staff here are very friendly helpful and caring. I came here after a terrible accident. The wound care team, Tiffany, Mary and Rani are very good and attentive wound care nurses. My wound got so much better! The CNAs just were so caring. Misha was just great. She always made sure I got cared for properly. I can't say enough good things about Mrs Betty. Some even popped in to say hi to put a smile on my face. I'm a vegetarian and food service team made sure to make accommodations for my vegetarian diet which I was very grateful for. Chris and Toni from PT and OT were simply amazing. Even on my bad days, I always looked forward to them coming by to help me get stronger. Andrea, the Director of Nursing and her admin was very very diligent, ensuring I had what I needed for getting better. If you have to go into a facility - I pray no one ever has too, BUT IF you do, choose this one.

I've been here almost 5 weeks. It was good, until a week ago. The same person has been my aid the whole time. He was the best. I bragged on him & he changed on me overnight. Now, he treats me like I'm trash. I'm really hurt. Thank God I go home day after tomorrow. I can't take the abuse any longer. Quanisha is a doll. She takes good care of me. She makes sure I'm very clean for bed. She bathes me and she's so pleasant to talk to. I had others who was nice to me, Monica B was another good one. Tabitha is wonderful with me. Crystal Jones , Jade & another Crystal took good care of me. Olu was nice & friendly. Jeremy was a sweetheart. Victor & Charlotte took care of my respiratory. Housekeeping was excellent. I hope KH gets his stuff together. I know he can do better. Therapy was great. Dan, Toni, Chris, Maria, Traci and the rest were great. Thank you all for taking care of me.

I am giving 4 out of 5 stars as for the most part I had a very good experience. Prior to arriving I had had 2 extensive hospital stays and 1 stay at a rehabilitation center. I am recovering from a vitamin deficiency. I chose this facility as they had a wonderful rating for their rehabilitation program. I worked with Phong for PT and Toni for occupational therapy. They were both phenomenal. Phong was able to instruct me how to safely learn how to walk again. Upon entering the facility I could only walk 80 feet on a walker and had to sit down many times. Upon leaving I was able to walk on the walker to the bathroom and continue to improve every day. I loved that they both encouraged me. Phong gave me examples and little sayings to remember so that while I am at home I would continue to not panic and remain safe. For instance; he told me to stand wide like a tree while walking on the walker. Toni was so encouraging to me. She helped with every day task. I had fallen from my bed at home and had a fear of it happening again. She measured the bed which helped me to know how to much my bed needed to be lowered at home. The nursing staff for the most part were awesome. Anytime Victor, Charlotte, Allison and Olu were working I knew my medication would be disbursed correctly. I was in the facility for just over month away from home and family it was hard they always had a kind word and encouragement for me. When other nurses were working I had to check what medication was being given to me. I had a mid line in my arm and had to remind one of the nurses to wipe the end with an alcohol swab before attaching it to another tube for medication. The CNA's were short staffed many nights but did their best. I loved seeing Carla in the morning and the other CNA's for other shifts. Housekeeping worked hard at keeping the rooms and the facility clean. The administrative staff went above and beyond to help with my stay as I had a special diet that was difficult for the facility to accommodate at times. The reason I did not give 5 stars is because of the food. I was in the facility a month before I met with the dietician. The root of my health issue occurred after having gastric sleeve surgery. It was essential for me to have proper vitamins and nutrition in order to recover. The food was hit and miss. Most of the time when I filled out the form to request food I did not receive what I ordered. Tish went above and beyond to try to correct the problem but unfortunately when she was not working the orders were typically wrong. I ended up door dashing food a lot. I did not care for the DON her name slips my mind. I had been in the facility a month when she came in my room with Tish and Mesha to discuss the quality of food being served. She stated she was unaware that I had had sleeve surgery as the reason why I was stating most of the food served I could not eat, that I had several food allergies and intolerances. I personally felt that had the time been taken to review my file or to speak with any of the other employees my dietary needs may have been met. The beds are terrible. They brought in another bed; it was better but still extremely difficult to get comfortable. Their medication is delivered from a pharmacy; my first night at the facility they did not have medication I needed to help with the regrowth of the nerves in my legs. The visiting policy was difficult; because of the pandemic they would only allow 2 people to visit and it had to always be the same 2 people. The visiting hours ended at 7pm which meant that mon- fri I was unable to see anyone most days due to traffic it was impossible for anyone to visit. At the end of the day I came for the rehabilitation and got exactly what I needed. If you are like me; I would recommend the facility for the rehabilitation.

Jeremy Parker is an excellent support staff member at the facility where I am doing therapy He is punctual in the mornings assisting me to get up and dressed. He sees that I have my meals on time and if not to goes the extra mile to correct the problem. He has on several occasions saw to it that I have my shower when needed. He is an over all great help in time of my great need and that is to get stronger in my legs to walk again.. You need to be glad you have him working here at North Houston Transition care facility. Beside his good personality you can tell by observation that he loves his work. Thank you Jeremy.

Things changed since my mother's arrival at North Houston Transitional Care... during the first weeks of December.. Thanks to the changes in the nursing staff the quality of the nurses and nurse aides improved with the guidance of Mesha (staffing) and Scott ( administrator) and Melissa (ADON). My mom's main therapist Sandra, and the other therapist were very consistent and very encouraging on what she was suppose to do and why she had to do it in order for her to get the strength back in her legs. The facility is kept very clean. There also has been a great improvement on the food services.. Depending on her injury or the need of therapy She would definitely come back for their services. Again thanks North Houston Transitional Care for taking good care of my mom.

My friend Chang asked me to share her story here and she highly recommended North Houston Transitional Care because her good care while she stayed in this facility about 4 weeks post total hip surgery. She had a very difficulty time at the beginning--- pain and weak and plus the language barrier and food differences... Luckily, one of the employees Asha Li who spoke Mandarin and also was her physical therapist. With her translation and coworker Shawn & Traci occupational therapist's help, things got much easier and Chang could had all the help she needed from all of the departments. And she said the kitchen even made her some Chinese food to please her stomach and not feeling so homesick. Now, my friend Chang is home independently again and she is sharing her good experience with her friends and neighbors about North Houston Transitional Care. And thanks for all the staffs to provide the good care!!!

My mom is here at this facility and she loves it ...Everyone is very nice and helpful.. The nurses and cna's make sure she get's everything she need.. The food is amazing and has a wide variety to choose from. Compared to other facilities, this place is top tier. I am so blessed to have such amazing staff to help me heal and go home.

I love the care and attention to detail the staff take to help my dad during his rehabilitation. Enjerrie (spelling error), the unit charge nurse, has kept our family involved on my dad's progress, was a positive force during coordinating an emergency management issue, and spent quite a bit of time going out of her way on a Sunday to obtain a therapeutic recliner for my dad's rehab...it was ordered and set up in only a few hours so that he could begin using it Jeremy, another staff member, has also been very compassionate with my father's care. My dad has been appreciative of the attention that Jeremy, and the many other staff members, have made towards his recovery.

I had a good stay at this facility. The staff is amazing. The nursing staff is caring and nice. All the staff goes the extra mile to make sure you have all your needs and that you are ok. I had a wonderful experience here at North Houston Transitional Care. I received awesome physical therapy with Blake. He worked me hard but it was worth it.I am so much better now than when I first came in 3 weeks. Janelle was my occupational therapist. She worked a lot with my arms - building strength in both arms. I needed a SNF with great PT and OT services for my 82 year old dad. He had fallen 4 times in one week. This facility gave him fabulous care, the food was so good and the nursing care was top notch. I felt good with the choice I made. The building is very clean and staff very friendly.

If you are feeling overwhelmed and unsure of where to send your loved one, I will share ours. Background, I work in a hospital in Houston and was trying to find a nice facility closer to the Cypress area that had good therapy and was closer to family. I have also worked in a skilled nursing facility. Let me share our experience to help you decided. Pros: 1) Private room. I was honestly impressed with the room when I first walked in. Nice and spacious bathroom as well. The beds are on the small side. If your loved one is taller or larger than a 5'6 190 lb woman/man then be prepared for a tight fit. 2) COVID. If you are extremely worried about COVID then THIS IS YOUR PLACE. They are only allowing 2 visitors for each patient for the ENTIRE stay. You have to gown up, use a N-95 and if you leave the building then you are not allowed back in. Oh and you are only allowed to visit for a total of 2 hours every day between the hours of 8 am -- 12 pm and 3 pm - 7pm. (Restrictions I was not aware of before.) 3) Good Food. Cons: 1) Brief/Diaper changing. This facility does not use chuck pads! I have no idea how that is even possible. Every time a patient wets themselves and it goes through the brief then the staff has to change EVERYTHING. This might cut "costs", but it certainly uses up other resources like, time staff could be using to helping other patients and not to even mention laundry costs. If they are wearing a brace of any sort there is nothing to absorb the urine, it just expands down and soaks the brace they are unable to remove per surgeon orders. If your loved one can get up and go to the restroom without assistance, then this might also be your facility. 2) Staffing. When the charge nurse came in to deliver the papers for my grandmother to sign, I informed them that my grandmother needed a brief/diaper change as the sheets were soaked in urine. He told me that he would send in a CNA. I was able to read, explain, and sign all the paperwork without somebody arriving. I pushed the call light AGAIN, and waited. No assistance came. I walked outside and told the charge nurse I only needed one person, and I would help to change my grandmother. He found a 2 CNAs to come in and assist. I told them new bed sheets would be needed. They both got the bed sheets, placed them on the bed and left. I again waited a time to only realize that they were not going to help. I again went into the hallway and told the charge nurse that I needed help which he thankfully provided. 3. Front Desk Lady. My interaction upon leaving the facility: She was upset that I was not wearing the required items. (I was only wearing a surgical mask and was confused as I was not informed of the rules despite speaking with multiple staff members). I asked if she knew if the visitor policy would change. In a rude and condensing reply, she touted how the facilities policies and told me I should be grateful I am even allowed to visit my loved one. (Even the hospitals, which have more critical patient and more visitors, are not as restrictive as this location.) She acted this way and got on to me about following the rules as she herself wore her N-95 dangling from one ear. We were not 6 ft apart and there was no glass protector. If you are still reading this novel, then this is what I gathered from my brief one hour in this facility. They are trying to cut costs and the rules are not for protection, but to prevent family from seeing the poor service our loved ones are receiving.

Good experience during the week but weekend sucks!!! Has activities going(which is fun while rehab). Chris in pt and Linda as social worker is great. So is Jeremy aid who is great,almost forgot Deborah at night is real supportive. You need up lifting people around and not negative people.

My mother was placed here twice. The first time was after she broke her hip and leg in a fall and was not able to put any weight on her leg for 4 weeks. After that for the next two weeks she was only able to put 25% weight. They worked with her physically and listened when she complained of pain and any time she needed to be changed since she was not able to get herself to the bathroom. Any time we had a question, someone was always able to answer it in a timely manner. My mother ended up back here a second time after a hip replacement when the pins in her hip were not healing correctly. My mother was very happy to be back since she knew the place and how friendly the staff was. The only complaint which could be better is the accommodations for a vegetarian. Basically she had salad for lunch and dinner s. The vegetables are all over cooked but it is feeding a lot of people at one time. The staff was always friendly and willing to work with us. My family and mother would recommend North Houston Transitional to anyone who needs them.

My father came to NHTC in December 2020 after having a stroke and stayed about 2 months. The facility is very nice, clean and newer. The nursing staff does a good (not great) job. The admin does an okay job. I believe the facility is under-staffed and it's really hard to make a comparison as I have no frame of reference and we were only here during covid. When my father first arrived, his nutrition was not being administered correctly and he was not receiving enough liquid and was dehydrated as a result. I know there is a delicate balance with tube feedings, but feel it could have been handled better. The nurses really only know the basics of care so if your loved one is here you must follow-up regularly with their doctor(s)and the therapists. The therapists were supposed to call at least weekly, and I probably got a total of 6 calls and I had to initiate all but 2. The nurses do not call unless something happens. It was nice to be able to visit, which many facilities were not allowing at all. They did a good job of keeping visitors covered up to protect staff and patients. Lastly, they called me 2 days before my father's insurance was going to stop paying after they assured me I would have plenty of notice. That was stressful on top of everything else With my father's health. The social worker, Ms. Scott, did end up helping me get him transferred after I guess she realized I needed help, but then they transferred my father without any of his belongings. Nothing. That was also disappointing. While they do well in some areas, they are in need of improvement.

Don't let the ratings on medicare and other review sites fool you. I researched extensively before sending my loved one to this SNF. I was relieved and grateful when my loved one was accepted. However, that relief soon diminished. If your loved one requires multi levels of therapy and Rx distribution, I would NOT recommend this place. The facility is beautiful and clean, food was good, admissions and the receptionists are great, and the physical therapists are good. However, the disappointment in care far overshadows the good. The distribution of medication was wrong, every single day! My loved one raised concerns daily, as well did I (when I was able to actually speak to someone). They make no consideration to medication that need to be given exclusive of any other medication, or medication that needs to be taken within a certain amount of time before eating. They just bring all the a.m. meds at one time, and p.m. meds much earlier than they need to be taken. The occupational therapist really needs to evaluate her technique and line of work. She said really ugly things to my loved one and walked out on her several times. The OT stated everything was in my loved one's head! She had absolutely no compassion. A PT overheard the OT berating my loved one and stepped in to tell her to back off. The nurses were hit and miss. There were a couple that were really good and nice, but they were overshadowed by the nurses who didn't seem to care at all. I left several messages for the various department heads and rarely received a call back. When I had the care plan meeting, the respective departments were very defensive when I raised my concerns. I was supposed to receive 1-2 calls a week with updates on progress, but only received one call the entire time. Again, left messages, no return calls. When it was time for discharge because insurance was up, I received the initial phone call, but all phone calls and text messages by myself and the facility my loved one was being transferred to went unanswered for over 24 hours. May have been coincidental, but my loved one was discharged on the day insurance expired, whereas the transfer could have occurred 2 days prior. All therapy was done in room, so no access to the gym...was told this was COVID protocol, but it is really a disservice to patients when other facilities have COVID protocols in place too, and still utilize their gyms for therapy needs. Was told equipment would be taken to the room, which was never the case. I am sure the facility is good if your loved one is there for assisted living, but not so much for skilled nursing.

My husband and I wanted to thank North Houston Transitional and their amazing employees! We have never been inside due to covid. On Christmas afternoon, we went to visit my mom and we were confused on how to find her window, and didnt want to peek in through anyone elses accidentally! A nice young lady asked us if we needed assistance as she was presumably walking into work. She said she didnt work on the bottom floor and wasnt familiar with my mother, but that she would find out. She went inside and just a moment later directed us to her window, walking us there so there was no confusion. My husband asked her name and thinks it was Breanna (short, blond shoulder-length hair, wearing a blue Christmas shirt). My husband had a few questions and she explained everything very well and eased our minds a lot! We have called the front desk several times and wanted to also give a shout-out to Sheryl (spelling?) who has explained to us the drop off procedures for supplies, etc and been so kind to us! Although we hope moms stay isnt too long, were glad shes somewhere where people are kind and thoughtful and willing to help!

My 4 week stay after back surgery has been terrific. The nursing staff has been great and the food is terrific and the therapy is also been terrific particularly my physical therapist Chris and also my occupational therapist janell has been very supportive and understanding. I made great progress in my recovery from my recent back surgery. I would highly recommend this facility to anybody.

My uncle was a patient at North Houston Transitional Care, and he received amazing care and was very pleased with his stay. He spoke very highly of CNA Tara and her dedication to making his stay very comfortable. Im so thankful to all the staff for all the help and care they showed my grandfather while he was recovering. Were so thankful to have him home and healthy. Im not good with names, but I remember a lovely young lady named Karla on the weekends that was very helpful and resourceful. She was just a breathe of fresh air.

I want to express my sincere appreciation for the promptness and professional care given to my great aunt during her illness. Our experience at this facility was of the best we have ever encountered. The nurses here were very kind and helpful. Especially Mrs.Rhonda Johnson, she was very attentive and she made sure my aunt was comfortable during her stay here. She was so sweet and was genuinely concerned about her. My aunt wouldnt stop talking about how much she enjoyed being here. We are very thankful for everything nurse Johnson did to help my aunt recover. I most definitely will be recommending this facility to anyone who may need it in the future.

This facility was amazing especially during this terrible pandemic. I want to thank social services for getting my grandmother all the tools upon discharge . Activities Director Tashna was amazing at keeping me in contact with my grandma via FaceTime . And Nurse Tan never failed to return phone calls and keep my family updated . Overall great stay and nana is home and strong because of therapy . I definitely recommend.

This place is so nice and the staff is friendly and very helpful. My mom was there for PT and she loved it and didnt want to leave. Lisa helped me through the admin process and even texted me with information I would need for my moms insurance. Mom is home now and I thank them for making her stay so pleasant.

My Grandfather has been staying here for about two weeks now and he has been receiving exceptional care since his arrival! Everytime I visit he loves telling me how great Mrs. Rhonda treats him. She always makes sure he is taken care of and even goes the extra step to make him smile and laugh on a daily. I really appreciate what Mrs. Rhonda and this establishment for the outstanding service they are giving my family.

My experience with pass health care facilities have been very hard to endure when coming to trust my moms delicate heart condition with just strangers. ...MRS. Johnson ensured my mom was in good hands and her actions showed every bit of her qualifications and promises made as a nurse to not only help my mother but form a compatible relationship in a short amount of time I will forever thank MRS. Rhonda Johnson for putting her heart on frontline for each patient to ensure the trust in families to know they are indeed in good hands .

I had lower lumbar surgery in June. Ive met with my attorney and godson regarding my health. I had noticed I was having difficulty maneuvering around my house. I had fallen down four times in my townhouse. My godson advised me that I needed assistance with every day activities which included bathing, getting dressed, cooking, cleaning, walking. My godson and I decided that I needed rehab. My godson has researched and see many facilities in him and his wife have decided that North Houston Transitional Care was the best option. Upon arrival I was impressed with the facility from the outside and inside the facility is kept very clean. The personnel are very kind towards me. Cindy the receptionist is nice and very cordial upon entering. Since my stay Chris with physical therapy, Shawn and Tracy with occupational therapy has improved my physical being enormously. Melissa in nursing is very attentive towards me and other patients. The CNAs help with my daily needs in a timely manner. I really enjoy the food here as well.I highly recommend this place to my friends and family members for rehab

Thanks so much to nurse Rhonda for taking care of my mom while she was in your care. You spent so much time with my mom — even when she didnt need anything. Since I live six hours away, I couldnt be there. That was so hard on both of us, but you really made her feel at home during a difficult situation. Ms. Scott and Nurse Tran have been taking excellent care of my dad. Everyone at NHTC seems to go above and beyond to take care of him. One evening I stopped by to see him and the lady working at the front desk dropped everything to help me connect with him in a window visit. It wasnt easy but she stuck with it. I can not remember her name, but I owe her a huge thank you.Very happy he is there and receiving the care and attention he needs.Thank you very much!

The staff at North Houston Transitional Care have been absolutely wonderful working with my brother who is recovering from a brain bleed. I would especially like to thank Asha and Jannelle from physical and occupational therapy and Adam from the nursing staff. Yall have been wonderful and kept me informed on my brothers progress in his recovery, And working with him to make it safe for him to come home. I would definitely recommend this facility and staff to anyone that would require their services.

Came to North Houston on Saturday afternoon and my bed was uncomfortable but the staff made one phone to Mesha to get me a better bed and my bed was here in 4hours... That was awesome... I love it the staff is awesome they are very attentive to your needs...The therapy team is awesome as well.. I just want to get well and go home dan your the best therapist .anyone could ask for your kindness is such a blessing your have taught me I to keep faith I will walk again thank for your patience Also janielle is great I so lucky to have your help thank you

Have been here for a month getting antibiotics by IV for my brain infection. The people here are great -the nurses, CNAs, PT people, and housekeeping. My biggest complaint is the food which is very institutional and flavorless. but they do allow outside food to be brought in The place is clean and well maintained

I recommend they talk to these lazy CNA or one in particular I dont know her name , if she dont want me to get coffee she should get it for me so I dont have to go get it but she does not mention to do that I told her to tell her boss so then I can explain to her boss how she is but no one has come to talk to me yet I guess shes too scared to go to and when she does tell to seams like shes just a co plainer instead of help . He nurse Tran is great 5 star And many more also would make this place a five star please And highly , very recommendable. The director is good at fixing things if he knows if theres lack of communication here sometimes you can speak to a doctor when he says to do something things dont get done like they should, or he is not that responsible to talk to me or call me back . I dont care if they make me leave today they read this I would be happy. The therapeutic gym is great the people in therapy are great five star people. This place is shorthanded so theyre also lackscommunication. , name is Eugene in 128 , my number 8322629438 if you or any one wants to call me . I can not name all the good people Who really listen and care . Like I said this place is very nice very clean as you can see from the pictures they deserve a five star rating all across the board with because the people from the kitchen and some people they dont deserve one the kitchen is going downhill its not everybody fault in the kitchen. You should bring more coffee dispensers especially since everybody drinks a lot and this is free I even bought my own but the Keurig is broken so you dont have to complain about me. 3 1, 2 stars ⭐ no more . Yes I would recommend this place to people but I would warn them first of what they might deal with. For the beauty of the place nothing can beat this place

I have received exceptional treatment and care while at North Houston Transitional Care for over the past two and half months. If it werent for Chris, a physical therapist who worked with me each day on my physical recovery I would not be going home today. Chris encouraged me each and every day that I worked with him in performing physical exercises and tasks that built me back up and that restored my ability to stand and and walk again following my significant medical emergency and problem. want to thank and recognize Adam, a nurse who provided to me my morning medications each work day. Every day Adam bestowed on me his professionalism, kindness, and consideration, and made me feel at home while I have been a the facility.I want thank and recognize La Tanya, the wound care nurse for her treatment and care of my wounds during the time that I have been a patient at the facility. Her daily treatments of my wounds have been successful in healing my wounds.I want to thank and recognize Kyle, the Engineering Director at the facility for his assistance in helping me achieve reliable Internet access on my laptop computer in my room so that I could perform my computer-based work that utilizes the Internet.Finally, I want to recognize and thank all of the employees of North Houston Transitional Care who provided services, help, and care to me for their professionalism, politeness, and in making my stay at the facility a very pleasant experience.

My wife is currently under the care of the staff at North Houston Transitional Care. There are two nurses, in particular, that I must comment about. Not knowing if it is proper to give the names of them, will simply note them them as nurse T and nurse M. They are truly exemplary. They are quick to assure that my wife receives the necessary support. They even helped arrange the commencement of treatment with an oncologist and my wife. Nurses A and M have definitely eased my concern for my wife.

My Mother moved to NHTC after a 7 week hospitalization and two major surgeries. She was so weak that just getting into a wheelchair was momentous. We never expected her to walk again since she is 90 years old. However, Asha and Chris and others in Physical Therapy and Occupational Therapy have been able to rebuild Mothers capacity over the 100 days of Skilled Nursing with continuous care into their Assisted Living Center. Mother can now walk around the gym two times and even down the hall with PT at her side to help manage the oxygen tank. It is a testament to their skill and excellent motivational spirits. The group encouragement of seeing others working hard is also part of the success. NHTC has also successfully protected residents from Covid 19 during these very dangerous times. Keshawnda, the director of the Assisted Living Center has also gone above and beyond to help us with our Mothers care. We are so very thankful for good people who work so hard at NHTC. There is no perfect place for care given current funding and Covid stresses, but NHTC has brought Mother to a level of near full recovery that seemed unattainable on January 30 when she was admitted. God bless the people who serve our Mother! We deeply appreciate you!

I had heard of this place from a lady I go to church with and how well her mother was doing after her stay here at North Houston Traditional Care. I recommended this place to my resident who had been falling in his apartment and had a slew of other medical concerns. I was nervous when they kept him so long but upon speaking to the social worker named Sanedra Scott, she put me at ease. She explained to me that it would not be safe for him to return home just yet and why is wouldnt be safe. Made sense.... but why hadnt anyone else done this for him before? BECAUSE SHE ACTUALLY CARES!!!! She has a reassuring calmness to her and spoke to me directly, not over me or at me but TO me. She is the best social worker in that place and has that special something that cant be learned or bought. My resident is doing great and if I could give this 100 stars I would!!

This is truly an outstanding Care Center. From the Administrators, Staff, Nurses, Aides, Therapist the housekeepers, cooks and clerks they all have a great personality. Showed concern and care and did a great job on teaching me how to care for my mom. Its true you get what you give. I did not expect a miracle but my mom received the care she needed and is back walking and talking doing wonderful. Thank You All at North Houston Transitional Care Center. We appreciate you all. Five Stars and more.

The staff is very friendly and the physical therapists are fantastic!! The WiFi wouldnt accept new adds so that was frustrating as I needed to look up websites to pick doctors for my mother. I also needed to send and receive documents for her healthcare but couldnt do it due to no WiFi. Food was good and consistent with feeding times. The people there want to do their best. I think you do have to wait too long for some services but I think the floor staff is stretched pretty thin. We are having an overall good experience with hopes of just a few adjustments.
