We could not be more satisfied with the care my mom has received at NHTC. One of the big and pleasant surprises is how good the food is. Even visitors mentioned how delicious it looked and smelled. Special shootouts to Loney, in the dining room, and Deidre, at the front entrance, who provide extra special care.

Overall experience was good considering the changes that took place during my stay. Paying for transportation to and from doctor visits was pricey. Not offering out patient re-hab was very disappointing. The staff and doctors were very nice.Thank You, Treva Gibson

Shout out to Sonia for her quick attention to Mom's needs! She sweet and helpful and patient with Mom as well as with our family. The staff is all very caring and interested in our experience. Sonia stands out. She has taught me so much during our first experience in a skilled nursing facility. I need to add to the list of service providers who have been so kind and helpful with us. Mom has been here now since January 7 and she is improving everyday. Our goal of returning home is in sight thanks to Monica, Iffy, Q, and Cheynette all go above and beyond in care assistance. The Nurses Kevin, Danielle and Ashley, and Marta are committed and bring their own personality and best judgement in caring for us. The Nurse Practitioner inspires confidence when we consult with her. She has helped us understand the ordeal this situation can be. These people work very hard and always have a smile on their face. I had a great time doing bingo with Sabina and meeting new people. The food was always fantastic and appetizing. The dining room staff was also attentive with my needs.

My son was admitted to North Houston Transitional Care for rehab. He is special needs and nonverbal. The nurses and staff treated my son with dignity and respect and were always there for his needs. The food was excellent and the cleaning staff were attentive. Also his PT and OT therapist were excellent with him they knew just how far to push him to get his strength back. I would recommend this facility for your loved ones.

This is a wonderful facility with a great staff. Sabina the activities director does wonderful decorations for the holidays and keeps patients busy with activities and games. Bingo twice weekly, group meeting and church services. Many veterans residents and remembrance here.Veterans day bbq with hot dogs and hamburgers were a treat. Medical staff are very interested in your care and needs. Administration are very attentive to making sure procedures and medical needs are met and exceeded. I'm a current resident at NHTC who would love to share my experience here. I would like to start by saying that they have a great group of employees that treat me and other residents with the up most respect and compassion. They are very professional and dedicated to their work and ensure all of our needs are met. The attention they give to details have been very appreciated and notice all the hard work they put into their roles. The facility has been great in terms of establishing a community and engaging residents in daily activities. PT staff is great and patient to engage the residents in motivated safe exercises. It’s important to advocate for your family who is living there to stay on top of meds and doctor visits. Really enjoyed a lot of the staff who have helped take care of my grandpa. Chris, Alfred, Phun and Blake from PT. Brittany from speech therapy. And Sabina making sure we’re comfortable and creating a welcoming environment to engage in daily activities for even my grandma who visits my grandpa daily. Such an amazing facility. Their staff is very attentive and very detailed to each resident and helped me with all my needs. They have a great therapy department. Their speech, OT and PT therapists are phenomenal (Phong, PT, Jeanelle OTA and Brittany, SLP). Their nurses and cna are extremely kind and helpful when asked to do something. Would highly recommend this facility for anyone needing a skilled nursing facility.

NHTC is a great facility. While they aren't perfect you can tell that the nursing staff truly care for their residents. There have been some bumps along the way however they were address timely. My uncle has been in two other facilities and this is by far the best yet. You don't get the feeling that they're just their for a check(if you know what I mean). The housekeepers ensured the room and hallways were clean and no foul odors lingered. And the rehab team is exceptional! Shout out to Blake! This place is headed in the right direction, I would most definitely recommend this team!

This place is amazing. Not only is the facility spotless and the accommodations spacious and well-designed, the staff are genuinely interested in how residents are feeling. The activities are well-planned, the meals are tasty and generous, and the professionalism is evident at every turn. My mom has been there for two months now and every time we visit she mentions how sweet everyone is and how comfortable she is.

It’s been a good experience there so far for my mom in rehab & assisted living. Most of the staff there are attentive to my mom, the food looks decent, and they have a nice Thanksgiving dinner where family could attend with their loved one and the food was actually good. I love how they have social activities with incentives for the residents every week, bible study, popcorn Wednesdays & last but not least, I love how the facility DOES NOT smell of urine. I’m there weekly to check on my mom and majority of the time it’s clean. My mom was there about 5 years ago for rehab. and it was a good experience then and still clean. My only complaint and this seems to happen in every facility my mom has been in & that’s missing clothes that were washed even with her name written on them while in rehab. But overall it’s been good, I feel like they care for my mom and any concerns I have I can voice them and it’s always addressed & taken care of. Plus the staff is very kind and friendly!

Safe, comfortable. PT does excellent work with patients in rehabilitation. Very welcoming staff and lots of activity, entertainment. Beds are comfortable and rooms are private provide private bathroom and shower in each room. Have option to eat in room or in a spacious dining room. Meals are very good.

N.H.T.C. is a really nice place. The facility is clean and the staff is friendly and helpful with my needs. The food is good, somewhere between home cooked and fine dining, with a few alternative meal options. The gym has a decent selection of equipment and weights and knowledgeable therapists. The nursing staff is very attentive to my medical needs and concerns. The activities director makes sure to ask each resident if he/she wants to participate in the days activity. Bingo is really spectacular. House keeping makes sure each room is clean and manages to keep the facility clean and fresh. The administration also takes the time to know each patient. The rooms are nice and the staff is great. Trust me, I know because I chose to go long term care here. Come take a look for your loved ones if they need to recover at a place that feels like a piece of home.

This is my third time here I’m here for the Rehab which is outstanding. I have Vertigo & fall frequently with a head bump as my most common injury. You can choose your meals or eat their regularly menu, not my favorite but acceptable. Nursing staff is attentive & caring. Activities are varied & well organized. If I have another fall or injury, I will come back here to a known facility.

This place is crazy. They have 34 patients and at night they have 2 aids to help. That means 1 aid is responsible for 17 patients. We have got to do better. Can't get anyone with any answers to return your calls. Plus most of the day to day employees don't know anything. They are over worked and most likely under paid.

I have been a patient at NHTC for 2 1/2 months and I want to praise the nurses, CNA’s, physical therapy department, housekeeping, the kitchen, and Sabina (activities director). My nurse, Deidre, took excellent care of me every day. She had an outstanding knowledge of medications, how to care for different types of patients, and what to do in difficult situations. I am a retired critical care nurse and I was impressed with her every day. She also made me laugh which was great! Kevin was my weekend nurse. He was kind and professional, following up on items needed, and he made me smile. My CNA’s cared for me day and night. Monica, Iffy, Sonya, and Sim me et were always kind and attentive. During the day shifts I came to know Iffy and Monica better as they cared for my needs. We had tender moments and hilarious moments. It means a lot to be able to cry with your caregiver, when that is what you need. And then, if I was feeling down, they always told me I could do it! I needed intense physical therapy here - first as I was recovering from my heart attack and next as I received my second prosthetic and was needing to learn to stand on my two prosthetics. I am a bilateral amputee and Dan, my physical therapist, has been incredible working with me from day one. He is very innovative and he introduced me to unique ways of recognizing the muscles I needed to engage for standing on two prosthetics. He encouraged me and made me laugh, too! The other therapists and staff were good as well. I can’t list them all but Shawn, Sandra, Jan, Maria, and the new occupational therapist are just a few of the staff who occasionally worked with me and treated me well, helping me progress. Housekeeping always kept my room clean and the halls sparkled. For dietary - the food was almost always good. And when the staff learned that I did not like sausage, they sent me bacon every morning! Delicious! Sabina, the activities director, works her fingers to the bone as she constantly runs multiple programs that patients need. She walks from one patient room to another to encourage patients to attend the activities. I enjoyed Bingo on Tuesdays and Thursdays. Sabina made it fun to win! She has an amazing memory for patients’ names, room numbers, and special needs. And she can read Bingo cards upside down as she helps patients. I’m sure that I have forgotten to list someone. I apologize. I’m grateful to have been a patient here. Thank you for your care!

I have been in North Houston transitional care twice and I have had wonderful care. Two of my nurses that I have just loved. I’ve been Savannah and MONICA. I have had other nurses that have been wonderful to me. Also, I can’t say enough. Sabina has been wonderful. I cannot say enough about how great these nurses have been and staff and I wish they could all win an employee of the month because they’ve been so good to me. Yvonne’s been good to me. I appreciate the care that my father has received here. He is a Vietnam veteran and has quite a few challenges. Initially, he was reticent to spend more time away from home, but he has come to love the staff and work harder during his PT sessions! The warm and friendly feeling begins at the reception desk. Everyone I have met has been gracious and welcoming, but Shawana stands out to me. She has been gregarious and attentive and is never without a smile. Even juggling multiple parties of visitors, assisting staff and vendors, and fielding phone calls, she has always managed to find time to help me when I needed it. The nurses and aides have been wonderful also, but my dad loves his afternoon visits from Diedra. It has also eased my mind that he has a private room, with frequent checkins from staff. He should be discharged soon, and I think we are both going to be sad to say goodbye. The reasons for needing a facility like NHTC are almost never good, but they make recovery and rehab a better experience than you could hope for!!

Wow, shout out to Kevin and everyone in Therapy department Tina my evening med nurse Joseph the overnight nurse always brought me a cup of coffee at 5am and Mike and the west end always had a very good coffee starting at 530 to7am Mrs Jones really took care and she took of my problems that I had Hailey really good with her bingo games. Terrance went above and beyond after i stuck my wheelchair and helped me after three other people walked by me and he worked in the kitchen. facility was always clean. Brandi did a great job.

My grandmother and mother-in-law have both stayed at this facility for rehabilitation. The majority of the staff are friendly and provide expert care. They allow pets (with updated shot records provided to them on file) to visit their owners in their rooms inside the facility. The physical therapists are amazing! I like that their workout room is at the front with windows on the entire wall to check on your loved ones and to be transparent in their care. I highly recommend this facility! I encourage you to take a tour to other nearby nursing homes, you'll see that there is not one that can measure up to this facility. There is also a rating system on Medicare's website, just do a google search for "Medicare rating of nursing homes."

I was very pleased with my rehab experience at this rehab facility. The staff was caring, efficient, and professional. The physical therapy program was excellent, and my dream team’ consisted of Phuong and Kevin who were dedicated to my success, working hard to help me get stronger, while remaining encouraging and fun. The nurses and caregivers were all excellent and professional, everyone of them friendly, professional, and efficient I would like to specifically commend Amador Torres who went out of his way to help me feel safe and well cared for whenever I needed to be transferred on the hoyer lift. The hoyer lift was specifically difficult for me because I was hurt by a caretaker at another facility while on it. Torres was so experienced, patient, and kind sensing my fear, he really took his time and went out of his way to help me feel comfortable. I am thankful for this special effort to calm my fears and make it so much easier for me. The place is kept clean, no need or request I has went unaddressed and taken care of. The food is good for institutional type centers like hospitals, I found everyone from management to housekeeping pleasant, hard working, professional, and friendly. They work as a team, everyone dedicated to the patient’s wellbeing. I highly recommend NHTC, and will request to go back if I should ever need this type of health care facility again.

It saddens me to write such the review but after receiving a phone call regarding a physical therapy bill for my deceased father that past away in this facility 2 months ago is quite ridiculous considering the staff asked for my dad by name (SMH) and was looking at his chart which means NOTHING has been updated. I chose North Houston Transitional because of the private rooms. My dad was a Army Veteran and preferred his own space, this facility provided that, however everything else about this facility need a redo. There's a high turnover rate in staffing especially nurses therefore there's no continuity of care. On two occasions I had to inform nursing of my father's symptoms that they assumed was bronchitis but was actually aspiration pneumonia and had they been more aware of his history they would have known antibiotic therapy would have been the first treatment vs cough suppressants and breathing treatments. Actually this starts with the physician. The uneducated nurses just carried it out without doing a stop the line therefore I had my father transported and treated appropriately through the VA hospital. Many other instances that I questioned but unfortunately I was not able to care for my father while working a full time job so I entrusted North Houston Transitional but became more attentive of the care they were providing. The day before his transition, by this time he's now under the care of hospice (reason there's a unpaid physical therapy bill) I entered his room and I could immediately notice a difference in my father . I asked the nurse did they notice anything different and of course they didn't. Informed the nurse of the familiar symptoms which is why hospice was involved and after updating the nurse on standing orders from hospice for comfort care the nurse finally carried out those orders. Had I not been there my father would have died suffering, because he was having difficulty breathing prior to my arrival with no interventions initiated until I advised the nurse. My dad passed away 6am the next morning. I was there the entire time as he transitioned peacefully. North Houston Transitional was not the cause of my father's passing but they surely are lacking adequate and knowledgeable staff. Even the med techs are unaware of medication being given and why. Please use this review as a basis in choosing a facility for your loved one.

Tghis is a bad place to stay, I would only give it a one star because that is all it deserves, Room mate playing blues all night or eithet the tv is on loud, the food is the only thing nice, never enough staff her to answer the call button when adk for the nurses station never get an answer and this supppose to be the nurses station
