My mother has gone here several times for after hospital stay rehab. She is very picky but loves this place! It is nice and clean and the staff goes out of their way to accommodate you. Hoping to get her in for assisted living!

My mother has been in this facility for about two weeks or so. Her stay started out good but as time is increasing they've lost her shoes and when ask about them everyone says they're in storage. Where ever "storage" is I'm not sure why they can't be picked up from there and brought back to her so she can use them during therapy. Also, the Chief nurse administrator, Sandra Woodbury-Hulitt is not a real person because she's NEVER available on the phone and when you're at the facility she just left. There was an incident on 05/13/2022 where she didn't receive dinner and when it was brought to their attention they showed no remorse. Also, 05/16/2022 during my visit with my mother she needed to be changed. I pressed the button for her to alert the nursing staff that they were needed. My mom waited about 45 mins. A lady came in and told my mom she was busy doing fingersticks on the diabetics and that she had three more and would be right back and turn off the call light. I made my way toward the front desk and saw two young ladies in the hall way and asked who was taking care of her room because at that point my mother had been waiting over an hour to be changed. One lady said we just got here and we don't even know what's going on. The other asked me well did she press the call light. I told her yes the call light was pressed and was turned off because we were told that she was gonna be right back. The nurse wanted the aide's name and I didn't have one so she started giving me descriptions of what two of the women looked like and I agreed on one. She then said oh that lady is gone for the day and it's shift change. She looked at me with no compassion or reassurance that someone would come to change my mother. After waiting well over 90 minutes from the first time we were told we were gonna get help, a young man finally came to the rescue to change my mother. I'm not sure if it's a staffing issue, pay issue, overtime issue or staff not having integrity and pride in what they're doing but something needs to be done and my mom wants hers shoes back!!!!!!

My husband was there for a week during late January, 2022. The Physical Therapy and Occupational Therapy department is excellent. However, I cannot say the same about the nursing staff. They do not check on patients at regular times, the moral among the nursing staff is not acceptable, nothing was done for my husband without requesting the service. Not a caring nursing and aids staff!

My 80 year old mother was at this location in April 2021. She was there to rehab her broken hip. Her level of care was more extensive than this place could obviously handle. She couldn’t walk so we needed bed rails. This place only has small rails at the top of the bed by her head. Needless to say, she fell out of the bed twice while there. After being dismissed, she went to the hospital and was severely dehydrated and had a UTI, mind you we immediately took her after her dismissal. There was never a consultation with the family upon admission to see what level of care she needed. They would not allow us to stay overnight even after she fell out of the bed. We don’t know how long she was on the floor either time. This place is not for patients that need a little more assistance than your normal rehab. 1 or 2 of the nurses were a little knowledgeable but we will not be returning because they did not check on her every two hours like they claimed. We never got to meet the night nurses because of their strict visiting hours, we assume because of COVID. Visiting hours were from 9am-5pm. During the night is when she fell out of the bed twice. We were told she has a “right to fall” and no bedrails because they have no right to be restrained. So please be careful with your elderly loved ones if deciding to come to this place.

My 91 year old father was released from the hospital with the provision that he move into assisted living facilities. He had previously been in another facility (after hip replacement) and it was not a good experience for him or the family. Bridgemoor was the complete opposite. The facility is clean and bright, the food that was prepared for him was excellent and plentiful. There are plenty of opportunities to remain active, both mentally and physically. Dad was also made more comfortable by having his own furniture. All of this made for a nice experience, however, the real difference-maker is the staff. The director (Luke Fritz) and the nursing care could not have been more caring or accommodating. The facility is small enough that everybody receives the personal touch, for instance, when my dad expressed a desire for some strawberries, Luke drove to the grocery store and got dad fresh strawberries.I have no hesitation in recommending Bridgemoor to anybody.

I recently had a family member in this facility. It was her second stay in the facility. Although no one ever wants to go to a nursing home, this place isNOT your typical nursing home. Its rare to find a facility where patients actually enjoy returning, and she loves the staff here. The nursing and therapy staff were kind and attentive, and the food was delicious. We were able to visit her, and that made a world of difference. Shes back home doing great! Special thanks to the therapy and nursing staff, and to Robert and the director. I dont know the last ones name, but she said they all showed her special attention to make her stay a pleasant one

I was placed at Bridgemoor for a week for physical therapy rehab. after back surgery. I was inpatient which was basically a hospital stay. The physical therapy facility and therapists were top notch and successfully helped me increase my strength and mobility for a safe transition home. However, I do not have such praises for the nursing staff. Initially, I was not put on a pain med. schedule, so getting the nurses to get me pain meds when I requested it was like pulling teeth. Average time of receiving my pain meds was 45 min. to an hour. Attitudes of some of the nurses were outright unacceptable. After I spoke to management about my concerns, things seemed to turn around. They put my meds on a set schedule, so I did not have to ask for it and attitudes changed for the better. It took the Chief Nursing Officer patrolling the area for that to happen. Overall, my experience was average considering the goods and the bads. I would highly recommend the physical therapy portion of the facility, but I would not want to stay there again as inpatient. I give it a three star only because of the positive physical therapy experience, otherwise I would have given it a one or two star.

A n old friend of mine recently came here after a hip replacement, and I couldnt believe how nice this place was! She had a private room (I think theyre all priate but not sure), her own shower, and a window view. She raved about how great ther therapy and food was, and Ive seen some of her meals. Defenitely the nicest facility Ive seen, and doesnt have the nursing home smell. They got her home in less than 3 weeks!!1 Would recommend

Rapid Recovery is now BRIDGEMOOR TRANSITIONAL CARE and truly an unbelievable facility. This facility is clean and very well managed. We were pretty terrified of placing my Mom in any Skilled Nursing Facility because of the horrible things weve heard. BRIDGEMOOR is NOT one of those. This is premium care above and beyond what she received at the post acute medical rehab facility she entered after her surgery from a fall and broken femur. They were actually going to send her home when she lives alone and needed 24, 7 assistance before her advocate pleaded her case and we were blessed to end up here. The staff to patient ratio is excellent! They are pleasant, never grumpy or complaining, very joyful employees, reassuring, and asking if theres anything you need. They have a chef who often appears to take your meal order.And their goal is to help you heal quickly to get you back home. They are short term stay only, averaging 16 days per patient they tell me. This place is run like Southwest Airlines. The staff appear to really like their jobs and it shows. Additionally, patients can actually get some sleep at night because all the rooms are private with their own bathroom & the call buttons are silent in the patient rooms. No dinging all night long, and very quick to respond to a call button. I know it probably sounds like I know someone who works here or Im in the medical field because Im raving about this place, but I didnt know anyone before my Mom ended up here and Im not in the industry. I am just so grateful for Bridgemoor and the people who make it work. This is a model ALL skilled nursing facilities should follow. Please dont change a thing. Thank you so much! You are Excellent!!!!

brother-in-law stayed here after a recent hip surgery and they treated him great. Rooms were big, had the room to himself instead of havin to have a roommate. Food was great, and if you didnt want what they were serving that day, they had other options. Not used to seeing appetizing food in nursing homes aftre seeing the places my grandparents have stayed and what they ate., but this was a nice surprise

Stayed here a few months back after recommendations from folks inmy church group. Therapy and nursing were so caring and kind, and I loved the food here which Ive never heard another person in a nursing home say. Gym is brand new and I got therapy every day of the week, though it was much more during thr weekdays. I recommend Bridgemoor to everyone!

The therapy people are Excellent. The food tastes good.Carbs and sweets are large portions. That is good depending on how you feel about carbs and sweets. Bad for diabetics and dieters.Need more employees for direct patient attention.Several issues arose during stay that I had to address with several staff members. Therapy is the only reason they are getting 3 stars

This facility has been open approximately one year and is the nicest skilled nursing or rehab provider I have ever experienced. The design is more similar to a hotel than a medical facility and is very inviting. The rooms are large, clean, and well-appointed. They have a very low nurse-to-patient ratio which allows them to be very attentive to your needs. All of the staff, from professional to housekeeping, is friendly and will work to accommodate any reasonable request. I highly recommend this facility for rehab or short-term care.

This is a fantastic facility for rehab. The physical therapy is 7 days a week with an aggressive care plan. My 82 year old mom has experienced amazing results in her strength and stamina in two weeks post double surgeries. Every staff member has been professional and mindful in executing her care plan. Not only am I extremely pleased seeing my mom improving everyday it has also been rewarding to see the remarkable progress in other residents that I see in the hallways during my daily visits . The staff is friendly, facility is well kept and the food is great. Thank you Bridgemoor team for helping our family navigate through our moms road to recovery .

My grandmother has been in this facility for 2 weeks recovering from back to back surgeries. The staff has been such an asset in my grandmothers road to recovery. Not only has our family been impressed with the care she has recevied, but my grandmother has been impressed as well, and it is not easy to impress a former RN. The staff is friendly and polite, and always making sure the patients are well taken care of.The results and care have been so great in the rehab facility that she will be moving into the onsite assisted living portion of the building to continue her recovery.

I am staying at the Ridgemoor Transitional Care in San Antonio. And I am so surprised at the service the food and the quality of the Personnel that work here from rehab to kitchen help everybody has been so very much helpful the rehab staff is so professional executive chef I met twice this food is great please if you have a loved one I have to have rehab for short time Bridgewater Transitional Care is a place to stay for your recovery back to home
