For our family, Legend at Council Road was a scary and very disappointing experience. If we could go back in time, there’s no way we’d ever leave Dad in their care. Their 2 head nurses, at the time, and their sales/marketing manager did an assessment of Dad while he was in rehab for a broken hip. Previously that year, he had a stroke. So, an 88-year-old debilitated from a stroke and now a broken hip, Dad needed a lot of care. Legend promised us they could take care of him and his needs and further reassured us by telling us he wasn’t even at the top of the “Level of Care” scale for their facility. They do a great job of making their facility look clean and inviting. The staff are constantly walking around with air freshener, vacuuming, wiping things down, etc. That is, in the main areas. Dad’s room was a pigsty. His dirty clothing and used towels were constantly thrown around his apartment. His carpet was vacuumed when we asked to have it done. His toilet was filthy. His clothing was constantly stained, and he looked like a homeless person. It was tremendously sad. The worst part was, they kept him so sedated in an attempt to keep his needs to a bare minimum, that 90% of the time he was asleep or sleepy. We would come to visit him at least 3 times a week, and the vast majority of the time, we had to try and wake him up, or resign ourselves to sitting next to him while he slept. He fell out of his wheelchair or recliner multiple times while at Legend. But the last straw for us, and Dad, was when they over medicated him and we watched him slump over in his wheelchair and lay across his side table. (We could see all of this on the camera we had installed in his room) I had been trying to get Dad to answer his cell phone for at least 30 minutes. When I checked the camera to see if he was perhaps not in his room, I could see him sitting in his wheelchair next to his phone and I could hear through the camera that it was ringing. Dad continued to slump further and further until he was laying sideways on his end table. After multiple attempts to get someone from Legend to check on him, I decided to call 911. When the paramedics arrived, they called me and asked if I wanted him taken to the hospital. I said yes. The director texted me and said he was fine, and just resting. She reassured me that her staff had checked on him and he was doing well. I’m so glad we had Dad taken to the hospital. He was WAY over medicated, per the emergency room physicians. The ER doctors even went so far as to tell us that with his condition and current medications list (which we had on file from Legend) they were surprised he was alive. Most of the pills they were giving him at Legend were sleeping pills, sedatives, mood stabilizers, antidepressants, and pain pills. Dad is now at Willow Creek in Guthrie and they are FABULOUS! They care about the residents there and treat them like family. Dad is aware, he can talk again, he is calm, he doesn’t yell out or cry anymore. I feel terrible, absolutely disgusted that I let him stay at Legend until “the event” happened, that probably could have killed him. I’ll never have anything good to say about that place, other than 1 or 2 employees who treated Dad with just a bit of dignity and respect, everyone else there sees a resident with needs as a nuisance. And Valor Hospice is complicit in this, as they approved almost all of the medication that kept Dad in a zombielike state for his time there.

My mother and I were greeted warmly by all, and it was very obvious that they cared a great deal for their residents. Legends has a very good facility providing five care levels. If a person requires care at levels one, two, or possibly three, the facility, personnel and residents are top notch. Levels four or five are needed, there is not enough staff or proper training to provide adequate care wo family aid or full time hospice. Sign on the front door states the facility is locked during the evening and a visitor must push a button to request entry. While my mother was here the door would not shut properly and lock would not work. The Director told me it was being worked on. Often I walked in after hours without getting 'buzzed' in. After mom passed I continued to periodically check the front door. It was not in proper working order in December of 2023 but starting Feb 2024, the door was locking as intended. 07/22/23 Mom got a pain patch at 10:05. The med tech, F__, said that mom’s norco was showing as discontinued. Mom was in severe pain not having a pain pill since 20:00 on the 21st. I contacted ADN 10:38, left a message and she called the Med Tech. He sent a photo of the med chart. ADN verified with Boomer Pharmacy that Norco was active, instructing F__ to administer dosage. Mom got her pill at 10:50. The explanation how this happened was F__ computer didn’t sync with main computer . Never got an answer on a resolution. 07/24 I visited at 17:15, my sister’s notes indicated mom got pain pills at 10:20. Around 20:10 I checked with the Med Tech to learn last dosage but was unable to locate the tech. At 20:38 the Med Tech returns, mom gets her meds at 20:51. At 21:16 ADN calls and we talk. ADN explains the two hour window for the 8 Pm meds, from 19:00 to 21:00. Later the Med Tech explains Mom had gotten her pain pill at 14:00 prior to the 20:51 dosage. 07/27 05:30. Mom called asking to get out of her chair and into bed. I went and spoke to the aide. Aide said mom was too weak to stand and transfer and asked me to help. I explained mom was susceptible to skin tears & couldn't be lifted by the arms. Lifting by mom’s waistband we got mom into bed around 0600. We repeatedly told staff to not grab or lift her arms. Based on the number of tears, this information was not properly spread to the staff as they continued to lift mom by the arms. 07/29 Aide struck mom’s arm on bathroom door frame, tearing the skin in two places. Hospice rewrapped the inadequate bandaging done by the aide. Approx. 15:40 my sister visited, found mom soaking wet in urine. She got mom cleaned up. 07/30 My sister got a call from DN between 2200 and midnight, stating that mom had awakened the man in the room next door by banging on the wall. DN requested we stop mom from banging. My sister went and found that mom was without access to her pull cord & was using the remote control for the bed banging the wall to get the staff's attention. They noticed but contacted the DN whose response was to contact us, rather than actually directing someone to see what was happening in that room. 08/06 Hospice had put mom on a status of two-person assist. During the entire day the only staff that came in the room was to administer meds. I learned later Two Person Assist indicates a patient should be checked every two hours to see if they need food, water or to use the bathroom. Again, there was a mix up with the pain pills . Mom was on a six hour scheduled pain pill with a As Requested pill on a four hour window. Somehow the scheduled pill was changed to four hours. I was told Boomer Pharmacy made the mistake. Mom was over medicated and sleeping constantly. I contacted hospice and they contacted the DN to get it changed. I stayed to 01:30 to insure that mom’s pain medication was properly scheduled. 08/07 09:00 I discuss with DN the status of mom’s care, pain medications mixups and how nobody checking for 24 hours. Without us mom would have been overmedicated & ignored. DN notified aides to set watch alarms every two hours to check on mom. Until my mom passed the next day, staff were moving mom, changing her, checking her every two hours.

We were blessed to find Legend At Council Road for our loved one. The staff is simply incredible. From management, to the aides that work directly with the residents, to the sweet folks working in the kitchen, or the person that greets you at the front desk, you will not find more kind caring people. The nursing director is one of the most incredible people you will ever meet. Stephanie Petty has made it her life's work to love and care for the elderly. We were in a very stressful situation transitioning our loved one into assisted living. Stephanie walked beside us each step of the way. Every question or concern we had, regardless of whether it was late evenings or weekends, was met with prompt and caring attention. Betsy Rodriquez, LPN has been an amazing resource and is a great asset at the facility. She has been there to assist us countless times. We have had family members in other facilities, but they did not come close to the level of care that is given at Legend At Council Road. You would be hard pressed to find any place better to care for your loved one.

My father lived at Legend the last 6 months of his life. I had visited several other facilities before choosing Legend. The determining factor in choosing this one was their Nursing Director, Stephanie Petty. She cares for the residents in such a compassionate & caring way on a daily basis. Any concern I had or need of my fathers was immediately addressed. It is a big decision to leave a vulnerable parent in other peoples hands. But with Stephanie there I knew he was getting the best care & attention possible. Desserie & Narnita were also exceptional in their work & care of my father & the other residents. A facility is only as good as the care team in place. As long as Stephanie Petty is there you can confidently know your loved one has the best.

My parents moved into Legend at Council Road in January, 2021. Despite staff turnover, I believe the care they have received has been exceptional. The menu selections need improvement. But, I realize they have had turnover, and they are making positive changes. I’m looking forward to the finished product. Through the changes, the one thing I knew my parents could count on was Health Care Director, Stephanie Petty. She has been so kind and caring with my parents. And I can tell her kindness and caring are genuine. Sadly, my father recently passed away. Despite her busy schedule, Stephanie made sure she took the time to attend Dad’s funeral. That’s just who she is. I know Mom can ask anything of her, and she will do all within her power to accommodate her. I am pleased with the overall care my parents have received. And I would not hesitate to recommend Legend to others looking for a quality Assisted Living experience.

We moved my dad in to Legend at Council Road about 4mths ago. The staff is great. Stephanie, the heard nurse, is wonderful. She loves the residents and their families. Stays in close communication with us even outside of "office" hours. Tommie the Director is great! She has a true passion for making sure the residents and families are well informed. They have gone above and beyond to help us know what is best for our dad. The reason for 4 stars and not 5. I will be honest, nothing is perfect! It doesn't matter where you put your loved one. I do believe he was getting the care he needed. We have to remember we moved him into a facility because we can no longer take care of him. At home nothing would be perfect either. We are thankful for the staff. Kris and Harmony work with the residents in memory care. They are so kind and helpful. Very sweet to my dad.

My mom's mind was sharp, so I also wanted a place that had friendly staff and residents. I could not have been happier with Legend at Council Road. I moved my mother to Legend At Council Road on June 12th 2022. This has been one of the toughest decisions our family has ever faced. My first visit everyone was extremely friendly and greeted me as if I were family. I took a tour of the facility. A few days later I took other family members to take a tour and we agreed this would be a wonderful home. Our first day we met Director of Nursing Stephanie Petty and I knew right then we had made the right decision. I have never met a person that felt so genuine. She made all my fears and doubts disappear. She is so gentle and caring not just with my mom but with the family. We have experienced a few bumps in the road along the way and I understand that no facility is going to be perfect. Every concern I have Stephanie has done her absolute best to work through the issues with me. Legend went through many changes with personnel right after we moved in and at one point I was looking to move my mother. I went in and talked with Stephanie in private and gave her all my complaints. She asked me to give her sometime to work on them and I have to say there has been improvements. We would have moved if it wasn't for Stephanie and I feel so blessed that I have someone like her caring for my mother!!!

The staff was very helpful with our family to make the difficult transition from a home of almost four decades to the Legend At Council Road. Obviously, this was a tough move, made much easier by the caring employees. The community is an active one and all were extremely helpful in getting a new resident comfortable in their new surroundings.
