Since I have received the State investigation back I would like to thank the very nice gal that listened to the nightmare story (12 days of hell) that my dad experienced and she knows much more than you guys at (Willow Haven Staff and nursing) got cited for and that the other things were uncitable events I guess. I have not commented to any of you or have talked to any of you since my dad passed in early November but now is the time to let people know not to take there loved ones to Willow Haven. I dont understand or know how all of you that are involved (there's 6 of you) can sleep at night...you know I know what went on and has gone on in the past and most recent believe it or not crazy and the [language removed] stuff you are all guilty of. My dad was in late stages of alzheimers dementia you guys!!! I trusted your decisions and thought we were like family. He was freaking there for almost a year!!! I tried so hard to make your jobs easier by doing the little things that you wouldnt have to worry about I was there every day. Im not about naming situations or people for humility only facts I have more respect unlike your decision you made that day!!! I would like to give a big shout out to the nursing and management staff on how you lied to me on the phone (my girlfriends phone mind you who worked there at the time but no longer does because of the incompetent and neglect that was going on) and you told me "YOUR DAD WANTS TO KILL HIS SELF AND HAS A PLAN." They wanted to send him to psych and I approved. If I only I new the truth what he actually stated at the time which was "HE WANTED TO DIE AND HAD NO PLAN" is what your statement in the report said. He should still be here.You are all complicit for the decision made that day (6 of you) for sending him out. There were other alternatives. You were not set up for a dementia resident to his aggressive extreme advanced nature or experienced enough to deal with that so you sent him to psych u Unexceptable!!! Theres so much to this nightmare everyone..and I do not want anyone else to have to experience my dads nightmare OR OTHER FAMILIES NIGHTMARES! The decisions being made by these 6 and or should I say not being made correctly (GOOD LUCK GETTING THEM TO ANSWERING THERE EXTENSIONS) are unexceptable. One last thing to all those that was complicit that day... My dad walked up that hallway himself with his walker got into a company van road in the front seat got dropped off at facility #1 because of having alzheimer dementia you guys thought he was psycho...NO HE HAS ALZHEIMERS DEMENTIA!!! He was fine for 1 1/2 there then the severity of his Alzheimer dementia kicked in there and there was an action taken and a decision made to send him to facility #2 he arrived there fell hit his head because of previous treatment at #1 and the neglect to see after him at #2 he was then transported to facility #3 where he would ultimately be put on hospice and transported back to Willow Haven on a cot on hospice. Pretty [language removed] walk out where he was living and come back on a cot 12 days later!!!! You can only lie, cheat and steal in life for so long until you get caught you all know what Im talking about and who Im talking about. Schooling, Paperwork or just something as simple as taking things that are not yours. I talked to headquarters/corporate the head of something she stated IM PERFECTLY HAPPY WITH MY STAFF AND SUPPORT EVERYTHING THEY DO...I have one thing to say to her on that if you are happy with what you have encountered over the past couple months at Willow Haven shame on you!!! GOD KNOWS!!!! Karmas a [language removed]!!! Think about my dad every once in a while will ya he was a really good person he just had a disease that you all didnt know how to deal with! Good luck Put a "D" on evil

My boyfriend is the ADON. We have placed his grandma there twice now and almost did a third time. The nursing staff is amazing and always give the up most attention to their residents. Each time "grandma" is there the staff treat her like family and make sure her every need is met.. big or small. I would recommend Willow Haven to anyone.. even my worst enemy... at the end of the day everyone deserves great care and I know Willow Haven will provide it.

I have worked at Willow Haven now for about a year and a half. I love my job and my residents. The staff go above and beyond to make sure the residents are well taken care of. The building has done a 180 since the new management team took over. Corporate is working on updating the building and it's starting to look great.

I used to deliver the medication to all the facilities here in Zanesville and by far Willowhaven was my favorite nursing home to stop at. So many great things I can say about Willowhaven from the cleanliness of the facility to the friendliness of the staff, I was always greeted with a smile, and even though at times the staff was super busy, they would always go out of their way to make my Job easier. I would definitely recommend this facility over all here in town.

My mom stayed at the facility a while back for her second stay at the facility. The staff were all very welcoming of my mom and treated her like she was family. The therapy department did an amazing job at rehabing her to the best of their ability. The nursing department was nothing short of amazing. I was notified of any issues with her care as she will as change of conditions immediately. I would truly recommend this facility to all my friends and family.

We really enjoyed our Trunk or Treat 2021. We had many new faces this year, that attended this event. All of the staff and residents really enjoyed seeing all of the kids in their costumes.

The staff at Willow Haven are the best. Our sister is a resisent there and she couldn't be in a more caring environment. She has special needs that the staff are more than willing to handle. She is back home in Zanesville and very very happy with her living arrangements. The staff keeps us well informed on everything and we love the way they get the residents involved with things and make them feel good about themselves. Being patient in the healthcare profession takes a very special, caring and loving staff and Willow Haven has it all. Amy makes you feel comfortable knowing your loved one is taken care of and the nursing staff are great. They call updating us on everything. My mom is in one of the most expensive nursing facilities in Pennsylvania and she doesn't get the care and attention that Joan gets at Willow Haven. It may not be the fanciest but it is the best when it comes to care. Thanks for everything you all do to make her happy and comfortable. We feel blessed to have found you all. PS Keep the pet visits coming and Joan will be the happiest kid on the block.
