April 2024
My Father was a resident at the Northwood facility and I wish I could / would have changed places for his final days. So much wrong with the Northwood facility. Other than a few caring aides, they forgot to get him for breakfast at times in his first weeks there. He fell requiring 15 stitches within his 1st week. They rarely cleaned his room. After meals, he was taken back to his room, left there alone, never checking on him until I would arrive. No interaction, no games, no trips no nothing. He vegetated in his room. They never offered or took him to places as they claim they do. (Ball game, zoo, museum, stores etc) Within 2 weeks, his bedding & his clothing were lost. Never found or returned. They didn't use the provided Tide soap, fabric softener and they washed his belongings with other resident's items. He got a bad rash. Frozen meals 9/10 times for dinner. A slice of toast and an egg with a piece of fake bacon was typical not catering to the residents. Fresh fruit and vegetables was rare. 1 prepackaged cookie for dessert and as an evening snack. Sometimes ice cream was donated by a family member for the residents. After having dental work, I supplied soft, microwavable foods for him as they didn't offer or supply. I had made because the aides forgot. Constantly I had to review his medications as they were being given incorrectly. Hospice also reviewed and still had problems. I hired his personal aide he had prior to becoming a resident for the duration who cleaned, showered him, took him to his meals or even for time outside. I took the afternoon/evening shift and usually most of the weekend. The kitchen was locked after dinner until they opened for breakfast at 8am. He wanted coffee and was denied at 6:30am. He was never taken outside to enjoy the sunlight unless I or his hired aide did. No beautician/barber, devotional, activities on site or offered. I had his barber come to the facility to trim his hair, his aide helped him shave. A Nurse/Doctor never came to see him because according to them, he was under Hospice care. 2 aides from the facility to care for a full house weekly during the day and 1 aide for the evenings. There were approximately 16 residents in his building who required showers, aide, room cleaning and it was a pick & choose who would receive. 1 day and 1 evening aide on the weekends. If after a meal the resident went to their room, they usually were there until the next meal. Otherwise, they were pushed into a common area until mealtime. The single, evening aide had to help all residents to bed by themselves and do evening medications if required. I literally witnessed residents sleeping in a wheelchair, a chair or on the couch until taken to bed. Bedtime routines began when the 7pm aide arrived and usually was completed by 11pm. I witness an evening aide's boyfriend in the facility who had showered there, was watching tv, eating what was served for dinner etc numerous times. I witnessed an aide leave their boyfriend in the facility alone while they were outside enjoying a cigarette. I argued with an evening aide who lied about giving my Father his evening medication that I went back to the facility at 9:30pm and verified he lied and demanded a solution with the manager. They switched houses with the aides and the lying aide stayed employed. I gave direction of visitors allowed and it was not followed by the aides that the police had to get involved at one point and the associated paperwork was taken out of the facility by an aide and was incorrectly written out before returning a few days later. I gave care instructions expecting them to be followed and repeatedly they were disregarded. I have called the owner numerous times because of additional issues with observing alcohol, drugs, my request for using of a listening device or an aide who took advantage of a situation with us and he has NEVER returned my calls. I helped almost every evening with serving meals, talking with the residents. I was asked to donate my father's slightly used motorized rocking chair for the common area only to find out they threw a $1,000 chair away within days of his passing away. I loaned personal items to a few aides and the facility only to beg for return of my items. I was lied about by an aide to management and I originally wanted to donate my time to just help because of the short staffing. I've never returned and I have told people to literally stay away from Northwood's facility. They are aware that an aide has removed personal property, medication and information from their residents. They didn't want returned and the aide retained their job. Major HIPPA violations. This is just a brief summary. The Toledo facility may be different. But this is my warning to be cautious and vigilant if placing your loved one at the Northwood facility.

The friendly staff always greet me upon arrival. I am extremely impressed with the kind, caring and very compassionate staff at The Parkcliffe. The caregivers and nurses are engaged with the residents. The nurses and management team are always communicating with my clinicians. This allows us to provide the best personalized care for each unique resident.

Rents are increasing effective January 2024. Houses are closing or have been closed since late Summer 2023, and residents being moved to two in a room. No cleaning or housekeeping staff. Filthy windows, carpets, and floors. Mold odor due to poor cleaning or lack of cleaning. Lunch is Steakumms sandwich or hot dog. In general fresh fruit is rare, fresh vegetables are rare, maybe half a banana once a week. Plenty of money thrown away on things like personalized blue duffel bags for employees and for rocks and silver rings around the trees in Summer 2023, my father in law’s rent provided that money. Zero explanation of where money is being spent.

My mom was a resident. We went to Parkcliffe because it was more of a home feel than other places and the web site said it was family oriented. Well in house 5 there were a few staff members who were really caring, however there are many who didn't seem to care. Felt we couldn't complain much due to fear of retaliation against Mom. When she was moved to House 7 there was a definite lack of interaction between staff and residents. Her wedding and engagement rings were taken (she could not have gotten them off herself due to arthritic knuckles). Clothes, cane, glasses and wallet went missing. Nothing is done about missing items. Employees do not take responsibility for anything, always blaming other shifts. Admin didn't seem to care when we did talk to them. No one from admin ever calls to ask how things are going. They get your money and leave you hanging. No I would not recommend Parkcliffe

I transferred my father as soon as I could from this substandard facility. The staff were not knowledgable or friendly. The facility was shoddy with a poor HVAC system and no designated housekeeping crew. The food served was mostly of a frozen variety. There were few activities or engagement and no individualized care.

Our Mom spent the last months of her life at Parkcliffe and I can't think of any other place I would have wanted her to be. The loving care she received was excellent and many times above and beyond. The staff is incredible and so many of them have been working at this facility for many, many years. It takes special people to work at these Communities and to the person, in my opinion, they are all very special. The communication was above and beyond and they kept us well informed. It is with the deepest gratitude I thank them for being so kind, loving and caring. Thank you to the entire staff at building 7. You are the best!!! Bill R

My father spent the last 18 months of his life at Parkcliffe. I cannnot express in words how wonderful the entire staff was throughout. Everyone from Darryl, who helped us move dad in to Q, his ever-present care giver to everyone in between. I owe eternal gratitude for their kindness and care. The world is a better place because of the people at Parkcliffe.

most caring aides nurses and administrators of all the assisted living places in Toledo. My Aunt worked at the one off of Airport Highway and was so impressed by it, she said that would be where she wants to go if she ever gets dementia. My Dad is there now and they are treating him very well. His health has improved. They are clean, caring, and positive and not at all impatient or intolerant. I thank God for this place.

Absolutely the best & most caring facility of the 11 places we checked after having a difficult (i.e., unpleasant) financial experience with Lutheran Village! Its not rocket science, and these folks know how to screen & honor their employees -- thank God we found such a caring place for Moms final two years! Thank you, Wayne Bucher for your caring mission!!!
