I'm reviewing based on having my mother there at the end of her life. While she wasn't there long, it was a positive experience. I've worked in long-term care myself, and my expectations were admittedly quite low going into this, but it was absolutely medically necessary for my mom's safety and health. I expected that some clothes and personal effects would be lost. I don't think there's a nursing home anywhere that is able to keep track of 100% of residents' items. While that could be bothersome at times, I'd estimate that 85%-90% of her items were accounted for, which is pretty good in my opinion. I found that bringing up specific missing items to various staff people helped as items were often able to be located. Although the building labels and inventories items, I labeled everything myself as well with a permanent laundry marker. Some of this is also just going to happen when working with people with Dementia or Alzheimer's, things are misplaced. Staff really bonded with my mother while she was there, and she quickly developed many favorites. She did not fully understand her condition or that it was a nursing home, so she would tell me, "So many of my new friends here are nurses!" At the end of her time living independently, many of her friends had pulled away from her due to her Dementia, and she had been feeling very lonely and rejected, so having "friends" there was so good for her. She also made friends with some of the other residents there and enjoyed eating meals with others. I loved hearing her talk about how much she liked various activities or the food. She was also not one to sugar coat things and could be overly critical at times, so I know if she was happy with it, they must really be doing something special. Prior to going to Foundation Park, she was not getting adequate nutrition. Food was placed in front of her, but due to her condition, she was unable to feed herself without help and prompting. I was glad they had a dining room designed to support her and others with feeding needs. On many occasions I would arrive during mealtime and witness staff kindly and respectfully helping her to feed herself to the greatest extent possible and helping her where she fell short. I got to know many nurses, aides, and other providers there during her time there, and found the vast majority of them to be kind and helpful. I appreciated physical therapy and occupational therapy working with her on individualized goals and seeing her make progress. I also found the MDS nurse, Social Work, nursing supervisors, and business staff to all be very responsive and helpful. At one point she and her roommate were not a good fit, and staff was able to adjust her room right away to accommodate both of their different needs. Sadly, her last week there she was on Hospice; however, staff were so supportive and caring for us during that time. They really made a difficult time more bearable, and I was able to rest easier knowing that even if I had to step out, they were taking great care of her. If I had it to do over again, I'd find a way to get my mother to Foundation Park sooner.

My husband is currently at Foundation Park it is in my opinion a hidden gem. He had been in another home in the Cleveland area, it was beautiful on the outside but didn't have enough staff and/ or trained staff. He was in trouble at least once a month finally got kicked out. We didn't want to come to Foundation Park but basically no choice. It is wonderful. The staff is always kind and know how to handle issues with patience & kindness. There is enough staff on site not just on paper I come about 3 times a week it is an hour & a half one way for me but it is worth it he has had no issues since being here. LOVE you Foundation Park.

The staff are caring and attentive. They know how to handle people with significant memory deficiencies. The rooms are larger than any other facility I toured. The food is better than other places I have had experience at. The layout is conducive for those who want to move around so that they don't feel closed in a limited space (the halls surround a courtyard so the clients can move around and around). It is clean and the laundry is well organized. The carpeting could use replacement, but overall this is an excellent facility

Oh HELL no! Never again. Ghetto fabulous at it's best! My mom had no privacy in this place, men barging up in her room all times of night, my sister and I had to spend nights because she felt unsafe and uncomfortable. To avoid catching a case, we pulled her outta there ASAP!!! Mom had no tv, the place was dirty and you could clearly tell they will hire ANYBODY! You might look up see Ned the whino chillin at the front office. NO! Don't do it! DO NOT!

There is not a facility around with a more caring staff. They have taken care of my father for almost 7 years and no other place he's been has been able to deal with him. They are patient and kind. He can be a handful and drives my mother crazy. The staff at Foundation Park just smile and take care of him gentle and kind, no matter how much he screams at them or has a fit. I've never witnessed a rude person or inappropriate care over the years. Thank you to the staff for taking such good care of my father over the years and hopefully for many more years to come.

I have never seen harder working people. They do truly care about their residents. As with any facility caring for people there can always be "moments". This particular facility is for Alzheimer's. The staff is so understanding and patient with the residents. I have witnessed the office staff as going the distance as well. I do recommend this place.
