The nurses and cna’s here are awesome, efficient and caring. In particular, Jazmin, Tonia, Jasmin, Jocelyn, DeBorah, Sharon, Ayam and Eboni. They run their a$$es off making sure everyone is well taken care of, even with the abuse and derogatory remarks they receive every day. They really deserve high praises and to be more appreciated for the commitment and long hours they spend here. Kevin Bosworth

I stayed at the Grand for rehab. Tonya brought me on board and executed the necessary steps to help me walk on my own again . Thanks to the PT staff, Thelma and Yolanda, they helped strengthen my legs so I was able to walk with a walker then on my own and I was home before I knew it. The staff was very kind and attentive and I would recommend it

This place deserves 0 stars My wife was an inmate (patient is not appropriate) Management is totally neglectful of their inmates needs Actually the upper management here disappears faster then the invisible man in the movies if you have questions or needs or concerns Place is filthy Staff for the most part is indifferent and lazy, which I’m sure makes upper management here proud since it reflects on their patient care philosophy If you actually hare the person needing care this is the place for you If you care about them stay far far away from this hellhole

I loved the visit i had at the grand, the unit nurse manager tonia on unit 200 is fantastic and the activities aides, Logen, Karlie, kellie, jackie and Olga are all fantastic, the transport and service they provide is very helpful and readily available, the nursing staff on unit 200 are fantastic especially jasmine, the nursing aides such as rosemary, patritia, melany and daborah are always there when you need them. All in all it was a great visit.

December 2025
I wanted to articulate some elements of my stay at The Grand rehabilitation facility. Nearly every aspect of my experience here exceeded all of my expectations. No one was more instrumental in my positive experience here than Tonia, the unit manager. From advocating for me regarding meals and laundry, to putting in place the mechanisms in place to facilitate the quick, seamless details of my discharge. I would gladly recommend this facility to anyone in need of care.

We have tried multiple other places to accommodate my grandmother, and she has not been pleased with most of them, she is very picky. The grand rehab rehabilitation and nursing center has finally made her happy, she enjoys the staff and accommodation is very much. Highly recommended.

May 2025
I have been here for three weeks and I would say that this facility has its good and bad let me start by saying that my experience here hasn’t been bad because I can basically take care of myself I bath eat go to PT OT and memory therapy on my own also Ruby from activities was nice sweet kind and thoughtful and helped me stay active and not be in my room Alison was my favorite nurse she was sweet kind and dedicated the therapist department was great

This place does NOT deserve the poor Google rating it has! Everyone here is so NICE! My room is cleaned spotless and sanitary daily, the kitchens are clean, the dining room is clean, and everywhere else is too! MUCH cleaner than St. Luke's Hospital I can tell you! Ok so the food kinda sucks, but the food sucks at EVERY institutionalized facility there is, from school lunches to prison food. We are treated like living, breathing human beings here! The staff is kind and helpful! Their PT and OT departments are TOP NOTCH!!! I have advanced more here in 2 WEEKS than I did in a MONTH at Taconic of Beacon, and NOBODY is lying to me or gaslighting me! 5 stars to The Grand Rehabilitation and Burning at Pawling, NY!!!!

Everyone was super nice and took good care of my sister. I felt comfortable with her being there. Thank you
April 2023
My mother is a resident at The Grand @ Pawling and the service she gets is impeccable. From Dillon house keeping cleaning her room, Gigi and Deborah changing her bed and getting her up and taking care of her, Allison LPN and Corey giving her medication, Christina and activities keeping her spirits up, Divya in physical therapy pushing her to make strides, Tish from SS helping us make tough decisions with compassion, and Brandon the administrator came in and introduced herself reassuring us that she is in the best of hands. I could not agree more with her. The service is non stop and met with kindness. This is my 2nd relative to have been there and I'm very impressed with the comfort and care we have received. Its a grand place to be. They are all definitely HEROES

Absolutely pleased with the hands on approach with the Activity Department. They are all very entertaining and the Attention given to each resident is outstanding.

I have been here for six years and overall I have a great experience. I am leaving next week as i am finally able to return to the community. I took great pleasure in serving as Vice President of Resident Council. A Grand shout out to Christina Meehan in Activities, Sharon the CNA on unit 200, Alexis LPN, Allison LPN, Julie LPN, Shannon LPN, Whitney LPN and the entire kitchen staff
June 2022
Good morning, My name is Linda LaSand and my older Brother is John LaSand who resided at your Center for the past few years. It's been hard on our Family as we watched his health decline. Especially for me, as I live in California. I was able to come out with 2 of my Grandsons to visit in the Summer of 2018. Your Staff was pleasantly accomodating, making our visits so much easier as we spent what I knew to be the last time I would see John face to face! It was excruciatingly painful when we headed home! John's daughter, Lorraine told me about this wonderful nurse Christina Meehan who was willing to help us FaceTime with him. I was so excited to still be able to still SEE my Brother's precious face! Christina has done this for the past couple of years with kindness and love in her heart. No matter how busy she is, she always makes time to help. During this time, we unexpectedly lost 2 of John's Adult Children! We were really going through it! The day sadly approached when my dear sweet brother was transitioning. My niece called me from your facility while I was out shopping. When she shared the news with me, I began shaking and almost passed out. Lorraine told me that Christina was going to put me on the phone with John. I was alone in a store full of strangers, drenched in tears as Christina gently explained the process and what to expect. She was concerned because her shift was ending and she would be off the next day which meant she wouldn't be there when he passed. After I spoke with my Brother, she spoke with me for a few minutes longer to help calm me before we hung up. He passed just a little while later. What can I say about my sweet dear Angel, Christina Meehan (Director of Activities)? The day you hired her was one I will ALWAYS be grateful for! She is in the right place for ALL the right reasons. I will never forget Your Staff and Christina for treating my Brother and our Family with Love, Respect and Compassion! THANK YOU! MAY GOD'S BLESSINGS FOLLOW YOU AND YOURS ALWAYS, Linda S. LaSand

Two years ago my uncle John Coraggio fell twice had to go the hospital. he needed rehab they gave a list of rehab places. Also I needed to put him into nursing. I saw the grand of pawling, I said to my self that the place because it was easy for me go visit him because I work at the grand. I know he would be taken care of. The place has alot great workers, the nurses, cnas, activities housekeepers, maintains the kitchen crew. The place was clean and the food was great. I made a great decision. The Grand of pawling is a great place to get better .I would not look any further than The grand of pawling.
February 2022
From the outside, this facility looks a little tired, but inside, the staff is anything but. The staff is attentive, kind, considerate, and accommodating. They took very good care of my mother after a two week stay at the hospital with Covid and brought her back from being very sick, unable to eat, to sitting up, eating pureed food, and learning to be mobile again with her walker. Each time I visited, the staff was more than willing to run down and get food I requested, or come in to assist with whatever my mother needed to make her more comfortable. The weekends saw a reduction in staff, as most places are experiencing, and getting a hold of someone was a bit trying, but don’t let that influence you regarding choosing this facility for your loved one. Thank you to the entire staff for the selfless work you do each and every day.
February 2022
My Mother-in-law is a resident here. I just want to say Christina Meehan goes above & beyond for everyone there. She is the kindest to everyone. She’s making her days there a little brighter.
January 2022
Our experience since working with staff at the Grand to place Miss Violet in for rehab has been phenomenal. Clearly, the entire organization is committed not only to make the placement process as easy as possible, but after being placed the attention given to make the stay as pleasant and rewarding for Miss Violet was superb. Special thanks goes out to Christina Meehan, Josephine in placement, all the staff on the floor, the housekeeping and food service staff, and all the doctors and nurses who attend to Miss Violet’s needs every day. Clearly, visitation during these times of COVID are hard, but it is evident that every precaution necessary is being taken to protect the residents, and staff. We can’t say thank you enough. Sincerely, Tom and Alice Pilch

The Social Worker Olivia McDougall went above and beyond to help me move my mother from this facility to one here in SC. Olivia helped me through the whole process and has even followed up since then. She definitely is a very valuable employee and very dedicated. Thank you so much. I am so happy to have Mom closer to me.

The Grand At Pawling delivers…MIRACLES! My 89-year-old Aunt Fran arrived at The Grand in September 2020 for Rehabilitation services. She has been there since and has never been happier! She is delighted with all the people who surround her and care for her. She never fails to tell me, “These are good people here. They take very good care of me.” Those words are a balm to my soul and would be for anyone who is aching for the best of care for their loved one. I have consistently experienced high levels of professionalism and responsiveness from all members of Fran’s Care Team. Be it Case Management, Nursing, Dietary or Therapy, they are well versed in her condition and needs. She has made tremendous strides under their care at The Grand. I know it because I SEE it! I SEE it, because the Activities Team, under the direction of Christina Meehan, faithfully connects me with Fran on FaceTime 3 times a week. And they are ever-willing to accommodate any change in my schedule to be sure we connect! As Fran’s Care Team reminds me, Fran loves her bed. They are correct. Fran did not move out of her bed for 3 months before being hospitalized last September. But, Fran also loves people and activities. At The Grand, Fran has the best of the both worlds she loves. The Activities Team brings the activities to her. They consistently go above and beyond! They even brought her Birthday party to her room including a staff member who could fiddle the Happy Birthday tune! (And yes, they did paint her nails purple because she wanted to look like her neighbor!) It certainly wasn’t my original intent to have Fran become a permanent resident at The Grand, as she was being cared for by caregivers in her apartment. As much as Fran loved her caregivers, something was missing. It seemed that no matter how hard I tried, my efforts came up short in meeting her needs at home, and there was an ever-present underlying level of discontent in Fran. I am beyond every measure of grateful that Fran has had a complete turn-around in her spirit. She is beyond happy. She truly is joy-filled. Words are inadequate to express my sincere appreciation for the MIRACLE of The Grand at Pawling!

I want to thank the GREAT staff at The Grand for taking such great of mother while she was there. My mother is on dialysis so that made her care even more challenging for them. She required a special diet and is on a fluid restriction. She also has difficulty sleeping in a bed all night. They made all kinds of accommodations to make her as comfortable as possible while she was there. Thanks again for all your patience and understanding. A special thank you to Beverly for almost a daily correspondence of my mother’s progress and condition. Also thanks to Jonathan the Director of The Grand for reaching out to me and being involved with her care.

*This review is specifically for Christina, from the activities department, at The Grand in Pawling, NY. While no rehab/nursing home facility is ever perfect, there are particular people who always go above and beyond to do their job the way it was meant to be done. Christina, from the activities department at The Grand in Pawling, is one of those people. My great aunt spent six and a half years there and Christina was one of the reasons that my great aunt thrived. Christina treated her like family. She checked on her, on a regular basis, even outside of scheduled activities. She organized so many functions like Dutchess County Fair trips, holiday activities and family barbecues, in addition to all of the many daily activities she provided for residents so they could win fake money to be able to spend at a store during their stay. My great aunt always spoke so highly of Christina every time we spoke on the phone or visited in person. Christina always put a smile on my great aunt's face not just because it is her job, but because she loves her residents and knows how precious life is. My great aunt passed away on April 11, 2021 and therefore she will no longer see Christina to make future memories. However, we are eternally grateful for what Christina did for her during her stay at The Grand and know that Christina will continue to better the lives of those who get to spend time with her. My great aunt loved when Christina was around! Thank you for loving my great aunt as if she were your own. "Grandma Bea" loved you, Christina! Keep up the great work! We will miss you! Much love, Brian and Kate LaSusa
April 2021
We thoroughly enjoy working with Christina and her team at The Grand at Pawling whenever we do community events together. They are the most giving group of folks and her residents are always willing to try new things and remain active because it’s hard to resist the opportunities they offer! You can tell that The Grand has created a positive culture for their staff, residents, and visitors. They always take initiative to find creative ways to benefit the Pawling community as a whole.
April 2021
My sister is in the Grand at Pawling for physical rehabilitation due to hip surgery. Since COVID-19 has restricted safe visits for both my sister and my family, Facetime has been our only lifeline. It is a blessing for both patient and family. The Recreation/Activities Department, especially Christina, is diligent, caring, and accommodating. At times, this is the only way I can see, talk and comfort my sister. This service is a breath of fresh air anytime, but particularly during this pandemic. Thank you Recreation/Activities Department for connecting us with our loved ones, until we can hug again, this is the next best thing.
March 2021
Shout out to Christina and the activities department for always coming through (even if it’s last min) and brightening up dad’s day with our FaceTime and window visits. Thank you all so much ! -Melissa M
March 2021
Moving a loved one into a nursing home is really a very emotional time. The staff at The Grand helped make the transition a lot easier for my husband and & I but more importantly for my sister in law Ann Corcoran to adapt to her “new home”. We had zillions of questions, and each one was answered. Marlene was a HUGE help in getting her on Medicaid, her patience was great. A shout goes out to all the staff that worked with Ann and a big Thank you. Sorry to be long winded but there is an employee who deserves the highest of praise, Christina Meehan, she is truly an Angel on Earth! She does an outstanding job with the activities provided to the clients. Prior to Covid, we visited Ann very often and would see the Ann we use to know before Dementia and Parkinson took over, Ann would be in the activity room smiling, dancing, singing loving life, she always lit up when Christina was around. It wasn’t only Ann, all the clients love Christina, as I said she is an Angel on Earth, Thank you Christina and God Bless !
March 2021
The Grand at Pawling has been a Godsend to our family. My Aunt and Uncle were both admitted to the Grand at Pawling for rehabilitation. They are getting stronger every day and and tell us how nice the staff are to them. The Activities Director, Christina Meehan, has gone above and beyond her job duties and has helped our family tremendously by getting them important information, assisting in getting clothing delivered, to helping us get important documents signed. Christina has made a very stressful family issue to not being stressful at all. I would not hesitate to refer a friend or family to the Grand at Pawling!
March 2021
Sue is a wonderful dedicated worker with vast knowledge and patience for her clients.
February 2021
My aunt was here at the Grand recently. I can't say enough good things about this place! The nurses, the nurse supervisor and staff were amazing. Attentive, professional and nurturing to say the least. We have been in other rehabs before but this one is by far the best. If you need a place for your loved one, this is the place. Your family member will be treated like family and you can rest assured that they are getting the best care available. I highly recommend this facility. Best nurses ever! Truly a home away from home 🙂
February 2021
Would like to give a shout-out to some of the staff here. Sharon, Nicolla, Veronica, Chris, Janet, Joyce, a big thumbs up! And a big smile to Sue for her charm, wit, making my day brighter and easier.
October 2020
The staff here is clearly overworked. The direct care nursing staff were helpful but you could see there were clear staffing shortages. Our largest issue for my father was that paperwork was done so poorly it caused problems with Medicare and they denied to pay for care despite there being an obvious need. I had to request copies of what Medicare received through the appeals process and the Grand's paperwork made no mention that my father was in a wheelchair compared to his original state before falling ill. Also the social worker Jonathan was more concerned about pushing my father out of the facility (because Medicare was refusing to pay) rather than planning a safe discharge. They did not care that by sending him home he was going to a house without a ramp, even though he was released barely walking. My father had to be carried inside the house and the Grand provided literally no assistance in even sourcing a ramp. Additionally, one year down the line we found out that the hospital bed that the Grand ordered as part of discharge to be delivered home was considered a "rental" status, even though our insurance would cover an outright purchase of the bed. We did not know that it was a rental because his employer's insurance had met "Catastrophic" levels of coverage the year he was at the Grand. When the catastrophic insurance level went away the insurance started to send us bill for the bed a year later. Additionally while my father was at the Grand the electricity went out due to a strong storm. The only thing that my father was served was literally bread and water. He and his roommate ordered takeout, so they were fine, but I can't imagine how the nursing home residents who could not advocate for themselves and had no cell phones fared.
July 2020
The Grand is a beautiful facility with kind and compassionate employees. I would highly recommend this facility. The Administrator goes above and beyond to make sure everyone’s stay is wonderful.
December 2019
My father came here for 3 months after being in the ICU for three weeks. Every facility in the area declined my father due to his weight. This place took him with open arms and started to rehab him immediately. I work in the health care field and work in a nursing facility myself and know what goes on behind the scenes. This place has phenomenal wound care my father developed a stage 4 ulcer from Vassar hospital and the grand was able to heal it. I'm so thankful for this place getting my father to walk again and being independent. Yes they are short staffed at times but what healthcare facility isn't. There is a national shortage for nurses and aides. The rehab department are phenomenal as well. My father and I are very thankful for this place and will refer anyone to the grand in pawling
December 2019
I just want to take a moment to commend your staff for attending to my mom, during her final few months. The staff in the 200 unit led by Rachel was terrific. Alexis, Susie and Lania were very attentive to mom’s needs and to mine. Rachel was always on top of things and wouldn’t hesitate to reach out to discuss mom’s status. Aimee worked hard to attend to mom’s dietary needs, especially as mom was chewing challenged. The aids have a hard job and I appreciate their doing their best to keep mom clean. When mom moved to the 300 unit led by Janice she received wonderful care in her final few weeks. Janice, Barbara and Lori were compassionate caregivers, tending to my mom and me as well. In closing, your entire organization made my mom’s last few months one of caring, compassion and dignity - which helped ease her gently into her final chapter. Thank You

I want to recognize Leslie, a supervisor at The Grand. I live in Colorado and try to reach my great aunt by phone every couple of months. Usually I am not successful because my aunt is 96 and likely isn’t able to reach the phone when I call. Today I reached Leslie who went out of her way ( I believe) to facilitate an opportunity for me to talk to my great aunt. It was wonderful to hear my aunt rave about the care and to hear her voice so strong. I so appreciate the care she receives. Thank you!

Assisted Living Center uses the term "assisted living" or "personal care" to inform users that a community provides assistance with some Activities of Daily Living (ADL's). It may not meet your state's legal definition required for that level of care.
