I was released on January 16 from 2 weeks of treatment at CareOne in your Bound Brook facility. I had a most successful and pleasant experience and found myself greatly strengthened from the excellent treatment I received from the many competent and pleasant professionals who worked with me to bring me back to my normal physical capabilities. I would especially like to commend Guiliana Lazarte who was very instrumental in my successful dialysis. Beatrice Acevedo who tirelessly provided me with round the clock care. Jessica Pentado and Cheryl Bukovich exhibited invaluable expertise in running the dialysis process. Andrea Pierre and her team were tireless in bringing me back to par and adjusting the therapy program to my needs. All of these people and their associates are a remarkably effective and helpful team and they do honor to your organization. It is my pleasure to commend them. Dr. Isaac Moked President, Itek Corp.

My name is Keisha Garnett-Jackson I lived at Care One in room 8w in Bound Brook for 30 days these were some of the kindest, caring most empathic people I have ever met. Darren, Nicole, Portia, Isabelle, Reshma, Karl, Katherine, Kristina(social worker) and Diane took care of me when I couldn't do anything for myself. It brings tears to my eyes when I think of how they cared for me, even the young lady who cleaned my room daily we didn't speak the same language but she found a way to make sure I was comfortable every day. I won't go into detail about the front desk (Lobby)staff along with Rose (Supervisor/Nurse) and Patricia the entertainment person who were all horrible along with having 0 empathy and were out of touch. To everyone else thank you guys for your kindness it meant everything to me!

My husband was placed at Care One Somerset Valley in September 2021. He stayed for 4 weeks. Visits were initially difficult to schedule, and one of the 2 receptionists with whom I dealt was nearly always grumpy and unprofessional. She also berated staff within earshot of me. I'm not sure she was a receptionist, but I generally saw her at the front desk. Once I learned to circumvent her and just meet my husband in the patio, things were better. I don't know whether Will is still there, but he was an awesome liaison; he would ask how my husband was doing, whether we needed anything, and he would bring my husband small snacks from the kitchen. He helped me find missing clothing items. He helped me arrange a time to bathe and shave my husband, too, since no one else apparently was doing it. He helped me arrange an outing, so I could take my loved one out for a day trip. HOWEVER, I noticed my husband's legs were swelling, and no one else seemed to be taking notice of it, though I mentioned it to each and every person I could. His socks were always wet, which initially I didn't realize was because his leg-skin was stretched to the leaking point. IF someone had been helping him dress each morning, as they claimed they were, would they not have noticed that? I even emailed about it. Then Covid struck again, and I was told not to visit until further notice. Later that week my husband was taken from Care One to the emergency room at the hospital. Four days later he was dead. I have been trying since early February 2022 to get my husband's medical records from Care One, and am constantly getting the run-around. No one seems to return their voice messages. No one seems to follow up. I've been through the medical records department, and from Giselle to Ariel to Bem to Adiza, and finally today referred back to Ariel. Surprise, she didn't pick up when I called. Please think twice before sending your loved one there.

My uncle had his first experience at CareOne in their rehab facility. Despite how many issues he had at the beginning of his stay, the Careone faculty and staff handled each situation very professionally and he made long term relationships with the staff and left with a huge smile on his face thanking everyone for their persistent care. Now, my uncle is back in their assisted living facility where the staff is continuing to take wonderful care of him. My family and I really appreciate how accommodating the staff is,, the follow up phone calls if there's a problem and how quickly the staff responds to messages and emails. Thank you CareOne for putting our family at ease and providing my uncle with the proper care he needs!

They sent me home disabled and with an unstageable bedsore after a five week stay. It has been 34 weeks of wound care and it is still not completely healed. They deny any responsibility for this abusive neglect. DO NOT SEND A LOVED ONE TO THIS FACILITY.

We were very happy with Care One. Not a typical sub acute. My husband needed PT after a hospital stay and we picked the perfect place. PT 7 days a week. Everyone was so nice from Samantha in admissions to Monisha his SW. the care was excellent. The director of nurses even called me about an issue they noticed ( which turned out to be fine) The facility is very clean and very Covid responsible. And my husband loved the food!! The last rehab he went to I was bringing 3 meals a day. The reason for 4 stars instead of 5 is an issue we had with transportation. My husbands is a dialysis patient and the transportation company was so unreliable. He actually missed a day. Care One did everything they could and were very responsive. They eventual switched to a different company for him. We would definitely use Care One again and highly recommended them

My husband was recently a patient at CareOne at Somerset Valley. His situation was very challenging to both him and our family. I cant thank the staff, the physical therapy and occupational therapy teams,Jackie the liason and most notably his Social Worker Bionca Z. Bionca was an endless resource of information and support. Our family could not have gotten through the very stressful and challenging situation we found ourselves in. I highly recommend CareOne at SomersetValley.

This rehab has changed our views on sub-acute rehabs for the better. We had been to other rehabs in the area with dad and were left hopeless and confused. Jackie was our first point of contact and she was absolutely amazing. She was a breath of fresh air and so kind to me and my family. Anytime we had a question about dad transferring from his current rehab to the hospital to care one she answered. She was always available and if she didnt pick up she would call or text back immediately. Once dad was admitted he perked up with the rehab team and actually wanted to participate, Alison was beyond helpful and informative. Bianca the social worker was so kind, helpful and resourceful. Any question we had she was always willing to help. We werent sure if dad would be able to return back home but he did and we couldnt be happier with the care he received at Care One. Thank you so much to the team there!

The 3 primary nurses I had Vonnie Angelo and Raul were outstanding but they were stretched thin due to patient load. Weekend shift staff were not always attentive or knowledgeable about my wound. The ot and pt staff were excellent The social worker Bianca went above and beyond standard assistance

I SPENT 4 MTHS AT CARE ONE AFTER A SERIOHS FALL.. NOT BEIING ABEL TO WALK AND BEING TOTAL DEPENDANT ON EVERYONE THERE WAS A VERY HARD THING TO DO. EVERYONE THEIR MADE IT SO EASY AND PLEASANT FOR ME.THE PHYSICAL THERAPIST WERE GREAT.ALL THE NURSES WERE EXCELLENT.VONNIE AND ANGELO WERE 2 OUTSTANDING NURSES I HAD.THEY WERE EXCEPTIONAL. CNAS RIGHT DOWN TO HOUSKEEPING.I CANT SAY ENOUGH GOOD THINGS ABOUT THIS FACILITY. THE SOCIAL WORKER BIANCO ALWAYS HAD WHAT WAS BEST FOR ME.SHE ANSWERED ALL MY QUESTIONS,PUT MY MIND AT EASE AND TRULY WENT OUT OF HER WAY FOR ME. I will say we had a difficult time making a decision to pick a rehab facility during covid times. Reading some of the reviews didnt help but I do take into consideration that people tend to only review negative experiences but I would like to tell you about our experience. From the start our first chats with Bianca was so detailed and assuring we decided to go with CareOne. We were assigned another social worker also named Bianca Zullo. She was so caring and kind. Made sure we were able to contact her at anytime. My dad went in for a leg infection after staying at the hospital for 21 days. He needed to regain his strength. The director of physical therapy, Elaine was beyond amazing! I can tell her goal was to get my dad better fast and safely. She asked key questions to how his living situation was to make sure he would go home and be able to function well in his own home. During my fathers hospital stay he ended up having to be on dialysis. They arranged transport to and from the facility and all went seamlessly. Art the director of food was amazing making sure his dietary needs were met. My dad actually said he liked a lot of the foods. The nurses were very diligent with his care. His wound was well taken care of and they did a great job with standard hygiene procedures. He said he always felt clean and clothes were changed regularly. Because of covid he was quarantined for his first 14 days. The facility visitation team, Enite and Will were absolutely amazing! They would schedule window visits for us and made sure we could see my dad daily. Knowing we could only see him though a window was tough but being able to see him when we wanted was amazing. Covid times make for difficult situation but we couldnt have felt more comfortable that he was in great hands. Communication from all his care team was top notch. We were very happy with our experience and my dad he said he was going to miss the staff. I really felt so much understanding from the team, Bianca, Elaine, Art, Will and Enite really took an interest in our family and treated my dad like family! A very scary and hard decision ended up being really positive and amazing. We wont forget the people that helped get him better and we are grateful for them. I hope this helps someone make their decision.

I spent 2 1-2 weeks at care One Somerset Valley. the stay was necessary due to my broken hip and femur. I felt very happy with the physical and occupational therapy, it was one of the best. Although I had a problem with the food - it was not right for my IBS, my nurse and Art the supervisor of the kitchen, came and worked out with me a new menu. He was very helpful. The nurses were professional but at times slow to respond when I needed my pain meds. Overall I needed the support and was grateful for their help.

Great leadership staff social worker Bianca was a wonderful asset in the coordination of my fathers care. Nurse Teresa is a kind, gentle, and professional nurse my father spoke wonderful things about how she delivered the best care while at CareOne. Aide Darin is also a great, dependable, and professional man who was one of my dads favorite aides. My dad spoke very highly of the kindness he encountered by all staff from nurses, aides, receptionist, and even the staff in the kitchen. Great people and a great culture of kindness.

My mother is been in there 2 times this is her 2nd time they seem to be more willing to accommodate hard they gave her are her own Room the only thing I dont like is that they dont check on her and the nurses arent fully on their game but mind you and today societee there are much worse places sadly Lake Princeton house mental health facility that place is Lake Trenton state hospital so I didnt give it 5 stars but Ill get it a good full 4 stars for me thats really good so if youre looking to put a family member somewhere and youre really close with them I do highly suggest this place they do have happy hour games and many other fun activities

Its a beautiful facility in terms of the grounds and support. Very nice staff and caring nurses. CNAs also trying their best to entertain and help residents. My only concern was the doctors who are visiting the facility, unfortunately some of them are very indifferent to the patients and their needs. But I think this is not uncommon situation for the rehab and LTC facilities.

Dear readers, I just want to give a huge thanks and express mine and my families gratitude on how well my father was cared for at Care One at Somerset Valley. He was admitted at the beginning of August 2017 and remained there until January 25th 2018 in which he passed away. My father was very prone to having strokes in the last part of his life. Having a severe stroke in August which paralyzed him on his left side, which sadly making it impossible to bring him back home again. The staff at Care One was remarkable. Management, aids, nurses, food preparers and servers, laundry associates, and house keeping. They allowed me also to still be hands on with my father, caring for him as well. They are a great group of people to work with and I also learned a lot from them. Towards the end of my dad's life he also had great hospice care from a company called Ascend. They too were remarkable. No complaints here. The last week of my dad's life was as important to them as it was to me and my family. Believe me, it was extremely important. Everyone from Care one to Ascend was at my fathers side making sure he felt no pain and was as comfortable as possible. The kitchen staff sent us food and drinks to keep us going, and the staff checked in on us constantly. The wonderful hospice nurse and Care One nurse was at our side and so caring. So I just needed to write and commend everyone for there compassion. Thank you, John Billian and Family

We could not be any happier with CareOne for my Mom! My Mom was living with us and her needs began to fall outside of what traveling nurses, aids and OTs could provide. We went to CareOne one day completely unannounced and their Director, Aly took us under her wing and hasn’t stopped being a constant presence and help to us for the last three months. We have been through some rough patches with my Mom’s mental health and they have responded to calls and texts from us within minutes, sometimes seconds. My Mom is a difficult patient even in the best of times and the CareOne staff is patient and kind even in the worse moments. The help and stress relief we have received is beyond anything we could have ask for. The PT and OT services are wonderful and very encouraging for Mom! We could go all day with the praise for the staff and administration! We can rest easy knowing Mom is in excellent hands.

What a great facility, especially the Assisted living section. The staff is very courteous and patient with the residents and the food they are served is gourmet quality. I have experienced this personally so I can state unequivocally that this is the absolute nicest and well run facility I have ever experienced .
