July 2026
My step dad received excellent care after a sudden hospitalization and emergency amputation. He flourished with the care and therapy and is now getting back to his life. My family and I could not be more pleased with Americare and the Quail Creek staff.
May 2026
Fabulous facility and staff that care! They also have a great bird aviary!!
April 2026
Many times when my husband needed help to get in/out of bed or repositioned it took the staff 20-30 minutes to help him. He was supposed to have PT and OT daily which he did not receive. I feel the rehab facility was under staffed creating problems for those who were trying to take care of patients.

I enjoyed staying there. I met some nice ladies in the dining room. The only complaint I had was the heating system. I am a cold blooded person so I was cold all the time. There's not much heat that comes from the bathroom.

The nurses and all caregivers on the Cedars Wing were fantastic!!!!!!!! 5 stars. Nothing but glowing reports for them. Not happy with the physical therapist. Zero stars. When she discharged my husband and said he no longer needed that level of care I adamantly differed with her. He was not ready to be released. She said home therapy was all he needed. The night we arrived at home Robert could not enter the house. Fell and required 3 lift assists men to get him in the house. 2 days later on January 8 the home service arrived for evaluation. After 3 hours with him she said he didn't qualify for home health because he was not capable of doing any of the therapy they had to offer. She confirmed what I told the therapist at Quail Creek. I went back to Quail and told her of his fall and not qualified for home care because of his lack of function. She replied well something must have happened. I told her nothing had happened since he left Quail. I even told them before he was released that I had not seen him function on his own at all. He couldn't even go 6 feet to the bathroom without full help. I cannot give any stars to that therapist. My husband died 4 weeks later totally bedridden.

A friend of mine was recently a resident at Quail Creek. Her room reeked of Urine, her room mate had used the porta pot at 6:00 am and I was there at 2:00pm. Yikes disgusting no one had been in to take care of it all day. She also complained that a male aid was verbally abusive and used fowl language. Another male would not step out of the bathroom to give her privacy when asked. The food was bad and the patients were neglected.

The staff at Quail Creek are awesome! The facility is clean and well furnished. When my dad arrived at Quail Creek after a long stay in the hospital, Will Creek has been a breath of fresh air. I can rest easy knowing my father is being taken care of. Meals are cooked to order I’m very much enjoyed. It means a lot that my loved one is being taken care of by a knowledgeable and kind staff. Also, thank you to Randy who greeted us upon arrival and made us feel welcome! Would recommend Quail Creek to anyone needing a rehab rehabilitation.

My mother recently stayed here after a fall. Everyone was extremely kind to her. She enjoyed eating in the dining room with others, the varied group activities and the great care by the therapy team. Very clean facility as well!

We had an absolutely amazing experience with our dads stay at The Neighborhoods at Quail Creek! The staff are so kind and attentive. Chandra could not have been any more helpful for the whole process. Jake was super helpful in securing home health for dad once released. This place feels like a community and is making a huge difference in the lives of those who just need a little extra help!

My husband is currently in rehab, following a heart surgery. He is fully aware of this surroundings. The staff are great, caring & efficient. One draw back is not all private rooms. We love Maranatha, but needed something on the south side. This place is amazing. I love the Aviary. You have food choices. I would definately choose this for myself as well.
February 2025
My mom was in Chestnut the last 3 years. She had some really good care givers that went above and beyond for her and the other residents. Then you had those that did just enough and then the rest of the time they say either on the sofa in front of the TV playing on their phones while residents sat close by. Or in the room across from the Nurses Station playing on their phones. While residents were hollering for help. (Which they were not in a medical crisis, but in distress). Weekends wasn't the best. Trash was not taken out, dirty towels and wash rags remained on the floor in the bathroom. And we needed to supply the bathroom with towels and wash rags. Seriously for the amount that is being spent per day there, you would think staff would only be able to use their phone during their break. For the Staff who took unwavering care for mom, Thank you so very much!!!

Hands down the best facility in Springfield. The staff is both caring and professional. Both my mom and stepdad have been residents of this facility and had great experiences. If you’re looking for a place that truly cares for their residents this is your place. Even my grouchy stepdad loves this place and the food is fantastic!
December 2024
My mother and sister both needed to go to a skilled nursing facility after being in the hospital. They both had a positive experience at Quail Creek. The staff was pleasant and knowledgeable. They received really good physical and occupational therapy, and my mother also enjoyed the food. It didn't seem like a typical "nursing home". If you need skilled nursing/rehab definitely check them out.
October 2024
I spent 3 weeks with some of the most caring and professional nurses and staff and would never hesitate to recommend Quail Creek for short term rehab. PT & OT staff were exceptional!

Great place for short term rehab and long term care. Leadership that are compassionate and dedicated to care of residents and staff
March 2024
My husband was there for rehab. While the therapists were good the care he received was horrible and his room was not kept clean, his call button didn’t work and they didn’t keep his supplies of toilet paper, wipes and depends stocked.
November 2023
My Mother was recently at The Neighborhoods at Quail Creek and she was treated with great care and concern. Josie and other Nurses and OT/PT & Speech teams were also very attentive to Mom's needs. Thank you all for making this part of her journey better. Thank you Dr M. for you Medical guidance.
October 2023
On 9/27 we had my dad scheduled for a transfer to see his Nurologist at 2:20pm. At approximately 11am I was notified that dad's transport was cancelled because someone else had an emergency. We had this transport set up for over one week as we were able to move dad's appointment up over one month earlier. That transport was the only way that dad was going to make his appointment so I thought. I called the administrator Eric to voice my concern about dad missing his appointment. Dad has Parkinson's and really needed his medication adjusted. Eric said to give him a couple minutes. In 5 mins Eric called me and said he would transport dad in his personal vehicle by himself. I was extatic and dumbfounded that the administrator Eric would take time out of his busy schedule and help my dad that day. The transport he performed was seemless. Thank you Eric for your dedication and willingness to go way above and beyond.

I received a report from someone who is going through rehab at Quail Creek that a member of the staff was rude and hateful to her. I find this behavior unacceptable and unprofessional. An 82 year old in rehab should not be treated that way.
February 2023
OT & PT will have you back to being independent in your own home if you’ll work with them. They also have a great amount of activities to fill your day. The nursing staff is kind & attentive. The meals were awesome! If you didn’t like the meal they would cook you something else. I had a great experience.
February 2023
We trusted my grandma in law to this care facility and I couldn’t be happier with the care she received. Everyone there was so kind and professional. She was nearing the end of her life and they took care of her exactly as she needed. The facility is clean, well managed, and had lots of activities. Plus birds in the atrium! It was so cool to pass by them when we visited her.
February 2023
My mom was treated with such respect and kindness! Therapist was were great. She was completely satisfied making improvementshowever insurance dismissed her . As we all know it always comes down to money. Who on social security can afford $281. a day?
December 2022
We are very pleased with the care, friendliness of staff, and cleanliness of the facility. Concierge labeled all his clothing to ensure we got his items back from laundry service--if clothes are taken directly to the patient room this critical step is missed. Dad's health and cognitive function improved noticeably after both of his 5 night respite stays at this facility. I was surprised to get bills from outside vendors, but it's a small price to pay in comparison to his 'nice little vacation' (Dads comment to me).

Lack of staff resulting in sub par care
October 2022
If I could, I would give QC 9 stars! From the Administration to the WONDERFUL Redbud wing Nursing Staff to the absolutely FABULOUS therapists the CNA's (Arlene in particular) and above all the Redbud wing "chefs" (Shout out to Kiana Cook) - this facility, in our experience is top notch. When admitted (after a 10 day hospital stay), mother was unable to walk more than 10 steps (with her walker) unable to dress herself nor use the bathroom. After 14 days with the AMAZING therapists, Jane, Tracy, Colton and Brandi (the four that I remember most) she was discharged to a long term care facility, able to independently physically take care of herself. The administration at QC was instrumental in helping us find a long term care facility that had availability for us with a memory unit. Thank you Shanda Bermudez (Social Services) from the bottom of my heart. Thank you Eric (Administrator) for having Judy (podiatry nurse) on staff to address those issues (I know that's not a position skilled facilities are required to staff). Jane (the memory book continues to be used, but confusion about the calendar and date is still a work in progress) Colton and Brandi (she practically races the halls with her "wheels") and Tracy, I can't thank you enough for all your support and "mental" therapy for my family. Naturally, no facility is flawless and we did have a MINOR, VERY MINOR issue, ie a buzzer that didn't function properly - so we got a bell, a TV that had remote issues (maintenance is still working on that one). I would HIGHLY recommend this skilled nursing facility. From top to bottom. BTW - if cleanliness is your concern - NO worries there either. Showers are given regularly and laundry is continual. Like I said...... 9 STAR
July 2022
Non responsive staff who ignored call requests from clients. Left clients in restroom and did not return for an hour. Food was bad. Rude and very uncaring staff. Care was minimal. The PT staff was very good and caring. I can not recommend this place due to my own ’ experience.
June 2022
I was at Quail Creek to recover from and receive treatment for a pressure wound. My care was excellent. The staff was compassionate and skilled.
May 2022
The staff were friendly and compassionate. The rehab people were great but the amount of physical rehab I feel was not given, probably due to my insurance. They had to do notes and charting as part of my rehab time so the hour of physical therapy was not an hour of everything physical. The occupational therapy in my case was not really needed but they had me doing it as well which could of been used for physical instead of seeing if I could wash and dry my clothes. Unfortunately in some instances not all of my dr instructions were done. Like he ordered a cream gir me and the nurses said he didn't. And also some other things gs he prescribed was not followed thru on. But the worse case was, the first full day I was there, while taking my shower someone got in my purse and stole something. When I reported it, the administration acted like it never happened. They approached me and said none of their staff would get into my purse. I had no visitors and no roommate. Not sure how they walked put by themselves.

I'm trying to think of positive things to mention, and there are a few. My mom went to QC for rehab after being hospitalized. The staff were pleasant and the aides were helpful for the most part. The food was fairly good, and I was able to visit whenever I wanted to. There were a lot more negatives, though. She was placed in a room with a bathroom that was so small she couldn't turn around with her walker. She fell twice in the bathroom, and we weren't notified the first time. She opened a serious wound on her leg. Communication was a problem too. We were not notified of her blood test results or the plan for PT and OT without repeated requests. We decided to take her home after 3 days, mainly because of safety issues. They neglected to tell us that the in-home rehab they ordered wouldn't accept her insurance, so we lost several days waiting to hear from someone. My dad was in rehab there several years ago, and it seemed much better then. I can understand some issues because of staffing shortages and covid protocols, but safety and communication are serious deficiencies at this facility. I would not recommend it.
December 2021
There are no easily accessible wheelchair bathrooms! Some of the staff was very kind. I told you who you were. Some of them were harried as they were usually short staffed.. some of them need to be brought up on abuse charges . (Don & his henchwomen that made the midnight rounds). The executive director was impressive. Filling in several nights. When I called before I went in I was told I'd get 2 hrs of therapy a day. Once I got there I was told no Saturday or Sunday. That took 6 days off of my 20 . No therapy on day in or day out now I'm down to 10 days of therapy. Only a few of the aides would allow me to pull my pants up & down in the bathroom. I was discharged after 17 days very scary when I got home .
December 2021
Loved rehab. The food was tough because I'm diabetic so I had to be a brat and ask for special things but they were kind. I liked all of the nurses rehab people. I had a couple of difficult roommates but I'm sure they thought I was difficult too. But no one's there for fun they're lovely people everybody just has a lot of problems as did I. I asked to go there and I feel like it was the right decision

My Mom stayed here for rehabilitation after an extended hospitalization. It was in the Covid-era, so a lot of these places have been getting whacked in reviews for staffing problems, ridiculous Covid restrictions, and the like. In my experience, this place was awesome. If they were short of staff, we didn’t feel it. The service was great, they provided excellent care and were on the ball with mom’s plan for therapy. I especially appreciated the team approach to the therapy they provided. Mom checked out in much better shape than she checked in, both physically and emotionally, and is now back at her regular assisted living. Thanks for all your great care a therapy for my mom.

The facility is very nice. While I did not try the food my father said it was good. The PT OT and speech therapists people was great, the rest of the staff not so much. I caught much of the staff in things that lacked the truth. Would be told one thing while they did the opposite. Then again that might be the way of all nursing homes.
July 2019
This facility didn't know me from Adam. But when I called them up looking for a place to park my semi truck so I could see my daughter that I hadn't seen in 18 years. They treated me like family. I had the best evening with my daughter and I didn't have to worry about my truck. If they can treat a complete stranger with this kind of love and open arms I can only imagine how well the staff here treats the residents. Everyone that I met at this place was super nice to me. Think you so much.
