James River staff took excellent care of my Mom. She was there for about 18 months before her death. Teri, Leslie, Heather and everyone (sorry I can’t remember all their names) are excellent care givers. Mandee makes sure all the residents are busy and stimulated. Sometimes they are short of staff is the only reason I give 4 instead of 5 stars. The techs and aides make up for the shortfall with lots of hard work. Love you guys!!

I couldn’t be more proud of what they do. I just love my nurses to death. Keep up the great work. Megan in the business office is so sweet.. Kelsey is amazing and Sydnee in social director is very nice she goes out of her way to help the residents out. Maintenance Joey is an absolute joy to to talk to. My mother resided at James River Nursing & Rehab for only a few months before she passed away, but she was a patient in both the skilled nursing wing and the long term care unit. The care she received in both wings was consistent, thorough, and compassionate. Every staff member was kind and knowledgeable. My mother was not an easy patient! She made everyone’s jobs more difficult than necessary with her negative attitude and refusal to cooperate. Yet, the goal to give her quality care remained unchanged. Many staff members went above and beyond to provide Mom with what she NEEDED, even if it wasn’t always what she wanted. In spite of the time-consuming demands of caring for the needs of the residents, communication was good with the family about what was going on. Many individuals went above and beyond the scope of their job description to help my mother’s experience there to be pleasant and life-giving, even though she was only there a short while. My sister & I are most grateful!

My grandmother was there for a short stay after a thyroid coma and hospitalization, she unfortunately passed due to the circumstances of her health but there was honestly no better staff that we could’ve asked for. They were supportive and kind and understanding. They had the best intentions with my grandma. Specifically those taking care of her were as follows Tish, James, Bethann, Barbie, Meagan, Macy, Henry, Jeb, and Tracy Lee. These amazing people ensured to follow exactly what was requested by my family and by my grandmother. Without question if she needed anything they were on top of it! When she passed they were there for us and gave us all the time we needed to say our goodbyes. They even let my entire family in the room and in the dining room that was next to my grandmas room so that we could have this time to be with her and to be alone. They were incredible. There’s nowhere else I’d trust with any of my relatives. Thank you to everyone at James river I know that she enjoyed having you all there for her. We appreciate everything you do!

While there were a few issues in my loved one's stay, I would remind all that it is a very difficult job that requires great patience and a desire to help those who cannot help themselves. Taking care of a loved one at home comes with issues as well. A difficult situation all around. The staff took care of us with love and concern, as they could. They got to know us and made Mom's last days as good as they could have been. I rated four stars because I rated on the level of CARING also, not just care. That 5th star will come with a little more support for the staff, as it was often needed, especially at night. Although, in fairness, staff shortages were sometimes due to Covid outbreaks. Overall, WONDERFUL job, with the family support. Thank you all, too numerous to list. I appreciate your challenging work.

My grandmother was a patient at the James River Care for the last six months of her life. I chose this facility because it is very close to my house and work. Deaf, unable to ambulate, and mentally impaired, my grandmother required a lot of supervision and care. Her care was exceptional. I purposely withheld information from the staff, that I, her grandson and only relative, was a healthcare professional. Each time that I visited with my grandmother, I was keenly aware that she was receiving the best care possible. I was with her on the Wednesday evening that she took her last breath and embraced her Savior and entered Heaven's Gate. What a joy it is for me to know that grandmother was treated with such professionalism and dignity while she was a patient at James River. May God richly bless the many wonderful staff members who cared for my grandmother. Thank you so much! Dylan S. Clifton, Professional Educator Emeritus

Warning: Long review - read the whole thing and I hope this helps you. This review is a challenge to write. The building is very nice and the front office personnel seem pleasant. My father recently stayed in James River for a short term rehab. The therapists were fantastic. They were professional, and approachable in every aspect, although not accurate in their predictions (more on that later), but I can't blame them for that. My struggle was with the care staff. The care my father received was not to the standard we had expected going in. My father was a "fall risk" meaning he should not get out of the bed by himself. When he needed to use the restroom, he would press the call button, and there would often be no response for a long period. He would become frustrated (didn't want to go in the bed) so he would try to get himself up. He fell multiple times. The staff seemed to become frustrated that he was getting up by himself. I would ask if he had pressed the call button, and was told "no". This led to me going there every day, and I experienced the same thing. I would press the call button, and get no response for a long period and I would end up helping him to the restroom. I certainly didn't mind, but I couldn't be there 24/7. We were also told he would be bathed twice a week, but often times that didn't happen until I brought it up in care plan meetings. There is a board by every nurses station that shows call light status for rooms on that section. I rarely ever saw this board when it wasn't lit up. Multiple times I saw staff congregating at the nurses station when lights were on the board. This tells me that the call lights are not a priority response for the staff. We selected JR because Cox hospital gave us a list, and they were well scored by medicare. I learned that medicare does an audit. The audit results must be made available to the public at the location. JR stores theirs in their library. I would encourage any potential family members to read this audit before placing a loved one in their care. During this audit, you'll see the same themes repeating. Patients not getting the help they need, or staff ignoring calls for assistance. This is from the patients themselves, that the auditors recorded. I asked if there was a record of times the call button was pressed, and the response time of the staff. I was told there was not. This seems like a very easy thing to implement to develop metrics on performance, but I believe JR doesn't want to know, because then, they might have to do something about it. I don't know if they are simply understaffed, or if they lack the managerial oversight to be effective, but they are not good. The care my father received was so bad, we considered hiring a sitter for him, at additional expense simply to do the job we believed JR was hired to do. In the end, the therapists stated that my father would be aspirating any food or liquid taken by mouth, and we took him to his home under hospice care. It has now been several months, and my father is thriving. Since leaving JR, he eats every meal, and is doing great! He is putting on weight again, and is alert and aware of his surroundings. Overall, I believe JR is NOT a good place to care for your loved ones. I would strongly urge potential families to read the audit, and then find another location. When the time comes, you're usually under some time pressure because the hospital wants the patient out, but take the time to do the research before hand. When you do, I think you'll pass on JR which is a shame because I think they don't need much to be MUCH better. If they made some basic changes to their business they could actually serve their patients well. The fact that they don't leads me to believe that they aren't focused on patient well being, but on money - and that's sad.

The best place ever! I've had my father there as well as my Aunt. You couldn't ask for better care that they both received. The staff was wonderful, you can tell that they love their job and really love their residents. The went above and beyond when they took care of my father in his last days as well as my Aunt. The compassion they had for my family made the transition so much better. They made sure my mother was doing ok when taking care of my father. And also did this when her twin sister (my aunt) was in her last days. My family said they were so glad they found James River Care. This facility is recommended often to family and friends. Thank you again to the staff of James River Care and Rehabilitation.

I have visited this facility several times over the past three years and I am always impressed when I walk in the front foyer area. The facility is nice and clean. There is always someone there to greet you when you walk in the door. The sitting area has a saltwater aquarium filled with several different Tang, Clownfish, and beautiful coral. It provides hours of enjoyment for our family members and all who come to visit. The CNAs work very hard here to care for all the residents. Nursing staff keep you informed of changes and provide great care here. The food is really good here and they have BBQ's once a month during the spring and summer. The y are really good and they serve a wide variety of dishes. There is always a friendly face here. Everyone has always been very helpful. 5 stars to James River Care and Rehab of Springfield.

Great facility they put a lot of money in making it look nicer than it was. A lot of private rooms and maybe more expensive, but in this case you don't get what you pay for. You get skipped or the wrong medicine. Staff also the staff seemed over worked and tempramental. It was still probably one of the best in town. After this last year of deal with care centers I will say this is the best in town. It goes to show how screwed our health care system is
