My mother came to Silverado a year ago - I've been impressed with the staff, and the consistent care they provide since the day she moved in. She came from a hospital - not hardly able to walk or communicate - and has made remarkable progress over the past year thanks to the care team and nurse's consistent care. Whitney did an amazing job helping me with the transition from hospital to Silverado - and even met me at the hospital to make a smooth transition for us from the hospital to Silverado. Ryan and his sweet dog were also there in Silverado van to transport Mom with Whitney by her side to her new home. The comfort and support Whitney & the staff provide is invaluable. The leadership team (Jordan, Nichelle, Whitney) is wonderful and has my full confidence; they always address concerns that pop up (like changing health needs, preferences, dining/seating arrangements, etc.) promptly and with detailed communication. Ryan has helped comfort Mom with making sure the HVAC is tuned to her liking, and made her initial move-in, and her move within the facility (from room to room) quick and seamless. Mom needed help with her TV remote - Ryan helped her and also drew her a picture of which buttons to press in what order. Whitney's monthly support group meetings have helped me tremendously to address emotional concerns, care, and to meet others with the same stresses & struggles of having a family member with dementia. Housekeeping does a great job cleaning, keeping supplies filled, and by helping mom feel loved - Kelly has left her encouraging notes that mean the world to her (and to me also - to know Mom is cared for in such a wonderful way.) Trish, Grace J., Maria and so many other special staff members provide special care and support. Family holiday events each month are a great way to celebrate, connect with my loved one and other residents & their families - wonderful music and food (Thank you Morgan!) Silverado brings in wonderful live music and combines it with weekly happy hour + snacks/drinks. The activities available are great and engaging (Mom has enjoyed jewelry making with Tatum : ) Silverado has a family feel - and anytime I visit I truly feel I'm visiting my mother's home, and not just a care facility. I know it sounds cliche but the staff feels like family to me. Thank you to each staff member and the part you play in caring for my mother - and myself also. You are loved.

It is not only a beautiful facility, but they have wonderful activities for their residence as well as family members! They have a kind and compassionate staff. My ex-husband has been at Silverado for almost a year. It's very scary when you have to put a loved one in assisted living. I made several calls to different places around the KC area. When I found Silverado I didn't look any further. They were kind, the place is very clean. It's all memory care so that's what it is completely geared towards which I think makes a very big difference. I feel like he is safe and very well cared for. They keep the residents busy. The staff is friendly. They got several resident animals. If there's been something that's come up, they always call. I would highly recommend Silverado Memory care in Lee' summit, Missouri to anyone that needs that type of care for their loved ones. With him being there, it's taken a lot of worry away because I know he's safe and happy there.

I moved Mom from assisted living to Silverado Memory Care a few weeks ago after she was briefly hospitalized. I have met many staff and I am amazed how well they interact with ME to ensure that they understand Mom's needs and better address her challenges. My in-laws moved into Silverado Lee's Summit, MO towards the end of April after their health declined at a previous facility. The staff at Silverado were welcoming to the entire family in helping our parents with this transition. Although my father-in-law's health continued to decline and he recently passed, everyone has gone out of their way to make sure my mother-in-law feels safe and comfortable with the adjustment of living without her husband of almost 60 years. Shout out to all of the staff for taking care of Joye!

I had to move my mom into a memory care facility. I am so glad that I found Silverado Memory Care in Lee's Summit, MO. This place is perfect for mom. Everyone is so friendly, they call her grandma, they help her with whatever she needs help with. The staff is super friendly and they always know where she is when I get there. Silverado feels like family. Mom is home.

We are so happy our mom is at Willowcreek.When we first tour the facility we were impressed with Kelly and the decor and loved the room, the room was furnished !! They made us feel welcome ! Some of these places are all about the business end and money but I did not get that from this place and I have visited many facilities trying to move my mom but none gave me the calmness like Willowcreek. Staff is truly friendly and caring. My mom use to say dont you put me in no bad place I always assured her that will never happen and I kept my word. She is happy there.When finding a place for your precious love one, you look for , good care for your mom or dad , a great price , if they can meet the needs of your love one , location , cleanliness , hopefully decor updated , and the best of care and to feel welcome. We found all of that at Willowcreek. Place is beautiful ! Clean !I was convinced my mom would be in good hands. Kelly was the person we met first and she is genuine , has a good heart, she is something ! rare type, very accommodating extremely caring and contentious.After listening to her thoughts and how they would do things we were convinced our mom should be there.What I saw was staff talking to our mom and being so sweet to her ! I cant thank them enougleaving at night I glanced in window and could see my mom in her room and witness them being so attentive and caring and I left thanking God he brought us to Willowcreek. They will treat your love one like their own and with love and understanding because with memory care residents you can have good days and bad days but I can tell they have patience ! I feel confident my mom is in good hands , i just hope she doesnt drive them crazy so I truly recommend Willowcreek.

Since having transferred my grandmother there, I can only say great things about Willow Creek! My grandmother had gone down hill at her previous facility. She is back to her spunky self and its a night and day difference with her personality and demeanor since coming to Willow Creek. I truly believe this has everything to do with the amazing care she is receiving. The staff is super attentive and they treat all their patients like family. They do crafts and fun activities, that my kids and I join in when visiting, as well.

The facility is very modern and clean, but most of all the staff is delightful, loving and attentive. If I could give this place no stars I would. We moved our mother here and she was healthy. Shortly after moving in she mysteriously died! They have had several questionable deaths one after another. If you love your family member please do not move them in here. From what I have seen the caregivers did not care for my mom properly and the management and head nurse are rude, don’t care what happens to your family member and should not be in charge of this place. This place should be shut down. I wish I would have looked up the state surveys before we decided on a place for our mom.

The activity director, Amy, has a special heart that pours love into all she does - from giving each resident one-on -one daily attention to the fabulous programs and activities. The living quarters are always decorated with seasonal themes and a home atmosphere is so inviting. Our Mother has found her forever home and is happy.

ZERO OUT OF FIVE! Too many things about this facility, previously known as Autumn Leaves, to even write as a warning. I will say, if Gary Holmes is the director at this facility, do not believe anything he says. He has managed to weasel his way out of paying a measly $155.71 pharmacy bill that my dad never received and wasn't even in the facility when he incurred the charges over a year ago. In fact, my dad never returned to the facility because he was hospitalized from neglect, and dehydration while in this facility, resulting in hospice, and his death. The DON and 3 cna's, 2 being my dads, walked out and never returned while he was there, and he went 3 days without required medications! My dad was a physically healthy man aside from dementia when he admitted, walking with no assistive device, minimal medications, and no O2 requirements; declined in less than 3 weeks here, hospitalized the other 5 weeks, and passed within a 2 month period!!! I have been communicating with this facility since Oct 2018 about this, even though $155.71 isn't a lot of money, my dad didn't receive these meds, so who did or where did they go???? Gary Holmes spoke to the pharmacy in May and agreed to pay, spoke to me afterward and agreed to pay, and conveniently waited until a new company took over in August to claim it was out of his control. He lied to the state when they investigated, by telling them he didn't know where to send the money and was unable to contact anyone, resulting in the state deeming it unsubstantiated. Poorest excuse for a human being, or director of a dementia facility, and 10 months after my dad's death with no results!!!! I've contacted the controller of Autumn Leaves, and waiting for a response. I won't hold my breath if they are anything like Gary Holmes. My advice, run from this maze trap, your family member will only decline. If you are looking for a place you can trust and a director with integrity, search elsewhere! UPDATE: The controller responded to me and resolved the medication issue with me in 4 days, and handled in a very professional manner! Why Mr. Holmes couldn't make the call to her in the past 10 months and get this resolved is a testament to my post, and his true lack of integrity, accountability, and professionalism.

I rarely leave reviews, but I feel an obligation to in this circumstance. Autumn Leaves is a memory care facility, and they advertise to specialize in caring for those battling dementia. My dad suffers from Alzheimer’s disease, and he lived there until I abruptly moved him out for his own safety. Exit-seeking is a common characteristic of Alzheimer’s, which is a reason why I selected Autumn Leaves as it is a lockdown facility. My dad routinely got out of the facility. I confirmed with another family their dad also got out, more than once. The last time, this gentleman was located nearly 2 blocks away. Mind you, this gentleman needs a walker for mobility, and exited the facility with it. Following this gentleman’s elopement, the building was still not secured, preventative measures were not taken, and my dad got out 2 more times (that I know of), within a span of approximately 10 days. My dad’s Alzheimer’s is severe enough he does not know my name. The other family I referred to moved their loved one, and after I was able to confirm what I had been hearing, I immediately grabbed my dad and took him out of their care. I don’t want to take away from some (note some) of the workers at the facility that are really good people, who know I genuinely like them. However, there has been a significant turnover in staff since the facility opened less than a year ago. The facility is simply not safe. I could detail other concerns I had, or have heard from others, but I think the residents’ ability to abscond is enough to support my concerns and my 1 star review.

Autumn Leaves of Lee's Summit although beautifully decorated is a complete fabrication and the leadership and staff lack ability and compassion to properly care for the aging let alone those suffering from dementia or Alzheimer's. Although they may have the occasional visitor fooled, for those families able to visit on a daily basis the poor quality of care is very apparent. It's not uncommon for bedding to be soiled and not washed unless brought to staff's attention by family members. Also, the lack of nourishment for residents is border line criminal. Future families do your research before sending your loved ones here.
