What a fabulous facility. My husband spent over a week there in memory care. From the minute we walked through the door I felt at ease, it was meticulous. A smiling welcome from Nekayla and all employees from the nursing side. Many activity options were given to him. The cleaning and food service personnel were outstanding. The beautiful outside garden was perfect for him. He was able to enjoy watching the birds, Water features and beautiful gardens. I inquired about future options for him and spoke to Ashton, she was, kind, friendly and professional. She gave me several options for the future which I appreciated. The most important part was my husband was happy. He loved the food and many options to choose from. If I could give more than 5 stars I would. I highly recommend Autumn woods Health campus.

My mother was discharged from the hospital and admitted to Autumn Woods. After having suffered a broken hip and in the final stages of dementia-She needed skilled nursing. What she received from Autumn Woods was nothing short of excellent care and a loving touch from employees that have been there for a long time.I can't recommend Autumn Woods highly enough. I just came home after spending 5 weeks in rehab after a serious illness. I chose Autumn Ridge because of previous experience there with both my parents. I was not disappointed. The Staff are wonderful caring people who work hard to help you yet convey they truly care and it wasn’t just a paycheck to them. The food was good, the company and fellowship also was good. It truly made this as pleasant as possible. I made new friends and will go back to visit. Highly recommend to anyone Needing in-patient care.

I was at Autumn Woods for two weeks for rehab and everyone was extremely kind and helpful. I had a very good experience from having the best roommate, wonderful nurses, aides, and even the food was very tasty. Bobbie Jo, Nyomi, and the entire staff in the dining area were great. My husband spent a month at Autumn Woods for rehab to help him with his progressive neurological disorder. He was well taken care of and was content to be there. His day nurses and aides were very good. (I never met the night shift but I am sure they were very good too.) A big thank you to Bobbie Jo who set up a wonderful 49th anniversary table for us. All the staff I met were friendly and seemed to enjoy working there.

My mother was in need of rehab after a 10 day hospital stay. After speaking with hospital staff and friends, Autumn Woods was the overwhelming recommendation. Though her stay was brief-because of insurance considerations-it was exactly what she needed. We immediately felt like family on day one. Big shout out to Bobbi Jo-activities director. She not only made my mom feel special, she helped me navigate the sudden changes in my mother’s health. She felt like an old friend within a couple of days. All of the staff, from housekeeping to food service to the administrative staff to the nursing staff, were kind and proficient in their care. I am so thankful and indebted to the incredible therapists-physical, occupational and speech. My concerns and questions were always answered clearly and compassionately. Thank you all.

Autumn Woods long term facility gave wonderful care to my Aunt Lorraine S. I live in Virginia and she lived there. They were all pleasant and most helpful in the care of my aunt. Communication was great and kindness was given to her and myself freely. She was treated with dignity and respect. I knew she was being taken care of. When I visited I was treated very kindly. The nurses and aides were wonderful in the most meaningful ways. I must mention that Nicole was excellent. Sean from hospice, Courtney and Leland were excellent as well! I can’t say enough wonderful things about this nursing home. From the moment you walk in, you can tell the place is exceptionally clean, well-maintained, and thoughtfully run. The environment feels bright, calm, and welcoming ” not clinical or cold like some facilities can feel. The staff truly make this place special. Every nurse, aide, and team member we’ve interacted with has been kind, patient, and genuinely caring. They treat residents with dignity and respect, and it’s clear they take pride in their work.

Autumn Woods Skilled Nursing cared for our mother for over a year, and so many of the staff became like family to her. She was always treated with respect, dignity, and genuine kindness. Nicole, Tammy, Shiela, and Athena stood out in such meaningful ways, but truly all of the aides were wonderful. When our mother passed away last week, she was surrounded by family, and we are deeply grateful for the care and compassion Autumn Woods showed her throughout her time there.

Autumn Woods was a wonderful place for my Grandmother to receive therapy and to later reside. All of the staff were so caring and treated her like their own family. They were very attentive to her needs and went out of their way to ensure she received the care she needed. They welcome visiting family with open arms and go above and beyond to make their residents feel at home. I feel very blessed my Grandmother was able to live here and am so thankful for all of the wonderful staff.

My mom has been at Autumn Woods twice after stays in the hospital for rehab so that she can get her strength back, and they did an incredible job through both of those stints. Autumn Woods is family to us. Both of my wife’s parents stayed there. She and I volunteer there and we love the folks who participate in our hymn singing and Bible study. The staff makes the difference in a place like this, and the folks at Autumn Woods are truly loved by the compassionate staff. Hopefully, I’ll never need to live here, but if I can’t care for myself, Autumn Woods would be my first choice.

I can not say enough positive things about this campus. The staff are so friendly and helpful. The nurses are very efficient and caring.The property is beautiful. The activity department goes above and beyond for the residents entertainment. The dining service is very nice and the food is really good. The therapy department is great! If you can't be home and you need extra help for yourself or a loved one this is definitely the place to be.

My Mother has been at Autumn Woods since Jul 2024. She is well taken care of and the staff is very attentive. They are expedient in addressing any health issues, as well as prompt notification to family members. Bobby Jo and Activity Staff maintain a full schedule of great local talent, to entertain the residents and keep them immersed in constant daily activities. The dietary staff is very friendly and provide high quality and varied meals daily. Great place for your loved ones!

The only issue I have is that I feel that the patients are not getting helped quickly enough. I will be visiting my grandmother, she will push the call button, and nobody comes in. I also am noticing when walking down the hallway, nurses just chatting, while the call lights are on meaning they need help. Other than that I would say it’s a pretty good establishment.

In the past two years I have had two loved ones at Autumn Woods. One in long term care and another in rehab. I cannot say enough great things about this facility. I have had terrible experiences with rehab at two other facilities in the Southern Indiana area so believe me, I know what I am talking about - this place is TOP NOTCH. One of the things that makes it so much better is their staff is much more stable - there are many people working there who have worked there MANY years. EVERYONE there is kind, inviting, and concerned about the wellbeing and care of the patients/residents. Most recently, my Dad was there for 2.5 weeks of rehab. The nursing staff was excellent. They came promptly when he needed them, his medication was provided timely, and in general they just provided great care. The therapy staff were genuinely interested in doing everything they could to help him progress. They have lots of activities and things to keep the patients/residents engaged. I noticed that everyone, all the staff from the Executive Director to the cleaning folks take the time to notice, talk to, and help with the patients. Icing on the cake is that the food there is good and the dietary folks strive to serve the patients/residents what they like to eat. Knowing that my loved ones were getting great care gave me peace of mind during a difficult time.

My Dad was recently there for hospice care. The place was very clean and the nurses, CNA’s and other employees were very nice. They always cheerfully greeted you. Their doors are open 24/7 so you can visit your loved one anytime. They offer complimentary soft drinks and ice cream. I can’t thank them enough for the care my Dad received. The employees were very attentive and always took the time to offer support, updates and to find the right person to address any needs or concerns. I would highly recommend them and very happy that my Dads final days were peaceful and done according to his wishes.

Amazing service! The manager, Taylor is wonderful and puts all the residents first before herself even if she has to make sacrifices. I would and I WILL recommend her to anybody that needs a nursing home for their loved ones. Don’t hesitate to make a visit and see for yourself why I put a 5/5 rating.

I highly recommend Autumn Woods. My mom has been at Autumn Woods for almost 2 years now. She is very happy and well cared for. I can't say enough about their activities. Bobby Jo and Bill Andrews go above and beyond to keep the residents active and engaged. Mom alway tells me how much she likes Autumn Woods. My mother-in-law has been a resident at Autumn Woods for almost three years. She considers the wonderful staff to be part of her family. She feels safe and and happy. I can say very genuinely, this is the best Skilled Nursing facility around Southern Indiana/Kentucky. Not only does every one go above and beyond for the residents, but for their fellow coworkers as well. The team work is what kept us going during the global pandemic. The world may have changed, but we haven't. I love this team and this place!

They don't automatically check sugar on diabetics, can't get private room so TV's are noisy making rest difficult, there is no therapy on weekends. The positive reviews are for the kindness of staff not for actual medical care in my opinion.

I cannot say enough about how much I love the family at Autumn Woods. I say "the family" because I started with intentions of naming everyone I dealt with who went above and beyond for my mom and me, but I quickly realized EVERYONE I dealt with did just that, just like family. My dad was on legacy lane for about a year, there are angels working here, they took excellent care of my daddy, thank you for caring for my dad in his last months when we could not be there ourselves.. staff made sure they sent pics, videos, updates regularly.. thank you each and everyone that took care him.. no worries if your looking to place your loved one..they are the best here.
