My 89 year old mother was sent to St. Charles Health Campus for skilled care and rehab after a successful surgery for a broken left tibia and fibula. In less than two weeks she ended up with a severe 2 inch necrotic pressure wound on the back of the left heel as a result of St. Charles' staff redressing and re-applying her surgical splint with no attention to protecting the heel. Next were severe pressure wounds on the top of her foot from dressings applied incorrectly and an ortho boot strapped too tight. I started weekly visits to Memorial Wound Care to try to turn this around. St. Charles was negligent and inconsistent in following the dressing instructions from Memorial Wound Care. I had to visit daily just to ensure her wounds had been properly attended to. I arrived one day to find mom missing all the skin on the back of the toe on her right foot! I was the first to notice and no one could tell me why or when it happened. Now she has severe injuries to both feet! Mom also had numerous falls there and lost nearly 10 pounds. The management team in the skilled services area is extremely lacking and present little accountability or leadership to the skilled staff that is trying to do a good job. There is a lack of a thorough patient information transfer process between shifts enabling mistakes and inconsistencies in care. However, I was impressed with the rehab group staff, the activities staff, the environmental services manager and several nurses and aids who really were trying to do a good job under bad management. I finally moved mom to another facility for her safety and for better care. I only wish I would have done it sooner. Not one person from the management team acknowledged mom leaving or offered an apology for all that happened to her there. An important Red Flag ---> the day after I moved mom my husband requested a meeting with the Executive Director to discuss the challenges with mom's care and to give him more insight into mom's and our experience. My husband left 3 polite messages over multiple days asking for a call back. When the Executive Director did finally return our call he was curt and refused to meet with us. He told my husband "I have complete confidence in my staff and have no interest in what you have to say". That is a direct quote I kid you not. If that is the way an organization is run from the top-down there is no way it is patient-centered or even safe for your loved one. Please consider my mother's experience and choose carefully. Mom will likely not walk again after all this and could even lose her foot.

I had the pleasure of visiting St. Charles this week, nestled in a quiet and gorgeous community. The building, cleanliness and decor was perfect. Then I met Jenny, SW and Lisa, RN. Both of them, have worked there for a number of years, were very involved and knew their building and residents. Most importantly, I could tell how much they cared for their residents and team. All of this is a perfect mix for a well-ran healthcare campus.

My father has been here at St Charles for several years. He is happy with the food, nursing staff, aides and activities. When we moved dad in for the first time we ask if they couldn't find him a group of people to play cards with and they were happy to get that started for him. We ask if he couldn't sit at a lunch table with other people that love sports, so they had stuff alike to talk about and they had it done right away. We have been so happy with the care he receives and the communication between the nursing staff and our family. They have such wonderful activities that include family and residents all the time. We always feel included. We would recommend St Charles to everyone.

My mother was a previous resident, twice before for rehab. It came time to find a permanent Assisted Living facility for both of my parents and my mother was not interested in looking anywhere else. She loves St Charles for the social scene and activities! They have a couple beautiful courtyards for their residents and guests to visit in. Plus they had friends that were already at St Charles so it was a no-brainer. Sherry, the Activities Director and her crew of volunteers work so hard at keeping the residents active and involved. They will come and get the residents or notify them when somethings going on. They also coordinate fun outings for the residents! Sadly, since moving in, my mother had unexpectedly passed away leaving my father there by himself. If you ever go to visit St Charles you might be greeted by my father at the West entrance. He enjoys sitting outside greeting everyone. If you ask him if he's happy he'll tell you he's content and this is the best place for him, for the time being. My father has level 4/5 dementia. He might need to go to a Memory Care Facility, eventually, or maybe not. Regarding the Healthcare side of things..are they perfect? NO. But they are approachable and they DO try. I couldn't ask for a more friendly, safer place for my parents to spend...their final chapter

This is the 4th facility my mother has been in and is by far the best. She's been there almost 2 years now. Compared to other places: She is always clothed, they make her to get out of her room and go eat in the dining room, they call anytime something changes/happens, the rooms are clean* and the people are happy to help with anything. *The bathrooms could be cleaner, but maybe it's because my mom doesn't care, they don't. They aren't nasty, but more like a fast food restaurant bathroom. Everything else is as clean as your avg home. Overall, I am glad we found this place and doubt we could find better. The last place in Franklin IN drugged my mother up on 3 different antidepressants and never dressed her.

I have had the pleasure of being employed here since 2001. We had a previous rehab to home patient visit us today, and these are his exact words: "This was home for 5 months. My care was excellent, and I'm doing things now that I never thought I would. I even miss this place. I still do my exercises everyday, and am grateful for care I received from St. Charles Health Campus.". Ron F.

My family was so blessed that my grandfather was able to call St Charles "Home" for the last four years of his life. The facility is well kept and the staff are awesome! My whole family felt at peace having him there, knowing he was well cared for!

St Charles is one of the best nursing homes I've ever been able to experience. The facility smells and looks clean, fine dining meal services, and they have resident retreats where the residents get to stay the night in a hotel and experience another city! They care for their employees just as much as their residents, quarterly wage increases, monthly parties, and free health insurance is offered. Voted number one nursing facility in Dubois county!

My grandfather was a resident at st Charles. He loved it there and he was treated like family. There's nowhere he would have preferred to spend his last days. I'd recommend this place to anyone. I knew he was well taken care of.
