My mother has been a resident there for 3 years. She is on the 3rd floor memory care. It is a very caring, well kept and clean facility. Her room is clean, bed made everyday, lots of activities (calendar of events) which she may participate it. She does not complain about the variety, quality or portions of food. Her nurses and staff are excellent, very caring and friendly. May, Jay, Eva and Rose are the best nurses anywhere. It is a two-way street of communication. They are always available to answer any questions or concerns. They will call with anything that they see is an issue. I couldn't have picked a better place to care for my mother. They have a difficult job and do it with kindness, caring, professionalism and respect.

The staff at Bella Terra Streamwood have been very helpful in taking care of my wife and seeing to her needs. The therapists have worked with her so she is able to come back home after her rehab. Everyone from admissions, nursing, and therapists were involved with evaluating her when she arrived and creating a care plan for her rehab to be able to return home. The facility is kept clean and neat.

My sister just spent several weeks here following a serious medical event. During that time, there were several aspects of the experience that we truly appreciated. Many members of the staff were kind, caring, and encouraging during a very difficult time. Several caregivers treated Andrea with patience and compassion, which meant a great deal to our family. The facility itself was generally clean and comfortable, and the environment felt safe for someone working on regaining strength after a major health challenge. One of the strongest parts of the experience was the physical and occupational therapy team. My sister truly thrived under their care. The therapists were knowledgeable, supportive, and instrumental in helping her rebuild strength and confidence. Their dedication made a meaningful difference for her. I recently spent almost two months at Bella Terra in Streamwood following a serious medical event, and I’m very grateful for the care and support I received during my stay. Many members of the staff were incredibly kind, encouraging, and patient with me while I worked on regaining my strength. It was a challenging time, and their compassion and positive attitude made a real difference in my day-to-day experience. The physical and occupational therapy teams were outstanding. They pushed me in all the right ways and helped me rebuild strength, confidence, and independence. I truly believe my progress would not have been possible without their dedication and expertise. The facility was generally clean and comfortable, and I appreciated the staff who helped make a difficult time a little easier. My sister is a resident at Bella Terra Streamwood for several years now and is well cared for by the nurses, CNAs, dietician and social worker. The entire nursing staff on her floor along with the DON and ADON are very compassionate, caring and patient with my sister and all of the other residents too. I'd like to recognize Rose, the restorative CNA, who has provided her own TLC as time permits which I appreciate. My sister's nurses and CNAs on each shift are top notch providing various levels of care for each resident as if they were family. 1/9/2026 - Update..Since my last review management reached out to me within a few days and I truly appreciate how quickly they responded and took my concerns seriously. POSITIVE changes took place and mother feels better about the changes. Thank you, Debbie & Anthony Old review- I wish I could give the facility more stars. Debbie in administration is great but she's very busy. Maritza at the front desk is also great, but everyone else is difficult to reach. I read alot of the reviews. My mother has been there since February 2025, 1st floor was good, 2nd floor not so good, there is no team work. If a patient buzzes and the CNA assigned to that room is busy with another patient, the ones sitting in the station ignore the buzz and goof off on their phones. I am tired of trying to solve issues. There is a handful of CNA's that are good kn that floor. I won't say her name but 1 CNA doesn't like working on the 2nd floor, she is lazy and rude to my mother and rumor is she is like that with others. Mother gets stressed out when this CNA is assigned to her room. I have to visit too much to keep an eye on what's going on which is challenging for me because of my own health issues. Her eye glasses have disappeared 2 times. The facility is short handed. I was originally told 8 patient ratio per CNA, not true. This location is convenient for me, but it's sad that I worry so much about how mother is treated when I am not present.

You are subject to THE RNs mood. Big PROBLEM. Some wonderful CNA ANGLES SAVE the day every day. No matter how tired they are they’re always kind and respectful no matter what. Eva who works on days on the 1st floor is angel and should be recognized as such. Maryann who works nights is also an angel. She sticks with you no matter how annoying you may be. Always kind always respectful. She has gotten me through some tough times. She’s a saint These ladies should be trainers. A lot of Good ones. They are so understaffed and need help.

Bella Terra Streamwood's staff was wonderful during my father's stay there after a hospitalization. The nurses and staff were compassionate, caring, and informative every step of the way in my father's care. They treated my father, my family, and myself with nothing but respect. They treated all of our concerns and worries as if they were their own. The Director of Restorative, Admissions, my father's caseworker, Nurses: C.N.A's, and the Therapy dept. employees are exemplary employees. My father has been in a few skilled nursing facilities over the last few yrs for various health reasons, and this by far was the best treatment he has ever received. My family and I greatly appreciate everything that they did.

I just need to say thank you all at Bella Terra in Streamwood. My Mom had a bad turn after receiving chemo and radiation for stage 4 head and neck cancer. It took away her ability to talk, walk and eat. Dakota (admissions) Julie (dietitian) Kathy(nurse). The therapy staff was amazing...all of them. Agnes(social Worker), Iris, Mary, Colleen, Loni and most importantly Katelyn, the speech therapist. She really worked on my Mom and it was so nice to see her progress every day she was there. Housecleaning was especially patient and kind to my Mom. They kept her area clean even though my Mom played her own version of "hit the basket" lol. My Mom is home now and so much better due to the diligence of the staff at Bella Terra. I know I'm forgetting someone so Thank you to the whole team for dealing with my Mom...she's not always easy and they took care of her like she was their own Mom.

My Brother was a patient at Bella Terra Streamwood. I live in Minnesota and was unable to physically get to Chicago to be an advocate for my brother. He has wonderful friends there who filled that role. My brother has never been married and has no family there so these friends were wonderful advocates. I tried to do as much as possible with phone calls and got to know Agnes Mika very well in all the conversations we had. I can’t begin to describe what a wonderful, kind, professional caring woman she is. It’s difficult enough to deal with an elderly loved one and then add Dementia/Alzheimer’s to that equation. Agnes never once made me feel like I was being a bother too much. My son is in healthcare and knew the right questions to ask and Agnes spent a lot of time with him as well. When I would call to speak to the floor nurses literally everyone that I talked to was so kind and incredibly caring. I am truly thankful for their kindness and caring as well.

I transferred my 86 year mom from Chicago Insight Hospital to Bella Terra in Streamwood on 01/31/24. Dakota Burke the Director of the Bella Terra was so helpful in getting mom accepted into the facility. Dakota took my family for a tour of the facility before my mom got transferred over to Bella. Dakota when thru all of the guidelines of the facility very thorough. Dakota have some very good staff that been working very hard taking care of mom. I would like give a Great THANKS to May, Rose,Marcy and Maria( on the first floor) for all you have help and done for mom. And a special thank to Monette the social worker. And Marisa the front desk receptionist is always so friendly with a wonderful smile to all incoming visitors. Mom is so… happy at the facility and made a lots of new friends. My aunt (75) was in residence at Bella Terra Streamwood for about two and a half months recovering from injuries on her legs. She was very nervous and uncomfortable coming out of the hospital and the staff at Bella Terra welcomed her and put her at ease very quickly. She had many CNAs that she would see on a daily basis, including her favorites Gabby and Georgina, and all of them were friendly to interact with. There were definitely staffing issues, but from what I understand, there are nursing staff issues everywhere these days. There were temporary staff that came to fill in the gaps and even these nurses acted professionally. The doctors were patient and explained what was happening at each stage of recovery, and caught a potential blood issue that could have proven more harmful if left unaddressed. My aunt also needed wound care and they have a wonderful professional there lovingly nicknamed “Mr. T”! He was very gentle when changing the dressings on my aunt’s legs knowing that the process caused her immense pain. He worked efficiently and methodically making sure everything was sterile and treated properly. The true standouts were the PT/OT staff, they were kind and encouraging to my aunt and made the difficult workouts run so much more smoothly. Peaches, Desi and Colleen worked with her most days and they pushed her to increase her strength and fitness. When she walked for the first time, everyone clapped and there were more than a few tears in the room. When she arrived at the facility, she needed to be lifted out of bed and into a wheelchair using a machine. When she left the facility, she was getting up out of bed with no assistance and walking confidently with a walker. I would recommend this facility to anyone needing physical rehabilitation, they do a wonderful job. I was only able to be there a few days every week but I trusted every member of their staff, even those I never saw. Even the administrators were helpful and easy to get a hold of whenever we had insurance issues. This is a very well-run facility, I am very happy I chose this for my aunt’s recovery.

We are a home care company that sends caregivers and nurses to homes to take care of the elderly and those that need it. A lot of our clients go into rehab facilities in the suburbs. It wasn’t until recently that I heard a lot of good reviews coming from Bella Terra Streamwood. I took it upon myself to go into the facility and I saw that they have reinvested heavily in the facility. The new head social worker Agnes is so empathetic and responsive. I cannot say enough how pleasantly surprised I have been from the care coming out of this facility. Top notch! Way to go!

I've visited many nursing homes during my professional career in the medical equipment sector. I never anticipated becoming a resident at one at a relatively young age. I believe Bella Terra has earned the title of a great rehabilitation center. When I arrived a year and a half ago I was in really bad shape. Bella Terra has proven beyond a doubt that they truly care about it's residents. There's always going to be some issues at any facility. However, I feel that they have done an outstanding job in an unforgiving economy. Special recognition to Maria and Sandra 1st floor for always doing an amazing job. Second floor, Nancy, Murna, Soni, and Jackie , for nurturing me back to health. Jessica, Ivan, and Tashandi in activities for always spreading cheer to all the residents. “I love this place!! It is one of the best nursing homes I have stayed at, compared to the horrible experience I had at another place. I was a recent resident of Lexington Health in Streamwood,IL and I was so impressed with the overall ambiance of the facility, as well as - most importantly - the professional care I received from the Staff. Recently, I had a surgery and was directed to choose a residential facility where I would receive medical care while recuperating, before I finally get home, and I was not mistaken in choosing Lexington, Streamwood. First of all, our home in artlett is close by, and my folks are just a phone away.It was so convenient for them to check on me, and pay me a visit until 8:00 p.m. It was as though I was in a Hampton guest house, except that I was with my aches and pains! My family, though, as well as myself was confident that I was better off there, for the meantime. But of course! Lexington is manned by an excellent Nursing Staff. If you were to judge their care with their promptness to answer your light call, they deserve my above rating. To complement, they are backed up with highly-competent CNA , Physical Therapy Food, Housekeeping , Administrative, Volunteer Staff, and most importantly Medical Personnel to coordinate with your primary doctor and respective surgeon. It's preally a home away from home, while you are on your way to recovery.
