Our loved one was treated with the utmost kindness and care during the final stages of his dementia. We were allowed access at any time of the day or night, for as long as we wished to be there. The entire staff was so helpful, kind, & caring of not only our loved one but the entire family. It felt like home with all the accessories you could ask for!!! The entire facility was ALWAYS immaculate, as was the staff. What a lovely place for our loved one to be able to spend his last days being so kindly cared for. Autumn Leaves is truly a God send!!! I wish I could give them all ten stars instead of just five!

Hi, we just moved to the area from Orlando. We were looking for a place for my mother. I dont know about you but walking into an assisted living facility, I need to smell clean!This place was clean! For sure! We met the director of nursing, Kam. She talked with us, showed us around and made us feel very comfortable with the transition ahead. My mom is currently in a rehab but look forward to visiting her at this facility in the future. Thank you Kam for answering all our questions and easing our minds. You are AMAZING!

My family and I did our research of the communities in St Charles and surrounding areas, when we walked in Autumn Leaves we immediately felt that this was the place for our mom. The facility was clean, smelled good, and we were greeted with warm smiles from the staff. Kam, the director, went above and beyond to help facilitate the move. This was not an easy decision to make to trust others with mom's care but she helped to calm our nerves and come to the understanding that this was not only the right choice for our mom but for us as well. I'm happy to report that mom is thriving. She's happy to see us on our visits. We get to spend quality time with her and still feel at ease when we leave. Thank you Autumn Leaves staff for providing such great care to our mom.

This is just a little note to thank everyone at Autumn Leaves for taking such good care of my wife Mary. You have kept her warm, dry, clean and well fed. Even though she has advanced dementia, I know that she benefits from the many mental and physical activities that you provide along with your kind words and friendly smiling faces. All the activities such as tai chi, bingo, trivia, storytelling, and interacting with outside guest performers are so much better than just to have her sitting alone in her room. Also thanks for the comfortable home-like setting that you provide with large comfortable sofas, plush chairs, piano, library, pleasant music, and fireplace in the Great Room. It has a friendly, calming feeling which almost seems like home. She also enjoys the large tv lounge and the lovely wind protected outdoor patio on warm summer days. I can't think of a better place for Mary; again, many, many, thanks for all you do. Rich K

Well over a year ago, making the decision to move Mom into a facility was the hardest decision our family has ever had to make. We chose Autumn Leaves because they concentrate on memory care with activities and socialization. It was a very good decision and we have been very happy with how Mom is cared for and treated. Many of the caregivers have been there for a long time and are exceedingly patient, kind and caring. The nurses are amazing. The office and support staff is responsive and knowledgeable as well as kind and caring. The chef provides excellent meals. We are able to visit and participate in activities whenever we wish. There is a monthly support group for families that has proven very valuable. All in all, I recommend Autumn Leaves.

Mom is so happy and contented at Autumn leaves of stc. Their care has given her a sense of well being and stability. She feels alive again and her fears have lessened. A special shout out to debbie in marketing she's such a dear. Melanie has decorated, soothed and even painted nails while covering her management duties. The activities director is so kind and patient. He often serenades the residents on the lobby grand piano. The nursing staff works so hard keeping their patients healthy, clean and calm. Autumn Leaves of St. Charles really understands the needs of people living with dementia. The administrators, the nurses, the care-givers, food preparers, the maintenances workers, offer the residents one-on-one attention. They are kind, respectful and always professional. The rooms are clean and decorated with the residents personality in mind. It is a smaller community. Everyone knows your name, be you resident or visitor. Theres always an activity for the residents to engage and the staff very encouraging. I highly recommend this community to others.

My husband has been at Autumn Leaves for just over a month and Im very pleased with my choice of facilities (and I visited all within a 10-mile radius of St. Charles). The staff is very caring; from the facility director to the activities coordinator...to the nurses and aides....to the restaurant staff. While the residents dont spend very much time in their rooms, the bedrooms are large and well-appointed. I was initially attracted by the smaller size (single story with only about 40 residents give or take) and the layout which is easy for residents to navigate. The common areas are bright and airy and welcoming. Looking forward to enjoying the outdoor patios once the weather warms!
