Good people. truly care for you while staying there. I witnessed it often with the residents. I believe my care was beyond superior. Therepy staff challenged me.which is what I needed. Facility was very clean. This is tough to only name a couple of staff members. Victoria and Armani. Are incredibly helpful and responsive. Thank you for the superor care I received. They work as a team, to assist their residents. They made my stay tolerable. I am happy to home, but I truly will miss the incredible employees. Please tell therepy I will move slowly:)

My first week here was hell. It's getting slightly better but needs improvement. No one sits down with you to explain the ropes. You better know all the questions to ask. Staff is rude and I have waited excessively long for call lights. Lots of bad attitudes. I was told I can always choose to go somewhere else. I don't know about you but that seems terrible to me. The rooms are small but clean. The food needs work. Most days I supply my own.

Thank you for speedy approval from OSF to PA. Working with Abby the DON and Theresa was nice pleasant interaction with speedy service. When a patient has no one at home to care for them but then when facility such as PA jumps in and can help care, it really helps. The family just have some ease with the transition. Michael, the Admin and Abby genuinely care for the patient as if its there own family. Also, Tanya the CNA is a gem ’. You are lucky to have such a kind hearted staff! Keep up the great work for our patients and our community.

I was very pleased with my experience. The staff was wonderful very hard working and very friendly girls helping me. I actually loved my 6 weeks there. PT was awesome..Nicole,Dana,Jen were so fun to be with and really got me up and on my feet! Gave me something to look fwd to every day Loved them. I actually miss my stay but home was good to go back to just too quiet. Would highly recommend this facility. Would definitely come back if needed. Happy holidays to all. I would never place my family member here. The front desk lady is rude and nasty. The staff have no idea what’s going on. One of the worst smelling facilities I have ever walked into. There are a select few staff members that seem okay. But I will never place my family member in this disgusting place! You shouldn’t either if you have the choice!!!

Been here for rehad after surgery afew times and a time or two with help for unhealthy wounds. Yes there are good staff members here and one or two bad ones. The bad ones get weeded out fairly quick. Now as far as the food goes. I've been there and had just blah food and have been there and had restaurant quality food. Again, turn over in staff. Good employees that know their job and ones that don't get weeded out. Anytime I have had a complaint I went through the chain of command, head nurse on my floor, management on my floor right down to the director of the facility. Always within an hour but no more than three hours they take care of problem. I had back surgery and spinal fluid leaking out my back. Nurse and staff checked in on me every 15 minutes to make sure I was OK and not getting brain damage from lose of spinal fluid. I soiled myself while bed ridden and staff cleaned me up everytime with in a few minutes of alerting them. My room was cleaned everyday by friendly housekeeping staff. No complaints there. I had P/T 3 times a day 7 days a week. It was exausting but necessary. Is this place the very best? Maybe not. It definitely is not the worse. Remember, if you see a problem, report it to the head nurse, that's part of the reason they are there. Be nice and not mean to the staff. They are doing the best they can do. And remember, if you don't like a facility ... you can always goto another one.

There is a nurse there named celena clark who is a wonderful woman! She not only does her job. BUT SHE LISTENS TO THE RESIDENCE FAMILY! SHE LOOKS AT YOUR EYES WHEN YOU'RE TALKING. SHE TALKS WITH THE RESIDENTS GRACIOUSLY! BUT MOST IMPORTANTLY.... SHE GOES JUST A LITTLE UP AND BEYOND OF HER WORK DUTIES TO MAKE SURE THAT THE RESIDENTS IN THE FAMILY ARE HAPPY. THANK YOU, CELENA CLARK! I APPRECIATE YOU! I had called and asked about something for a resident there. I was told that I would have to wait and speak with somebody, because it was the weekend. Celena, started the process early to make sure tell my family member was safe and content, knowing that somebody was helping her to feel comfortable.

I came to them after being in the ICU for 3 months. I was unable to walk, dress myself, or do any of the things needed to be home. Now on oxygen I was barely able to stand without becoming extremely short of breath. They fast tracked my care, I was doing sometype of physical or occupational therapy upwards of two or three times a day, every day. The staff was wonderful, caring, professional. They were this way with the others in our groups too. I remember a car they have in a large room. Just to teach us how to get in and out. When it was time to finally get out I rang the bell. Letting everyone know I was ready for the next steps. When it was time for my discharge they made certain that my wife had what would be needed for me at home. I can't thank the staff there enough. For helping me to get back home to my loving wife and kids.

I want to express my gratitude to this facility.... especially the staff. My father was until yesterday a resident on the 3rd floor dementia Alzheimer award. He actually was driving there he felt that he was worked there and was carrying to everybody that was there and they allowed him to do that. The girls there on the men too let him do that they give him a sense of duty when he would act out they were kind and loving and when he had a fall I was contacted and they stood by the whole time they took care of my mother while she was there and they actually cried tears in my arms and held me they were always willing to give us love and support and they answered every time I made a phone call. It means a lot to me that these girls really treated him like their own father I guess for lack of better words but they were able to be soft kind gentle and kind of make it seem like his idea when he didn't want to do something but they could talk him into it and he would comply he love them and that was very comforting to me when I went home to go to sleep every night. They're grooving along with myself and my family over his passing but it was time it had nothing to do with the facility they were there to the end and in fact I bet they're going to be at the funeral actually... I really have faith in them and I think as long as you're diligent too on let them know that you are present for your family member that is there they will be totally totally loving an extra special as they are anyway but I really appreciated them and I made sure that they knew that and I think they appreciate that. It's so easy to cast out negativity and be upset because things aren't going the way they should be but I really generally a hundred percent feel that this place on their employees really really do care. And I will say thank you to them.

After a long month of being treated at UW Health's (Swedes) ER/ICU for covid pneumonia. My dad was admitted at PA Peterson at the Citadel from Oct 22 -Dec 8, 2023 for the purpose of health recovery and yes one on Park View Ave in Rockford, Illinois. He was still in very bad shape and needed a lot of care for physical, mental and specialty care for newly found illness acute kidney failure. Dyalisis treatment was required three times a week. I would very much like to Thank Sherri from social services and her entire staff (NP, Nurses, Hemodialysis Technicians, Physical Therapy, Receptionist, patients, cafeteria cooks, cleaning staff) for doing a Great job. He was very well taken care of and we could come visit anytime. My dad especially liked the physical therapy and dialysis techs and the fact that he could have his room as warm as possible. My dad was discharged on Dec 8, 2023 and is doing Great! He's even driving now and at the present time does not require dyalisis treatment. Thanks be to God and PA at the Citadel's entire staff. Merry Christmas to all and my dad being healthy is the only gift I need. -azj

nurses and aids and helper are all very wonderful..but my daughter had had her room changed a few days ago and everything was in boxes..nothing had been put away..she has clothes missing and her laundry had not been done since she first got there and that was over a week ago..her blanket from home was still in a box and the TV has no remote and I think it's broken anyway..the girls work hard and are very sweet but must be stretched to thin..we put her stuff in the dresser but still some cloths are lost..I am distressed about that..we'll be back soon and I hope the lost clothes show up.

Husband was here for rehab. Only had rehab maybe 3 times in 3 weeks. Leg lifts only. Was told of more but no they lied. Sent him home ,saying insurance would not pay for more. He was treated badly.

The staff was extremely accommodating and very friendly. All the nurses and nurses aids were great and so were the PT’s and OT’s. They all went out of their way to make my stay as pleasant as could be. I’m glad I had chosen them to transition myself into getting well enough to go home.

The nurses were the only positive thing I can say about this facility. They did seem to care. Every person in the business office, front desk, or basically anyone in other departments were inconsiderate, unresponsive, and just did not care. It's like they didn't want any business, or because of Covid they aren't being checked on so they're lazy. So disappointing in a retirement facility that are supposed to be there to care for our seniors.

I just visited a family member on 7-28-18. I was ashamed that her family has their family member in this place. The room has wall paper coming loose in multiple locations. The window looking out is overgrown with ivy growing all over the building. One corner of the room has a plastic cover to protect the corner from being damaged, but the plastic molding is broken in such a way that someone could be injured or clothing torn. I took photos so don't deny it. My wife was present when the family member was brought in and the room was that way 4 months ago. How long does it take to get minor repairs and upkeep done? I couldn't believe that the TV Cable in the room was just nailed to the wall. I was also present when their food was being served in the dining room and was appalled at the filthy condition of the hand washing sink in that area, as well as the nasty condition of the working water fountain. Your elevator could use some work. I have those photos as well. You know the building looks okay from the street, it's right across from a golf course and hey you have huge fancy TV mounted on the wall when you step into the 1st floor lobby. I've been told by the immediate family that it costs $7,000 a month. For that kind of money, you haven't impressed me. 7-30-18 I did email you immediately after I listed this review. In my email I stated that I do no want to email back and forth. I gave you my name and my cell phone number to contact me. I'll keep checking to see if you answer my email or contact me. 8-19-18 Still no contact from anyone at this facility. I left you my name, email and phone number. You did remove the vines from the window. You did replace the corner protective strips, but that was a safety hazard and you could have been sued for any injuries. However, the wall paper is still not corrected. You only have 1 working elevator. The 2nd elevator has not worked at all and it has been this way for over 4 months. Why?? If there was a fire, there is no way you could get those patients out.

Some of the staff are really really nice and caring but some of them aren't sometimes takes over an hour to get a call light answered The meals are horrible. Most the time we don't even know what is on the plate. My husband was given oatmeal on a plate at breakfast they don't give milk or sugar or salt or pepper with the meals the food/meal menus need fixed asap .I have to bring my husband's food to him daily

This facility is by far one of the best in Rockford, IL. They have warm, empathetic, caring staff. The building is an older building with so much character and is just beautiful. The facility is by far not what it used to be. You can look around and see that at one time it was the apex for what you want for the care of your loved ones. Now that a Chicago based heathcare company has brought their quality of care to Rockford it has declined in it's appearance and the CNA's aren't much help. that is not to say they are all bad, but a few can make an impression on the majority view by those visiting. Perhaps they are underpaid and over worked. The nursing staff (RN's) are still vary good aswell as the therapy staff. It is only slightly better than being in any other nursing home. The true score would probably be 2.5 stars but they don't fraction the total.
