My friend Creighton was in rehab at Accolade in Paxton for 3 months. He asked me to relay this review: The nurses and CNAs are very nice and helpful. The therapy staff was excellent and helped me a lot. It’s a nice clean place and the food is decent. I will add that my friend is extremely disabled, and this stay is the first one that was exceptional compared to other facilities he’s stayed in.

My name is Chris, I am 46 years old, and the journey that I have been on this past year to date, lead me to the Paxton Senior Living Center. To go back in time, I sustained bilateral quadriceps tendon rupture in October of 2024. Multiple surgeries later and I am able to walk with the assistance of a walker and leg braces . In November of this year, 2025, I fell and landed on both knees, this ended up causing a staff infection in one of knee joints. I went to the ER, had surgery, when my stay in the hospital was completed with, I went to Paxton Senior Living Center for acute rehabilitation. The rehabilitation was geared to help me gain strength back so that I could go back home and live on my own. I was not to sure about going to a facility like this, but after speaking to a representative of the Paxton Senior Living Center that visited me at the hospital, I decided to go. She, the representative, said they have a great rehabilitation program there. So, on November 26th, 2025 I was transferred there from the hospital. As soon as I was brought to my room I was greeted by the staff even the director of admissions (Holly). The nurse that completed my intake was great and understanding of my situation. From that day on, everyone of the care giving staff, the food service staff, the housekeeping staff, the administration staff greeted me by my name and was always concerned about my wellbeing. The second day I was at the facility I was evaluated by OT and on the third day I was evaluated by PT. I arrived on a Thursday and the following week was in OT/PT rehabilitation. The CNAs and Nurses made sure that all my medical needs were met as needed. I had PT/OT Monday through Friday. The facility itself is very clean, housekeeping cleaned my room and bathroom everyday. The meals provided by the food service were good. The medical staff was great including the nurses and CNAs. I would recommend this place to anyone in search of care facility as such. I was there for a total of 20 days and it was really sad to go. It felt like everyone there kinda became family.

Mom has been in and out of the hospital for several months and each time I have requested and been fortunate to have her placed at Accolade of Paxton Senior Living for rehab. The staff are wonderful and attentive. The speech, physical, and occupational therapists and their staff worked with mom each day to get her back to a place where she could return to her home. Any questions or concerns I had were responded to quickly. I recommend this facility to anyone who needs to find a facility for their loved one to rehab.

Mom received great care from the physical therapist and occupational therapist almost daily while there for 2 wks. The staff were kind and also provided many opportunities for activities and socializing. They are the reason mom was able to return to her apartment in a senior community.

Even with her busy schedule, she always answered my calls and provided updates with professionalism, patience, and care. Her knowledge and attentiveness gave me peace of mind knowing my dad was in good hands. Every staff member we interacted with displayed an impressive level of professionalism, knowledge, and heart. Brought my 97 year old Mother to the Accolade health care in Paxton inAugust of this year. I have been very pleased with the care she is receiving. Whenever I come everyone is very welcoming to me and all the nurses seem to know my mom and show lost of love for her. There is away someone around. I feel she is never left unattend . My Aunt was taken under the care of the Accolades Team (Paxton, IL) on July 5th in the rehab unit. Over the next 45 days, the care and attention she received during this time was exemplary. From the day she got there until the day she left, I felt complete confidence that she was getting the best care and medical support possible. The social services staff were phenomenal in getting her settled through a very efficient and organized process, and her nurses treated her like family. Everyone I encountered made everything as easy as they could for me considering I live several states away. A huge shout out to Lane (Social Services Director) and Jordan (Activity Director) who were so unbelievably supportive that I don’t think I could have gotten through this journey without them. After my Aunt left for the next leg of her journey, I really missed communicating with Lane and Jordan and the guidance I received from them. Their support meant everything to me. Their activities staff cares deeply for the residents and creates a space where their involved in several community activities: the Easter Egg Hunt for children of families and community, trick-n-treating at Halloween .... the wonderful Christmas Dinner and program. The residents are blessed to have a wonderful staff that cares. This is our family’s second experience with Accolade and both times PSL has done a wonderful job caring for them. Thank you! Families considering long-term care facilities might be discouraged by the appearance of the facility, it might seem old, but the caring of the staff and the activities, they engage the residents in out way the appearance. It’s what's inside not outside that counts.

When I first got here my start was not off to a great start, but as time went on, I got to know more of the staff and the better it got. Most days the food was good, of course there is always going to be a few dishes that are not the best. I then met the activity director Jordan, and he was a ball of fun! He highly encouraged me to attend the activities. At first, I was hesitant. but I stopped by one day and enjoyed the Music Trivia game little did they know I was a Music Buff let just say I scored 2,000 points. The Therapy department was wonderful, honestly the only thing I can say is THEY WERE AMAZING! Overall, it's a good facility is their room for improvement, of course there is, nobody is perfect and anybody who says they are, are lying.

Accolade Healthcare of Paxton Senior Living has been the source of my elderly father's care for the past many months. This is our first experience in dealing with a skilled nursing facility and while it's been a learning process, the administrators and support staff have been wonderful to work with, always treating us with respect, and showing kindness and compassion towards my father. I firmly believe that contentment and satisfaction with a skilled nursing facility depends on family being fully involved in care decisions, while exhibiting patience and understanding, since staffing can be limited at times (especially during a pandemic) and all of these homes have to deal with the balance between what families want for their loved ones while maintaining the rigid requirements of state regulators. Everyone we've communicated with at Accolade Healthcare of Paxton Senior Living has been courteous and responsive to our requests. Dad is always clean and presentable, he is well fed, and since he's immobile, they take the time to make sure that he's out of his room throughout the day at various times to spend social time among the other residents. I also appreciate the way the staff shows humor to my father, since he has always been one to joke around with people. It makes his days brighter. You've all made his new temporary home, a place where where he feels comfortable and taken care of and I'll always appreciate that.

My father stayed at Accolade for approximately one month for rehabilitation after hospitalization from a fall. The release from the hospital happened rather quickly, late on a Saturday afternoon. However, the staff at Accolade responded well and were able to make him comfortable, even with such short notice. Unfortunately, he went into hospice care late in his stay at Accolade and died shortly after entering hospice. We found the staff to be very supportive, responsive and they did everything they could to make him comfortable during his last month of life. While this was not the outcome anyone wanted, we were glad he was in Accolade's care at the time. The staff was very clear about his treatment, they were quick to reach out to us and keep us informed, and they did a fine job of explaining the expenses and insurance coverage for his stay -- ahead of time. We recommend you ignore the mediocre Medicare ranking and go to Accolade for the care, which was excellent.

My 90 year old mother came to Accolade during the pandemic after breaking her hip. As the family member attending to the situation from afar, I was pleased with the attentiveness and care given by all, from the front office to the business manager to the nursing staff to the activities staff. As mom recuperated, she enjoyed the food, participated in activities, and was generally well cared for. Under the difficult circumstances of lockdown, it was reassuring to know her physical and social needs were competently met.

In July of 2020 our mom was admitted to accolade senior living on Fulton street in Paxton, IL. She had suffered a severe stroke after living a very independent lifestyle and always maintained contact with what seemed hundreds of friends. We were very concerned about her adjustment to being placed there for therapies, along with the strict rules of visiting her with the COVID pandemic. As it has turned out, we are very happy with her stay at this facility. All of the employees at this facility are absolutely wonderful with helping mom and making sure her needs are met. The hardest part has been our fear of not getting to see her due to Covid, but Jordan Duffin and many others have devised a way to make that happen. He has arranged visits for us and moms friends from the beginning along with adhering to the strict Covid rules. It has been great and we are so grateful for the hard work he has done to make this happen. Thank you so very much Jordan! You are the best!

My mom was at Accolade on Fulton for 3 weeks for rehab and had great care. Jordan, the activity director, set up straight talk so she would have a phone while she was there. Thank you Jordan! This was crucial for my mom because she has low vision and we couldn’t visit because of Covid. She was scared and confused and everyone there was so nice and treated her with the utmost care. Anyone that came into her room would dial my number so she could talk to me. I could hear how the staff would talk to her roommate and it was always sweet and caring. One night was particularly bad and Shawna was there to settle my mom down. Thank you Shawna! Idk names, but thank you to her rehab team as well. I highly recommend Accolade on Fulton.

Went to visit friend. She hadn't been showered in 3 days, also found several pills of medication laying on her chest. My friend was satisfied with food. Her night shirts are missing after 2 days. Used to work there yrs ago and quit due to behind the scenes care and concerns. Doesn't seem like much has changed, unfortunately. Its now 5 days later. Still visiting friend after 1 week. Things have gotten much better.. The facility smells and looks clean.,so I raised it 2 stars,and staff did seem more helpful.
