I truly hope NO one is giving this facility a 5-star rating. My son was a patient and died there. Of course, not their fault. I will give you an example of what happened. I show up to visit. My son's room smells like urine. His urine bag "exploded" and was all over the floor. When I asked about this "situation" I was told "the CNA" was JUST there and checked on his urine bag. REALLY. I think not. If you value your loved ones, send them elsewhere. Not that any other place would be better,

Everybody is very nice and polite and take good care of my mom. They listen to my concern and make sure that things are taken cared. Shout out to Stivy and wound staff, Macy, Tess, Shene, Agnes, Neli, Dr. Singh, Felix, Raymond, Houda, Sonia, Rosetta, Rick ,Sylvia , moon ans Yonny and countless others that cannot think of their names. My best friend is a resident of Zahav now and we sat out on the patio today. The other residents were lovely and when it's time for me to go into the home I think I'll chose Zahav, however, I'm only 56 so hopefully not too soon

This facility is excellent. I've seen great improvement with my husband. I am working two jobs and I know that in my absence , my husband is in good care. Jenny , Sarah the nurses always gives me update. Nelly from administration is so nice to me and very easy to talk to. Tess the assistant Director is always there for any questions or concerns. I would also want to thank the CNAs and Agnes. I recommend this facility.

My Experience at Zahav was the best from me gaining another resident as my best friend too many more friends. The residence were friendly and fun. We attended activities every day Barb and Angie gave us lots of fun things to do and activities that made us want to participate from management Tess and Nelly answering every question and needs that I had, and was very attentive, not only to me, but I seen with others to my favorite Nurse Neena who was always getting this done in a Timely matter to my absolute favorite CNA Bridget who treated everyone like we were her ONLY resident and laugh and spent time to get to know me. I can say i recommend this facility 100 times over the caring and compassion of everyone here makes a difference. - Helen PointDext

As an employee, Zavah is a great place to work! I travel over an hour to get to work, yes there are lots of facilities to work at in healthcare, but I'm very picky, as a person who has a 83 yr old mother with dementia and bedridden, I look at places can I see my mother or any family member in the facility as for Zavah I would check the yes box, from administrator (Nelly), management, DON ADON, nurses, CNA's and therapy, activities even housekeeping and maintenance . I see everyday where people gain more from the help of everyone one here. The residence might not see it for themselves, but as an employee, I reassured them from where they were to where they are now So much and how much they have improved. Everyone is team players and very professional and compassionate. I love my job and what I do and Zavah has made me want to go further. They respond to residence needs and employee needs to find a solution. Thank You for letting me be a part of the team this is definitely a second family to all~ Bridget CNA

When I come to check on my aunt, she's always appropriate and clean.I want to thank the ADON, she's a people person and she fixed whatever problem arises. Zahav takes excellent care of the residents. The nursing staff are top notch. Physical and occupational therapists increased my stamina and strength to where I now work out daily. I call myself a therapy junkie. The therapists work with the residents on the skills they need. That's what they did for me. In Activities we celebrate all the holidays even the more uncommon ones like Mimosa Day and Ice Cream Sundae Day. Access to the heads of all the departments is available for all residents and that is highly unusual. Having been in several rehab and nursing settings, Zahav is the best.

My name is Lashawn Washington.I came to this facility really scared because I don't want to be in a nursing home. But staying here for almost 2 months changed my perspective of a nursing home. The administration headed by Nelly , Tess and Justin always listens to my concerns. The dialysis people are very kind , his name is Alex and his assistant with the red hair , I forgot her name. The therapist are great , Moon is compassionate . I love Chammy , I call her my baby , she is giving me extra care when she gives me a bath. The Social worker Agnes is always checking in me and arranging my discharge. Thank you all for a great care.

I highly recommend this facility. I have been here in this facility for a year and I have nothing but great experiences. I want to mention the people that always goes above and beyond, let's start with Monsarat, Theresa the CNA, theres Two Theresa, Nurses Sarah , Jenny, and Joel the CNA , Sony the CNA is great too. Raymond and Cristine from Therapy. Tess the Assistant Director of Nursing is very helpful and funny . Elizabeth Nurse too is very helpful. Nelia the administration is very accommodating. Cheryl the receptionist .I hope I did not forget anyone.

"My daughter is attending activities at Zahav of Des Plaines, and she thoroughly enjoys the group activities. The staff, including Barbara (Activity Director), Angie, Anju, and Danny, are all loving and caring. Their support brings joy to my daughter, especially during times when she needs someone caring and attentive. Thank you so much to Nerissa, the assistant admin, consistently provides exceptional assistance whenever I have concerns, and her approachability makes it easy to discuss any issues. Additionally, Agnes from social services and Carlos, the first-floor nurse, have been very helpful. Thank you to the entire team."
