My husband had a very severe stroke and has been here for over a year. Most direct care workers are pretty good. I know they care but they seem to have limited resources. Loretta is an absolute rock star. She cares, she listens and she always tries to help.

Lady I lived with was sent here after a hospital stay. Nurses were sweet and caring but the facility itself was run horribly. No communication, can't get in contact with anyone and sometimes are just hung up on. They almost didn't give her her blood pressure meds or insulin (as they don't get the patient meds until the day following being admitted) until I had to call and demand she get the meds she needed. Again not on the nursing staff, they were awesome and the only staff to help in any fashion.

I just want to thank the 3rd floor staff for the amazing care they give my loved one on a daily basis. From nurses, cna's, housekeeping and activities. They are honestly superstars. Colleen the supervisor is always running around helping wherever she is needed and making sure residents are well attended to. Thank you all for loving Jack as your own family.

The following CNAs are excellent: Loretta, Jayden, Jada, Martha, and Maria. These nurses also give great care: Lucas, Viola, and Denise. Restorative staff are all great, as are the Physical Therapists. Administration should be let go.

My mother was there over 2 months for rehab then waitlisted pending medicaid for assisted living. I will say the nursing staff, CNAs and everyone caring for her directly were excellent. I would like to give special attention to Devonte, Melrose, Alivia, Lucas, Diamond, Marisol and especially Lorretta. Lorretta really cares for her patients and can often motivate them to do what they need to, she also really brightens their days. These people are the reasons you can be sure your loved one is getting cared for. Now for the bad. I could never get a copy of her OBRA form from the administration, I kind of needed that to get her to assisted living. The Medicaid person finally contacted me 2 weeks ago, after I had already navigated the medicaid process myself and had already applied for my mother, forwarding on the application to the facility. Medicaid is slow here, just do it yourself. Another sticking point is the social worker Gabriel. I think he is doing his best, I hope, and just has an unmanageable too large case load. The state REQUIRES a care meeting 72 hours after admission. I was not able to get one scheduled for almost 2 weeks, then it was cancelled as I arrived because people from the state were there, which is strange. Contacting him was difficult, and he never responded to emails or voicemails. The documents for the assisted living were not sent over until April 29th. Let me tell you he did call super quickly after I advised of discharge though. We set a time and date and no surprise here, the nurse did not have the discharge paperwork, it had to be tracked down. If a facility ever needs to be selected, this would be ok for my mother because she enjoyed Loretta and Devonte so much as people and I could tell they cared about her as well. Your parents have rights, call the Ombudsman if necessary, I did.

My mom landed at Meadowbrook after basically being kicked out of other places that said they were not "equipped" to handle her. Meadowbrook from the administrator to the social worker to the activities director, to the nurses to the CNAs to the PT and OT people...to housekeeping! My mom was treated with a lot of affection and respect. She died with dignity. They understood the disease and were able to handle the type of outbursts that you see with this disease. The place isn't pretty. And the food isn't great but the care received was above and beyond what was expected.

The nurses Viola, Denice, Terrence Joy Diana are the best they took great care of me and all my needs. They are all awesome the physical therapist Adrian was great the CNA's Chanel Christina, Loretta April, Oscar Felicity Maruk Alexis, Alexa Jada Benard were great. They took care of me got me whatever I needed. Rosa and Jessica are totally awesome Dr. Mohammed and nurse practitioner. Melissa are the best adon Kourtney is the greatest if I forgot anybody, I am sorry you guys are all the best and the greatest. I would recommend this place to anybody thank you guys for taking such great care of me.

Here are the staff that make this place run smoothly, efficiently and with heart: Colleen. Barbara, Rochelle, Tina, Manny, Charles, Faith and Trudy. Consider yourself blessed if one of them has their attention on you. The place would be nothing without any one of them. There is an atmosophere of love and caring and it is directly stemming from these nurses and CNAs. I am especially grateful to Colleen for the many times she has gone above and beyond for me.

The nurses and CNA staff are fantastic at this facility. I am a little less impressed with the fact the MD staff HAS NO CONTACT with the patients primary medical doctors. The changing of medications, without consenting the patients Primary care doctor, cardiologist, nephrologist and other specialists, is absolutely unacceptable and potentially dangerous to the patients health and well being. On a more positive and personal note, I want to comment the CNA staff and the CNA staff supervisor, Loretta Smailis, for their tireless work and continued excellent care, of my Mamma.

The long and short of this review is this: take your loved one somewhere else. My father spent 10 weeks at Meadowbrook following a fall. We did not have a lot of time to consider options for rehab when he was released from the hospital. We were limited to facilities that had openings and accepted his insurance. It seemed most facilities had some bad reviews and Meadowbrook was a more convenient location. We figured, "How bad could it be?" It is exactly as bad as the reviews said. We should have heeded the warnings. Rather than chronicle every offense my father endured during his stay, I will summarize the grievances: ”The facility is poorly managed. The top administration is horrifically ineffective. There is an overall feeling of disorder and disorganization. The lack of leadership has a ripple effect throughout the entire facility. Policies are inconsistently enforced (are masks required or not?) and there seems to be no discipline or urgency among staff. It was not unusual to see staff sitting around on their cell phones while nearby patients were ignored. ”The facility is understaffed. The nurses and CNAs complained regularly about their high patient load due to lack of staff. This results in poor care for patients and a demoralized workforce. On countless occasions, I witnessed patients, including my father, wait for more than an hour to receive basic care like pain medication or toileting assistance. This is particularly dangerous if the patient is vulnerable and unable to move on their own. ”The communication is astonishingly poor. This was especially problematic during the transfer from the hospital but also throughout my father's stay. No one seems to be on the same pagenursing staff, PT staff, social workers, external agencies, families. It was not uncommon to ask a question more than once and get different answers each time. ”The food is awful. I know institutional cooking is a challenge but nutrition is a key part of healing. We had to supplement my father's meals heavily to prevent him from losing any more weight while he was there. I would be remiss if I allowed you to believe there are no redeeming qualities to this facility. The physical and occupational therapists were great. Their tireless work helped my father gain the strength he needed to be released. There are a couple of nurses and CNAs who were absolute rockstars. They gave the best care possible under very trying circumstances. These people are the reason we didn't transfer my father to another facility. If your loved one ends up at Meadowbrook, know that you will need to check on them daily. You will need to advocate for their care constantly. Between my mother and siblings, someone was with my father for several hours every single day. I'm sure they were relieved when he was released because we were relentless. It should not have to be that way. If you have other options for care, use them.
