Sunrise at Five Forks has spent the last 7 months caring for my elderly mother. She first started in independent care & then moved to memory care with their assistance & professional concern. I had the joy of finding Sunrise at Five Forks. My husband was there for 7 months. He just recently passed away. I could not have picked a better place for him to be. From the very beginning when I first entered Sunrise, I felt a warm and inviting feeling and greeted with a wonderful staff. Everyone there in every position were so very kind and very helpful. I could see that what was most important to them was the care of my husband and all of the other residents there. I visited a lot and I saw a lot. I saw a place where my husband was very well taken care of and loved. The food is delicious and an incredible amount of choices at each meal, served in a lovely dining room. I enjoyed eating lunch with my husband and getting to know some of the residents there. There are many different activities planned for each day to meet the interest for all. There is a newspaper printed for each day. Outing are planned frequently. I met the most remarkable people in those 7 months. It became a family for me. I can't thank each and every one of them enough. I know that I gave my husband the best choosing Sunrise at Five Forks.

I have limited experience at Sunrise at Five Forks but I did have a wonderful tour with Greg and was very impressed. The facility looks first rate, clean, well organized, and well kept. The staff appeared engaged with residents and were friendly. The facility is located in a safe, quiet area. My mother lived in a few other local senior centers and I would rate Sunrise at Five Forks head and shoulders above those.

Our mother has been a resident at Sunrise Assisted Living for 2.5 years, and the experience has been outstanding. We initially moved her from another assisted living facility in a different metro area, and the difference was apparent immediately. From the moment we walked through the doors, we were struck by how genuinely friendly and welcoming the staff were”a warmth that has remained consistent throughout her time here. The level of care our mother has received has been exceptional. The meals are of excellent quality, and the staff maintain her room and the entire facility in immaculate condition. Staff is excellent and friendly. Food was really good. Room was nice and overall floor plan is good (not a long walk for anyone with mobility issues). My only negatives are minor and mom was only there for about 2 months so we couldn't settle into a routine. 1) billing is based on a daily rate so it's hard to budget for fixed income and the online bill payment system from corporate in KY is confusing. 2) mom used Express Scripts Pharmacy for her military discount and it appears that the local pharmacy vendor Guardian filled a couple of prescriptions intended for Express Scripts. (I haven't even started working on the dispute with them yet.) Overall I would still STRONGLY recommend Sunrise for your loved one.

The care my husband received was very good, which was my main objective. The care manager and head nurse were extremely helpful and available to an anxious wife when needed. The staff treated my husband with dignity and respect and did everything within their power to tempt his appetite, keep him comfortable and make him feel at home. The activities with other residents were a source of stimulation and I was pleased to have him participating in that in so far as he was able. He was in memory care and that is more complicated than assisted living. The staff treated all the residents with great patience and preserved their dignity and I found that very comforting.

I have worked with Greg Lawrence, who has been so pleasant and accommodating, to place our mother at Sunrise. Placing a loved one , a mother, can be very stressful but Greg calmed a lot of fears. Sunrise at Five Forks is beautiful, clean with a warm atmosphere. My niece worked at Sunrise in another location in Atlanta. She was so impressed with Sunrise that we pursued this one at Five Forks and have been very pleased. Mom moved in! We moved our mother into Sunrise about a week ago. She is 97 and cognizant with some dementia. I stayed with her two nights and 4 days. The care is exceptional. Each caregiver is conscientious, patient and understanding. professional. The atmosphere is so welcoming and the residents are happy, enjoying companionship and fun .

Sunrise Five Forks Tricum is an amazingly wonderful place. My husband, who has Alzheimer’s Disease, has recently spent a short stay in the Memory Care Neighborhood. The staff are all experienced, knowledgeable, long-tenured, and extremely patient and caring (with residents and families!) My husband received individual attention and a specific plan of care to fit his needs. The meals are nutritious and served in a family setting. The activities staff plan fun and stimulating projects for those able to participate but are always available for extra encouragement for all the residents. All of the staff: administrators, nurses, caregivers, medication and activities assistants have made my husband’s stay a home-away-from-home. And including support staff not directly in Memory Care, I felt embraced and supported. I would highly recommend Sunrise Five Forks Tricum as a Memory Care Facility.

My Mother-in-law moved into SunRise at Five Forks about five years ago and the team that cares for her have been wonderful. There have been changes to the personnel but each time they have gotten excellent new team members. Veronica, the Executive Director, and her team do a fabulous job of caring for all the residents. Millie and Linda are so compassionate and help out where ever they are needed, in addition to their assigned jobs and treat everyone like they are part of their family. I would like to thank the staff at Sunrise/Five Forks for their hard work. They work hard to make the residents comfortable. My father has been there almost a year. He has had health issues that seem one after another. The nursing staff has worked hard on the various issues to get the medications correct. The Events Coordinator has worked to get participation from my Dad. Dining room maintains a special diabetic diet that he enjoys. Not to mention the receptionist, cleaning staff and hall workers that keep him straight. Also a plus that the coordinator checks in with me periodically just to make sure all is good. It's nice to rest assured he is enjoying life to the fullest, in a caring and safe environment.

Covid restrictions were difficult for residents, families and staff, but everyone tried to make the best of the situation, while keeping the residents safe. Communication with families is excellent. The staff is also very friendly.

My mother lived here in the Memory Care section (Terrace Level) from Aug 2019 until she passed in June 2020. My father could not have chosen a better place and based on our experience, I don't hesitate to recommend it. The staff and the quality of care was exceptional both for my mother's needs and ours as a family. She was treated lovingly as an individual and always with dignity. She was carefully monitored and the staff communicated well with each other and with us. The slightest change in her condition was noted and treated appropriately. We visited every day, always welcomed warmly, until COVID-19 rules restricted visitation. Even then, the staff were in touch with us almost daily. After my mother passed, we were very touched that the section director and one of my mother's nurses came to her memorial service. Several staff members called in the weeks following to check on my father. This sincere concern for us just underscored our belief that my father chose the best place for my mother when the hard decision was made. If you are looking for a safe place for your loved one, especially in need of memory care, take a look.
