My husband, who is in late stages of dementia, was there for a week during respite. Upon calling everyday to speak with him each employee answered the phone with an upbeat voice, knew where he had been earlier, and had just seen him rolling by the desk. My husband was always happy sounding and had things to share with me during our call. There are no smells in this facility. No bleach, pine sol, urine, or bowel movement. However, when we picked him up on Friday the smell of fried chicken was wonderful. While there when dropping him off for his stay I saw an aide navigating a wheelchair in front and behind her; she and both ladies she was maneuvering to activities had big ol smiles and were giggling. My husband’s medication was gone over throughly with me by the discharge nurse, and she gave me a hug bye. I do not know why this facility’s rating isn’t a 5 star. My experience certainly was and I am awaiting eagerly when my husband’s number 3 on the waiting list moves up!

The 2 CNAs that worked days were great. The activities director, dietary manager, and the nursing director (not sure of her title but her name is Marie) are very nice and tried to help my father. However, the doctor vetoed a UTI test I requested (and the nurse said would be taken care of) for my father without any communication with me and my father ended up in the ER a week later. When we arrived, no one had notified the floor staff we were coming in and no one came to give us any information. I was told by a nurse that a service was not available and the social worker later offered the service to us. It seems like a lack of communication exists overall, or at least between management and staff. It wasn't the worst place, but it wasn't a great experience.

I was very against my grandmother ever being in a nursing home, but the way everything has been handled here is truly from God! Curesa is awesome at organizing activities that includes the families. Mary Beth is a ray of sunshine and helps tremendously. Any problem that has arisen Joy or Charlotte has handled it! The nurses are good and the custodial staff is always cleaning and super friendly when they see me. If my grandmother isn’t at home with me, I’m glad she is here!

The students learn valuable caring lessons and connections from the interdisciplinary team and the patients/residents themselves. We also appreciate how clean the facility is and that there are daily recreational and therapy activities for the patients.

Intake wasn’t good. We met the ambulance bringing my mom to The Oaks from the hospital for rehab. She was put into her room with no instructions, orders, advice, or any information around 5:00 pm. When we asked what the process was, information about meds, etc, the nurse just told us we were free to go and everything would update the next day. The staff were never hurried, or seemed to be bothered at times when we asked for help. My mom developed a UTI while there that the staff said could take 8 days for results, so her symptoms (including delirium and chaffing) got worse and worse. We finally decided to have mom taken to the ER when one of the staff said she developed an odor- which we assumed was infection. Pretty clear mom was not bathed or cleaned properly while there. We had her transported to the local hospital where antibiotics were started immediately. Prior to her transport, a staff nurse was asked by the paramedics what’s the reason for the transfer. The nurse stated no reason, just that she was having surgery follow up. We had to relay the fear of infection. The nurse then questioned why we wanted copies of all my mom’s medical records, as if we were bothering her to get them and we shouldn’t need them. The paramedics even said stated the nurse should be reprimanded for this behavior. Some of the staff is kind and helpful, but most just seem to move slowly and show zero empathy. But families of patients shouldn’t be made to feel belittled or insulted when just asking questions about care of family.

Awesome team. It started off rocky, but EVERYTHING WORKED OUT AMAZINGLY. Having a great attitude coming in, makes things happen for you. My husband was treated exceptionally well. I would highly recommend the rehabilitation services. Christie and Joy were very pleasant to work with. The PT’S did what they needed to do to get my husband’s stability together. My husband complained about the food, but I explained to him that this is a rehab, not a 5 star hotel.

I really don't like visiting nursing facilities, but I went with my girlfriend to visit her mom. I was really surprised at the cleaningness of The Oaks! The office workers were very friendly. If I ever have a family member that needs rehabilitation, I will try to get help admitting them to The Oaks. I was very impressed with my visit.

My mom was there for a month 30 days . She fell and broke her hip. They treated her with respect and dignity. The Rehab got her back in shape in 30 days your hear me. I RECOMMEND THE OATS. The only reason I am giving two stars is because the facility itself was clean and the nurses my father had were amazing, even the grumpy one. There are a few women in administration who should rethink their roles, in my opinion. We were told multiple times that my father, who's health is deteriorating and was recovering from total hip replacement surgery, completely immobile and was nearly immobile before getting to the Oaks, that he "can do these things (therapy, use the bathroom on his own, feed himself, etc) he just won't". We were also lied to multiple times, information was withheld, referrals were not sent to other facilities when asked, and we were threatened with adult protective services. He has since moved to another facility thankfully, where his paperwork states he needed no help going to the bathroom (which isn't true) and they had him on a pureed diet (also not true). He also acquired two bedsores while under their "care". The last situation was the Director of Nurses telling us to consider hospice because my father wouldn't eat and was severely dehydrated. Keep in mind that if we didn't come and fill his water cup, he had none. If we didn't help him eat, he didn't eat. The food was just left on the table and taken away later, no help was offered to feed him. There are so many things that could be said about our experience with the Oaks. And this was our second time in the facility for rehabilitation. If this is the best facility in Carrollton, its only because of the cleanliness and nursing staff. NOT administration. I'm sure they won't be happy to see my name attached to a negative review, but I do not care. They did not care about my father and that was evident.

The outside grounds looks better than the interior of the rehab center. The rooms are dated badly. I found the majority of the staff was nice but some were not tolerate it appeared in doing their job for the patient. They injured my dad's arm moving him in a lift and didn't inform the family. It was only when I noticed a bandage on his elbow did I get a report on the injury. They did not bath him when it was time, I had to inquire and complain to get him a bath and a shave . The medical staff in my opinion did not try to make his pain in his body at a reasonable level so he would be able to complete his therapy. The administration was cold and all business and I was told several times when I inquired could they control his pain in his knees or hip better so he could do better in PT but I was told that he was not sent to them because of his knees or any other pain but he was there for PT. They should change their signage and remove skilled nursing because I didn't experience much skill or caring. If you have other choices where to go for PT for yourself or your love one, go there! I can't believe that this place is affiliated with Pruitt health! I'm not sure how they got 4 star reviews based on what I saw and experienced with my dad.

The staff at The Oaks skilled nursing facility exceeded our expectations! They were very professional and caring. Staff members took the time to get to know our mother and customize her care to her individual needs. From the moment we had our intake meeting until we left the facility, we felt that we were in the very best setting for Mother to make a full recovery and return home to live independently. Each member of the team very lovingly attended to her needs and wants. Should we ever need this level of care again, I hope The Oaks will have a bed available.

They just got a new Executive Director Robert Day and just the past couple of weeks has been a major improvement to the building. He is willing to take the time and listen to family concerns and actually address them and get results. If you’re looking for a place for a loved one don’t hesitate on trying here. The staff and faculty at The Oaks have been amazing to my mother. The decision to place mom was an emotional and difficult and took months however once we found The Oaks we knew it was the perfect place for us. They made the process smooth and they were compassionate. They have become family. The nursing staff are kind and professional. They always communicate and explain every aspect of her health in an understandable manner. They go above and beyond to take excellent care of mom. I cannot say enough about The Oaks. It is a calm and loving environment for the residents and I thank them every day for being the great place is is.

Great up to Friday 17th. Bobble called her sister and said she needed to go the doctor. I called one of the nurses and ask I needed to call 911, she said no Bobbie was fine missing her family. That night they found her unresponsive and called 911. She still in intensive care. Need to pay attention to their patient.

The facility is very clean and smells good. The clients looks happy and the staff seems very nice and concerned about the patients. However the food is bad. They need to take more pride in what they are serving. People are already sick so you don't want them to feel even worse. They vegetables are cooked to death the potatoes are hard the cabbage stinks. In the last three weeks the only good thing I notice about the food was the ham was good the ice tea tasted good and the dessert was decent.

My dad is currently at The Oaks nursing home! This is my dad's 2nd nursing home. I come every day to check on my dad. My experience with the first nursing was not a good one, the place smelled like urine, I would walk in to find my dad soaken wet. I just can't say enough about the staff at The Oaks, so professional and very caring. The nurses, CNA's, maintenance, cafeteria workers and even the custodians are super sweet and caring. What is so impressive is how clean and how good this facility smells. I really feel it must be hard to accomplish having a good smelling facility. To me I feel like the staff is one big family. The staff is very helpful and considerate. If you are looking for a nursing home I highly recommend The Oaks!

I am one of the fortunate people who gets the opportunity to see the true inner workings of long term care facilities all over the state as a medical vendor. I must say this is one of the greatest facilities that I’ve ever been to. Their staff truly works as a team. They have the best attitudes and are eager to help in any way. I stayed at the Oaks for most of 6 weeks. No staff members have the covid, or residents. Thanks go out to members like Sarah and Amber and the team. The laundress was so kind.The afternoon shift was beyond compare on how they treated us with compassion, humor, and listening skills. Nurses Denise and Tracy-Stacy are out of this world! Miss Kimberly short and strong deserves an award if nothing else for her quick response and understanding. I stood at least a foot over her and she put what she had to help me. Christy thank you for the do's, I have since cut it all off. Ms. Pat, the director goes around, from her you can get the best coffee! I thank her for requiring the CNA wash my hair and body because of the way the hospital left me. A physical therapist can teach us many things. Jackie and Edwina ( OT), there are no word I can express how grateful I am for your help. Abby and JR, thank you for responding to my concerns.Those names I have forgotten round this place out as the #1! I would highly recommend the Oaks.
