I was real nervous at first because of bad experiences in the past with other facilities, but was pleasantly surprised how easy this facility and staff made the process for me. Easy and fully explained and walked me through the entire process from start to finish - that was huge for me. The food is a 4, but everything else was fantastic. The CNAs and nurses were some of the greatest. Nora the therapist I worked with was fantastic - and therapy was so amazing in my recovery I cannot praise it enough. And Jonah my social worker was extremely helpful because I was nervous about a discharge - and he made the process easy and explained it to me, he sat with me and listened and understood my concerns even while he was busy and made sure I had the discharge papers in my hand. Katie the nurse was fantastic and attentive, she was very helpful with getting things I needed. Anytime I needed something she made sure it was taken care of. The CNAs were very good and helpful, even though they were busy they were always polite to me. All in all. I highly recommend!!

Thankful for the care and support given to my loved one at this facility. The CNA’s are hard working and very professional in performing their duties. I am so grateful for their service. Management answered all my questions in a timely manner and this made me feel comfortable and confident in choosing this facility. Mrs. Kendria Williams-Bennett (Business Office Manager ) is an amazing lady. I met her at my lowest point and she absolutely did everything in her power to make things happen and I am forever grateful and thankful to her. She is an outstanding human being and an asset to the facility. God bless her for her dedication and commitment to getting things accomplished for the needs of the patients.

I want to start this review by saying I have had the unpleasant experience of my mom going to other nursing facilities. That is why I requested Villages Nursing & Rehabilitation in Lady Lake Florida from the doctor who performed my mom’s hip surgery. She broke her bone, she was an active 91 year old person. She had stayed there once before to be rehabbed, quite successfully. I also asked her case worker at Leesburg Memorial Hospital to place my mom there as well. I was afraid of what kind of care she would or would not receive elsewhere. I thanked God they could take her. I want to publicly thank Amy and Amanda in Social Services. They were a phenomenal key whenever I had a request, an issue, or even a complaintwhich were few. My calls were returned, my messages were answered, my husband’s calls and requests were answered, respectfully as well. Peggy, the director of nursing, was patient and responsive to our inquiries and concerns. She helped us greatly when we had an issue concerning my mom’s care. This was addressed immediately. My mom’s Physical Therapists, Tanisha and Inez were kind, caring and thorough, obviously dedicated to their job. Tanisha even gave me her phone number, in case I had any requests, questions or concerns regarding my mom’s care. She genuinely cared about her patients! Nora, yet another physical therapist was also involved greatly with my mom’s recovery, my mom really liked her. She worked my mom hardmy mom walks without a walker or cane again because of these three physical therapists! Ruby in admissions helped us keep track of my mom’s Medicare Day limits, as my mom therapy was getting close to the 100 day mark. The Unit Manager Kimberly, got my mom a new phone within hours when I complained I couldn’t reach my mom. This might seem minor, but it is not! When I couldn’t get ahold of my mom, her prompt response meant a lot! And lastly I much mention Dustin, my mom’s RN who she adored, was always positive, polite, friendly, very knowledgeable, always taking the time to answer my questions. He even brought me a sandwich, as I traveled the entire day to be there, without a meal. Justin, I believe his title was LPN. My mom spoke highly of him as wellagain taking the time to answer questions and to be reassuring. Village Nursing and Rehab’s Physiatric Nurse, Darcy, also called me with my mom’s report promptly after it was completed. I cannot remember the young lady’s name that helped me from one end of the facility to the other, with my mom and her luggage, getting her into my rental car, she went above and beyond! Wow what a long Google review! I will never see or talk to these wonderful people again, as I have moved my mom to a permanent senior living facility in another State. The only reason I am reviewing Village Rehab and Nursing of Lady Lake, is they deserve it! Other people caring for their mom or dad should know about this place, they would want them placed there for recovery, as well. The people who care for our elderly family members are truly Angels, performing the toughest jobs there are, compassionately. Thank you all!

Upon our initial visit with rm 404, gloves, medicine container and tissues all over the floor around the waste can near the door as if staff just threw it at the waste container. Bedside can had no liner in it but was empty. Dirty soiled laundry was on one of the visitors chairs. Saturday morning staff and house keeping came in to take care of the issues. Water in cups was from day before so we asked staff to dump, fill with fresh ice and water and took it back in to patient. Breakfast tray was still next to bed and staff removed it during our visit. We have noticed staff at least 4 just outside of rm 404 chattering at nurses station twice during our visits. Patient claims he calls for assistance and can never get someone just outside his door to respond. Patient claims he fell off bed and doesn't know how long he was on floor until help arrived at 3am.

She made me feel like a human being, making me laugh and lifting my spirits. My family member was admitted here a few days ago. I have had poor experiences in the past with skilled nursing facilities so I had a low bar set, however, I was hopeful that the good reviews here were warranted. Indeed they were! Each and every staff member that we have encountered over the last 3 or 4 days have been so caring, quite professional, & very timely in reaponse. My family member has cognitive problems but the staff have adjusted accordingly, without complaint, providing safe & appropriate care for her. I would feel confident telling others to send family here as well. Thank you to the staff for all you do for the people here. You don't just say nice things - you do it! My partner has been very ill which has led to a long stay at the The Villages Rehabilitation and Nursing Center. His stay at their facility has been covered by Medicare thus far but in 30 days his insurance coverage will run out and he will have to go on Medicaid for the remainder of his stay. Applying for Medicaid can be an overwhelming experience because of setting up a durable power of attorney, establishing a qualified trust and setting up all the accounts necessary for Medicaid approval. Fortunately for me, I was directed to their Business Office Manager, Kendria Williams-Bennett, who helped me get the paper work done in a timely fashion, helped guide me through the Medicaid process, the bank accounts necessary to get Medicaid approval, as well as answer a litany of questions I had regarding how the Medicaid process worked. She even went so far as to write directions for the steps I needed to take and the order in which they needed to be done. She was there for me whenever I had a question or concern and I am so appreciative of all the guidance she provided during this very stressful time. I

I was only a visitor but my friend indicted the she was being treated nicely and professionally. Her rehab was great for her and she was very happy with every treatment. She also has happy to have her nails done and was waiting to have her haircut. I was happy at seeing her being content being there. She said that everyone there was very nice to her. Furthermore, she was happy with the menu. Just want to add, she is a 95 or old woman who has all her faculties and deserves being treated well. Amen.

The facility itself was first class. I was disappointed in the amount of staff available to assist the patients, definitely lacking in number. Several had very bad attitudes, unwilling to assist if the patient wasn't "their" patient. They would just ignore your plea for even a slight assist.Most were kind but definitely overwhelmed. The food was not good and went down hill as my stay continued. I did appreciate the chef salads. The Physical and Occupational Therapists were fantastic, every one of them! They encouraged but didn't let us be lazy either. Generally the facility was clean and tidy. Only once did we have to request that the shared restroom have the floor cleaned which was taken care of right away. I was VERY disappointed in the money spent to honor nurses and staff. Balloons, tee shirts, and all the extras for the two weeks would have been better spent providing bonuses or additional staff. You would keep staff longer if you paid them more.

I can’t say enough good things about the physical therapy department here. All of the staff is wonderful, very informative and encourage you while working on making yourself better. My Personal/Occupational therapists were Tess and Rebecca both who were wonderful to work with, and didn’t act like robots, they actually engaged in conversation and made you feel strong. They were very flexible when I needed to change my schedule and worked with me on my special needs. Can’t recommend the physical therapy department enough, but especially the two ladies I worked with the most.

My moms stay at the Villages rehab was good. It served its purpose to help mom get back home and on the road to recovery. The PT dept was excellent, led by Peggy, and facilitated by Shakira, Nora, Theresa and Jennifer The food services dept was also very good. Michael, Juicy was professional and personable. He made mom laugh. Which was very important! The custodial staff was outstanding keeping the room clean! Thanks to the great lady (I can’t remember her name) on the 500 wing! Samantha, with her colourful hair and bright smile, brought joy to everyone and planned the greatest activities! The reasons that you didn’t receive a perfect score is that the communication between the change of shifts CNAs and RNs was lacking in my opinion. It made it hard for consistent care. It seemed that we had a different person for every shift. They weren’t familiar with my mom’s injuries or pre existing issues which would have helping in aiding her. It was better when Kerry and Yvonne were on duty. Also, Wilma, Sher, Selena, and Brian were very good. Patricia was also a good leader and patiently worked with my stepdad to get things done. The CNAs, whatever their ratios were, seemed slow to respond. It would help to create a system that would help the CNAs to be able to check to see what the patient needs when they press their button and to communicate when they can return to assist. At least then there would be some time frame. Obviously the more emergent needs would come first. Lastly, it might benefit the facility to have mandatory customer service training for all staff to periodically remind them of the care and compassion the patients needs in a very difficult time of their lives. I would like to also thank the front desk and administrators (Sue, Amanda, and ) for their help. The only issue was that the admission forms were brought in to be signed shortly before mom was discharged. I thought that it was a very delayed action on the business offices part. Overall, the Villages Rehab Center was a good place to help mom attain her outcome to heal enough to return home. Thank you for your attention to this comment card. Sincerely

This facility presents very well on the surface, but our experience raised serious concerns about the actual level of care. To start with the positives”the building looks and smells amazing. Housekeeping does an outstanding job and absolutely deserves recognition for maintaining such a clean and welcoming environment. The therapy team is also exceptional”knowledgeable, attentive, and clearly dedicated to patient progress. Unfortunately, that’s where the positives end. Communication with social services was poor and frustrating. Getting clear information or coordinating care felt unnecessarily difficult. At discharge, no proper paperwork was provided, medications were left sitting on the patient’s dresser, and morning medications were not administered or communicated as missed prior to discharge. This lack of coordination and accountability is extremely concerning. More concerning was the lack of basic personal care. There were repeated instances of soiled linens not being changed, and linens were left in place with dried urine stains. Bed was left unmade, and at times there was dirt left in the bed. The resident frequently smelled of urine, increasing the risk for skin breakdown and other complications. On more than one occasion, a CNA/MA was observed watching videos on their phone/tablet while resident remained in wet/dry dirty clothing. That is unacceptable in any care setting. Meal service was also handled poorly. Due to a stroke and weakness, assistance with meals is required. Despite this being clearly communicated, food was routinely delivered without being cut into bite-sized pieces, creating a choking hazard. Plastic utensils were provided even though they were difficult to use, and no assistance was offered. There were also times when meals were left in the room during outings and remained untouched and still not prepared appropriately upon return. While therapy and housekeeping are truly top-notch, the lack of attention to basic care, communication, and patient safety is deeply concerning. These are not minor oversights”they directly impact a resident’s health, dignity, and well-being. Significant improvements are needed. I thank God I was able to tend to my relatives needs-others were not so fortunate.

The Villages has some excellent team members that deliver 5 star service. Ann the nursing unit manager listened to our needs and delivered excellent care. Katrina , Minnette went the extra mile for my Dad they ensured his linens were changed and he got to the dining area. , Johnny and Nora theOT/PT assistant gave us daily updates and delivered timely updates and provided excellent . The coordination care was fractured at discharge. The home care orders weren’t sent out for authorization . Plus, patients are evaluated weekly for discharge and not daily. The weekly evaluation was frustrating as my Dad should have been able to leave earlier than he discharged.

I have been here for about two weeks, and I am progressing well compared to how I first arrived. They have all been absolutely fantastic and I cannot praise them enough. Danny and Ines in therapy make the sessions fun and also very helpful which is exactly what I need, I wish there were more words to use to say how helpful they have been. Jonah in social services sees me in the morning and listens to concerns and fixes them immediately, and he is an absolute delight. Zamaria is one of the nurses during the day that I see and she's sweet and funny and you can tell they care. Sam in activities is so fun and funny, she makes me look forward to every event they do - she makes it fun and enjoyable!! This is a busy building but they get to you as soon as they're able and they follow through!

I once again need to commend the outstanding staff that patiently, lovingly, competently & consistently has worked with my sister, JoLynn Mazzari, Rm 506, who in the beginning was too weak & sick & probably confused to fully participate. But, particularly Chiffon(OT) & Inez(PT) incredibly helped her find her MOJO, for which I am incredibly grateful! She is now progressing & even eating more, despite nausea. The nurses Deb & Atrial (sp?)have done a great job. And her CNA’s especially Learissa, have been PERFECT. Administrative staff have all been willing to listen & counsel me, as well as JoLynn,during difficult times. My mom was brought here in early December, after some initial issues, the staff, therapist, dietary couldn't be better. 7-3 Katy/Laura (in the 100 section) 3-11 Mia/ Henry, and 11-7 Jorge/ Douglas did an outstanding job helping take care of my mom. Therapist Kelly PT, Shakita ot, are perfect at pushing there patients beyond there limits and listen to the needs. Dave the cook is outstanding and willing to help. The administrator, Lee was responsive when I had an issue. During that cold spell the facility spared no expense to rent extra heaters, keeping the facility warm. Lisa has the beauty salon, she's very personable, great beautician and competitive prices. Pumpkin, was outstanding, I fell and she helped me. The director of nursing was very helpful and Ashley is incredible. I would highly recommend this facility to anyone needing recovery

My father was there for a few weeks after being discharged. We were so excited for him to be going to a place that looked wonderful. The first few days were pretty good. However he quickly learned how short staffed they are. He cannot walk, so if he had to use the bathroom he would have to call for assistance to get a bedpan. During the time he was there he ended up having a mess in his depends, because they never got there in time. They moved him closer to the nurses station, I assume in hopes they could get to him quicker, which didn’t help. When they moved him, they lost his hearing aids. They grabbed the case, and the charger they were sitting on, but the hearing aids didn’t make it. Now we have to figure out how we can get that $3k back to get him a new pair. The social services person was somewhat helpful, but rarely returned calls. They ordered home health and PT, but they used a company that does not employ a home health aide. (Crown Health). I’m sure it’s a wonderful place for people who are mobile, but not for anyone who is unable to walk, or may need assistance with opening things to eat.

The place is very lovely.. The lady I went to visit today was upset because they gave her the wrong medicine.She note 428 hours before she could take any different one.Why did that happen. Hello my name is Yvonne Walker and I arrives at the Village Rehab with a Left knee Replacement Surgery from West Marion Hospital Center in Ocala on 12/17/2025 know because all of the staff members here at the The village starting with Physical Therapy and Ocupacional Therapy team for the good job that they are doing by giving you a fast recovery with the help from all the Doctors and Nurses keeping up with all the medication around the clock also I want to give thanks to Dr. Bair for the good care she gave me thanks to Ms. Wendy and the morning crew for breakfast and lunch it was always on time plus thanks to the nurses aid for checking in on me at all time making sure I had everything I needed also thanks to the evening team for making sure I always had dinner on time thanks to the nurses aid for checking on me before bed time and all during the night when is time for shift change. Know I m been discharged on 1/15/2026 Thanks to all the staff members from the village rehab and nursing center for a fast recovery. I give it a five star and I will always recommend this facility to anyone who needs rehab and nursing center for they love ones

Outstanding Care from the Physical and Occupational Therapy Staff at Villages Rehabilitation and Nursing I would like to express my heartfelt appreciation for the exceptional Physical and Occupational Therapy team at Villages Rehabilitation and Nursing. My wife, who suffers from advanced Alzheimer’s disease, was admitted here for rehabilitation after a stroke and a month-long hospital stay. The transition has not been easy, but the care and compassion shown by the therapy staff have made an incredible difference for both of us. A few moments stand out that truly demonstrate their dedication and humanity. COTA Clay, for example, went out of his way to make my wife feel comfortable by singing Christmas carols while performing her stretching exercises. His kindness and creativity helped calm her agitation and turned what could have been a stressful experience into a moment of peace and even joy. COTA Shaquita has also shown remarkable empathy and skill. During sessions that can sometimes be uncomfortable, she gently diverts my wife’s attention with conversation, easing her discomfort while keeping her calm and cooperative. PTA Nanette impressed me with her attentiveness and genuine care. While walking through the hallway to pick up another patient, she noticed that my wife was sitting crookedly in her wheelchair. Without hesitation, she stopped and helped realign her, ensuring she was safe and comfortable ” even though my wife wasn’t one of her assigned patients. That kind of thoughtfulness speaks volumes about her character and the culture of this therapy team. My wife’s physical therapist, Holly, has been incredibly patient and kind throughout all of her sessions. Her steady encouragement, understanding, and professionalism have made a real difference in my wife’s progress and overall well-being. I’ve spoken with many other patients and families at Villages Rehabilitation and Nursing, and everyone has nothing but praise for the gym staff. It’s clear this team provides not only excellent rehabilitation care but also heartfelt compassion and respect to every person they work with. I am deeply grateful for all that they’ve done for my wife and for the comfort they’ve given both of us during this challenging time. Villages Rehabilitation and Nursing is truly fortunate to have such a dedicated, caring group of professionals.

I really enjoyed my stay here. I came here for rehab after having a procedure done. The nursing staff has been great. Therapy was amazing my angel Adrian was a blessing and I enjoyed all the activities.
