From the moment you walk into this senior living community, you feel it — a warm, welcoming, family atmosphere. It doesn’t feel institutional at all. It feels like home. The new management team has truly made a difference. They are caring, approachable, and always available to answer questions or address concerns. Update: it is now 7 months since Dad moved in and we are still having a great experience. Thank you all! My dad moved into Diamond as an ALF resident a month ago today. He was in a different Assisted Living prior to Diamond as a result of left side paralysis after his stroke. He had almost nothing good to say about his prior living arrangements; however, I don't think I've heard him say one negative thing about Diamond yet. When he had to go to the hospital for a medical emergency, the staff were waiting to welcome him when I brought him back. I'm amazed at how helpful they have been. Diamond has a wide range of visiting specialists which makes it convenient for your loved one to get the medical care they need. The receptionist Derek actually returns voicemails (unlike prior establishments I've worked with), and the maintenance technician Chris went out of his way to help me with Dad's TV and other things. This facility is smaller (fewer residents) than where Dad was before and I think that helps. Somehow they actually coax Dad out of his room to participate in activities which, knowing my Dad's personality, is an accomplishment. I know Dad has only been there a month and it's hard to draw conclusions quickly; but given we have experience to compare to, I'm optimistic and hopeful that this continues to be a great home for Dad. Thank you Sandy, Chris, Chris, Veronica, Bree, Charlene, Derek, Jessica, Marty, and the rest of the team!

Crystal is an incredibly caring and compassionate person. She cares for her residents like her own family. Crystal and her team take the time with each family and resident to meet individual needs. Diamond ALF is undergoing an incredibly beautiful remodel with Crystal leading the change for a beautiful ALF with a warm family feel, welcoming new residents. Go take a tour, it will be worth it to see the wonderful changes!

Ignore all the old reviews. They have new owners and they have begun an amazing renovation of the building. The majority staff and volunteers have been there for years and make it a home for the next step in your loved ones journey. Exhausted by the corporate sales pitches? Crystal was amazing in our time of distress, making sure to answer any questions we could dream up and helping us with parts of the process that were completely foreign to the regular average everyday Joe.

**This facility is in EVACUATION ZONE A in case of a HURRICANE** Please ask very specific questions on how they are transported, and what type of mattresses are used, and where the mattresses are stored while not in use. This facility does not have its own transportation. My mom WAS a previous resident at this facility. DO NOT BRING YOUR LOVED ONES HERE, especially if they cannot communicate to you what happens on a daily basis. This Assisted Living Facility is under new management and ownership. Newly hired management is very unexperienced on how to communicate with residents or their caregivers. Follow-up is always necessary. The GOOD: -- The receptionists are very welcoming and go above and beyond to help you. --The activities department is a blessing to this facility. They spend countless hours outside of their jobs to plan events, and make sure the residents are happy. --There are only a select few GOOD employees remaining at this facility. They remain because of the residents. --The housekeeper does a great job. She is very detailed, and the room always smells nice after she cleans. The BAD: -- Management has created a very toxic workplace. This facility has a very high turnover. Poor management decisions, and constant changes have caused this dynamic chaos. Happy employees, Happy residents and caregivers. --The current management constantly blames the owners for its poor decisions. --The housekeeper does a great job, but the caregivers do not do their part to keep the rooms smelling good..... Dirty pullups, urine-soaked clothes laying around, and the laundry is not washed as often as they say. --The food is poor quality and bad tasting....mostly processed meat products and pre-packaged foods. The bread is always crusty and stale. Foods are high sodium, high carbohydrate, and low nutrient dense. Special diet accommodations don't exist. --Residents often go unsupervised for up to 30 minutes in the afternoon. This creates a very serious problem. A bunch of crazy stuff happens. Residents try to help other residents get up out of chairs, and push other residents around. Residents sit in around in urine soaked clothes while the caregivers argue over who is going to change the resident this time. The caregivers disappear to avoid caring for residents. --The night staff from 7pm 7am, do not check resident rooms every 2 hours as they say. --Residents are constantly moved to different rooms with different roommates, causing depression, anxiety, and confusion. The UGLY: --Caregivers do NOT change the sheets when accidents happen. They allow them to dry, and the residents sleeps on the same soiled sheets. The caregivers excuse....they wash sheets on shower days. Unacceptable. Some rooms smell horrible. --Diamond cannot get a handle on the huge roach problem!! I have seen roaches crawling everywhere. I have personally had to kill a few crawling on residents, and equipment in the facility. --The biggest problem...MOLD... In the air vents, shower room, and some residents rooms. No one has notified the caregivers and POAs of the issue. The maintenance person was instructed to suit up, mask up and spray the vent covers with bleach, so the appearance of mold was gone. He did the exact opposite. No mask on, and sprayed bleach on the vents with the resident around. He had no mask on, and residents were walking around the area while he sprayed bleach. There was no regard to resident safety. Regardless of the clean looking vents, the mold is still there folks. This is a beautiful facility, and I hate to see quality of care decline at the expense of the residents. They have been upgrading the facility by painting and adding bright lights to the space. It will probably be a nice place when all is finished, however, the lack of care will remain the same unless the facility starts valuing the most important assets of any facility..... Its employees and its residents.

We put our sister in diamond assisted living a little over a year ago and she has loved every second of it. When we first put her in there she was skinny and not doing good health wise. After meeting all the residents and staff she began to gain weight that she needed and instantly became more happy than she was living by herself. I cannot say enough nice things about the staff at diamond. All nurses and CNA's are extremely passionate, caring, and loving. As soon as we went to diamond I could instantly tell how amazing they were. I would recommend diamond over and over again to anyone. I have never seen an assisted living like this. It is hands down amazing. The staff, management, residents. very clean and put together. They also have activities for all of the residents almost everyday, they have a volunteer nail lady that comes in once a week and also a person to cut hair! If you are considering putting a loved one in an assisted living, diamond alf is absolutely the best one!

The facility is very clean and safe. Everyone there is very kind and attentive not only to the needs of the residents but to the family members as well. We had tried other facilities before Diamond and Diamond was definitely a blessing to our mother and our family. I recommend this for family members who need assisted living and/or memory card.

Why is there a lot of bad reviews on here, it saying that the staff smoke in side and that it smells, and that the employee sleep so do I have to come in there un invited to find out
December 2019
Our experience with Diamond ALF in Green Cove Springs, Fl has been positive. The staff is very attentive and the rooms and grounds are clean and attractive. The meals are good and mealtime always seems very inviting and pleasant. I'd like to recognize the special attention that Sandy, Robyn, Susie, Pat, and Dana give to my mother, and also say that the director, Jonathan has always been willing to go the extra mile in finding ways to help make things as comfortable as possible. My mother enjoys the regular activities offered to the clients, and she particularly enjoys having her nails done on site. The medical support is very professional, and regular visits from a dermatologist and podiatrist have been convenient. My mother and father lived together at Diamond when the time came that my father needed daily support. When my father passed away, we asked my mother where she would like to live. She said she wanted to go home. We asked if she meant her previous home or if she wanted to live with us in our home. She said no, I want to go home where I am living now, at Diamond. Tina McRa

I've come to know Johnathan and the staff there over time and at all times, they have worked towards doing what's best for the patients there. I have nothing but the upmost respect for the facility and what they do there. Constantly trying to improve the care and provide top quality care for patients at quite affordable prices.
June 2018
My great aunt stayed here a few months before being put on hospice. When I notified the staff in person about her passing another few months after being on hospice, they were touched that I took the effort to tell them in person. They took turns telling me stories about her for around 30 minutes. It showed the staff really cares about their residents and remember them. Also, the facility smells nice and is clean
