It is nearly impossible to express my appreciation for the amazing people who work here with patients who often have extremely challenging health situations. My 90+ year-old father has been in both this Columbine Health Systems facility and their other facility on Lemay Ave. He landed here for rehab after a fall and emerged a couple weeks later walking like a champ. I can't speak to the long-term care administered here, but the entire team in the rehab side of the facility was top-notch. The professionalism of staff extends across the board, from admissions to physical therapy, occupational therapy, nursing, doctors, case workers, finance, custodial, cooking, and more. Put simply, the people working here are the unrecognized heroes of the healthcare profession.

Typically I went to see my dad in the afternoon. Saturday July 22 I went in at 10:00 a.m. I found my dad without any clothing and flat on his back with no water or oxygen within reach. He was dying from congestive heart failure. Everybody knows you do not lay a person flat that has congestive heart failure. It was a complete nightmare and I removed my dad 2 days later. A pathways hospice nurse (Triva) was incredibly unprofessional and talked me out of removing him immediately saying he "has hours to live" which was not true. I do have photos of this but obviously I will not share them with the public. This place is definitely a place not to take your loved one.

The staff at Colombine were wonderful. I enjoyed my stay at Centre Ave health and rehab. Everyone was thoughtful and helpful. Please give my regards to everyone.

Although most staff seems friendly, they didn't seem interested in admitting my LO. They didn't seem concerned with her needs and requirements. Once I picked up my LO to bring her home, her cath bag was so full it was about to explode, she was left to sit in feces and they ripped her toenail off and nobody knew how. It was a heartbreaking experience that I refuse to ever let happen again. I do not recommend this place. I did give 2 stars because a couple of the nurses seemed to make a heartfelt impact on my LO.

They rarely have the same staff working with the same residents, this leads to isolation and miscommunication regularly. Have to call back at the beginning of shift changes to ensure continuity of care

My mother had a fantastic experience. The staff was so kind, attentive, and friendly. She just could not say enough about the care she got from every person. The OT, PT and doctor that attended to her listened closely and worked diligently to get her healthy. The nurses and aids were so kind and seemed to genuinely enjoy their work. And big bonus was the food was "sooo good." My mom inisisted that I sit right down and write a review.
