This building is the original Grossmont Hospital and they made a nursing facility out of it. The staff is so warm and friendly. You have to feel compassion when you enter they took the best care of my dear friend. So much care bear team going on. I found this place that was listed as care centers and we did walk ins to see which place to put my friend. We looked at three that were listed. The other two had all the bells and whistles we needed but this place although old, parking was very limited got me from the first greeting by Shelly. We visit my friend often and we are never disappointed. Very accommodating friendly and if my mama had to go to a place, this would be it

My daughter was there for 3 and a half months. they released her with a feeding tube in her stomach!! the night nurses [name removed] ns her sister [name removed] were horrible!! she asked for water [name removed] laughed!! my daughter had a hard time walking and needed help with getting to the bathroom. [name removed] sat at a computer and said to her what are you looking at? Daughter said I need to go to the bathroom. [name removed] laughed and did not help her!! Do not send your family member there!!

Community Care Center Review My 105 year old grandmother came to CCC for PT/ OT a week ago. The staff here is nothing less than remarkable. They are very attentive , kind, polite, and respectful in every way. Every single staff member we have encountered has been extremely helpful. The place is kept very clean and the residents are all well looked after. There is not an hour of the day that goes by that we are not asked if we need anything for our grandma.The staff is always checking on her to make sure she is kept clean, warm, and fed. I have seen them redirect wandering patients with compassion and kindness. We as a family are so thankful for the quality of care our grandma is receiving here. This place is one of the best places to have your love one. I visited several places and this is heaven.. I will recomen this place with my eyes closed. Speak with Miss Edna she will work something with you and place your love one on the best hands if you can take care of them. You will have peace of mind. Bless your hearts you ladies work so-so hard and it didn’t go unnoticed. To think I wasn’t going to stay here my first day . CCC has left an impression on my heart deeply. Here’s a secret I used to tell my mom&sisters I never wanna leave this place. I’m sure you guys/ladies can’t wait till 7:15- 3-15-11:15 LOL. Hope I left a positive impact on all of you. Would name a few special people here but that’ll be unfair to the rest of the staff . From the kitchen(Dietary) to maintenance, to the entire Nursing staff for the unlimited hot Chocolate ,PT,OT, can’t forget about Activity’s for everything,Social Services and Environmental services I’m forever great full. Together in synchronicity you all make this place work. I understand this Job can be mentality and physically demanding but hard work pays off Luxuries and you ladies understand that and for that I have the upmost respect. The staff is friendly and wonderful however having to work with lots of limitations. Supplies are often out of stock ie: wipes to be used to clean a patient when they need a diaper change, fresh laundry as it has not been brought up yet to replace wet linens changes Hello , no extra pillows are available when by accident yours disappears or is removed from you bed, creams for butt redness and irritation. The food is mostly all way over salted or not salted at all. Vegetables are all overcooked to mush with no salt. Eggs are also not salted. There is no salt provided with any meal you could add on your own to food that would need it. Unusual food combination: pasta with red sauce and ground meat is topped with cedar cheese? Flan came topped with apple sauce. Toast is never crisp, always served moisture logged, sometimes without fake butter and almost never with jelly at breakfast. The cream of wheat comes with no sweetner and dry breakfast cereal comes with no milk. Oddities to normal meal services. Staff is spread too thin. Someone can sit for an hour with a wet diaper and be told that your CNA is on break and will get back to you. When they do return to the floor they use a system of starting at one end and doing patients room to room down the hall instead of dealing with the ones reporting wet diapers. Worst case scenario a patient can wait almost 2 hours to be changed out of wet diapers and bedding. Staff are also forgetful and promise to bring you something or report on appointments that require transportation. If is forgotten it is not fixed and or the issue even addressed. OT and PT schedules are not shared with patients. A therapist will show up in your room and expect you to be ready to go to the gym. You never know when they are coming for you or if you will have therapy that day or not. Those are of the ongoing issues in this care facility that need fixing.

So my grandmother has been here for almost a month at the time of writing and I feel like it is time to post a review about this place. To start off: The staff are pretty friendly people, they are compassionate and seem caring for the family & patients (especially elderly). Food is just your typical nursing home food, nothing special or anything, they serve 3 meals a day and everyday is a different meal, and they also do dietary accommodations. Entertainment wise TVs are honestly pretty small which I think is weird but it’s not a complaint, they also have daily activities which I think is pretty cool. There’s a call button board right by my grandmas room which is REALLY loud and honestly seems like it can get really annoying especially for patients trying to sleep. Rooms are a little old and may be a bit dirty depending on the patient, rooms are also shared meaning that you or a family member may have/get a roommate. In Conclusion: It’s a good nursing home, there’s definitely a few things here and there that could be changed but care-wise it seems pretty good, but it’s only been close to a month so far so things may change in the future.

In 2021, I came here for rehab after an injury. It was horrible and a very traumatic experience. However, as of typing right now in 2024, I came in for wound care and since the change of ownership, this place is by far the best facility i have been in. It has completely changed, the CNAs are kind and quick to help and so are the nurses. I was unfortunately in seclusion so I can’t say too much about the activities but they did provide me with coloring and writing supplies. The food was really good and as someone with allergies they really accommodated for my needs. The staff was very patient and when faced with new things was willing to listen. The only thing i was annoyed about was the sudden choice of sending me home, it felt like there was a lack of communication to the people who support the person (ie me) outside of the home. It was handled but as i said, I think that was the only real problem i have qualms about. The lack of communication between the social workers and care team outside of the facility is the only thing that should improved upon. I recommend that if this happens, stay on top of them and they will help. This said, once I told them that I was a bit annoyed about this they jumped right on to fixing it. I highly recommend this facility to anyone needing rehabilitation help, I can’t express how amazing the staff are here. No one is perfect but the fact they listen and care is exactly why I think that this is an amazing choice for your loved one. (Written: Wednesday, February 21, 2024)

Community Care Center is a wonderful rehab. A very close relative was there and received top notch care. The staff was attentive to her needs and always with a smile. This was noted the moment we first walked in the door and we were warmly greeted by the receptionist with a smile. The staff in every department has been kind and caring. I was new coming to a place like this. At first I didn't know what to expect and I hated it. I was a little worried but after a few days of being here I became comfortable. Yes, you have to put your part into it. Take your most comfy tenis, PT clothes, pillow, blanket, snacks, sodas, etc etc etc. I couldn't agree with the food here, so I started ordering through Uber eats. This has helped so much. The rehab team (PT & OT) has been great and they are helping me get stronger so I can go back home. *after you learn how to live in a place like this, then you become comfortable. I had surgery on both my femurs and telling a 20 year old CNA's not to move you too quickly because it hurts, is almost impossible. I couldn't walk so I had to use a bed pan and trust that they would clean you correctly. They never did... I asked for some wipes and was told that they didn't provide any of this. I had to ask my family to buy me wipes so I could finish the job of cleaning myself off. The food was terrible , there was times when it was ok but I had to depend on Uber eats and door dash. I'd order items on Amazon and Walmart and got it delivered there. My wife would take me snacks and sodas because you could not get anything in there. There was some staff that really tried to help but they were few. The PT and OT staff was the best.
