I only have great things to say about the nursing, rehab, kitchen and maintenance staff. Management is a different issue. I wish admissions and marketer were more friendly to the patients and their families. The Administrator, who appears to be having an inappropriate relationship with the Director of Rehab, is very unprofessional. It’s highly unethical. Director of Rehab is benefitting from this relationship monetarily. Shame on SunMar for allowing this to continue. Admin cuts corners when it comes to the facility. SunMar manages this place and their CNE (Chief Nurse Executive) does not seem to care about the way the Admin runs this place.

Citrus Nursing Center has provided me with excellent service the business office is always willing to assist with any financial issues. The social services team is always willing to assist. The admissions team did a great job at addressing any concerns about placement and gave a tour of the facility. The Nursing team does a great job at assisting with care and communicating with the doctor.

My mom has been their quiet a while and she loves that place even tough I don’t live close to her she always tells me she loves the cna Destiny she always makes very good comments about her that she does a very good job with helping her with all her essentials and is a very caring cna to her and she adores her personality and does gets upset when Destiny isn’t working.

My mom has been in this facility for many years due to the amount of care needed to manage her ilnesses. I have seen both good employees and bad employees come and go. I don't live in the area so I depend on the staff to make sure my mom's mental and physical health are being taken care of. The activity department and majority of the CNA's are very caring. Especially the ones who have been there for many years and have personally cared for my mom. I would personally like to name a few individuals that have stood out to me who deserve some extra attention. Zulema Palencia has got to be one of the most caring people that I have ever met. She will go and see my mom at times when she is off or during her breaks to reassure me that my mom is doing good. She will also FaceTime me so I can see her. She should be given a pay increase for going above and beyond. Jessica Cintron is a nurse that also goes above and beyond. She reassures me all the time about my mom's condition. I am going to name other people and I hope I don't leave anyone out that is deserving. David Palencia, Mercy, Alice, Gabby, Reen, Pierson, Rosita, Rosa, Ines, Brittany, Kayla, Kyla, Miranda, Crystal and Jennifer. When I've had problems, The Administrator, Walter did his best to try to help me solve them. Thank you to all of the people who have made a sad situation more tolerable. The facility does the best they can with the shortage of staff. I love you guys!

Physical therapy mike and Linda are absolutely the best social workers are excellent with the exception of one name Linda who is rude and will destroy you outpatient paperwork the nurse are fun and pleasant the two top dogs Walter and yenni and excellent listener and make sure your very well taken care...I had a excellent stay would recommend this facility to anyone best so so far

My stay here at first was like the Twilight Zone . Had some medicine problems at first but once I was in the system and meds were plenty I snapped in ..The Nurses were groovy and my IV nurses were on spot at times...The office staff is awesome.. Kinds became my family after a couple of months there ...They treat the veterans like they should be treated! BEST NURSING HOME IN SOUTHERN CALIFORNIA... ’’

My dad has been at this nursing home.just like any daughter or son and patient are nervous about it. Since he arrive all staff made welcomed him well and made his stay super easy and comfortable. Any concerns or questions they always get answered with a fast timing. My dad left the first time very grateful for everyone and even made friends. Second time he had surgery and had to go there for a couple of weeks. He wanted to come back to this nursing home and with the grace of God he was able to. All staff remembered him and made him feel comfortable once again. He left the nursing home yesterday and let me tell you he didn’t want to leave. lol He made sure we gave this place good reviews and 5 stars. He is thankful for all the staff and residents. I hope he can stay as healthy and God protects him but if he ever needs to go to a rehab with no hesitation we will sign him to citrus nursing home once again.

Update: Because I was there so often visiting my mom, I started to see the bad things going on. There are amazing employees but there are employees that should not be around people who cannot defend themselves. I saw 2 of my mothers roommates being mistreated or not taken care of properly by certain employees. I spoke up and they stepped up at that time. They need to replace a lot of employees. Original review-My mom has been there for several weeks now. They have taken good care of her. Any time we have a complaint, it's addressed quickly. My mother has been given physical therapy regularly and is improving quickly after an infection made her extremely weak. The reviews scared me but we have had a good experience with my mom so far. They are going to get her home soon.

On my first day when I came and I’m admitted in CitrusNursing Center I am amazed.I know that I will had speedy recovery from my Lession Surgery and I need to PTEmployees are very friendly and always had a good smile Once u press the call they came in to your room.I am giving more than 10 stars. Let me start by saying, everyone’s entitled to their own opinion. I give a Big thank you!!! To all the staff that took care of my Dad Celestino Martinez jr. I can’t remember everyone’s name. The ones I can remember are Billy at the nurses station, Maria and Maria (Grace). Great Job!!! He didn’t want to come home. The times I went he was always clean shaved and always had clean clothes!!! Also he was always in good spirits!!! Once again thank you and Blessings to each and everyone of you!!!‘

When I first was admitted to citrus I was frustrated and concern related to my stroke and weakness. Nursing,case manager,social services,therapy,business office manager,dietician, maintenance all departments were cool and treated me fair. Very attentive to my needs.Staff provided friendly astomphere,knowledge,and assistance to all my needs and concerns. I would recommend Citrus to my family and friends. I am very pleased with the outstanding care I received at Citrus Nursing.I thank the entire staff for making my stay memorable and positive.

My friend was due to discharge today. I helped her get her things together and she has been waiting all day for the staff. She gets notified at almost 5 pm that she is not going to be discharged today. Waisted my time as well as poor communication. They need to do better.

My Grandpa was a patient at Citrus Nursing Center and my family and I had a great experience. All of the staff was kind and really seemed to care. I would recommend this place to anyone who wants for their family member to recover quickly. The only complaint that we had was the strict visitation policy but towards the end of his stay at the center they opened their visitation and we were able to see him without making an appointment.

This facility is how all facilities should be. The staff provided prompt, skillful and caring support to my dad for his short term stay. He was living on his own prior and the transition to needing more support went smooth, thanks to the nursing staff at Citrus. My family and I are so appreciative to everyone there.

I am a covid 19 survivor. When I came for physical therapy here at Citrus Nursing Center I couldn't get out of bed. Within 2 weeks I have relearned walking and my oxygen requirements have dropped from 6lpm to 3lpm. CNC has an expert staff of trained pros working 24/7 to help achieve independence. The food isn't bad either. I highly recommend this facility for recovery from hospital stays.

We were referred at this place for my mother in law’s rehab needs prior to Covid and it’s an awesome facility, very clean and inviting. The Administrative staff is excellent and the girls in the front office are very helpful and sweet. Great therapists and nursing staff, very competent and attentive to the patient’s needs. I will definitely recommend this place for any rehab needs.

I enjoyed my stay at the Citrus Nursing Center for several reasons, which I have stated below. First, I should say that I spent two weeks at Citrus due to suffering a stroke on my left side (I have never had a stroke before). When I arrived, I had no mobility in my leg and arm. This is the placement my health provider provided thus, we did not have chance to research, it was a true learning experience. Positives: Caring Nurses and CNA’s (Certified Nursing Assistants) even during the most difficult times Many bilingual staff Clean basic living and facility areas (beds changed daily) Food is what you would expect, low salt/fat. Appropriate portions Access to management when needed Excellent therapists, very knowledgeable (A+ for me) Excellent Covid-19 rules are in place for all who are in the building Staff always took my wife’s phone calls and deliveries Concerns: Time it took for Nurses or CNA’s to come and help/answer questions, I had to be patient. (I will add, the average CNA had anywhere between 12-18 patients to attend too at any given time. What I found, some of these patients required constant monitoring, thus other patients where left to wait, this also pertains to the nurses. I highly suggest to the management that more staff is hired due to the type of high-need patients Citrus serves) This facility serves both high and low functioning patients. At times, the needs of the low functioning patients had a negative effect on the ability of the staff to attend to the high functioning patients. Inside gray wall paint is very institutional (dark). Wall pictures of old Fontanawhy, who cares! All staff wear gray institutional scrubs (yuk). I viewed this atmosphere as counterintuitive to patients wanting to get healthy. Suggestion: a new paint job with positive colors, scrubs for the staff that are bright, cheerful, place slogans of positivity on the walls and hang pictures of famous people who have overcome tragic health/life events. Make sure to educate all staff about the slogans and pictures so they may share and possibly inspire their patients. Final thought: I have read the other reviews, I see their point of view. What I observed and this is from my point of view, families are dropping off family members and thinking that the Citrus staff has a magic-wand that will automatically make their loved-one well. This is not the case, the patient has to want to try and families need to be involved as best as they can (I understand, Covid rules make this difficult). For me, my goal is to be healthy, self-reliant and get back to what I consider normal. The Citrus made a difference in my life now I’m getting better! Many thanks to Citrus and thanks for reading.

I don’t care what anyone says about this place because we all have our own opinions. My uncle was here for several years and they treated him like a King. We made and developed many relationships with nursing staff. They were always good to him, answered all our questions during the Covid-19, provided hot meals, very clean and made sure he had his cell phone charged to receive phone calls from us. The nursing staff would notify us right away if anything wrong. My uncle passed away due to old age, and underlying conditions. This convalescent home will always have a special place in my heart for taking care of my loved one who is now with the Lord.

I've been a resident at citrus for about 5 years now and during my experience I've had a lot of ups and downs but mostly ups. Unfortunately when I first came in the staff wasn't very welcoming.. now that I have warmed up to everyone I've grown to have great friendships with them, staff and residents. Citrus felt like home right away. The people here really care about you. Like any facility you know your good nurses from your bad ones. At citrus I was able to go out In the halls to talk to everyone, I was able to go to the stores, I participated in activities like bingo and bowling and much more. Since covid started, all of that is on pause now. Things are different but I hope it's temporary. The facility itself is very clean and organized. As for the food, it's not going to be the greatest but it is definitely edible! Out of my whole experience and 5 years here I'd have to say I would definitely recommend.. no one's perfect but this is the closest you'd get to it.

My grandma was a patient here at citrus and unfortunately it was around the time citrus got covid thankfully my grandma recovered and is feeling a lot better.. the staff was very good with keeping me in the loop.. I knew when she had good days and bad days. My grandma has no complaints the staff did what they could and I couldn’t be more grateful.

My mentally and physically handicapped son recently spent two weeks in this facility to get rehab for a broken hip. I was with him daily for several hours. It is not the Hilton, but the best place to recover I have come across in my 82 years. I would highly recommend it. The staff is dedicated and friendly to everybody. The place is light and clean, the meals are appetizing. I am grateful for the good care my son received. As I mentioned before I can highly recommend this nursing home. I do not think there are any better ones. At least I have not seen any when I had to visit several of my elderly friends.

First impressions are lasting. No one at front desk to start, had to ask administrator for assistance, this facility needs to have better housekeeping, dust on furniture (alot), trash on floors, etc. The only good thing was staff. Very friendly and accommodating. Don't have much confidence in this place.

My 94 yr old father is currently at Citrus, and is receiving excellent care. Their pureed food is delicious, I've tasted it. Pork loin with herbs, mashed potatoes with gravy, green bean salad, chicken chili. He is learning to eat and swallow. Kaiser would have let him die, drugged and starving, because "he's 94 after all". WHAT?!!! Dr. Constantino was the only Kaiser Dr. that believed my father needed respect and proper care, and sent him to Citrus. He will be coming home soon, within a week. He is clean, shaved, receiving physical therapy and is doing awesome. I see him every other day, and am always impressed with the staff and the cleanliness of the facility. This is a good place for your loved one should they need to go to a facility like this. Highly recommend.
