We entertain monthly at Avista Senior Living in Prescott, AZ as The Phil and Mary Show. Every time we are there, the building is neat, clean and well managed. The residents seem happy and engaged. We enjoy performing there and interacting with the people.

we love the atmosphere at Avista/Evergreen senior living. they really seem to care about their residents. the staff is very attentive and responsive to their needs. they seem to have alot of activities to keep the residents happy. We are transferring my mother there as soon as we are able to. the food is the best of all the senior facilities in the area and their front office is efficient and easy to work with. i would highly recommend them.

We have served the facility for 3 years in transportation. Staff is always pleasant to work with as well as tentive to the needs of their private community residents. Always a pleasure to be able to serve the community and give back to those who served and are serving in other medical fields.

My wife and I toured many of the assisted and senior living facilities in Prescott before we moved my mother into Evergreen Village Prescott 8 months ago. We were all very pleased with it and strongly recommended it to my mother-in-law who then moved in 2 months ago, now also a happy resident. It is located on a quiet side street right in town, so it is very convenient to frequently visit. The facility is well maintained and attractive. The resident rooms are larger than those we saw at many other facilities. The dining room, lounge, activity room, library and many other indoor and outdoor locations to relax or commune are attractive. Both my Mom and mother-in-law have great views from their apartments. The list of the weeks' worth of assorted daily activities is prominently displayed at multiple locations, as is the daily menu which is distributed to each resident. My Mom and mother-in-law have said they are happy with the food and the various choices and changing menus; the menu always gives 2 choices for each item comprising each meal, and there is another list of foods that are always available. A facility bus goes to Prescott doctors' offices and some Prescott shopping locations such as Walmart. The facility hairdresser has many happy customers and stays busy. Since moving in, both my Mom and mother-in-law each have been warmly greeted by name when seeing staff members, all of whom are eager to be helpful when needed. Even the staff that cleans their apartments and takes their laundry every week knows them. The excellent nursing staff is aware of and remembers the healthcare status and needs of each. My wife and I have been impressed by the personalized warm greetings we receive by all the staff upon visiting. When my Mom needed further medical attention after being seen by the Evergreen Village nurse, the nurse arranged for her to be seen by a nurse practitioner that regularly frequents the facility who then referred her to the appropriate specialist. Though also readily seen as needed by the very capable and responsive facility nurse, the NP is now the primary care practitioner for both my Mom and my mother-in-law, which is very convenient for all of us. My wife and I are very happy and relieved that our mothers are part of the Evergreen Village Prescott community.

The facility is great and the staff is amazing. My brother and I chose Avista for our Mom because the facility was nice and her apartment was the best I have seen. She has a living room, separate bedroom, a patio with a great view (see pic) and a storage closet. We were worried about being in a different state but the staff at Avista is so helpful. We have constant updates on our Mom (non-urgent and urgent), they help her to Dr. Appointments, help her/us with prescriptions, helped a lot when she had a small heart incident, essentially help with everything. Everytime we talk to our Mom, she talks about how much she likes it there. We couldn't be happier and luckier with the support our Mom receives and the communication we get from Avista.

This place is amazing and the staff is so caring. The view is beautiful, the rooms are very nice and the dining hall is put together very well. All in all an amazing experience.

I am more than satisfied with the excellent care my Mother is receiving at Avista-Prescott. They keep my Mother healthy and happy with a truly collective and caring effort by their entire team. Her room is cleaned and the bedding changed weekly, trash is emptied daily, meds are timely and I am amazed at the daily variety of menu items to please all palates. The individual attention provided deserves mention…stopping in to remind my Mother its lunchtime, giving her yogurt out of her little fridge at breakfast to take with her meds (shes not a morning person!) and shampooing her carpet when needed are just a few ways they care for her and all the residents. They truly shine when it comes to keeping her active and engaged with the wide variety of activities and one-on-one attention. Theres the normal bingo, jewelry making and coloring but she can also learn sign language, attend dance get-togethers, meet for cheese and crackers and this week they are bringing horses to the facility to interact with the residents! My Mother is now addicted to Wii bowling, which for her is a challenge with her tremors. She is encouraged and receives patient and loving attention by the Activities Director and fellow residents to improve this muscle memory while she enjoys great camaraderie. I wish to express my sincere thanks to the director, Alyssa Branson, and her entire team who contribute daily to my Mothers good care at an extremely reasonable price.
