My husband was a patient at Fair Haven Rehab Facility from 6/26/2026 to 7/22/2026. The facility itself was very nice. It was the perfect location from my house since I went to stay with my husband from 9:00 am to 4:00 or 5:00 every day that he was there. The nurses, CNA’s and therapists were all very nice and helpful. They truly made you feel like they genuinely cared and were concerned for your well-being. He received Physical Therapy, Occupational Therapy and Speech Therapy during his stay. Prior to Rehab at Fair Haven, he had been in the hospital for 24 days. During his hospitalization the Speech Therapist detected that he was showing signs of Dysphasia (he couldn’t swallow liquids without aspirating it into his lungs. This is a very dangerous condition that can cause pneumonia or even death. Not good for an 81 year old man.) Because of this condition, he was not allowed to eat or drink by mouth and had to have a PEG feeding tube and receive liquid nutrients continuously 24/7. He was also very weak and could not walk. Due to the care, knowledge, exercises and professionalism of all the staff, he is now walking with a walker, has strengthened his arms and passed his swallow test and can now eat food by mouth. We are extremely pleased with the care he was given and the progress he has made. He gets discharged tomorrow after 26 days and couldn’t be happier. Many of the staff actually came to his room to congratulate him and say how proud they were of him. You don’t get that everywhere! We wholeheartedly recommend Fair Haven Facility to anyone in need. My dad was placed here after leaving his hospital visit at Princeton Baptist medical. Arrived on 3/6 for 20 days, however, due to my concerns over his lack of therapy, meals and hygiene, I was told to get him after 16 days. Whenever, I got him home, he was so weak and lacked strength that he couldn't stand at all! They kept him in the hospital bed as long as possible, probably thinking he would just die (Dad is 96). That is not their call to make but God's! I am thoroughly disappointed and disgusted at the place. Please don't send your family here!

Fair Haven cared for my sister. She received great care from the staff on 2 North (the memory care area). We had only one facility to compare her care with and the care at FH far exceeded our expectations from the previous facility. The staff was very caring to my sister. I would highly recommend Fair Haven.

My dad moved into Fair Haven assisted living and I found his apartment to be spacious and inviting. It made the transition to a new place really easy. The nursing personnel and staff were wonderful. My dad also spent some time in their rehab facility after hospital stays and they got him quickly back on his feet. He continued to have therapy once he was discharged back to assisted living. Everyone kept me informed and genuinely treated my dad like family. I highly recommend fair haven assisted living.

My father-in-law recently stayed in Fair Haven Rehab floor 2. Many people were very attentive to him and always treated him with kindness. This was certainly a hard time for our family going through this, but Erika the social worker and the nurses like Simone and Jessica made it so much easier on us. Therapy really helped my FIL. I would certainly recommend Fair Haven Rehab especially floor 2 I commend them for all of their hard work.

Mother was recently discharged from fairhaven rehab in room 6201 k2 . Throughout her time there she was well taken care of by staff . The cook Keke Coleman was very exceptional and efficient when it came to mother’s meals .

My husband's stay at Fair Haven Rehab went very well. The third floor team was very interested and caring to the needs of my husband. Both therapists, Josh and Henely worked diligently with him. Our family had a very positive experience with Fair Haven nursing home where my uncle stayed. From the beginning, the staff was kind, professional, and attentive to his needs. They treated him with dignity and respect, always taking the time to listen and make sure he was comfortable. The nurses and aides showed genuine compassion, and it gave our family peace of mind knowing he was well cared for. I want to recognize Tierika (The Cook) for her tasty food and kind, friendly spirit to my uncle!!!! The facility was clean, welcoming, and had a calm atmosphere. Fairhaven at Montclair Rehab is horrible. Also, UnitedHealthcare is even worse and kicked my Mother out on the 13th before her 20 paid days were up. It was okay in the long run as I'm fairly certain we couldn't have kept her in Fairhaven another day! PT has been good, but they started working on her balance and transferring herself from walker to shower today. Which she certainly needed more work on before coming home. Mother enter Fairhaven late 6-30-25 and they vacuumed her room once last night (7-10-25) at 8:30pm for the first time since she was in there. The cheerio that was there the night she checked in is still there today. I was told they clean every 2 to 3 days. Half the time they didn't bring her water until she asked or late in the day. She only got coffee a few times and was told once they didn't make any. Her bathroom was cleaned once after I complained and i had already cleaned a nasty floor, so she didn't bathe and then step unto the dirty floor. Mother asked for stomach medicine one day as she had diarrhea, but she never got any after asking for it. She called the first couple of days for help to the bathroom as the order was, she needed someone to walk with her as she was a fall risk. However, she ended up just going by herself as an elderly person can't wait an hour to go to the bathroom. Most of the food was nasty and mystery meat a couple of times with Green Beans almost every day. Her dinner today was a turkey sandwich. Her TV had to be manipulated every day to get it the cable to work. A couple of the nurses could get it on but several days she set and looked at four walls. On Saturday, 7-5-25 I was there and ask a nurse to fix it she said she could put in ticket, so I said fine, we kept talking and said it again and I said Well put in a ticket or go get her a different TV from a vacant room as she said they didn't all do this. I finally showed her the button that the other nurses had pressed to get to work and she was able to get it working so she obviously knew what i was talking about but didn't want to be bothered. After that i was able to fix it but work full time and couldn't come every day. I certainly would NOT ever put another family member in FAIRHAVEN Rehab even for a temporary period and certainly not in the nursing home.

I enjoyed my stay at Fairhaven all the nurses and nursing assitant were great and helped out a lot. I also enjoyed working with Jeff Physical Therapist was great and made PT fun and Josh from OT was excellent coming up to my room early morning exercises were fun both Jeff and Josh were very compassionate. The food was excellent. My nurse Anne J needs a daisy award she is very compassionate and listens to your needs. Alexi’s is also great she cares about all the patients and what their needs are and helps all the patients. I would recommend Fairhaven to anybody.

My Aunt was there on rehab second floor through the holidays and I must say that her care taker Wanda was very attentive and caring, and not to mention the awesome cook Jennifer is a exceptional cook and very polite as well as professional. Fair Haven is a not-for-profit FAITH based community. The facility has been met with incredible adversity during the COVID-19 pandemic. All AL SNF continue to push back against debility. As in the community outside the facility COVID continues to rear its head. This is a ™ bunch of people. I trust what is happening and they keep working to be better every day. The community needs Fair Haven too badly for its FAITH to FAIL I know I do. I am an only child. If it wasn’t for Fair Haven who would pray with and care for my MOTHER when I can’t 24/7? We are blessed and thankful ™ for this community within a community.

I am beyond grateful to the entire staff on the second floor of Fairhaven’s skilled nursing rehabilitation facility. When I brought my mother there 3 weeks ago, she had very poor cognition and was unable to care for herself. I chose to take her to Fairhaven after several other supposed skilled nursing rehabs had failed her miserably. The thought initially was that my mother would have to be placed permanently in a nursing home facility due to her lack of daily living skills. Blessedly, the managers at Fairhaven rehab felt she could improve and wanted to give her a chance to return to a more independent life. Fairhaven’s therapists and staff did a miraculous job and she will be moving into assisted living next week! The meticulous care given to my mother, especially by Eric, Jenean, and Jenifer, were the reason for this miracle. They spent countless hours just pouring love and time into my mother and I could see her blossom from this kindness and compassion. They never gave up on her. Carie, the executive director, Emma, Jenean and Eric also patiently guided and loved me and supported me as her only child and caretaker during this very difficult time. There are so many others who were part of my mom’s recovery and I am grateful to every person on that floor. Just not enough space! Thank you everyone, especially Eric and Jenean! Thank you for all your love and support. I truly have grown to love you and will miss you terribly.

It is difficult to rate a facility until you have a comparison. After looking at many facilities at which to place my wife, Fair Haven was an excellent choice. Initially, there were many staff issues and housekeeping was non existent but improved slightly although it was far from being acceptable. Given time, the new facility manager, Alexia, will bring the staffing to a very high level. William was one of the real bright spots.and really enjoyed his job. All of the new staff, brought in by the new facility manager were efficient, careing professionals . The food almost always looked good but was rarely hot. I would go back there at any needed time. We were in the Rehabilitation Community for more than two months.

The rehab center was very good. Once they hired a new person to clean, the floors were vacuumed and the bathrooms were cleaned. The food was good and the cook was so sweet and friendly! So glad that the facility had physical therapy. Everyone was so knowledgeable and helpful to all of us working on getting better! Finally, I can’t say enough good things for the overworked RNs. They are so kind and efficient!!! I needed a lot of care and they helped me as quickly as they could! Fair Haven Rehab is a good place to go if one is not ready to go home after being in the hospital. How could I have forgotten to say something good about the nursing assistants! They are the work horses of the whole facility! They can do most everything! They also motivate one too by encouraging one by being positive! Lovely people!!!

This place is kind of clean, but that’s the end of my positive remarks. The staff, which seems pretty overwhelmed, and like they need more help, is pretty rude. When we toured, they had doordash and other delivery people walking through the dining area to get to residents doors. I wouldn’t waste my time with this one.

I love how upscale that this place is; but some of the staff's not the most courteous! A perfect example would be how one of the RNs (an Asian nurse by the name Cheri) had mentioned a comment that I found highly offensive one mentioning, "How my mom didn't want anymore fried chicken?" To make matters even worse, it was right while I was in her presence! With that being said, I fronted her on how racist & highly offensive that was!

Whether you are looking for yourself or for a parent I highly recommend Fair Haven Independent Living! My mother enjoyed her spacious and beautiful 1 bedroom apartment where she was surrounded by her lovely antiques and favorite things she brought from her home. She made many friends and memories while participating in the many activities provided by the staff. Any maintenance issues were quickly resolved. She enjoyed the meals provided in the dining room as well as in the Ice Cream parlor and the Bistro. The Holiday events were always fun! Everyone on campus was warm and friendly. She lived there over 7 years until she passed away and that sad event was handled with such grace and compassion for the family.

We have had superb service from Fairhaven and its staff. They have been very helpful, and communicaions were outstanding. The rehabilitation staff and caregivers were suooer and my Mom finished reganb from a broken hip in record time. Thay had her back to 80-90 win a couple or weeks. Keena and the staff in the rehabiltation section are the best around. We are truly grateful fo rthe care and service they provided.

Merry Christmas to the staff who are doing the best they can. I have seen genuine care and exhaustion in the CNA's who are the front line here. Crystal, Jackie, Joan, Jean, Jessica & Barbara, and a few others who showed caring and love when caring for my mother. The facility has a good process, but is understaffed. Those that I met are superb and are attentive. Emma, as the clinical director, is the bright light who provides comfort due to her professionalism, genuine care and detailed, helpful well throughout support. I do hope they increase the staff and they are responsive to suggestions and concerns everytime. Thank you to all we have interacted with and for the care provided. I give 5 stars to the individuals I named. 4 stars is because they were short of staff I thought they needed to provide the level of care they desire to provide.

My mother is a patient there at this very moment. She has been there going on 2 years. Sometimes it's good sometimes it's bad. They do not have enough staff each shift only has two CNAs. Each CNA from what I understand has over 30 patients. Lunch breakfast and dinner are always late. My mother has not had a bath in 4 months and no one has even asked does she need one. Also the nurses are very rude. It's sad that you're relative has to be there anyway, and it would be nice if the people that have to take care of them were not so rude. It hurts her feelings and it brings down her spirits daily. But I cannot give her the care that she needs at home. Not enough staff, the food is not that good, they have roaches and bugs. There has never been an exterminator there since my mother has been there from what I understand. I have even had to go up there and clean my mom and her roommates room and bathroom myself because it was not getting done properly. Now we are going to start having someone come in and bathe her and pay them out of pocket for it, which is ridiculous. And I seen in other comments where they say contact the head nurse, the head nurse does not do anything except what she wants to do. If they do not like your relative they will not be taken care of properly. And the ones that they don't like are the ones who are needy and need a lot of assistance and the ones who complain. I'm not giving any name at this point as to who my mother is and that is because she is still a patient there.

The staff is doing an amazing job rehabilitating my eighty-three year old father. Yes, there are signs of the strain on healthcare these days, I get it. But his color is good, his main nurse is fantastic and I really get the impression that they care and they are trying, despite the obstacles that have been ever present these last two years.

Bad experience all around. No integrity in what they tell you. My father was treated as a burden. The nurses I encountered seemed to be dedicated and good. They are the only redeeming quality of the experience but the nurses can’t do it all. My father’s clothes were left in wet piles on the shower floor. He was served the same meal every day. Sometime twice. No one ever answers the phone. Ever. Even the hospital transferring him in couldn’t reach them to discuss my father’s care. I signed the admission paperwork the day before but when the ambulance delivered dad, the floor didn’t seem to know he was coming. His room wasn’t ready. The day he checked out, I had set a time to pick him up. When I arrived, nothing had been done. It took 2 and a half hours for me to get him. He wasn’t dressed when I got there. Nothing was packed. His clothes were still in a pile on the shower floor. After I packed him up, I made multiple trips to the car to get his things loaded. He didn’t have much but he did have a Rollator, suitcase, and meds. I even got dad to his wheelchair and pushed him to the car. The night before he checked out, I visited him. His hearing aids were on the night stand charging. The next day they were gone. I searched everywhere for them. I know dad didn’t touch them. He wasn’t able to do so. When I asked about them, what I got were shoulder shrugs. When I persisted, I was told they would go check the trash. That person never came back. When I pushed dad out in the wheelchair, the person was sitting in the main room at the table laughing and talking with a colleague. I called the director multiple times and received empty promises to look into it. Fair Haven was a waste of insurance and financial resources. The two stars are for the nurses, who by themselves would have gotten 5 stars. But, again, it takes more than nurses to care for a loved one.

Patient neglect ... if the staff don't like your family member they will not take care of your family member. If your family member is sound of mind but not sound of body they will just ignore them as much as they can and do as little as they can get away with. You have to be in contact with your family member everyday and coming up behind the staff every day making sure your family member gets their meds everyday and making sure that your family member gets whatever special care they were prescribed .. physical therapy, the number of times they're supposed to have physical therapy, special meal requirements, etc. it really is ridiculous and shameful and neglectful!

My uncle who has dementia has been a resident of Fair Haven's SCALF section - special care assisted living facility - for several years and I could not be more pleased. There appears little turnover with the staff and they see to genuinely care for the residents. A special shout out to Roslyn and Loretta. They are the best!

My mom had a great experience at Fair Haven for PT rehab after a fall. Every member of the staff who worked with her was stellar. From the young lady who came in early in the morning with water to the nursing staff, physical therapists, social worker, and everyone else, Mom’s care was excellent. I am so grateful to Fair Haven for getting her back on her feet.

My mother has been a resident for 2 years in the Assisted Living section at Fair Haven. My mother and my family could not be more pleased with the care she receives there- from the CNAs, homemakers, activity personnel, front desk receptionist, nurses, Director of Nursing, Administrator, to the CEO. My mother knows them all by name, and they know her by name (and I know them all by name as well). My mother is treated with respect, kindness, attention, and love. Concerns are addressed seriously, and I feel comfortable talking with any staff member or administration about anything that may arise. Many times when I am visiting (I live out of state), the DON or administrator will drop by during meal time to say hello to the residents. The CEO is visible around the facility as well. My mother receives good care from the onsite doctor who is a UAB gerontologist. Dr Fix returns my calls in a timely manner. If Mom cannot be at home, Fair Haven is the next best community for her. (Additionally- the food is OUTSTANDING!)

I am writing a new review just because the staff on East wing has been wonderful! I know working in a skilled nursing facility is not an easy job so I highly commend the nurses and staff who help care for my Dad at Fairhaven.
