Oh my goodness! These CNAs (Certified Nursing Assistants) here in the 200 hall on the 2-10:00 PM shift! I feel like i'm at a spa! Since I'm paralyzed, they give me a bed bath three times per week. After the awesome bed bath, they cover me with lotion. My body is having a time with this attention! LOL! but it's true! Thank you girls for taking good care of us! We sure need you! God bless you!

I have been going back and forth on what and how I should rate the experience I had here with my mom. My mom stayed here about a month and there is a lot of positive things. The facility is always clean, most of the staff are nice and friendly (especially the nurse Tanya), and they were overall good to my mom and took great care of her. But my mom was there for rehabilitation to walk again after a lengthy stay in the hospital. I got 2 updates of her progress in her physical therapy in the time she was there. I was told she could get up on her own and even walk a certain amount with a walker. There came a point that insurance wouldn't cover her stay any longer and the doctors that decide this use the same reports I received for the decision. When my mom came home she is absolutely not able to get up on her own, she can not walk at all with or without assistance. The update reports I received and the doctors that decide coverage received does not match the condition my mom is currently in. I want to give the most honest review because when I was choosing places for my mom I didn't have a lot of time to do research and basically had to use Google reviews and what facility helped the most to get my mom admitted. This place worked quickly and did everything to get her admitted (I actually did not have to do much work at all for the admission) and when she left they did make sure that we got a wheelchair, oxygen machine, and any other supplies that we needed to care for her at home. But I had to give 3 stars simply because I don't feel she improved at all in her rehabilitation and I don't understand how their update reports we all were getting was so drastically different then how she really is physically.

I highly recommend the facility for rehab. The staff is caring and efficient. Rehabilitation is excellent and effective. They make sure you receive what will be best for your recovery.

Two star rating so it can’t be removed. Absolute worst facility coming from down to earth people that are hard to get upset. My grandmother received the worst care here. She arrived on a Saturday with a fractured hip and UTI. Saturday dinner/Sunday breakfast dishes were picked up when they brought Sunday LUNCH. Sunday they changed her soiled bedding but not her clothes. Three hours later (after several requests) they put the soiled clothes on the floor along with soiled towels. Pushing the call light is a signal for the nurses to flat ignore you. She finally got moved to decent facility Monday after she developed a RASH from her short stay here. Thankful to have had family with her (couldn’t imagine the care she’d receive had she been alone). I’m just baffled there wasn’t a singular nurse seeing a brand new, immobile, incontinent patient and didn’t have a thought in their brain about caring for them. Apparently it’s par for the course given similar reviews we wished we had seen before. Just bare minimum stuff guys. Ya’ll are playing with lives. I’ve seen a preschool class accomplish more with a chore chart than what was done over a weekend here.

My stepdad was treated with respect by everyone who came in contact with him. Everything went well with no complaints. He enjoyed his time in therapy which helped him so much. He loved the food especially breakfast. I would highly recommend Legend Oaks.

This was my experience. My mom was here for 21 days in order to receive rehab after fracturing her right hip. She was non-verbal and paralyzed on the right side, therefore; needed to be monitored all the time. Day one, we sat in the room for four hours before anyone came to speak with us. I had to find someone to ask them if anyone was coming. Admission came and stated that she didn't know she had arrived. (I looked confused like wth). For the next 20 days, I was the one cleaning her up, doing her personal hygiene (they weren't washing her face or brushing her teeth) and making sure she was eating. I had to change her linen almost every day. When she was able to shower, I had to shower her and change her clothes. Their idea is just putting her in a hospital gown. I had to TELL them to get her out of the bed and have her sit in the chair to keep her from losing strength. When I questioned if she was actually receiving rehab, no one could give me a definitive answer...so that told me that she probably wasn't. They would leave her in urine-soaked pull ups. On one occasion when she had an accident, the soiled clothes were thrown and left in the wall locker. I would pop in at different times of the day and I'd check on her two to three times a day to ensure that she was being cared for. I had only two requests, that she always has water and that she's dry. The requests were not fulfilled. Sometimes the water/ice wasn't replenished until late in the morning and she was always wet. I was so pissed because she is nonverbal and couldn't walk so she depended on them for everything and they failed. In my opinion, the staff (nurses, assistants, rehab) just wasn't accountable. Most of the assistants that were assigned to my mom were 19/20 so maybe that's why the care wasn't there. Again, this was my experience but yours may be different.

I recommend Legend Oaks. They took good care of my mother. They have a gym on site for rehab and the facility is kept in good shape. The employees were always very friendly. Special thanks to Nikki Littleberry as she was a godsend for helping me through the Medicaid process.

The facility is clean, and everyone is friendly and genuinely seems happy to serve the residents. Ms. Tammy was amazing during our tour and has remained just as attentive and supportive after my dad’s check‘in. The rest of the medical staff ” Robert, Ben, Shay, and Regina ” have treated my dad with such respect and tender, loving care. Ms. Nikki in the finance department was friendly, comforting, and extremely knowledgeable about the resources available to help us secure the best care possible. Everyone we’ve met truly seems to have the heart of a caregiver. I’m confident my dad will have a great experience at this facility.

My mom was there for a broken femur. The staff were very friendly and helpful. The only thing she complained about was the food and she did have a couple of things that came up missing.

Mom stayed here for a few weeks and was recently discharged. Her experience was great, with very friendly and helpful medical care. The rehab experience was new to her and she mentioned that it was very good for her. From admissions, business office, social care, and nursing it was an overall positive experience for her stay. If she finds herself needing another SNF, she will be back here without question.

LOVE THE STAFF THEY GREAT WITH MY MOM. MY MOM DOESN'T CARE FOR THE FOOD. AND THE PHONE NOT VERY GOOD YOU CAN ONLY GET FAMILY WHEN RECEIPTIONIST THERE AFTER 5 PM .THEY HAVE TO USE THERE CELLPHONE. IT'S HARD FOR FAMILY MEMBER TO USE THERE CELLPHONE IF THEY NOT USE OF USING IT. OTHER THEN THAT THE PLACE EXCELLENT. VERY CLEAN, THE NURSE , CNA STAFF...THEY ARE VERY POLITE .SOCIAL WORKER ,ADMISSION ARE VERY HELPFUL IN HELPING YOU WITH YOUR FAMILY MEMBER. GREAT JOB LEGEND OAK KEEP UP THE GOOD WORK.

I enjoyed my respite week. I had a broken leg and needed assistance getting in & out of bed or the wheelchair. All of the aides were very helpful. The ones that stand out are Tyria Kayla Kensley & Cameron. Charlotte the nurse who distributed medication was awesome. Nursing staff in general was good. Meals were always on time and I enjoyed the options to select my meal. Housekeeping was efficient and there every day. My room was always clean & the temperature was comfortable. The facility is beautiful both inside and out.

My mother was moved to Legend Oaks for hospice care and we couldn’t have been more pleased with the care that she received. The staff at Legend Oaks was absolutely incredible. They were compassionate and so caring to our mother in her final days. Specifically, Christine, Tiffany, Tazz, Charlene, Eric and Robert were amazing! Additionally, the facility was beautiful and very clean. All of the residents that I encountered seemed genuinely happy to be there. I hope I don’t ever find myself or family in need of this type of care in the future, but if I do, Legend Oaks will be first on my list of options for care. Thank you for all that you did for our mother and family. We are forever grateful!

Review for Reception and Business Office Management at Legend Oaks. My review specifically focuses on the exceptional service provided by the reception and business office staff at Legend Oaks. Every time I’ve had to call the facility, the receptionists have been consistently helpful, courteous, and highly professional. My father has been a resident at Legend Oaks for a year and a half, with approximately 16 months as a private pay patient. Each month, I contact Nikki Littlebury to make payments, and when she is out of the office, I speak with Gloria”both of whom are always reliable and courteous. I want to take a moment to especially commend Nikki. I’ve worked with her for quite some time, and she is truly a gem. Nikki consistently goes above and beyond to provide outstanding support. For example, just last month we encountered an ongoing issue involving an in-house supplement sent to MChest Pharmacy, which had persisted since October/November 2024. After a single call to Nikki, she promptly resolved the matter. She always ensures I receive a receipt for every payment and is a pleasure to work with. Nikki is not only extremely professional and efficient but also kind and approachable. Thank you, Nikki, for your exemplary service”your dedication does not go unnoticed.

I Would NOT recommend this Facility to any loved one. My mom arrived at 6p. BY 10p we had her transferred out. It only took 4 hours to know that the Staff were, ILLEQUIPPED, UNPREPARED, UNSKILLED, UNPROFESSIONAL,UNQUALIFIED, UNFIT, INCOMPETENT, LAZY and CLUELESS. Thought we were going make a 911 call due to two staffers boisterous shouting match about a resident (client). ABSOLUTELY no hospitality from ON Duty Staff. Could not get any assistance from staff. ABSOLUTELY WOULD NOT RECOMMEND THIS FACITLY. DIRECTORS OF THIS FACILTY SHOULD BE LISTED ON WEBSITE. THERE IS A REASON THEY ARE NOT! (HIDING IN SHAME) I rate them a 1 star because 0 was not an option.

The nurses, CNAs and other staff were pleasant , professional and friendly. They made my stay there comfortable. The therapists worked effortlessly (once they got the medication mess fixed) to get me to the point where I could go home. All the OTs and PTs (who worked with me) were truly awesome. Except for a minor issue with the cafeteria, my stay there was good and would definitely recommend them.

My grandma has been on the rehab hall for therapy. Her CNA Shay has been such a saving grace during this time. If it weren’t for her I don’t know what I would think. She cares and takes phenomenal care of her patients. Legend Oaks is lucky to have her.

I am currently a resident here, after surgery. The idea is to recover the use of my hip after surgery, and be able to go home. The PT and OT staff are great, but the nursing staff is understaffed. Badly. As I sit here, I’ve been waiting an hour and a half for a pain pill. I’m not going to be able to do anything in therapy like this. It hurts too much!

My Daddy has been at this facility since July 2021 and I can honestly they have become family! The staff are more like my cousins, aunt's, and uncles. They have gone and and beyond for Dad. From taking my letting me know when he's having a rough day, to calling just to communicate they miss me because I haven't been the visit in a few days. They held my Daddy together when I lost my youngest daughter, and I could barely hold myself together. I love the LO FAMILY!!
