I would strongly caution families considering The Citadel at St. Anne Place for a vulnerable loved one. A close family member of mine was a resident there during the 2025 management/ownership transition period. In my family’s experience, the quality of care declined noticeably after that transition. What concerned us most was not one isolated incident, but a pattern: delayed basic care, poor responsiveness, inadequate communication, and what appeared to be a lack of urgency during a period of obvious decline. During the final stretch of this resident’s stay, there were repeated concerning vital signs and signs that, in our view, warranted closer attention. Family concerns were raised directly with nursing staff. We did not feel those concerns were taken seriously or escalated with the urgency they deserved. The resident was later transferred to the hospital and died the same day. Afterward, I requested records so our family could understand what care had been documented and provided. The facility produced portions of the clinical chart, but did not provide several categories of operational documentation that would show care delivery in real time, such as call-light response records, detailed CNA/point-of-care documentation, hydration/intake records, rounding records, meal delivery documentation, and staffing/assignment records. Multiple written follow-ups were not answered directly. Through the long-term care ombudsman’s office, the administrator stated that the facility had already supplied everything they had. That response only deepened my concern. When a vulnerable resident is declining, families should not have to wonder whether basic care was provided, whether warning signs were acted on, or whether anyone was truly paying close attention. In my view, dignity in long-term care is not just about having a chart that looks complete afterward — it is about timely response, clear communication, and staff who notice when something needs attention. Based on my family’s experience, I personally would not place a loved one here again. If you are considering placement here, ask very specific questions about staffing levels, call-light response times, hydration, toileting, rounding, meal assistance, escalation procedures, and how the facility can show that care is being documented and delivered — not just summarized later.

My mom was at a place on mulford and my mom called me crying. I called the ladyin charge and had a meeting with them and my parents. Needless to say i got her transferred to st. Anne. My mom was happy and comfortable. It made me very happy after she cried to me. I got her out of the other place the same morning.
