My Dad has been at Oak Creek Terrace for rehab. He had RSV and needed to regain his strength. The facility is very clean. He has been treated with kindness. We would return there if he needed rehab or skilled nursing care.

I was here for about 2weeks. ALLof the people I dealt with were very caring and very patient and helpful. I was kept updated by the social worker and everyone else involved in my care and progress. The physical therapists encouraged me daily with a variety of things to help me regain my strength and mobility. The ONLY reason I didn’t give 5 stars is because like most medical rehab centers the bedside aids and nurses were responsible for too many patients so their response time wasn’t as quick as they’d want it to be. But when they came they always greeted me with a smile and a willingness to do everything they could to help me! If I ever have to go to rehab again, this is where I will go.

My Mom has only been here for a little over a week but the entire staff has treated all of us like family. They genuinely care about their residents, treating them with love, kindness, respect, and the greatest understanding. They keep us informed and explain everything they are doing to help. We could not ask for a better experience and I would highly recommend Oak Tree Terrace to anyone looking for quality care for a loved one.

The staff provide excellent care, are very patient and informative. The patient's care is their priority and keep family updated with important information that helps in the patient's overall improvement.

Staff is very hit and miss. Most staff is very helpful, but others not so much. It is not uncommon to be left on a bed pan for 1.5 to 2 hours. Management agrees this should not happen, but does. The physical and occupational therapists are outstanding, could not ask for more. We have enjoyed all the friendly staff and wonderful atmosphere they provide. We especially wanted to thank Roe for her very caring help and patience, she truly takes pride in her job. Photos and videos will be added soon!!!

Oak Creek took care of our father for an end of life stay. The staff were incredible at all levels. The nurses, nurse aides, social services, rehabilitation services staff were all great. The food was very good. I would recommend Oak Creek without hesitation. A+ in every way.

My dad was in rehab after a fall. He received good care overall. The staff was nice and friendly. The only thing I was not fond of was after office hours there’s no one particularly tending to the phones. Therefore you receive their voicemail. Also, at times there were odors in the common areas. All in all, I would highly recommend Oak Creek Terrace.

My mother is currently at Oak Creek Terrace she is getting good attentive daily care only thing I would say negative is the therapy isn’t enough she mostly sits in her room with about 30 minutes of therapy per day that isn’t enough to get her back on her feet. The place could really use an update also rooms are basic and the carpet is very worn and dirty.

My dad received excellent health care and also I appreciate the social worker’s ( Susan) 20 yrs of experience , she was very helpful. Overall I have no complaints. they were wonderful in finding my dad, a place to receive his rehabilitation after his fall and release from the hospital.

My husband convalesced at Oak Creek Terrace after both a hospital stay and time in a physical therapy facility so he could be ready for knee surgery. He had a single room with a comfortable and safe bathroom. The staff was kind, efficient and busy but took time with him. The highest praise I can give OCT is that I would willingly stay here. I was even allowed to bring our dog when I visited.

After an extended recovery and rehab of several months there were only minor annoyances not worth noting. At first I was very concerned about going to ANY facility with things you hear. I was quickly relieved of my fears. The care is excellent from the doctors to the cleaning staff. I didn't want to be there but they made it about as pleasant as it could be. Even an emergency that required a ambulance back to the ER was handled well. Everyone seems to honestly be vested and concerned about you. They are professional and kind. I can honestly say I would choose the facility again.

My 95 year old mother suffered a fall and needed care. Oak Creek Terrace had the best ratings, so we wanted Mom there. As well as being in pain, Mom has some dementia. We wanted to be with her daily . We have been accommodated with warmth and are around to see how Oak Creek operates. Mom is in the Memory Wing. It is so enlightening to see the interactions betweem staff amd patients. I'd expect patience , but it's so beyond that or even simple friendliness, the staff seem genuinely interested in interacting with their very personable patients. Some patients have a bad day and express it at times and in ways that would probably stress me. But I haven't observed any stress. Tracy, the nurse, operates with the philosophy that every day is a new day. It's a very positive atmosphere. We were there for Mom's 95th birthday and Christmas. Residents came in with cards and song on the birthday. Most days they gathered near the nurses station,,often I'd hear them singing carols along with a recording.There was warmth, a family feeling. Tracy is the leader, but everyone echoed the friendliness. The rehab staff has been fantastic. My Mom may be fixated on her bowels, they can work that into their plan. They roll with the situation and get Mom working on her plan in the most positive way. I've yet to have anyone suggest that they just come back tomorrow; as a result Mom has made such strides! One of my sisters lives out of state and 5 days passed between visits. She kept exclaiming over the progress that Mom had made in those days. Mom wants to be home, but she likes the people at Oak Creek and works hard for them. Meredith, one of the physical therapists spent 30 minutes with my family showing us how to assist Mom which we now do with confidence. In the few days before Mom was discharged she was moved to another wing. Everyone there is competent and pleasant. But we miss the Memory Unit, the friendliness, the warmth and sense of belonging to a group. Several of the staff have stopped into Mom's new room to say hi. I'm glad Mom was there.

The rehab section is where my father was. It was clean, spacious, and had everything needed for recovery. The staff was all very nice and was always willing to help when needed. Therapy was top notch. My dad really enjoyed everyone there. It made his stay easier. The one thing I can say that I did not like was his family physician was not allowed to be involved in his care as they have their own physician staffed here. I 100 percent did not care for that as no one can possibly know the patient more than their own primary care physician. Medication was stopped without his primary cares opinion. Other than that I would recommend this place.

Oakcreek Terrace took excellent care of my Dad. Their rooms are large making it easy to access with wheelchairs and walkers. Their staff was kind and courteous. I could always call them and get my questions answered. The food was good. My Dad especially liked the breakfasts. My Dad could walk around the facility, to the dining room and to visit with friends without any problems towards the end of his stay. So the Physical Therapists did an excellent job getting him back on his feet.

My Marmee’s stay there was pretty good however her discharge was a mess! It was like the insurance stopped paying and they shut down! They called in meds the dr discontinued for her to the pharmacy. Didn’t call ANY OF THE MEDS SHE NEEDS TO LIVE! She has been w/o her meds for 2 1/2 days! No one assisted us when she left. It was like they no longer cared. I would not send my loved ones there!

Had an ankle fusion that required non-weight bearing for 6-8 weeks. Prior to surgery I checked out 4 rehab centers close to my house and decided on Oak Creek Terrace. Theresa gave me a comprehensive tour and after visiting all 4 rehab centers I called her to make a reservation. The day after surgery I was transferred from Sycamore to Oak Creek. My room was ready when I arrived at 11PM, the head nurse checked me in and I hit the sack for the night. During my week at rehab I was able to work on all the things I would need to be able to safely do when I got home. Excellent PT, OT, and ST on site. Room was very large, I’ll try to post a few pics. Ate in the dining room for every meal except the first morning breakfast. Always had at least two options for entree and alternative of hamburger, hotdog, grilled cheese, etc. food was institutional, but quite good and you could have as much or as little as you like served by very friendly servers who always remembered your name. All of the staff was very friendly and willing to give you and your wheelchair a push if you looked like you were running out of gas. Next time I need to rehab I will definitely be going back here.

My mother was in there for rehab after being in hospital due to a stroke. While the rehab services were wonderful the facility lacked in other areas. My mom is diabetic and also supposed to.be on low sodium diet. They brought very small portions of al.ost nonedible food loaded with salt followed by a nice sugary dessert. She mentioned it to the nurse and was told yeah they aren't good with diets. She said they were very short staffed and it took them like 5 days before trying to bath her. She co.plained it was cold but they were unable to control Temps in specific rooms. The only reason it got 3 stars was because of the rehab team otherwise would be a 1.

My mom was here for about 4 months after a pretty bad infection in her body. While I agree that they are definitely understaffed, i had to take the good with the bad. What was important to me was feeling like my mom was SAFE when we weren't there. At no time did I feel like she wasn't. Because of the shortness of staff, there were times that I had to bathe her, and help her to the bathroom. I can't say, however, that her call light went unanswered. She would press it often, and they would come- answer her question- and go back out pretty quick. IF they were fully staffed or even more consistently staffed with regular nurses they would have the potential to be 5 stars. The few (regular) staff that they did have was pretty good. My mom is home now ( thanks to their therapy staff), but if necessary in the future I would feel comfortable sending her back.

What a great place for a loved one. They gave great care to my mother. She was up and moving in no time flat. She was home within a month, and living along once again. I could not thank the staff enough for her quick recover.

We moved my father from another facility in Centerville to Oak Creek Terrace. We could tell within days this was the right decision. They treated him like he was a member of their family. The care was wonderful. They went out of their way to make my mother, who came to visit daily, welcome and at home too. They treated him with kindness and dignity. I could not have asked for more.

I cannot be happier with the care my mother receives from Oak Creek Terrace in Kettering OH. She has advanced Alzheimers disease. There is very little staff turnover, we are always welcomed there, I am notified immediately of any changes for my mom and she is very well cared for both physically and emotionally. Care conferences are done often which is a great thing to have for the care of a loved one. Thank you so much for what you do for her!

My husband was at Oak Creek for only three months before his death. He transferred from another nursing facility and the difference was like rising to heaven from hell. I can't say enough good about this facility.

Everyone is very nice but they are 'GROSSLY' understaffed!!!! When call light is pushed it's anywhere from 15-40minutes before someone comes. Food is excellent and the facility is always super clean. Facility is NOT secure. Anyone can walk in at anytime 🙁

My experience wasn't the best. I had to constantly stay on the nurse/director of nursing/aids to take care of my dad properly. I would find him wet from not being changed/dirty from food spills. Had to get them to clean my dads wheel chair, it was very dirty. There are 2 FANTASIC AIDS. Those were the only 2, I would trust. The best advice I can give anyone is to constantly watch your person and I mean head to toe. The last 2 weeks of my dad's life was bedsores, bleeding on the foot that I wasn't told about, I saw it when I had the aid change his socks. You will have to stay on them Continually to make sure your person is taking care of. The money that is charged and the care you get..........
