Very Happy Former Resident

Meadow Creek is an awesome rehab facility. I stayed there twice, one knee replacement and one broken leg so I chose it for a second visit, and I live out of town! The therapy team was amazing in 2023 and 2024. Robert, Reed, Dave, Reese, Chris, and everyone else was wonderful. Sheila is a very caring, professional administrator and I hope she stays on with the new owners of this facility! I was always treated respectfully and enjoyed engaging with everyone there. I count them all as friends! Meals are wonderful along with alternative menus if desired. I was very pleased with everyone's patience and help to regain my independence to walk again. I have made several long time friends who still reside there and I really enjoy visiting them as a Meadow Creek Alumni. Aides and LVNs are very caring and Helen, the Activities Director is so very, very dedicated. All residents should participate in all the daily activities! God bless all who serve the Meadow Creek residents. May they all focus on those precious residents and all their physical and life enriching needs, Always eat meals in the dining room to make great new friends!!! Another reason Meadow Creek is special ---- there is at least one former resident volunteering to enrich lives at Meadow Creek. I encourage all to help Meadow Creek have the happiest residents in Texas!!!!

This is the worst nursing home ever. For starters my grandmother was discharged from the hospital and place to meadow creek, when picking her up from the hospital they called the nurses told them they were there and literally left my grandma at the hospital. It took almost 2 hours for them to come back and get her. Once she was there she was left in soiled clothes, was not served her food a few times ( she's diabetic ), was never out of her bed even though her doctor visited multiple times and said she needed to be up with her legs propped up for multiple hours each week. Dropped her multiple times, wouldn't bathe her when it was her day or even if she asked them to because she had not been bathed in days. We submitted her paperwork for Medicaid in December and never even filed it or reached out to let us know if we were accepted or anything. When we would try and call them no one ever answered. 90 days later they call her PCP stating she will be kicked out the next day, they made up lies saying that they had reached out and tried to contact us when they never once did, said we did not provide all the paperwork and we showed proof that we did and they got defensive. They change office staff more than they changed my grandmas diaper. This is the worst nursing home I have ever seen operate. I feel bad for the people that are in there, especially if they need assistance with everyday tasks. They will literally ignore a patient begging for help or assistance or with a need that they are asking to be met. This place needs to be SHUTDOWN!!!

I am incredibly grateful for the exceptional care my father has received at Meadow Creek Nursing Rehabilitation. Admitting him to a facility was a daunting decision for our family, but the compassionate support from the nurses and CNAs has made all the difference. Despite some changes in leadership, there remains a consistent commitment to quality care that shines through. Nurse Ramona Essary and Georgianna Escamilla, the Director of Nursing, have been instrumental in making this transition smoother for us. Their open communication has alleviated our concerns, especially given our distance from the facility. While some may share negative experiences, we can wholeheartedly say that our journey here has been overwhelmingly positive. The dedication of the entire staff deserves recognition, and we extend our heartfelt thanks to them and the administration team for their unwavering support. My dad went here for rehab after back surgery and we were very well pleased. The staff was very friendly and focused on dad’s recovery, most especially the therapy team. Kudos as well to Willie, the chef, as dad always bragged on the food. Special thanks to Bekha for providing a tour and getting us ready for dad’s admission. This facility has been nothing but the upmost best for one of my loved ones while being there. The staff is always so kind when I am in the facility and they are always so eager to help without hesitation. The aides do an amazing job at making sure my loved one has everything that they need and feel cared for on a personal level. The nurses do such a great job at giving medication and catching any potential health problems with assessments. My loved one raves about the food and how everyone there makes him feel so welcomed and like he is at home. It gives me comfort to know that he is surrounded by people who truly do care and do this job because they genuinely want to help others. I really do recommend Meadow Creek if you have someone in need of their services.

This facility was the best place for rehab after hip surgery for my loved one. The rehab team, nursing staff, and administrators were all professionals. Any questions or concerns were followed up by respectively by the rehab therapists, RNs, or administrator. I would call daily and get updates on my mother. I never felt like I was imposing.

I was at Meadow Creek North for rehab, the aides work so very hard. The nurses were all very sweet. Ro is amazing I was there a little over a month and he did my toenails twice!!! I can’t say enough about Kathy ! She is AWESOME!!! Therapy is GREAT! They work you hard but are very compassionate! Reed is GREAT! I wanted to go to Therapy!!! I am my worse enemy, but they showed me I am stronger than I know. Activity director Precious lady! Patients of a Saint! Love them all

Meadow Creek, a place packed with hospitality, love, gentleness, eager to serve their elder generation with acts of kindness and support. The staff are very family orientated, professional, experienced, united, friendly, and take the initiative. With a strong understanding kitchen team that cooks wonderfulmeals, selfless nurses on the move to accomplish their next task, outstanding physical therapy team! Dedicated is an understatement. The Administration and management team are not only united, but in constant communication to implement new or developing ideas. Making Meadow Creek a place where activities are important to daily living. The building is very clean and is currently being updated on needed repairs and maintenance. Heard a new patient say, "the food is so great here and the people are so nice!" Excited to be in a nice clean place. If everyone knew about Meadow Creek they would want to be here!

I don't trust these people. I strongly suspect that they may be getting kickbacks from certain doctors or clinics. Therefor it's greed for money, not the best interest of patients that drives them. They were unwilling to order a Hepatitis C confirmation test for my 88 year old mother. It was known that she was hepatitis C positive on the standard antibody test for the condition. We needed to know for sure if it was a currently active infection that demanded immediate treatment. The possibility exists that Hep C is the cause of some of her recent and current medical problems. The hospital that discharged her to the nursing home facility specifically asked them to run the test. They refused. These people were unwilling let my mother get a second opinion as to whether amputation of both feet for severe frost bite injuries was the only reasonable solution. Surgery and amputations are highly lucrative and many doctors are greedy so no one should immediately get amputations without a second opinion. Of course amputation itself is a very extreme solution, and you should not have it done unless there really and truly is no reasonable alternative. This nursing home was not willing to let her investigate possible alternatives offered by doctors or clinics other than the one they sent her to. Update as of 3/19/2021: This complaint appears to have been effective. Unfortunately valuable time has been lost and it could adversely affect my mother's hope of avoiding amputations. An appointment with a doctor who looks for ways to avoid amputations had to be scheduled 3 days later than originally planned due to the past attempts by the nursing home to block attempts to set appointments with wound care doctors other than the one my mother saw on 3/16/2021. Update as of 5/11/2021. I will give credit where credit is due. I think this nursing home has done a good job on the daily wound dressing and the nursing staff has done a good job of preventing injuries when transferring my mother to or from the wheelchair. But I have problems with other things that have happened. An appointment that I had arranged with my mother's general care doctor had to be cancelled only one hour from the scheduled time because the van driver said she was unaware of the appointment. She mistakenly thought the appointment was at the nursing home, not the doctor's office. The instructions given by my mother's GI tract doctor (technically a nurse practitioner functioning as a doctor) to avoid Tylenol were ignored by the nursing home. My mother unquestionably needed daily pain medication but in view of an active hepatitis C infection, recent problems with low albumin, elevated AST, ALT, and BUN, this was no time to gamble with drugs that are really hard on the liver. I had successfully gotten the office of Mother's general care doctor to approve Alpha GPC for Mother's brain function issues. I even gave the nursing home a bottle of pills that I purchased. They gave only the first 1 or 2 pills out of the bottle then stopped giving her the supplement. She recently complained that her memory was fogging up really bad. That's exactly the condition that Alpha GPC is designed to prevent or reverse.

We had our mom there during the month of August 2019.... There are pros and cons to this place. I think the staff is very hard-working and nice but I believe that there's not enough staff to cover some of the patient's needs at times. Very few of the staff members speak Spanish. There is no accessible Wi-Fi. Which is a big set back in my opinion. My mom in particular had a lot of different and difficult health issues and I don't think this nursing home was her best fit. But, I think it would be great for people who don't have as many health issues. I think the fixtures, beds, equipment should and need to be updated. For the most part the nursing home smelled decent but there were times where it did have a strong urine smell. Again, that could just be that it had just happened at the moment that I was there. If a patient is a little bit more independent I would say this would be an okay place for them but if they are more high maintenance you might want to look for a different place. But again I just want to reiterate that the staff was excellent and even the office workers were great. Not gonna lie, my mom is a very high maintenance lady in general and the staff was very patient. As far as the food.... It was basic nursing home food.

We have no complaints with the hard working staff and nurses. However, in the last two months, the food being served has gone from fresh and balanced to utter slop. And let's be honest. When you are of age where you can do very little and are under someone else's care, good food becomes something you look forward to. Not to ignore the amount of money these types of facilities cost per month or the proven studies that link poor food nutrition to shorter life expectancy in these facilities. Fresh fruit has been exchanged for canned. Gone is the fresh salad bar. There used to be fresh food and dining choices. Now most of it comes from a can or bag and everyone must eat the same exact thing. Today's dinner? A boiled hot dog weenie on a plain bun, coleslaw, and a sugar cookie from a bag. Chicken with peas alternates with chicken and carrots every other meal it seems. We've been told 'oh we had to change food suppliers; were working on it'. Would love this to be true and food provided improve to match the price we pay for our relative' care, but anyone reading this knows nothing is likely to change. For staff wishing to follow up on this review, you have only to return the calls of those who have already called your facility to complain. We'll be glad to update or change the review once food is improved.
