My stay here was excellent. Got here in 2023. Everybody was very nice. Some of the best nurses there is. Houda & Sonia are AWESOME! The way they run the place is good. I was the President of the Resident Council for almost 3 years. Activities/Ms. Barb and her team are fantastic and really care about the residents. Administration since we became Rivaya in May 2025 has also been great. I got here in 2023 and just left back to my own apartment. Social service team helped me with the entire discharge process and now I have my own apartment. Everyone is really helpful. Food is also really good and since Rivaya took over the kitchen been more smooth. Overall, this place is great.

Since the 1st day till today my mother been treated so well from everyone in Rivaya Care. From the Nurses & CNAs & Doctors & all the staffs. They're treating my mother as a favorite patient & like a family. They're priority is mom's health & care & support. They have been very patient with my mom & they know mom has Dementia stage 7th & lots of health issues. They're performing loving & caring to mom to make her feel safe just like home. Im blessed that my mother in safe place & with people really loves her & care about her. I would recommend this place for Nursing Home & i would like to thank everyone & from the bottom of my heart I wish you guys more success. Big thanks to Josh & Maria & all the stafs there. I won't say that there are some bad apples that work there because I think at any of these places there are but in my personal experience there were a lot more of the good apples then there were bad. It's a clean facility and a lot of the staff are or try to be attentive to your needs as far as facilities like this goes this one has to be one of the better ones and they are trying to improve so that's saying a lot.

I truly hope NO one is giving this facility a 5-star rating. My son was a patient and died there. Of course, not their fault. I will give you an example of what happened. I show up to visit. My son's room smells like urine. His urine bag "exploded" and was all over the floor. When I asked about this "situation" I was told "the CNA" was JUST there and checked on his urine bag. REALLY. I think not. If you value your loved ones, send them elsewhere. Not that any other place would be better,

Last week, I was sent to IL from WI to look at facilities for my mom. I arrived with my two little kiddos and was immediately greeted by the receptionist with a smile. The staff were all extremely pleasant and the facility was clean and well kept. My story is a little different but so important and shows the character of the staff here. My mom who lives in WI was told by her SW at the hospital that there were no places willing to accept her in WI, we were given two places in IL to tour, this was one of them. I went to look here and thank gosh I did! I met Josh, who was beyond kind and knowledgeable, not only that, he reviewed my mom's records and said he might know of a place closer in WI that could help us. My mom was in the hospital for 4 months and was told IL was her only option and this kind soul, helped us in 1 hour find an amazing place in WI to accept my mom. This demonstrated how much the facility cares and values a human life and truly wants to help people. I will forever be grateful for the knowledge and compassion that was shown to my family in this difficult time, and the willingness of Josh to go above and beyond to help a complete stranger so that her loved one could be closer to her family. 10 out of 10 recommend!

Man where do I even start? I came to Rivaya (it was called Landmark back then) back in 2022. I was in a coma, on a vent, couldn’t move, nothin'. I was in bed for over a year. Thought I’d never leave. But the people there they saved my life. Ownership changed a couple times... was Zahav for a bit, now it’s Rivaya. Gotta say, Rivaya’s been great. They really seem to care about doin things right. The respiratory team was just top notch. I mean, really knew what they was doing. The CNAs... God bless 'em... always treated me good. Nurses too, always there when I needed something. Everybody just cared. Therapy there was amazing. I had a G-tube for a long time.... couldn’t swallow, couldn’t eat right. They worked with me every day, helped me learn how to swallow again, get stronger. I'm even walking short distances now. I mean, I’m still in a mobility scooter most of the time, but I can get around some on my own. Feels like a walking miracle, if you ask me. I was even the president of the resident council for a bit, which meant a lot to me. Activities were amazing too... always somethin' going on. I even got a certificate once for 'Resident of the Month,' which was awesome. Made me feel real proud. Gave me a purpose while I was healing up. Food was actually real good too. I looked forward to it every day, no joke. Now I’m writing this from my own apartment. Been out 3 weeks. I'm out, movin' around, doing my own thing. Got my life back. I can’t say enough how grateful I am. I still come back almost every day to read the Bible with the residents. It’s just an 11-minute ride on my scooter! Feels good to give back. My mom was once a resident at zahav she receive great care from the staff here one thing i can say the she got great care from the team. And also the wound care team did a Great job healing her wounds, my mom improved so well that she was eventually able to go home and be in her own home. Thank you zahav staff for caring and helping my mom.

I am a friend of Jerry on room 125, this is my first time visiting and am impressed with the cleanliness , no smell , that is a big thing. Very friendly environment , from reception , speech therapist, very attentive and impressive with details, he is clean and well taken cared of, he told he loves it here. The Assistant Director was extremely friendly and caring . I highly recommend this place. Very impressive. I have a massive email with my friends and I will recommend this place. My Experience at Zahav was the best from me gaining another resident as my best friend too many more friends. The residence were friendly and fun. We attended activities every day Barb and Angie gave us lots of fun things to do and activities that made us want to participate from management Tess and Nelly answering every question and needs that I had, and was very attentive, not only to me, but I seen with others to my favorite Nurse Neena who was always getting this done in a Timely matter to my absolute favorite CNA Bridget who treated everyone like we were her ONLY resident and laugh and spent time to get to know me. I can say i recommend this facility 100 times over the caring and compassion of everyone here makes a difference. - Helen PointDexter

This facility is excellent. I've seen great improvement with my husband. I am working two jobs and I know that in my absence , my husband is in good care. Jenny , Sarah the nurses always gives me update. Nelly from administration is so nice to me and very easy to talk to. Tess the assistant Director is always there for any questions or concerns. I would also want to thank the CNAs and Agnes. I recommend this facility.

Zahav takes excellent care of the residents. The nursing staff are top notch. Physical and occupational therapists increased my stamina and strength to where I now work out daily. I call myself a therapy junkie. The therapists work with the residents on the skills they need. That’s what they did for me. In Activities we celebrate all the holidays even the more uncommon ones like Mimosa Day and Ice Cream Sundae Day. Access to the heads of all the departments is available for all residents and that is highly unusual. Having been in several rehab and nursing settings, Zahav is the best.

My name is Lashawn Washington.I came to this facility really scared because I don’t want to be in a nursing home. But staying here for almost 2 months changed my perspective of a nursing home. The administration headed by Nelly , Tess and Justin always listens to my concerns. The dialysis people are very kind , his name is Alex and his assistant with the red hair , I forgot her name. The therapist are great , Moon is compassionate . I love Chammy , I call her my baby , she is giving me extra care when she gives me a bath. The Social worker Agnes is always checking in me and arranging my discharge. Thank you all for a great care.

I highly recommend this facility. I have been here in this facility for a year and I have nothing but great experiences. I want to mention the people that always goes above and beyond, let's start with Monsarat, Theresa the CNA, theres Two Theresa, Nurses Sarah , Jenny, and Joel the CNA , Sony the CNA is great too. Raymond and Cristine from Therapy. Tess the Assistant Director of Nursing is very helpful and funny . Elizabeth Nurse too is very helpful. Nelia the administration is very accommodating. The staff, including Barbara (Activity Director), Angie, Anju, and Danny, are all loving and caring. Their support brings joy to my daughter, especially during times when she needs someone caring and attentive. I really was skeptical of this place especially the reviews. But any rehab short-term long-term facility in general speaking. Because of recently moved to Des Plaines Illinois closer to visit my mother-in-law there I had personally installed a two cameras in my mother in law room since she has a room to herself due to her cognitive outbursts. What I seen on the camera very outrageous disturbing behavior employees stealing from a resident. Her simple snacks she likes bathing items her bath and body works who does that no conscience whatsoever. And my mother in law yelling for help due to her falling out of her bed trying to use the restroom hours later laying on her bedroom floor the cleaning lady helped her into her bed. But I will be relocating my mother-in-law to a much better facility end of September petitioned upon request of my attorney.

My family and I can not thank management and the staff enough for their unwavering dedication , and commitment, to keeping residents safe and for continuing to compassionately care for those who rely on landmark of Desplaines . These quality services during this unprecedented time especially in the most vulnerable population is needed , is greatly appreciated from many of us who had sick relatives taken care there! My brother was admitted to Landnark of Des Plaines with a severely debilitated disorder following an infection . He was not able to walk , stand , talk ,or eat . He had a tracheostomy .He was very serious ill patient with West Niles virus , had meningitis from it ,which it paralyzed him . He was in ventilators , heart monitors you name it . He was perfectly healthy man prior to being inflicted with this disorder . The staff of Landnark of Desplaines immediately worked with him with progressive clinical respiratory , which is the best team in the area . Extensive Physical therapy took place to bring him to quicker return to health . The sweet angels of Landnark of Desplaines will always be in our hearts, and we will never forget what they did for my brother to bring him back to life ! God bless them ! Debbie Diniotis !

I am a resident at Landmark. For the most part, this is a nice place. The activities, dialysis, NP, maintenance, PT/OT & scheduling departments are all top notch. The nurses and CNA’s are very good, and provide excellent care. Although, just like a lot of places , staff shortages occur due to COVID & attrition. The management at LoD are seeking new employees to fill the positions. Take a tour, and check it out.

Wound care does a exellent job the Administrator and Don are so awesome.The staff makes me feel like i am home, anytime i had a concern it was adressed right away. The employees @ Landmark are wonderful ‘. Every time I visit my sister, I am welcomed like an old friend. The food could use some more variety; and it would be especially nice if family members were invited for holidays--just a suggestion.

Landmark Des Plaines is one of the best rehab and nursing facilities I have worked with and visited over the past number of years. The have tremendous nurses who are skilled and caring, an administrator that is on top of every patients care in the facility and the best social service staff in the business. I am happy to recommend them to anyone needing rehab or long term care.

I'm scratching my head at some of the negative or misleading reviews that I see. I think some of these people just enjoy making cynical and hurtful remarks and without putting everything in perspective. No place is perfect. This place is similar to an Emergency ICU and the staff work extra hard to take care of some of the most clinically complex patients around. They do a very good job keeping my mother alive given all her ailments. My family is blessed to have come across this caring place. Landmark, thank you for all that you do!!
