Although dad’s time at Autumn Leaves was short, we believe that it was a good place for him. It was close enough to our homes, so that someone from our family could visit him every day. With his memory declining and being physically weakened after a recent hospitalization, dad could not take care of himself. And as much as we wanted to, we couldn’t adequately care for him at home. The personal interest in our dad by everyone at Autumn Leaves made our decision to move him into memory care easier. Everyone was so welcoming, and we were allowed to visit dad whenever we wanted to see him. The transparency and great communication of the staff helped us to be comfortable leaving him there at the end of our visits. From our tour with Bernee, Jessica’s evaluation of dad at his condo, Bethany’s welcome at the front door, Lupe’s fun daily activities, the care of the nursing staff and CNAs, until our dad was promoted to heaven, dad was well loved by the personnel and caregivers at Autumn Leaves. We loved being able to visit with dad knowing that his needs were taken care of by others. When Nurse Joy called to tell us that dad was no longer with us, it was apparent by the quiver of her voice that she truly cared. The facility was state of the art, designed specifically for those experiencing mental decline. It was well managed, very clean and bright. The family- like atmosphere was welcoming and inviting. There were several places to go including outdoor patios to spend time together. Our family came to love the residents and caregivers at Autumn Leaves and plan to continue to stop in and visit from time to time. Thank you for being there for our dad and others that need care when their loved ones are no longer able to take care of them.

I had a tour with Bernee who was extremely helpful, compassionate, and knowledgeable. I especially like the design of the building and how residents can walk in a complete circle end to end and not get lost in long hallways. I will definitely consider this community for my mom when the time comes.

My wife has been to several facilities for rehab reasons, and Autumn Leaves is by far the best facility she has been to, especially for her memory care. The staff understands what it takes to deal with the ups and downs of dementia care, which can be challenging at times. Having been a full time caregiver the past couple of years, I understand this all too well. The management is very approachable, and they don’t mind when a resident wonders into their office to chat. One thing I like is that the number of residents is at a manageable level where everyone can get to know each other. No long hallways filled with patients who mostly stay in their rooms and are ignored throughout the night. Yes, there is always room for improvement (really wish they could get that fountain in front fixed or filled with flowers, at least (you’re welcome, Bethany)), but should you have to make that hard decision for your loved one, I highly recommend this memory care facility.

My father has been at Autumn Leaves for about four months now and I am very happy with placing him there. I feel that he is being well taken care of and he is safe. Whenever I go visit my father he always looks good; he is clean and looks like he’s eating well. The facility is always clean and the staff is always very helpful in anything I ask of them. I really like the fact that Autumn Leaves is a smaller facility because it has the feeling of a home environment and that is very important to me. I think it’s very nice that they are always having special get togethers for the residents and their family; it is a very caring thing to do for both the residents and their families. Our loved one was treated with the utmost kindness and care during the final stages of his dementia. We were allowed access at any time of the day or night, for as long as we wished to be there. The entire staff was so helpful, kind, & caring of not only our loved one but the entire family. It felt like home with all the accessories you could ask for!!! The entire facility was ALWAYS immaculate, as was the staff. What a lovely place for our loved one to be able to spend his last days being so kindly cared for. Autumn Leaves is truly a God send!!! My family and I did our research of the communities in St Charles and surrounding areas, when we walked in Autumn Leaves we immediately felt that this was the place for our mom. The facility was clean, smelled good, and we were greeted with warm smiles from the staff. Kam, the director, went above and beyond to help facilitate the move. This was not an easy decision to make to trust others with mom's care but she helped to calm our nerves and come to the understanding that this was not only the right choice for our mom but for us as well. I'm happy to report that mom is thriving. She's happy to see us on our visits. We get to spend quality time with her and still feel at ease when we leave. This is just a little note to thank everyone at Autumn Leaves for taking such good care of my wife Mary. You have kept her warm, dry, clean and well fed. Even though she has advanced dementia, I know that she benefits from the many mental and physical activities that you provide along with your kind words and friendly smiling faces. All the activities such as tai chi, bingo, trivia, storytelling, and interacting with outside guest performers are so much better than just to have her sitting alone in her room. Also thanks for the comfortable home-like setting that you provide with large comfortable sofas, plush chairs, piano, library, pleasant music, and fireplace in the Great Room. It has a friendly, calming feeling which almost seems like home. She also enjoys the large tv lounge and the lovely wind protected outdoor patio on warm summer days. Well over a year ago, making the decision to move Mom into a facility was the hardest decision our family has ever had to make. We chose Autumn Leaves because they concentrate on memory care with activities and socialization. It was a very good decision and we have been very happy with how Mom is cared for and treated. Many of the caregivers have been there for a long time and are exceedingly patient, kind and caring. The nurses are amazing. The office and support staff is responsive and knowledgeable as well as kind and caring. The chef provides excellent meals. We are able to visit and participate in activities whenever we wish. There is a monthly support group for families that has proven very valuable. All in all, I recommend Autumn Leaves.

Mom is so happy and contented at Autumn leaves of stc. Their care has given her a sense of well being and stability. She feels alive again and her fears have lessened. A special shout out to debbie in marketing she's such a dear. Melanie has decorated, soothed and even painted nails while covering her management duties. The activities director is so kind and patient. He often serenades the residents on the lobby grand piano. The nursing staff works so hard keeping their patients healthy, clean and calm. We moved our dad here almost 8 years ago and have been so thankful for the love and care he receives each and every day! The staff here really care about the residents. They treat everyone with such love and respect - they are such a blessing to our family. The decision to move a loved one away from their home is never an easy decision, but dad now considers Autumn Leaves his home and the caregivers an extension of family. They do activities daily to keep the residents engaged and involved. My dad has always commented on the food being good - although most days he doesn’t remember what it was - but it was good! . My father-in-law moved into the facility in January of 2022. We have visited on a regular basis during weekdays, weekends, and at all different times of the day. What do like about Autumn Leaves of St. Charles? First, they welcome you with open arms, learn and use your names, and know all the residents' names. Second, they build a sense of community. The staff is friendly, patient, and kind. They have a challenging job. Finally, it is very clean. They have done a great job of communicating with us and have been receptive and responsive to feedback when we have questions and/or concerns. We feel lucky to have a facility in our community that provides such excellent care.

My husband has been at Autumn Leaves for just over a month and I'm very pleased with my choice of facilities (and I visited all within a 10-mile radius of St. Charles). The staff is very caring; from the facility director to the activities coordinator...to the nurses and aides....to the restaurant staff. While the residents don't spend very much time in their rooms, the bedrooms are large and well-appointed. I was initially attracted by the smaller size (single story with only about 40 residents "give or take") and the layout which is easy for residents to navigate. The common areas are bright and airy and welcoming. Looking forward to enjoying the outdoor patios once the weather warms! If you're considering memory care for your spouse or parent, definitely check out Autumn Leaves!

Autumn Leaves of St. Charles really understands the needs of people living with dementia. The administrators, the nurses, the care-givers, food preparers, the maintenances workers, offer the residents one-on-one attention. They are kind, respectful and always professional. The rooms are clean and decorated with the residents’ personality in mind. It is a smaller community. Everyone knows your name, be you resident or visitor. There’s always an activity for the residents to engage and the staff very encouraging. I highly recommend this community to others.
