I had the privilege of visiting my cousin again at this community. WOW what a difference from last year. New management was brought in, and new Administrator she is very nice and attentive to the staff and residents. She introduced herself right away and was actually out on the floor walking around during lunch checking on things. Big change since the last administrator was there. I am liking this new change and Applause to them for buying this community and turning it around. I see all new faces right down to the manager. Maybe that was the change they needed. New paint new smells. His building was horrible dingy and dirty before smelled like urine and cigars now it looks amazing. New manager has really done her job. My cousin we won't say his name is always talking about the food in the past and the old manager always just sitting in her office, he said he likes the way things are running now. I am very happy for him.

Kudos to Nucare for stepping into this community, the changes that i see are absolutely mind blowing. This community has done a complete 360 in just a few months. The new administrator is amazing. I can't wait to see what else they do for the residents at this community.

I am so happy they have new management, and a new team at this facility. Nicole is absolutely great, knowledgeable and very friendly. I was not wanting to leave my cousin here when i came to visit back in January. He moved in a year ago and it's not been great. Last manger had been there for a few years, and the building smelled horrible like cigars, and it was dirty and just didnt look like a healthy environment. I have visited the last few days since I came to town on Business. WOW what a difference they have really turned it around. New paint and new staff and new management team it looks and feels great. I can sleep at night now and not worry. Kudos for getting new managers as the old ones needed to be arrested for neglect.

Another night of [name removed] not answering for myself or the staff my dad was missing in the building and they did not know where he was he was found in the emergency exit asleep on the floor not ok asked staff for details and they did not speak English to answer me so how in the world are they communicating with residents if they cannot speak English.

MY MOTHER MISSES APPOINTMENTS AND I HAVE PICTURES OF EDIBLES (THC) THAT SHE HAD ON HER DESK THE MORNING STAFF WITH THE COLORFUL HAIR IN THE MAIN BUILDING SELLS WEED TO THE STAFF MY MOTHER IS FRIENDS WITH STAFF AND HAS VIDEO OF THIS ALSO. COMMPASSION AND LOVE ARE NO LONGER AT THIS FACILITY ITS ALL FRIENDS AND FAMILY THAT COVER FOR EACHOTHER THEY LIE ABOUT FALLS WHEN I KNOW THEY ARE NOT BEING REPORTED INDIVIDULAS SUCH AS RESIDENTS AND STAFF OVER DOSING ON HERION THAT WAS NEVER REPORTED THE DIRECTOR COVERED IT UP. THEY NEED TO BE SHUT DOWN IM LOOKING FOR PLACEMENT FOR MY MOTHER TO MOVE OUT ALONG WITH CONTACTING OTHER RESIDENTS FAMILY TO GET THEM OUT. THEY DONT HAVE SUPPLIES TO CHANGE PEOPLE USING TOWELS OR PAPER TOWELS INSTEAD OF WIPES. DRUG ADDICTS AND TIME STEALERS BECAUSE THESE STAFF DONT CARE OR CLEAN. ALL BUILDINGS HAVE DRUGS IN THEM OR USERS AND THE STAFF ARE SUPPORTING AND GIVING THE RESIDENTS DRUGS. HOW IS THIS HELPING LIVE A FUFILFUL LIFE? My father was in this facility for 3 months, he stated he was not taken care of, staff would not prepare a decent meal for any of the residents. He had a special diet and it was not honored. He ended up in the hospital due to the lack of good, decent healthy food. The aides did not take people to the hall for the meal or take them back to their room. The left people sitting on the toilet for at least an hour or more while they were on their phones or doing their hair. I was told it was like a prison, and no one gave a darn if you lived or died!!!!! I wouldn't put my worst enemy in this establishment. It is abusive, negligent and all down to the manager [Name Removed] who is never there, along with who ever owns this facility should be prosecuted to the full extent of the law. This happened in 2022, they have not done anything to improve!!!! Negative Zero Rating!!!!!!

I would not recommend The Cottages after our experience with a family member with terminal cancer at this dangerous facility for a total of a month between diagnosis and passing away. The rooms are sparse and small. When they moved in, there was only a bed, nothing else. We had to provide furniture. Our interactions with the manager and owner were confusing. They promised things like a better diabetic diet when we complained, but in 4 wks it never changed. We complained about no call button and that bedridden residents had to yell for staff -- twice they told us they'd provide a baby monitor but throughout the month it never came. One night they had to sit on the edge of their bed for the entire night because no one responded to their yelling for assistance to lie down. Our family member said there were staff that were mean. There was no chair in the room nor space for one. The staff bickers and swears at each other so loudly I could hear them in the background of phone calls. The owner said up front she did not like the hospice we'd picked and didn't work well with them -- it's the biggest, most used hospice in our city. When an aid judged pills couldn't be swallowed by our family member, he just skipped administering them. He skipped two doses in a row of all the meds (half a day of dozens of prescriptions) without telling us or the resident's Dr. of the problem. The aid and manager claimed that was because there were no orders to crush medicine, but upon investigation of this we found out that there had always been orders to crush medicine since day 1 there. Twice our family member went to the ER because of medicine not administered while at this facility. We were told during COVID all residents could only have window visits, but when we called ahead for our visit, we were told the manager said we couldn't because the terrain was unsafe outside their particular window. We arrived and took a look and it was flat grass so we insisted on visiting (it was Christmas). The kind staff member on duty went to a lot of trouble, getting them to the window, using a lift to place him in a wheel chair. Afterwards, she told us she was the only staff member on duty that shift for the entire building (that usually had several on each shift). Privacy locks on patient's doors had been turned around so patients could be locked inside their rooms. When confronted on this, the manager said she thought it was an allowable"form of restraint". There were no activities at all. Managers claimed it was the general staffs' responsibility to plan activities but if you asked staff, they didn't know that. The diet was insufficient calories. A breakfast consisted of one small biscuit with cold gravy. A lunch (for a diabetic with high blood pressure, mind you) was two pieces of white bread with a thin slice of ham and a thin piece of cheese and a small bag of potato chips. We recommend staying away from this facility until state inspections find it adequate. The failings of this facility have been written up and can be viewed as a public record at the Wisc. Dept. of Health Services. Check for complaints and health department violations for any facility you are considering, particularly The Cottages. I wish I had done so before my family member experienced negligence and abuse here. A staff member at this WI Health Dept. number can talk you through multiple steps needed to find the complaints on the web site, should you have any trouble. Hi Rachelle, Its been a few weeks now and Kathleen seems to be settling into a routine at Cottages of Madison-Oakwood. I just completed a phone call with Jasmine regarding questions I had about Kathleen. I want you to know that you have a gem of an employee with Jasmine. She is thoughtful, caring and extremely helpful. As you can imagine moving Kathleen at a time when visitation is prohibited and having extremely minimal contact for the last year, has been stressful for the family as well as Kathleen. Its people like Jasmine and her staff that help ease the concern so we can breathe a sigh of relief. So, starting with the warm, friendly and informative phone calls I first had with you, to the ease of Kathleens move and finally to the excellent care she is receiving, we (the family) couldnt be happier. Making the choice to put my mother in a facility wasnt an easy choice but the staff has been helpful in the transition. Getting regular calls and updates from the staff has helped reassure me that this is the best place for my mother. Shes a notoriously picky eater and the cook there has taken the time to get to know what she likes and makes sure to have some of her favorite snacks for her. Thanks Cottages for all you do!

Cottages of Madison has always surpassed our expectations with care and communication about my dearest friend. When she moved in she had completely given up on life and had no desire to eat. She had a feeding tube and had refused to let us use it. The staff were patient with her and took the time to feed her and sit with her while she ate. It was a very long process for her, but she started gaining weight and eventually was back to her normal weight. They continued to work with her on strengthening her muscles and with physical therapy involved she was able to start walking, which was something that we never dreamed possible. They allow the family to bring her the favorite meal for her and have window meetings wither her. She was admitted on hospice and is no longer on it. We were able to have so much valuable time with her during the holidays. Anytime I have called to ask questions about her health and wellbeing they answer all of my questions and are very diligent about anytime she has to go to the hospital to make sure to call me to let me know of her status. I am positive that had we not chosen Cottages of Applewood that my friend would not be here. Thank you Cottages for everything you do for her!

I have had nothing but great experiences working with both Cassie Wolf and Rachell Atchinson-Bennet. Both are extremely professional, creative, and quick thinking! They ensure that everyone stays as happy and healthy and safe as possible and truly have the best interest of all of their residents that they serve. I have had nothing but positive conversations and interaction with both of these hardworking, loving, and thoughtful ladies. They are always there to lend a helping hand and get you through any difficult situation you or your family may be facing. Their dedication, background knowledge, and willingness to work with familys has been nothing shy of impressive but admirable as well. I would recommend Cottages of Wisconsin for these amazing two ladies alone along with all of the other loving, compassionate and hard working employees .

I have always had incredibly high standards for placements through the years and Cottages surpasses those expectations by far. The moment you walk through the door you immediately feel at home with the decor, staff, and other residents. Every staff Ive had the pleasure of seeing always has a smile on their face, and the residents couldnt help but rave about how much they love living at Cottages. Rachell, the Administrator, has always been nothing short of amazing to work with. She is detail oriented, compassionate, polite, and easy to work with. she has gone above and beyond to ensure their move is as smooth as possible. She is more than just an administrator. She will jump at the opportunity for improvement within her facilities, staff, and self. Its hard to find placement these days where the Administrator shows their unconditional support and build relationships with their residents, and I am so lucky to have found Rachell that can provide exactly that! I would recommend Cottages to anyone, including my own family. Thank you Cottages for being such a wonderful placement!

My father has been getting excellent care at the Cottages of Madison since they took over ownership a few months ago. I have been extremely impressed with the level of service and communication. Not only does my dad seem to have a higher level of engagement from staff, Rachelle has seen to it that we have frequent updates & finds ways for me to participate in his life via phone, Skype. And I know she genuinely cares about him, his well-being, and possible ways to elevate his experience there. Living far away from my dad is difficult, but because of the extraordinary care & service they provide, I am able to maintain a relationship with him. I am reassured hes in good hands!
